    Microbial polysaccharides with nutraceutical potential and bioactive properties have been investigated in detail during the last few decades. There is an increasing demand in food industries for live microbes or polysaccharides produced by them which assert health benefits other than dietetic constituents. Although there are a large number of exopolysaccharide (EPS)-producing bacteria, the titers are low for commercialization. This manuscript deals with the polysaccharides produced by probiotic strains, with major emphasis on the EPSs, their properties, applications and some of the strategies adopted which would be helpful in better understanding of the process in the near future. Research on the improved EPS biosynthesis is essential for obtaining high yields. Therefore, to reach commercialization, metabolic engineering must be applied.Posljednjih su nekoliko desetljeća potanko istraživani mikrobni polisaharidi koji imaju bioaktivna, hranjiva i farmakoloÅ”ka svojstva. U prehrambenoj industriji postoji sve veća potreba za primjenom živih mikroorganizama ili mikrobnih polisaharida koji, osim hranjivih, imaju i ljekovita svojstva. Iako postoji mnogo vrsta bakterija koje proizvode egzopolisaharide, proizvedene su količine nedovoljne za komercijalnu uporabu. U radu su prikazani polisaharidi iz probiotičkih sojeva, a osobito egzopolisaharidi, njihova svojstva i primjena, te navedene strategije razvoja proizvodnje Å”to bi mogle pridonijeti razumijevanju procesa.     HIV-infected presumptive tuberculosis patients without tuberculosis: How many are eligible for antiretroviral therapy in Karnataka, India?

    For certain subgroups within people living with the human immunodeficiency virus (HIV) [active tuberculosis (TB), pregnant women, children <5years old, and serodiscordant couples], the World Health Organization recommends antiretroviral therapy (ART) irrespective of CD4 count. Another subgroup which has received increased attention is "HIV-infected presumptive TB patients without TB". In this study, we assess the proportion of HIV-infected presumptive TB patients eligible for ART in Karnataka State (population 60million), India. This was a cross-sectional analysis of data of HIV-infected presumptive TB patients diagnosed in May 2015 abstracted from national TB and HIV program records. Of 42,585 presumptive TB patients, 28,964 (68%) were tested for HIV and 2262 (8%) were HIV positive. Of the latter, 377 (17%) had active TB. Of 1885 "presumptive TB patients without active TB", 1100 (58%) were already receiving ART. Of the remaining 785 who were not receiving ART, 617 (79%) were assessed for ART eligibility and of those, 548 (89%) were eligible for ART. About 90% of "HIV-infected presumptive TB patients without TB" were eligible for ART. This evidence supports a public health approach of starting all "HIV-infected presumptive TB patients without TB" on ART irrespective of CD4 count in line with global thinking about 'test and treat'

    Oxidative stress, resulting from dysregulation in the secretion of adrenal hormones, represents a major concern in human health. The present review comprehensively examines various categories of endocrine dysregulation within the adrenal glands, encompassing glucocorticoids, mineralocorticoids, and androgens. Additionally, a comprehensive account of adrenal hormone disorders, including adrenal insufficiency, Cushingā€™s syndrome, and adrenal tumors, is presented, with particular emphasis on their intricate association with oxidative stress. The review also delves into an examination of various nutritional antioxidants, namely vitamin C, vitamin E, carotenoids, selenium, zinc, polyphenols, coenzyme Q10, and probiotics, and elucidates their role in mitigating the adverse effects of oxidative stress arising from imbalances in adrenal hormone levels. In conclusion, harnessing the power of nutritional antioxidants has the potential to help with oxidative stress caused by an imbalance in adrenal hormones. This could lead to new research and therapeutic interventions

    Knowledge, Attitude and Practice of Mothers regarding Diarrhoeal Illness in Children under Five Years of Age: A Cross Sectional Study in an Urban Slum of Delhi, India

    Diarrhoea is one of the lead killers of children worldwide. This study was done with an aim to determine the knowledge, attitude and practice regarding diarrhoeal illness, its prevention and management in mothers of under- five children. A descriptive cross- sectional study; 125 mothers were randomly selected from an urban slum of Delhi and were interviewed using a structured questionnaire. 96% mothers defined diarrhoea appropriately. Most common perceived causes of diarrhoea were contaminated food and drinking water (80%). Though 83% mothers believed that clean drinking water prevents diarrhoea, yet water treatment was practiced by only 36%. Most mothers believed in (90%) and practiced (88%) hand washing with soap post-defecation to prevent diarrhoea. While only 31% had knowledge on importance of use of latrine, fewer (19%) accepted that safe disposal of stool was very important and just 58% reported practicing it. Less than a third of the mothers recognized critical signs of dehydration. 79% mothers recognized importance of increased fluid requirement and 70% practiced it for management of diarrhoea. Though 76% mothers used ORS, only 26% considered it as the mainstay treatment of diarrhoea. Also, 42% mothers had incomplete knowledge regarding proper preparation of ORS. Though only 22% were aware of the role of breastfeeding in prevention of diarrhoea, a positive attitude towards (74%) and healthier practice (90%) of exclusive breast-feeding was observed. Thus, we conclude that though the community knowledge, attitude and practices on diarrhoeal illness and its prevention is marginally satisfactory, the poor knowledge regarding signs of dehydration and relevance of ORS as primary management component is a matter of concern for child survival
