Next, argues Schuman, comes the curriculum; the context of the learning that takes place when honors faculty and honors students come together is framed by the curriculum. Honors curricula provide opportunities for honors students to endeavor challenges beyond what traditional undergraduate curricula provide. For faculty, honors is a unique opportunity to blend research and teaching and to provide a curricular laboratory for experimenting with varied topics and pedagogical approaches

    Gout in a 15-year-old boy with juvenile idiopathic arthritis: A case study

    Joint pain is a common complaint in pediatrics and is most often attributed to overuse or injury. In the face of persistent, severe, or recurrent symptoms, the differential typically expands to include bony or structural causes versus rheumatologic conditions. Rarely, a child has two distinct causes for joint pain. In this case, an obese 15-year-old male was diagnosed with gout, a disease common in adults but virtually ignored in the field of pediatrics. The presence of juvenile idiopathic arthritis (JIA) complicated and delayed the consideration of this second diagnosis. Indeed, the absence of gout from this patient’s differential diagnosis resulted in a greater than two-year delay in receiving treatment. The patients’ BMI was 47.4, and he was also mis-diagnosed with osteochondritis dissecans and underwent medical treatment for JIA, assorted imaging studies, and multiple surgical procedures before the key history of increased pain with red meat ingestion, noticed by the patient, and a subsequent elevated uric acid confirmed his ultimate diagnosis. With the increased prevalence of obesity in the adolescent population, the diagnosis of gout should be an important consideration in the differential diagnosis for an arthritic joint in an overweight patient, regardless of age

    Favorable outcomes with reduced steroid use in juvenile dermatomyositis

    BACKGROUND: High-intensity glucocorticoid regimens are commonly used to induce and maintain remission in Juvenile Dermatomyositis but are associated with several adverse side-effects. Evidence-based treatment guidelines from North American and European pediatric rheumatology research societies both advocate induction with intravenous pulse steroids followed by high dose oral steroids (2 mg/kg/day), which are then tapered. This study reports the time to disease control with reduced glucocorticoid dosing. METHODS: We retrospectively reviewed the records at a single tertiary-care children\u27s hospital of patients diagnosed with Juvenile Dermatomyositis between 2000 and 2014 who had a minimum of 2 years of follow-up. The primary outcome measure was time to control of muscle and skin disease. Additional outcome measures included glucocorticoid dosing, effect of treatment on height, frequency of calcinosis, and complications from treatment. RESULTS: Of the 69 patients followed during the study period, 31 fulfilled inclusion criteria. Median length of follow-up was 4.58 years, (IQR 3-7.5). Myositis control was achieved in a median of 7.1 months (IQR 0.9-63.4). Cutaneous disease control was achieved in a median of 16.7 months (IQR 4.3-89.5). The median starting dose of glucocorticoids was 0.85 mg/kg/day, (IQR 0.5-1.74). The median duration of steroid treatment was 9.1 months, (IQR 4.7-17.4), while the median duration of any pharmacotherapy was 29.2 months (IQR 10.4 to 121.3). Sustained disease control off medications was achieved in 21/31 (68%) patients by the end of review. Persistent calcinosis was identified in only one patient (3%). CONCLUSION: Current accepted treatment paradigms for Juvenile Dermatomyositis include oral glucocorticoids beginning at 2 mg/kg/day and reduced over a prolonged time period. However, our results suggest that treatment using reduced doses and duration with early use of steroid-sparing agents is comparably effective in achieving favorable outcomes in Juvenile Dermatomyositis

    The Effects of Heterospecific Mating Frequency on the Strength of Cryptic Reproductive Barriers

