    Yo Cayetana: The Woman in Her Own Words

    Much has been said and written about Cayetana de Alba, “Grandee of Spain” and a woman who took an important part in some of the historical moments of her lifetime. The images the media conveyed of her – that of a frivolous woman - is, yet, contradicted by her autobiography. The aim of this paper is, therefore, to investigate Cayetana’s discursive gendered positions from an interdisciplinary standpoint: based on a lexicometric approach of the data collected, the semio-linguistic interpretation highlights the possibilities and modalities of her Self represented in images and emotions involved in the real world and in a historical conjuncture in which the present and the past are articulated. Her words enable us to understand the strategies she uses to construct her identity and her own meanings in the representation of her lived experience in the interstices of the Self and of social relations (Probyn, 1993)

    Chronic wasting disease risk assessment in Portugal: analysis of variability and genetic structure of the Portuguese roe deer population

    Among the Transmissible Spongiform Encephalopathies, Chronic Wasting Disease (CWD) in cervids is now the rising concern in wildlife within Europe after the first case detected in Norway in 2016. CWD shows a notable horizontal transmission, affecting both free-ranging and captive cervids.     The multiple meanings of "wheezing": a questionnaire survey in Portuguese for parents and health professionals

    <p>Abstract</p> <p>Background</p> <p>Most epidemiological studies on pediatric asthma rely on the report of "wheezing" in questionnaires. Our aim was to investigate the understanding of this term by parents and health professionals.</p> <p>Methods</p> <p>A cross-sectional survey was carried out in hospital and community settings within the south of Portugal. Parents or caregivers self-completed a written questionnaire with information on social characteristics and respiratory history. Multiple choice questions assessed their understanding of "wheezing". Health professionals (physicians, nurses and physiotherapists) were given an adapted version. We used bivariate analysis and multivariate models to study associations between definitions of "wheezing" and participants' characteristics.</p> <p>Results</p> <p>Questionnaires from 425 parents and 299 health professionals were included. The term "wheezing" was not recognized by 34% of parents, more frequently those who were younger (OR 0.4 per 10-year increment, 95% CI 0.3-0.7), had lower education (OR 3.3, 95% CI 1.5-7.4), and whose children had no history of respiratory disease (OR 4.6, 95% CI 2.5-8.7) (all ORs adjusted). 31% of parents familiar with "wheezing" either did not identify it as a sound, or did not locate it to the chest, while tactile (40%) and visual (34%) cues to identify "wheezing" were frequently used. Nurses reported using visual stimuli and overall assessments more often than physicians (p < 0.01). The geographical location was independently associated with how parents recognized and described "wheezing".</p> <p>Conclusions</p> <p>Different meanings for "wheezing" are recognized in Portuguese language and may be influenced by education, respiratory history and regional terminology. These findings are likely applicable to other non-English languages, and suggest the need for more accurate questionnaires and additional objective measurement instruments to study the epidemiology of wheezing disorders.</p

    Humanities in the Future Tense: Mapping Interdisciplinary Approaches in the Research Project Landscapes of the Self

    The project entitled “Landscapes of the Self” emerged from the feeling that research is undertaken in a world that no longer breaks down according to existing disciplinary boundaries. The approach to the study of autobiographies covers a range of areas, enables the understanding of contexts of experience of the self, and is grounded in the inherent paradigms of the disciplines that frame and sustain this research—discourse analysis and cultural studies. An open-minded and sustained form of communication between these different fields of knowledge has underpinned the production of knowledge on discourse and representation in political autobiography. This article focuses on the two volumes of Cavaco Silva’s “Political Autobiography,” highlighting the representation of the emotional profile of the former Portuguese prime minister (1985–1995) in the construction of his political identity. Based on Cavaco Silva’s words and on the frequency with which he uses them, the article shows how emotions unfold in his discourse and reveal his intimate and public identity both as a man and as a statesperson

    Resumo: Introdução: Pretende-se determinar se a utilização do método INSURE em recém-nascidos de muito baixo peso, com idade gestacional (IG)â¤30 semanas se associa a menor morbilidade e mortalidade quando comparado com uso de nCPAP isolado. Métodos: Estudo de coorte retrospectiva dos RNMBP nascidos num hospital de apoio perinatal diferenciado entre Janeiro/2002-Agosto/2008. Incluíram-se as crianças registadas na Vermont-Oxford Network com IG â¤30semanas e com uso de nCPAP logo após o nascimento (N=96).Estabeleceu-se Grupo nCPAP â uso de nCPAP isolado (N=40) e Grupo INSURE (N=56). Consideraram-se outcomes precoces: síndroma de dificuldade respiratória (SDR), necessidade de administrar surfactante com intuito terapêutico ou recurso a ventilação invasiva. Compararam-se igualmente outcomes tardios: doença pulmonar crónica da prematuridade (DPC), mortalidade e o outcome composto DPC-mortalidade. Resultados: No grupo INSURE verificou-se menor IG, não se registando diferenças relativamente ao peso ao nascer e indução da maturação pulmonar. Verificou-se SDR em 21/40 (53%) casos do grupo nCPAP e em 17/56 (30%) casos do grupo INSURE (OR ajustado [IC 95%] â 0,2 [0,1-0,6]). No grupo nCPAP todos estes casos realizaram surfactante com intuito terapêutico, não tendo sido efectuada nenhuma dose adicional de surfactante no grupo INSURE. Verificou-se maior número de casos com necessidade de VI no grupo nCPAP do que no grupo INSURE (11/40, 28% vs 9/56, 12%). Não foram encontradas diferenças estatisticamente significativas relativamente aos outcomes tardios. Conclusão: Em RNMBP com IG â¤30sem, o uso isolado de nCPAP poderá traduzir-se numa ligeira desvantagem em outcomes precoces, sem evidentes repercussões em termos de DPC ou mortalidade.Rev Port Pneumol 2010; XVI (5): 779-795 Abstract: Background: Evidence suggests the INSURE strategy (INtubate-SURfactant administration and Extubate to nasal continuous positive airway pressure [nCPAP]) is superior to mechanical ventilation (MV) with rescue surfactant for the management of respiratory distress syndrome (RDS) in very low birth weight (VLBW) neonates. There is limited data, however, to assess whether INSURE is superior to nCPAP alone. We aimed to compare these two strategies regarding early and late outcomes. Methods: Retrospective cohort study from Jan/2002 to Aug/2008. We included VLBW neonateswith gestational age (GA)â¤30 weeks registered in the Vermont Oxford Network that used either nCPAP alone (nCPAP group, N=40), or with INSURE (INSURE group, N=56).We assessed early (RDS, need for rescue surfactant, and CPAP failure with MV) and late outcomes (chronic lung disease of prematurity (CLD), mortality, and composite outcome of mortality and/or CLD). Results: Neonates in the INSURE group had lower mean GA, while other baseline variables were comparable. The proportion of RDS was significantly superior in the nCPAP group (53% vs. 30%; adjusted OR [95% CI] â 0.2 [0.1-0.6]). Therapeutic surfactant was administered to all cases with RDS in the nCPAP group, but to none from the INSURE group.
