    thesisMethods have been developed for the study of amino acid incorporation into a specific protein under established conditions of net synthesis. The results of these studies have been reported and discussed. Synthesis and storage of the secretory proteins of mouse pancreas have been shown to occur between fifteen to twenty-one hours after pilocarpine injections. The concentration of amylase, ribonuclease, and total pancreatic proteins has been shown to increase rapidly during that period. Deoxyribonucleic acid as well as ribonucleic acid content of the pancreatic tissue does not increase during the period of protein synthesis. Injection of the labeled amino acid, valine, into pilocarpine treated mice eighteen hours after stimulation, followed by chromatographic resolution of the components of an extract of pancreas, has allowed a demonstration of radioactive labeling of the enzyme ribonuclease under conditions of net synthesis. The ratio of radioactivity to enzymatic activity has been used to define a specific activity for that enzyme. Analyses of the total radioactivity present in the pancreatic tissue as well as the total radioactivity of each of four cell fractions have been carried out. It was found that ten minutes after intraperitoneal injection of isotope approximately ten per cent of the radioactivity is present in the pancreas. Radioactivity appears most rapidly in the microsomal fraction of the cell during the first ten minutes but at later periods the soluble portion of the cell becomes more intensely labeled. The distribution of labeled ribonuclease in the various parts of the pancreatic cells has been studied as a function of time. The data indicate that the microsomal portion of the acinar cells is the site of formation of the secretory enzyme, ribonuclease, in the living mouse. The distribution of labeled ribonuclease in the other cell fractions suggests that the newly formed enzyme is transferred to the zymogen granules by way of the soluble portion of the cell. From a consideration of the rates of isotope uptake into the total homogenate and the microsomal fraction, as well as the rate of appearance of labeled ribonuclease in the microsomal fraction, it has been estimated that the labeled amino acid is present in the microsomes for about three minutes before it appears in the pancreatic enzyme. It has been proposed that this time lag is the period required for the elaboration of the finished, enzymatically active protein, ribonuclease, in the microsomal portion of the cells

    Properties of Hot Stars in the Wolf-Rayet galaxy NGC5253 from ISO Spectroscopy

    ISO-SWS spectroscopy of the WR galaxy NGC5253 is presented, and analysed to provide estimates of its hot young star population. Our approach differs from previous investigations in that we are able to distinguish between the regions in which different infrared fine-structure lines form, using complementary ground-based observations. The high excitation nebular [SIV] emission is formed in a very compact region, which we attribute to the central super-star-nucleus, and lower excitation [NeII] nebular emission originates in the galactic core. We use photo-ionization modelling coupled with the latest theoretical O-star flux distributions to derive effective stellar temperatures and ionization parameters of Teff>38kK, logQ=8.25 for the compact nucleus, with Teff=35kK, logQ<8 for the larger core. Results are supported by more sophisticated calculations using evolutionary synthesis models. We assess the contribution that Wolf-Rayet stars may make to highly ionized nebular lines (e.g. [OIV]). From our Br(alpha) flux, the 2" nucleus contains the equivalent of approximately 1000 O7V star equivalents and the starburst there is 2-3Myr old; the 20" core contains about 2500 O7V star equivalents, with a representative age of 5Myr. The Lyman ionizing flux of the nucleus is equivalent to the 30 Doradus region. These quantities are in good agreement with the observed mid-IR dust luminosity of 7.8x10^8 L(sun) Since this structure of hot clusters embedded in cooler emission may be common in dwarf starbursts, observing a galaxy solely with a large aperture may result in confusion. Neglecting the spatial distribution of nebular emission in NGC5253, implies `global' stellar temperatures (or ages) of 36kK (4.8Myr) and 39kK (2.9 or 4.4Myr) from the observed [NeIII/II] and [SIV/III] line ratios, assuming logQ=8.Comment: 16 pages, 7 figures, uses mn.sty, to appear in MNRA

    Upper gastrointestinal bleeding in Scotland 2000-2010: Improved outcomes but a significant weekend effect

    Aim: To assess numbers and case fatality of patients with upper gastrointestinal bleeding (UGIB), effects of deprivation and whether weekend presentation affected outcomes. Methods: Data was obtained from Information Services Division (ISD) Scotland and National Records of Scotland (NRS) death records for a ten year period between 2000-2001 and 2009-2010. We obtained data from the ISD Scottish Morbidity Records (SMR01) database which holds data on inpatient and day-case hospital discharges from non-obstetric and non-psychiatric hospitals in Scotland. The mortality data was obtained from NRS and linked with the ISD SMR01 database to obtain 30-d case fatality. We used 23 ICD-10 (International Classification of diseases) codes which identify UGIB to interrogate database. We analysed these data for trends in number of hospital admissions with UGIB, 30-d mortality over time and assessed effects of social deprivation. We compared weekend and weekday admissions for differences in 30-d mortality and length of hospital stay. We determined comorbidities for each admission to establish if comorbidities contributed to patient outcome. Results: A total of 60643 Scottish residents were admitted with UGIH during January, 2000 and October, 2009. There was no significant change in annual number of admissions over time, but there was a statistically significant reduction in 30-d case fatality from 10.3% to 8.8% (P &lt; 0.001) over these 10 years. Number of admissions with UGIB was higher for the patients from most deprived category (P &lt; 0.05), although case fatality was higher for the patients from the least deprived category (P &lt; 0.05). There was no statistically significant change in this trend between 2000/01-2009/10. Patients admitted with UGIB at weekends had higher 30-d case fatality compared with those admitted on weekdays (P &lt; 0.001). Thirty day mortality remained significantly higher for patients admitted with UGIB at weekends after adjusting for comorbidities. Length of hospital stay was also higher overall for patients admitted at the weekend when compared to weekdays, although only reached statistical significance for the last year of study 2009/10 (P &lt; 0.0005). Conclusion: Despite reduction in mortality for UGIB in Scotland during 2000-2010, weekend admissions show a consistently higher mortality and greater lengths of stay compared with weekdays

