Different Attitude toward the Concept of "Baras" in Traditional Persian Medicine
زمینه و هدف: برص اصطلاحی پزشکی و قدیمی است که امروزه از آن به تعابیر مختلفی یاد میشود. گروهی آن را پیسی، بعضی لک و پیس و برخی دیگر ویتیلیگو میدانند. تعریف صحیح این واژه، ردّ شبهات وارده بر مفهوم آن و مقایسه یافتههای حاصل از مرور منابع طب سنتی و طب رایج در این موضوع، از مهمترین اهداف این تحقیق میباشد.
مواد و روشها: در این کنکاش علمی سعی شده است که علاوه بر بازنگری تحلیلی و ادبی به برص، این مفهوم در طب سنتی ایران و طب رایج مورد بررسی، مقایسه و تحلیل قرار گیرد.
یافتهها: بر اساس منابع طب سنتی ایران، برص بیماری پوستی مزمنی است که در برخی موارد، تا عمق بدن نیز نفوذ میکند و با خارش و سوزش دردناک همراه است و به دلیل عوارض ظاهری، باعث دوری شخص از اجتماع میشود.
نتیجهگیری: مفهوم برص تنها معادل لکههای سفید صِرف و یا ویتیلیگوی امروزی نیست.Background and Aim: The Old Persian medical term of "Baras" is called variously today. Some people know it as "Pisi" and some others as "Lak-O-Pis" and/ or "Vitiligo". Providing an appropriate definition for the term "Baras", rejecting the uncertainties and comparing the findings of the Traditional Persian Medicine (TPM) to the Conventional Medicine are the most important objectives of the present study.
Materials and Methods: The present research attempts to study the definitions of the term "Baras" comparatively in TPM and the Conventional Medicine, in addition to the analytical and literal review of the concepts.
Findings: According to TPM, "Baras" is a chronic skin disease, accompanied with itching and painful burning, which in some cases it is possible to penetrate to the depth of the person’s body and make him/ her unsociable, for its unpleasant apparent symptoms.
Conclusion: "Baras" is not simply a disease accompanied with white spots which are mistakenly called "Lak-O-Pis" and/ or "Vitiligo".

Topical application of Cassia fistula L. fruit gel in management of cutaneous lesions of pemphigus vulgaris: A double-blind, placebo-controlled clinical trial
Objective: Cassia fistula L. fruit extract has been traditionally used in the treatment of pemphigus vulgaris (PV) lesions in Iran. The aim of this study was to determine the efficacy of C. fistula fruit gel on healing time of PV lesions in a clinical setting.Materials and Methods: This was a randomized, double-blind placebo-controlled clinical trial that was performed in dermatology ward at Saadi hospital, affiliated to Shiraz University of Medical Sciences, Shiraz, Iran. Right- or left- sided lesions of PV patients on standard systemic treatment were randomized for treatment with either C. fistula fruit gel or placebo prescribed twice daily. The largest diameter of each lesion was measured at the baseline (day 0) and on days 10 and 20. Epithelialization Index (EI), as outcome measure was calculated and compared between the two groups.Results: The present study comprised 20 patients, with overall 82 cutaneous lesions including 41 lesions in the C. fistula fruit gel group and 41 lesions in the placebo group. The EI in the C. fistula fruit gel group was significantly higher than that of the placebo group both on day 10 (65±28vs 30±34; p=0.001) and at the end of the study (91±22 vs 69±49; p=0.003). Conclusion: Topical application of C. fistula fruit gel can be considered as an effective adjuvant therapy in treatment of PV

مروری بر کاربردها و عوارض حجامت در طب سنّتی ایران