    Heterospecific mating frequency is critical to hybrid zone dynamics and can directly impact the strength of reproductive barriers and patterns of introgression. The effectiveness of post-mating prezygotic (PMPZ) reproductive barriers, which include reduced fecundity via heterospecific matings and conspecific sperm precedence, may depend on the number, identity and order of mates. Studies of PMPZ barriers suggest that they may be important in many systems, but whether these barriers are effective at realistic heterospecific mating frequencies has not been tested. Here, we evaluate the strength of cryptic reproductive isolation in two leaf beetles (Chrysochus auratus and C. cobaltinus) in the context of a range of heterospecific mating frequencies observed in natural populations. We found both species benefited from multiple matings, but the benefits were greater in C. cobaltinus and extended to heterospecific matings. We found that PMPZ barriers greatly limited hybrid production by C. auratus females with moderate heterospecific mating frequencies, but that their effectiveness diminished at higher heterospecific mating frequencies. In contrast, there was no evidence for PMPZ barriers in C. cobaltinus females at any heterospecific mating frequency. We show that integrating realistic estimates of cryptic isolation with information on relative abundance and heterospecific mating frequency in the field substantially improves our understanding of the strong directional bias in F1 production previously documented in the Chrysochus hybrid zone. Our results demonstrate that heterospecific mating frequency is critical to understanding the impact of cryptic post-copulatory barriers on hybrid zone structure and dynamics, and that future studies of such barriers should incorporate field-relevant heterospecific mating frequencies

    A Land of Plenty? Colonial Diet in Rural New Zealand

    Colonial New Zealand was built on the ideal of creating better lives for settlers. Emigrants came looking to escape the shackles of the class-system and poor conditions in Industrial Revolution period Britain. Colonial propaganda claimed that most emigrants achieved their aims, but the lives the colonists actually experienced upon reaching New Zealand remain relatively unexplored from a biosocial perspective. In this paper we present a pilot study of stable isotope results of bone collagen from seven adults interred in the St. John’s Cemetery (SJM), Milton, New Zealand (ca. AD 1860–1900). We interpret the diet at Milton and broadly compare our isotopic results with contemporaneous samples from Britain. We show that, like contemporary Britain, the diet of our studied individuals was focused on C3 crops and terrestrial meat sources. Despite higher ????15N values in contemporary UK populations (which can simplistically be interpreted as indicative of higher meat intake), consideration of different local baselines makes it likely that this New Zealand population had relatively similar levels of meat intake. Interestingly marine resources did not form an important part of the Milton diet, despite the site's proximity to the ocean, hinting at the possible stigmatisation of local resources and the development of a European New Zealand (pākehā) food identity

    The Foundation for Building the Conservation Capacity of Community Ecology

    Ecology is a young discipline that needs to develop into a predictive science to confront the challenges of human population pressures and habitat degradation. Basic ecology has disproportionately focused on undisturbed, charismatic ecosystems, species and academic questions, leaving gaps in its ability to inform the conservation and management of degraded, threatened ecosystems. Foundation species-dependent organisms have been studied at the expense of the habitat-forming species that build and maintain communities. We used cobble beaches as a model system to discuss the consequences of this disparity on translational ecology. We suggest that the historic development of ecology has led to an academic discipline ill-suited for proactive conservation. We propose that the incorporation of foundation species and a hierarchical organization theory, into the conceptual framework of ecology, will improve its predictive ability and successful application in conservation and the restoration of degraded ecosystems

    The First New Zealanders: Patterns of Diet and Mobility Revealed through Isotope Analysis

    Direct evidence of the environmental impact of human colonization and subsequent human adaptational responses to new environments is extremely rare anywhere in the world. New Zealand was the last Polynesian island group to be settled by humans, who arrived around the end of the 13th century AD. Little is known about the nature of human adaptation and mobility during the initial phase of colonization. We report the results of the isotopic analysis (carbon, nitrogen and strontium) of the oldest prehistoric skeletons discovered in New Zealand to assess diet and migration patterns. The isotope data show that the culturally distinctive burials, Group 1, had similar diets and childhood origins, supporting the assertion that this group was distinct from Group 2/3 and may have been part of the initial colonizing population at the site.
