    Estado, escolas e famílias: públicos escolares e regulação da educação

    O texto apresenta alguns resultados de uma investigação recentemente concluída que, genericamente, se inscreve nos complexos processos de (multi)regulação da educação. O estudo desenvolveu-se num concelho do norte de Portugal (convencionalmente designado Vila Formosa), circunscrevendo-se às escolas com oferta de ensino secundário. A análise dos dados sugere que, na topografia complexa dos processos de (multi)regulação do campo escolar, a combinação de medidas de política educativa com recursos e cursos de acção que as diferentes escolas e as diversas categorias de famílias e jovens detêm e adoptam constituem uma tríade de fontes de dinâmicas, umas vezes convergentes, outras vezes em tensão que, por seus efeitos sociológicos cumulativos, geram a reordenação do campo de lutas concorrenciais em torno da educação, lutas essas que parecem penalizar sobretudo as famílias que já sofrem de outras desvantagens.The text presents some results of a recently concluded investigation that, generically, is part of the complex processes of the (multi) regulation of education. The study was developed in a district in the north of Portugal (conventionally designated Vila Formosa) and was limited to schools that offered secondary education. The analysis of the data suggests that, in the complex topography of the processes of (multi)regulation of the school field, the combination of measures of educational policy with resources and courses of action that the different schools and the diverse categories of families and young people possess and adopt constitute a triad of dynamic sources, sometimes convergent and sometimes tense, that, by their cumulative sociological effects, generate the re-ordination of the field of competing struggles in education that seem to penalize, above all, the families that already suffer other disadvantages.El texto presenta algunos resultados de una investigación recientemente finalizada que, genéricamente, se inscribe en los complexos procesos de (multi) regulación de la educación. El estudio se desarrolló en un consejo del norte de Portugal (asignado convencionalmente Vila Formosa), circunscribiéndose a las escuelas con oferta de enseñanza secundaria.     Para o estudo da evolução do ensino e da formação em administração educacional em Portugal

    Estudos sobre a evolução do ensino de disciplinas, na formação de professores em Portugal, são recentes. O controle burocrático centralizado reteve as dimensões do controle político-administrativo. De certo modo, protegeu a esfera educativa das influências modernizantes, do capitalismo industrial e das lógicas mercantis e gerencialistas. Defendeu a educação do domínio político, da intervenção de movimentos sociais, das propagandas de ideais democráticos e da cidadania. A utilização da designação "Administração educacional" ilustra as dificuldades sentidas, ao longo dos últimos anos, em termos da construção acadêmica de uma área, seja pela falta de tradição, seja pelos antecedentes históricos.In Portugal, studies about the evolution of disciplines teaching in the teachers formation are recent. The centralized bureaucratic control has held back the dimensions of politic administrative control. In a certain way, it has protected the education against the new-fashioned influences, manufacturing capitalism, and mercantile and managerial logics. This centralized bureaucratic control has also profected the education against the politic dominion, the intervention of social movements, the advertising of democratic ideals, and against the citizenship. The use of the term "Educational administration" shows the difficulties met by the searchers along the latest years, since there is no tradiction nor historic antecedence

    Quasi-iatrogenia vascular – dois relatos de caso

    INTRODUÇÂO: As lesões vasculares iatrogénicas são uma causa importante de morbilidade e mortalidade após intervenções cirúrgicas ortopédicas ou neurocirúrgicas. Neste artigo, os autores apresentam dois casos clinicos de quasi-iatrogenia vascular. CASOS CLÍNICOS: Dois casos, um procedimento ortopédico e um procedimento neurocirúrgico, são apresentados. O primeiro caso refere-se a uma mulher de 80 anos com migração do componente acetabular de uma prótese da anca, associado a trombose da veia ilíaca externa. O segundo caso reporta um homem de 27 anos com compressão medular ao nível de D6 por motivo de um hemangioma ósseo. Após a operação, verificou-se a presença de material de fixação (parafuso) fazendo protusão para o lúmen da aorta torácica. Este foi removido num segundo momento. Ambas as operações decorreram sem complicações. CONCLUSION: Os autores consideram que uma abordagem multidisciplinar contribuiu significativamente para o sucesso dos casos apresentados

    Fístula Arteriovenosa após Nefrectomia Radical: A Propósito de um Caso Clínico Arteriovenous fistula after Radical Nephrectomy:: a case report