There is an increasing trend to CAM therapies worldwide. Cupping is one of the oldest medical practices which are very common intervention in Asian societies like China and Iran. There are 5 way for removing extra blood from the body in the resources of Iranian traditional medicine. Wet cupping is more common in Iranian culture. Despite the general acceptance of the bloodletting in our society, unfortunately, there is very limited information about side effects and precautions of this method among the people, even academics. Cupping is divided into two groups. Cupping with scarification and Cupping without scarification. The dry cupping is without bloodletting. Cupping with scarification and bloodletting works in the way that the morbid matters evacuated through the scarified skin, and cupping without scarification deviates the morbid matters from the compromised organs. In Iranian traditional medicine, wet cupping is recommended for 2 groups. Wet cupping in the health state is just for preventing the overwhelming of the blood humor in the susceptible individuals. In disease states, the wet cupping is a treatment for diseases in them, the patients confront with the abundance of the blood humor. Therapeutic effects of wet cupping considered as cleansing the morbid matters especially when there is abundance of sanguine humor with or without the other humors. The people who are under 2 years of age and above 60 years old are not suitable for bloodletting. Wet cupping is contraindicated in the people who are emaciated and anemic. The primary conditions for prescription of the wet cupping are determine based on the patient's physical and mental status, age, habits and environmental conditions. Specific requirements will be determined based on the type of disease and the symptoms of organ involvement.روند فزایندهای در استفاده از طب مکمل در میان جوامع گوناگون به وجود آمده است. از این میان، حجامت یکی از شیوههای طب مکمل است که در فرهنگهای گوناگون آسیایی مانند چین و ایران رواج بسیاری دارد. در منابع طب سنّتی ایران پنج شیوه برای خارج کردن خون از بدن ذکر شده است. از این میان، حجامت بیش از سایر روشها استفاده میشود. باوجود اقبال عمومی بسیار زیاد به حجامت در جامعه ما، متأسفانه اطلاعات درباره عوارض و احتیاطات این شیوه درمانی در میان مردم و حتی دانشگاهیان بسیار محدود است. حجامت دو گونه است: حجامت با شرط، که در این نوع همراه با گذاشتن لیوان حجامت تیغزدن نیز صورت میگیرد. در نوع دوم که حجامت بدون شرط است، تنها از لیوان حجامت یا بادکش استفاده میکنند و خون گرفته نمیشود.
در حجامت همراه با شرط یا تیغزدن بسته به تشخیص درمانگر و وضعیت بیمار میزان متناسبی خون گرفته میشود. در طب سنّتی ایران حجامت برای دو گروه توصیه شده است: گروه اول افراد سالمی هستند که در معرض خطر غلبه خون در بدن هستند. دوم، بیمارانی که به علت تجمع ماده فاسد در یکی از اعضای خود نیاز به خارج کردن خون فاسد از بدن دارند تا طبیعت بتواند بر بیماری غلبه کند. سن زیر دو سال و بالای 60 سال برای انجام حجامت مناسب نیست. افرادی که ضعیف و لاغرند و کمخونی دارند، نباید تحت حجامت با شرط قرار گیرند. شرایط عمومی انجام این شیوه درمانی بر اساس وضعیت جسمی و روحی بیمار، سن، عادات و شرایط محیطی تعیین میشود. شرایط اختصاصی نیز بر اساس نوع بیماری مشخص میشود و بر اساس نشانههای بیماری محل خاصی برای حجامت در نظر گرفته میشود

Massage therapy is considered as an important part of Iranian traditional medicine for preservation of health and treatment of diseases. This intervention scrutinised by Iranian physicians including Ghotb-ed-din Shirazi, hakim and physician of the 7th hijri.
Massage therapy is considered as a prioritized way of treatment for special disorders in Iranian medicine. These include reopening the abnormal trapping of substances, fitness of organs locally, removing the windy substances, overcoming the coolness in temperament, changing the flow of substances, preservation of body humidity and establishing a balance between softness and stiffness of body.
With respect to different application of massage in Iranian medicine including increase the warmness of a local area, undermine the substances and preservation of humidity; stretching, low, long, short, harsh, soft and mild massage are considered.
In fact, quantity and quality of massage is in coordination with individual differences, age and place; and the technique including time, stress and harshness of the massage is planned based on mentioned circumstances.
