Stability of Ceftiofur Sodium and Cefquinome Sulphate in Intravenous Solutions
Stability of ceftiofur sodium and cefquinome sulphate in intravenous solutions was studied. Chromatographic separation and quantitative determination were performed by using a high-performance liquid chromatography with UV-DAD detection. During the stability study, poly(vinylchloride) minibags were filled with a solution containing 5 mg of ceftiofur sodium or cefquinome sulphate and diluted to 0.2 mg/mL with suitable intravenous solution depending on the test conditions. The solutions for the study were protected from light and stored at room temperature (22°C), refrigerated (6°C), frozen (−20°C) for 30 days, and then thawed at room temperature. A comparison of results obtained at 22°C and 6°C for the same intravenous solutions showed that temperature as well as components of solutions and their concentration had an influence on the stability of ceftiofur sodium and cefquinome sulphate. It was found that ceftiofur sodium and cefquinome sulphate dissolved in intravenous solutions used in this study may be stored at room temperature and at 6°C for up to 48 h
Podstawy medycyny personalizowanej raka jelita grubego
Personalised treatment which is a dynamically developing branch of medicine, is based on individualisation of diagnostic and therapeutic procedures. Its aim is to optimise treatment by increasing therapy effectiveness, while minimising side effects. It is designed both for patients with a diagnosed hereditary cancer syndrome, as well as those with sporadic cancers. In the case of a diagnosed colorectal cancer, personalised treatment requires patient selection based on predictive factors. This involves determination of the genetic status within the epidermal growth factor receptor (EGFR) signalling pathway, including assessment of the cancer tissue genotype with respect to RAS gene mutations (KRAS, NRAS) and BRAF gene mutations. In patients who do not respond to anti-EGFR targeted therapy, chemotherapy aimed at vascular endothelial growth factor (VEGF) is introduced. In personalised medicine it is also essential to introduce prophylactic and therapeutic measures, both in carriers of germline mutations, and members of their families who have not been diagnosed with this mutation, but who meet family history and clinical criteria of hereditary cancer syndrome.Leczenie personalizowane, jako dynamicznie rozwijająca się gałąź medycyny, opiera się na indywidualizacji postępowania diagnostycznego oraz terapeutycznego. Ma na celu optymalizowanie leczenia dzięki zwiększeniu skuteczności terapii, przy jednoczesnym zminimalizowaniu działań niepożądanych. Przeznaczone jest zarówno dla pacjentów z rozpoznanym dziedzicznym zespołem predyspozycji do nowotworów, jak i pacjentów z nowotworami sporadycznymi. Założenia leczenia personalizowanego w przypadku rozpoznania raka jelita grubego wymagają selekcji chorych na podstawie czynników predykcyjnych. To oznacza, że należy określić status genetyczny w obrębie szlaku sygnałowego receptora naskórkowego czynnika wzrostu (EGFR), w tym ocenić genotyp tkanki nowotworowej pod kątem mutacji genów RAS (KRAS, NRAS) oraz genu BRAF. U pacjentów niewrażliwych na chemioterapię zawierającą elementy anty-EGFR do leczenia wprowadza się chemioterapię, której celem jest czynnik wzrostu śródbłonka naczyniowego (VEGF). W medycynie personalizowanej istotne jest również wdrożenie działań profilaktyczno-terapeutycznych, zarówno u nosicieli mutacji germinalnych (dziedzicznych), jak i u członków rodzin, w których mutacja taka nie została zidentyfikowana, spełnione są natomiast kryteria rodowodowo-kliniczne, które pozwalają na rozpoznanie zespołu dziedzicznej predyspozycji do nowotworów
Differences in sweet taste perception and its association with the "Streptococcus mutans" cariogenic profile in preschool children with caries