    Inhibition of delta-like ligand 4 induces luteal hypervascularization followed by functional and structural luteolysis in the primate ovary

    Using specific inhibitors established that angiogenesis in the ovarian follicle and corpus luteum is driven by vascular endothelial growth factor. Recently, it has been demonstrated that the Notch ligand, delta-like ligand 4 (Dll4) negatively regulates vascular endothelial growth factor-mediated vessel sprouting and branching. To investigate the role of Dll4 in regulation of the ovarian vasculature, we administered a neutralizing antibody to Dll4 to marmosets at the periovulatory period. The vasculature was examined on luteal d 3 or d 10: angiogenesis was determined by incorporation of bromodeoxyuridine, staining for CD31 and cell death by staining for activated caspase-3. Ovulatory progesterone rises were monitored to determine effects of treatment on luteal function and time to recover normal cycles in a separate group of animals. Additionally, animals were treated in the follicular or midluteal phase to determine effects of Dll4 inhibition on follicular development and luteal function. Controls were treated with human IgG (Fc). Corpora lutea from marmosets treated during the periovulatory period exhibited increased angiogenesis and increased vascular density on luteal d 3, but plasma progesterone was significantly suppressed. By luteal d 10, corpora lutea in treated ovaries were significantly reduced in size, with involution of luteal cells, increased cell death, and suppressed plasma progesterone concentrations. In contrast, initiation of anti-Dll4 treatment during the midluteal phase produced only a slight suppression of progesterone for the remainder of the cycle. Moreover, Dll4 inhibition had no appreciable effect on follicular development. These results show that Dll4 has a specific and critical role in the development of the normal luteal vasculature

    Vocal Cord Dysfunction Etiologies and Treatment

    Abstract: Vocal cord dysfunction, a syndrome of paradoxical inspiratory closure of the vocal cords, is becoming more frequently recognized and diagnosed recently since its initial modern description 30 years ago. Initially described as single case reports, the first case series in 1983 helped to clarify the typical patient and findings of vocal cord dysfunction. Recent investigations have elucidated specific etiologies such as gastroesophageal reflux, exercise, and irritants as causative factors in addition to the numerous associated psychologic factors. Speech therapy and psychotherapy have been used extensively with vocal cord dysfunction patients, but the optimal treatment has yet to be prospectively studied.     Impact of Sleep and Circadian Disruption on Energy Balance and Diabetes: A Summary of Workshop Discussions

    A workshop was held at the National Institute for Diabetes and Digestive and Kidney Diseases with a focus on the impact of sleep and circadian disruption on energy balance and diabetes. The workshop identified a number of key principles for research in this area and a number of specific opportunities. Studies in this area would be facilitated by active collaboration between investigators in sleep/circadian research and investigators in metabolism/diabetes. There is a need to translate the elegant findings from basic research into improving the metabolic health of the American public. There is also a need for investigators studying the impact of sleep/circadian disruption in humans to move beyond measurements of insulin and glucose and conduct more in-depth phenotyping. There is also a need for the assessments of sleep and circadian rhythms as well as assessments for sleep-disordered breathing to be incorporated into all ongoing cohort studies related to diabetes risk. Studies in humans need to complement the elegant short-term laboratory-based human studies of simulated short sleep and shift work etc. with studies in subjects in the general population with these disorders. It is conceivable that chronic adaptations occur, and if so, the mechanisms by which they occur needs to be identified and understood. Particular areas of opportunity that are ready for translation are studies to address whether CPAP treatment of patients with pre-diabetes and obstructive sleep apnea (OSA) prevents or delays the onset of diabetes and whether temporal restricted feeding has the same impact on obesity rates in humans as it does in mice

    A prospective evaluation of the predictive value of faecal calprotectin in quiescent Crohn’s disease

    Background: The faecal calprotectin (FC) test is a non-invasive marker for gastrointestinal inflammation. Aim: To determine whether higher FC levels in individuals with quiescent Crohn’s disease are associated with clinical relapse over the ensuing 12 months.&lt;p&gt;&lt;/p&gt; Methods: A single centre prospective study was undertaken in Crohn's disease patients in clinical remission attending for routine review. The receiver operating characteristic (ROC) curve for the primary endpoint of clinical relapse by 12 months, based on FC at baseline, was calculated. Kaplan-Meier curves of time to relapse were based on the resulting optimal FC cutoff for predicting relapse.&lt;p&gt;&lt;/p&gt; Results: Of 97 patients recruited, 92 were either followed up for 12 months without relapsing, or reached the primary endpoint within that period. Of these, 10 (11%) had relapsed by 12 months. The median FC was lower for non-relapsers, 96”g/g (IQR 39-237), than for relapsers, 414”g/g (IQR 259-590), (p=0.005). The area under the ROC curve to predict relapse using FC was 77.4%. An optimal cutoff FC value of 240”g/g to predict relapse of quiescent Crohn’s had sensitivity of 80.0% and specificity of 74.4%. Negative predictive value was 96.8% and positive predictive value was 27.6%. FC≄240ÎŒg/g was associated with likelihood of relapse 5.7 (95% CI 1.9-17.3) times higher within 2.3 years than lower values (p=0.002).&lt;p&gt;&lt;/p&gt; Conclusions: In this prospective dataset, FC appears to be a useful, non-invasive tool to help identify quiescent Crohn’s disease patients at a low risk of relapse over the ensuing 12 months. FC of 240”g/g was the optimal cutoff in this cohort.&lt;p&gt;&lt;/p&gt