Iranian physicians paid special attention to sport massage and they recommended specific massages before and after the exercises. They also mentioned the foot massage in their manuscripts and promoted it as an important health issue in their community. To revive these valuable techniques in the present time, we have no choice but reviewing the scientific legacy and treasure that we have forgotten it for a long timeماساژ درمانی از جمله روشهای پیشگیری و درمانی مطرح در پزشکی ایرانی است که از سوی پزشکان ایرانی از جمله «قطب الدین شیرازی» دانشمند و طبیب ایرانی قرن هفتم هجری مورد مداقه قرار گرفته است.
ماساژ درمانی از منظر پزشکی ایرانی روشی برگزیده برای مقابله با اختلالات خاص، شامل اجتماع موضعی مواد در بدن، پروش موضعی اندام، تحلیل مواد ریحی، دفع غلبه سردی، تغییر جریان گردش مواد در بدن، ممانعت از تحلیل رطوبتهای ضروری و معتدل کننده نرمی یا سفتی بدن است.
در طب ایرانی با توجه به کاربردهای مختلف ماساژ اعم از افزایش گرمی موضعی، تحلیل مواد یا ممانعت از دفع بیش از حد رطوبات بدنی سبکهای مختلف پرفشار، ضعیف، طولانی مدت، کوتاه مدت، خشن، نرم و معتدل ماساژ تعریف شده است.
در واقع کمیت و کیفیت ماساژ در طب ایرانی متناسب با تفاوتهای بدنی، سنی و شرایط محیطی افراد انتخاب میشود و بر این اساس است که مدت، قوت و خشونت ماساژ و نیاز به روغنمالی یا نوع روغن تعیین میشود.
پزشکان ایرانی توجه ويژهاي به ماساژ ورزشی داشته و ماساژهای خاصی را قبل و بعد از ورزش توصیه کردهاند. حكماي ايراني مالش کف پا را به عنوان یک امر بهداشتی- درمانی در سطح جامعه خود ترویج میکردند. به نظر میرسد برای احیاي دوبارهي «دلك» و «غمز» در جامعهي امروز، بازیابی نظرات دانشمندان ایرانی و مرور علمی میراث به یادگار مانده از گذشتگان این سرزمین کمک کننده باشد
"Plague" in Tradition Persian Medicine and Modern Medicine
زمینه و هدف: طاعون یکی از قدیمیترین بیماریهای عفونی با مرگ و میر بالا است که از طریق باکتری یرسینیا پستیس (Yersinia Pestis) ایجاد میشود و با شروع ناگهانی تب، درد و تورم غدد لنفاوی بروز پیدا میکند. هدف از این مطالعه، بررسی بیماری طاعون از منظر طب سنتی ایران و مقایسه یافتههای حاصل با رویکرد طب رایج به طاعون است.
مواد و روشها: در این مطالعه، ما منابع معتبر طب سنتی ایران در قرون مختلف شامل کتاب «قانون در طب»، «ذخیره خوارزمشاهی»، «اکسیر اعظم» را مطالعه و مطالب آنها را با یافتههای اخیر در خصوص طاعون مورد مقایسه قرار دادیم.
یافتهها: طاعون در طب سنتی ایران تعریفی مشابه طب رایج دارد و یک بیماری واگیردار همراه با تب و تورم گوشتهای سست یا غدد لنفاوی بدن توصیف شده است. تعفن هوا و استعداد بدنی افراد از عوامل مستعدساز ابتلای به این بیماری شمرده شده است و جا به جایی از هوای عفونی گرم به هوای لطیفِ معتدل، مصرف داروهای گیاهی مانند انار، آلوبخارا، سرکه و استعمال عطریات و بخورات مانند عود، صندل و گلاب از مهمترین توصیههای اطبای ایرانی در پیشگیری از این بیماری میباشند و مصرف گل ارمنی و جَدوار در درمان آن مفید بیان شده است.