The aim of the study was to verify the hypothesis about differences in sweet taste perception in the group of preschool children with and without caries, and to determine its relationship with cariogenic microbiota and the frequency of sweets consumption in children. The study group included of 63 children aged 2–6 years: 32 with caries and 31 without caries. The study consisted of collecting questionnaire data and assessment of dental status using the decayed, missing, filled in primary teeth index (dmft) and the International Caries Detection and Assessment System (ICDAS II). The evaluation of sweet taste perception was carried out using a specific method that simultaneously assessed the level of taste preferences and the sensitivity threshold for a given taste. The microbiological analysis consisted of the assessment of the quantitative and qualitative compositions of the oral microbiota of the examined children. The sweet taste perception of children with caries was characterized by a lower susceptibility to sucrose (the preferred sucrose solution concentration was >4 g/L) compared to children without caries (in the range ≤ 4 g/L, p = 0.0015, chi-square test). A similar relationship was also observed for frequent snacking between meals (p = 0.0038, chi-square test). The analysis of studied variables showed the existence of a strong positive correlation between the perception of sweet taste and the occurrence and intensity of the cariogenic process (p = 0.007 for dmft; and p = 0.012 for ICDAS II), as well as the frequency of consuming sweets (p ≤ 0.001 for frequent and repeated consumption of sweets during the day, Spearman test) in children with caries. Additionally, children with an elevated sucrose taste threshold were more than 10-times more likely to develop S. mutans presence (OR = 10.21; 95% CI 3.11–33.44). The results of this study suggest the future use of taste preferences in children as a diagnostic tool for the early detection of increased susceptibility to caries through microbial dysbiosis towards specific species of microorganisms
Guidelines from the Polish Surgical Society and Polish Society of Oncological Surgery Concerning Quality Assurance for Centres Performing Cytoreductive Procedures and HIPEC Procedures in the Treatment of Primary and Secondary Peritoneal Tumours
Surgical treatment of patients with peritoneal metastases in combination with Hyperthermic intraperitoneal Chemotherapy (HIPEC) and systemic treatments is applied with increasing frequency and, with correct patient qualification, allows for obtaining 5-year survival at a level of 32–52%. The conditions necessary for positive results of such treatment include the high experience of a given centre, its appropriate infrastructure, and appropriate patient qualification for the procedure. As a result of the debate connected with the need to evaluate treatment quality and results, at the request of the Peritoneal Cancer Section of the Polish Society of Oncological Surgery, the conditions for quality assurance were worked out and a Quality Assurance Commission was set up for the centres performing cytoreductive procedures and HIPEC procedures in the treatment of primary and secondary peritoneal tumours
Recommendations for diagnostics and therapy of gastrointestinal stromal tumors (GIST) in 2008
Nowotwory podścieliskowe przewodu pokarmowego (gastrointestinal stromal tumors – GIST) są najczęstszymi nowotworami
pochodzenia mezenchymalnego w obrębie przewodu pokarmowego. Nowotwory te, dzięki postępom w diagnostyce
patologiczno-molekularnej, powszechnie rozpoznawane są dopiero od kilku lat. W pracy przedstawiono zalecenia dotyczące
diagnostyki i terapii tych nowotworów, opracowane na podstawie danych naukowych oraz doświadczenia ekspertów,
które są powszechnie akceptowane i warte rekomendacji. Nadekspresja błonowego receptora KIT, będąca następstwem
mutacji protoonkogenu KIT, jest wysoce specyficzna dla GIST, i możliwa do wykrycia metodami immunohistochemicznymi
(CD117) w preparatach histopatologicznych z guza, stanowiąc najważniejsze kryterium w jego diagnostyce mikroskopowej
i wskazaniach do leczenia drobnocząsteczkowymi inhibitorami kinaz tyrozynowych. W każdym przypadku zalecane jest
przesłanie materiału do badań molekularnych (w celu analizy mutacji KIT i PDGFRA). Radykalne leczenie operacyjne jest nadal najskuteczniejszą metodą leczenia pierwotnych GIST, jednakże nowotwór ten cechuje duży potencjał nawrotowości.