    Existem três tipos diferentes de fístulas arteriovenosas (FAV) renais, as congénitas, as idiopáticas e as adquiridas, sendo as últimas as mais frequentes. Dentro destas, as resultantes da nefrectomia são uma raridade. A FAV entre a artéria renal e a veia cava inferior (VCI) é uma complicação rara após nefrectomia, estando descritos cerca de 10 casos na literatura. Os autores descrevem um caso de um homem de 51 anos, insuficiente renal crónico (IRC) em hemodiálise, enviado à consulta de Urologia por nódulo sólido do rim direito sugestivo de neoplasia, detectado em ressonância magnética (RMN). Foi submetido a nefrectomia radical direita, tendo sido efectuada laqueação conjunta do pedículo renal, sem intercorrências cirúrgicas. Quatro meses depois, inicia clínica de sobrecarga hídrica, de difícil controlo com a hemodiálise e surge um sopro abdominal localizado no flanco direito, contínuo. O estudo por eco-doppler abdominal e angiografia demonstrou a presença de fístula entre a artéria renal e a veia cava inferior. Foi submetido a embolização da FAV com coils, tendo-se registado migração dos mesmos para a artéria pulmonar. A sua recolha foi efectuada imediatamente com basket, sem outras intercorrências. Posteriormente, foi submetido a laparotomia com laqueação simples da artéria renal justa-aórtica, com resolução e estabilização do quadro clínico. A propósito deste caso clínico os autores discutem a abordagem diagnóstica e a hierarquia terapêutica da FAV reno-VCI nomeadamente na cirurgia directa e endovascular.<br>There are three different types of renal arteriovenous fistula (AVF), congenital, idiopathic and acquired, being the latter the most frequent. Within these, those resulting from nephrectomy are rare. The AVF between the renal artery and the inferior vena cava (IVC) is a rare complication, with about 10 cases described in literature. The authors describe the case of a 51 year old man, with chronic renal failure (CRF) on hemodialysis, sent to a Urology consultation because of a solid nodule in the right kidney, suggestive of malignancy, found by MRI. He underwent radical nephrectomy and conjoined ligation of the renal pedicle, without surgical complications. Four months later, fluid overload symptoms appeared, difficult to control with dialysis, associated with a continuous right abdominal flank murmur. Eco-Doppler and Angiographic studies showed a fistula between the renal artery and the inferior vena cava. He underwent embolization of the AVF with coils, and migration of the coils into the pulmonary artery occured. The collection of these was performed immediately, and without complications, with basket. Subsequently, the patient was subjected to a laparotomy with simple ligation of the proximal aortic renal artery, with stabilization and resolution of clinical status. Regarding this case the authors discuss the diagnostic approach and therapeutic hierarchy of renal-IVC arteriovenous fistula, in direct and endovascular surgery

    TFIIH-dependent MMP-1 overexpression in trichothiodystrophy leads to extracellular matrix alterations in patient skin

    Mutations in the XPD subunit of the DNA repair/transcription factor TFIIH result in distinct clinical entities, including the cancer-prone xeroderma pigmentosum (XP) and the multisystem disorder trichothiodystrophy (TTD), which share only cutaneous photosensitivity. Gene-expression profiles of primary dermal fibroblasts revealed overexpression of matrix metalloproteinase 1 (MMP-1), the gene encoding the metalloproteinase that degrades the interstitial collagens of the extracellular matrix (ECM), in TTD patients mutated in XPD compared with their healthy parents. The defect is observed in TTD and not in XP and is specific for fibroblasts, which are the main producers of dermal ECM. MMP-1 transcriptional up-regulation in TTD is caused by an erroneous signaling mediated by retinoic acid receptors on the MMP-1 promoter and leads to hypersecretion of active MMP-1 enzyme and degradation of collagen type I in the ECM of cell/tissue systems and TTD patient skin. In agreement with the well-known role of ECM in eliciting signaling events controlling cell behavior and tissue homeostasis, ECM alterations in TTD were shown to impact on the migration and wound-healing properties of patient dermal fibroblasts. The presence of a specific inhibitor of MMP activity was sufficient to restore normal cell migration, thus providing a potential approach for therapeutic strategies.     Thinking beyond borders: how can humanities and social sciences help to deal with climate change health hazards in the XXI century? Outputs of the transdisciplinary network "Composing worlds: humanities, well-being and health"

    Climate Change (CC) is the most noticeable evidence of the “Metamorphosis of the World” and it is changing regimes. The unpredictability of climate phenomena and their multidimensional impacts on human and environmental health has contributed to making our understanding of the world increasingly difficult. Furthermore, history proved that technoscience alone is not enough to deal with these issues. CC, diseases linked to poverty and human migration, health systems increasingly required by democratic societies, and artificial intelligence, are all technical issues, but also social, which require a broad understanding for creative solutions. A global call is now waking up for the humanities and social sciences visions to deal with the human, and non-human matters raised by CC, namely through the integration of health and sustainability areas. This chapter aims to present and discuss some of the first outputs of the transdisciplinary network “Composing worlds: humanities, well-being and health in the 21st century”, which consists of a network of experts in the humanities and social and health sciences who think about the issues of wellbeing and health in contemporary technological societies.     Climate Change and Health: Essay on the Limits of Naturalism

    One of the main problems of the twenty-first century is climate change and its impacts on global health. Nonetheless, there is a certain optimism that is rooted in the new world opened up by the possibilities of technoscience, namely in the control of health and climatic and environmental hazards. But on the horizon of these possibilities, there are increasingly clear signs that technological and scientific naturalism alone is running out of capacity to contain its own challenges. Naturalism has problems in resolving climate change impacts on health, founding ethics and epistemology, and defining relations with non-humans. In an era that is largely dependent on the actions that result from our thinking, there is a call to imagine multiverses or different versions of what naturalism seems to present as a single reality.