نتیجهگیری: شواهد اخیر نشان میدهند که توصیههای بهداشتی درمانی طب سنتی ایران در خصوص طاعون با موازین علمی روز منطبق است و بررسی بیشتر منابع طب ایرانی میتواند منجر به بازشدن دریچههای تحقیقاتی جدید در مقابله با اپیدمیهای عفونی از جمله طاعون شود.Background and Aim: "Plague" is one of the oldest infectious diseases with high mortality caused by the Yersinia pestis bacteria. It is characterized by the sudden onset of fever, followed by pain and lymphadenopathy. The aim of this study is to review the viewpoints of Traditional Persian Medicine (TPM) concerning the plague and to compare the findings of the study with conventional medical approach.
Materials and Methods: In this study, we reviewed the reliable resources of TPM in various centuries including The Canon of Medicine, Zakhireh kharazmshahi and Exir-e-Azam to make a comparison between traditional and conventional approaches concerning the plague.
Findings: Plague in traditional resources has a similar definition in comparison to conventional medicine; as a febrile disease accompanied with warm swelling of soft parts of the body. Septic air and body dystemperaments (abnormal changes of the whole body or an organ temperament) are predisposing factors of catching the disease. Moving from hot septic air into fresh air, eating some medicinal fruits and preparations such as pomegranate, prune and vinegar, also using perfumes like joss stick, sandalwood, and rose water are the most common hygienic recommendations of Persian traditional practitioners for the disease. Applying Zedoary (Curcuma zedoaria) and Armenia's clay are the other therapeutic recommendations in dealing with plague.
Conclusion: Recent findings show that Persian traditional approach on plague meet today's scientific standards. New research ideas can be derived from reviewing written resources of TPM in dealing with infectious epidemics such as the plaque.

A review on botanicals with wound healing activity for pemphigus vulgaris: perspective of traditional Persian medicine and conventional medicine
Objective: As a rare autoimmune disease, pemphigus vulgaris has a poor prognosis especially in lack of proper medical support. This blistering disease involves both the skin and mucus membranes. The challenge is improving the healing process of skin lesions of which, superimposed infections are among the main causes of the disease mortality. Accordingly, we aimed to assess the treatment options suggested by traditional Persian medicine (TPM) and compare them with current findings. Materials and Methods: We studied the main clinical and pharmaceutical textbooks of TPM (Kitāb al-hāwīfī al-tibb, the Canon of Medicine, Eksir-e-Aazam, Tuhfat al-mu'minīn, Makhzan al-adviyah (focusing on the skin chapter and respective herbal remedies for the inflamed skin and ulcers. Additionally, scientific databases such as PubMed, Science direct, Scopus, and Google Scholar were searched for the current pharmacological evidence. In the studied books, the term “hot ulcers” was found close to what is known as “Pemphigus vulgaris”. Results: Reported medicinal herbs possess anti-inflammatory, antioxidant, wound healing, and antibacterial activities reported by recent studies. Therefore, they could be introduced as novel natural remedies for pemphigoid wounds. Conclusion: Taken as a whole, the review of traditional remedies for hot ulcers in Persian medical and pharmaceutical literature may open a new window toward developing new topical treatments for this disease
Massage Therapy in Iranian Traditional Medicine
Background and Aim: Massage training and how the body is influenced by massage are common issues which are seriously under study and discussion in Iranian traditional medicine. Iranian physicians considered motion and massage as major principles of health maintenance.In this study, we examined the available literature of traditional medicine to evaluate location, purpose and use of massage therapy in Iranian medicine in comparison with other popular conventional styles.The aim of Iranian massage is to regulate the core body temperature and aid to eliminate the waste products from the body. This type of massage is divided into five categories including solid, soft, moderate, great and aggressive, based on the intensity, speed, duration and techniques of massage.Iranian physicians proposed general body massage or massage of a particular area based on subjective complaints. They recommended specific massages in particular groups including children, pregnant women, the elderly and athletes. In some cases, the effects of these recommendations have been studied in clinical trials.Conclusion: It seems that the major difference between Iranian massage and other styles of massage is special attention of Iranian massage to the individual circumstances, and the cause of the problem rather than technique of the massage