W przypadkach zmian nieoperacyjnych/rozsianych leczeniem z wyboru jest zastosowanie inhibitora kinaz tyrozynowych –
imatinibu, leku, który stanowi pierwsze efektywne leczenie systemowe w zaawansowanym GIST CD117(+). Zalecana dawka
początkowa powinna wynosić 400 mg raz dziennie (800 mg dziennie w przypadku mutacji w eksonie 9 KIT). Monitorowanie
leczenia musi opierać się na powtarzanym badaniu tomografii komputerowej jamy brzusznej, z uwzględnieniem zmian
wielkości i gęstości. W przypadku progresji zaleca się zwiększenie dawki imatinibu do 800 mg na dobę, a w przypadku
braku skuteczności zastosowanie sunitynibu w dawce początkowej 50 mg dziennie. Obecnie trwają badania kliniczne nad
ustaleniem roli leczenia chirurgicznego w skojarzeniu z imatinibem oraz skuteczności innych leków celowanych molekularnie
w przypadku występowania oporności w czasie leczenia imatinibem. Dostępne dane odnośnie leczenia uzupełniającego
wskazują na poprawę przeżyć wolnych od nawrotu, ale wpływ tej strategii na przeżycia całkowite i optymalna długość czasu
leczenia są nadal przedmiotem badań. Przedstawione zasady postępowania diagnostyczno-terapeutycznego powinny być
praktycznie wprowadzane przez lekarzy zajmujących się chorymi na GIST w Polsce. Zaleca się rejestrowanie przypadków
chorych na GIST w ramach narodowego Rejestru Klinicznego ([email protected]) oraz standardowe leczenie chorych
w wielodyscyplinarnych zespołach, mających doświadczenie w terapii GIST i włączanie nowych przypadków GIST do

in the understanding of the molecular mechanisms of GIST pathogenesis have resulted for last years in the emerging of
GIST as a distinct sarcoma entity. The paper describes guidelines for diagnostics and therapy of these tumors based on
scientific basis and experts’ experience, which are commonly accepted and worth to recommend. Overexpression of KIT
receptor, as a consequence of mutation of protooncogene KIT, is highly specific for GIST and enable for detection by
immunohistochemistry staining (CD117) in tumor specimens. It is the most important criterion in microscopic diagnostics
and indications for treatment with small-molecule tyrosine kinas inhibitors. Sending material for molecular analysis is
strongly recommended (for KIT and PDGFRA genotyping). Radical surgery is still the mainstay treatment for primary,
localized, resectable GISTs, although high percentages of the patients after potentially curative operations develop recurrent
or metastatic disease. In inoperable/metastatic lesions the treatment of choice is tyrosine kinase inhibitor – imatinib mesylate
– the first effective systemic therapy in advanced CD117(+) GIST. Recommended initial dose should be 400 mg daily
(800 mg for exon 9 KIT mutants). Monitoring of the therapy should be based on serial computed tomography imaging of
abdominal cavity with the assessment of changes of tumor size and density. In case of disease progression the increase of
imatinib dose to 800 mg daily is recommended and if further progression exists – sunitinib in the initial dose 50 mg daily
should be introduced. Clinical trials evaluating the role of surgery in combination of imatinib and the efficacy of other
molecular targeted drugs in resistant cases are ongoing. Existing data indicate beneficial role of adjuvant imatinib therapy
in terms of relapse-free survival, but the impact of this policy on overall survival, optimal duration of adjuvant therapy and
criteria for its implementation are currently investigated. Presented recommendations for diagnostics and therapy of GIST
should be practically implemented by physicians involved in management of GIST patients in Poland. The including GIST
cases in national Clinical Registry ([email protected]) and standard treatment of patients in multidisciplinary team with
expertise in GIST therapy, as well as enrollment of new cases to prospective clinical trials, are recommended
