    Stabilization of Linear Systems Over Gaussian Networks

    The problem of remotely stabilizing a noisy linear time invariant plant over a Gaussian relay network is addressed. The network is comprised of a sensor node, a group of relay nodes and a remote controller. The sensor and the relay nodes operate subject to an average transmit power constraint and they can cooperate to communicate the observations of the plant's state to the remote controller. The communication links between all nodes are modeled as Gaussian channels. Necessary as well as sufficient conditions for mean-square stabilization over various network topologies are derived. The sufficient conditions are in general obtained using delay-free linear policies and the necessary conditions are obtained using information theoretic tools. Different settings where linear policies are optimal, asymptotically optimal (in certain parameters of the system) and suboptimal have been identified. For the case with noisy multi-dimensional sources controlled over scalar channels, it is shown that linear time varying policies lead to minimum capacity requirements, meeting the fundamental lower bound. For the case with noiseless sources and parallel channels, non-linear policies which meet the lower bound have been identified

    Neuroinflammatory Triangle Presenting Novel Pharmacological Targets for Ischemic Brain Injury

    Ischemic stroke is one of the leading causes of morbidity and mortality globally. Hundreds of clinical trials have proven ineffective in bringing forth a definitive and effective treatment for ischemic stroke, except a myopic class of thrombolytic drugs. That, too, has little to do with treating long-term post-stroke disabilities. These studies proposed diverse options to treat stroke, ranging from neurotropic interpolation to venting antioxidant activity, from blocking specific receptors to obstructing functional capacity of ion channels, and more recently the utilization of neuroprotective substances. However, state of the art knowledge suggests that more pragmatic focus in finding effective therapeutic remedy for stroke might be targeting intricate intracellular signaling pathways of the 'neuroinflammatory triangle': ROS burst, inflammatory cytokines, and BBB disruption. Experimental evidence reviewed here supports the notion that allowing neuroprotective mechanisms to advance, while limiting neuroinflammatory cascades, will help confine post-stroke damage and disabilities.Peer reviewe

    High risk behaviors of injection drug users registered with harm reduction programme in Karachi, Pakistan

    BACKGROUND: Surveillance data of Sindh AIDS Control Programme, Pakistan suggest that HIV infection is rapidly increasing among IDUs in Karachi and has reached 9% in 2004–5 indicating that the country has progressed from nascent to concentrated level of HIV epidemic. Findings of 2(nd )generation surveillance in 2004–5 also indicate 104/395 (26.3%) IDUs HIV positive in the city. METHODS: We conducted a cross sectional study among registered IDUs of a needle exchange and harm reduction programme in Karachi, Pakistan. A total of 161 IDUs were included in the study between October–November 2003. A detailed questionnaire was implemented and blood samples were collected for HIV, hepatitis B & C and syphilis. HIV, hepatitis B and C antibody tests were performed using Enzyme Linked Immunosorbent Assay (ELISA) method. Syphilis tests (RPR & TPHA) were performed on Randox kit. Besides calculating frequencies univariate analysis was performed using t tests for continuous variables as age, age at first intercourse and average age of initiation of addiction and chi square for categorical variables like paid for sex or not to identify risk factors for hepatitis B and C and syphilis. RESULTS: Average age of IDU was 35.9 years and average age of initiation of drugs was 15.9 years. Number of drug injections per day was 2.3. Shooting drugs in group sharing syringes was reported by 128 (79.5%) IDUs. Over half 94 (58.3%) reported paying for sex and 64% reported never using a condom. Commercial selling of blood was reported by 44 (28%). 1 of 161 was HIV positive (0.6%). The prevalence of hepatitis B was 12 (7.5%), hepatitis C 151 (94.3%) and syphilis 21 (13.1%). IDUs who were hepatitis C positive were more likely to start sexual activity at an earlier age and had never used condoms. Similarly IDUs who were hepatitis B positive were more likely to belong to a younger age group. Syphilis positive IDUs were more likely to have paid for sex and had never used a condom. CONCLUSION: Prudent measures such as access to sterile syringes, rehabilitation and opiate substitution therapies are required to reduce high risk behaviors of IDUs in Pakistan

    In 1971, Fontan and Baudet described a surgical technique for successful palliation of patients with tricuspid atresia. Subsequently, this technique has been applied to treat most forms of functional single ventricles and has become the current standard of care for long-term palliation of all patients with single ventricle congenital heart disease. Since 1971, the Fontan procedure has undergone several variations. These patients require lifelong management including a thorough knowledge of their anatomic substrate, hemodynamic status, management of rhythm and ventricular function along with multi organ evaluation. As these patients enter middle age, there is increasing awareness regarding the long-term complications and mortality. This review highlights the long-term outcomes of the Fontan procedure and management of late sequelae

    Impact of maternal respiratory infections on low birth weight - a community based longitudinal study in an urban setting in Pakistan

    Background: The health of mothers and their newborns is intricately related. The weight of the infant at birth is a powerful predictor of infant growth and survival, and is considered to be partly dependent on maternal health and nutrition during pregnancy. We conducted a longitudinal study in an urban community within Karachi to determine maternal predictors of newborn birth weight.Methods: Four hundred pregnant women were enrolled in the study during the period 2011-2013. Data related to symptoms of acute respiratory illness (fever, cough, difficulty breathing, runny nose, sore throat, headache, chills, and myalgia/lethargy) in the pregnant women were collected weekly until delivery. Birth weight of the newborn was recorded within 14days of delivery and the weight of \u3c 2.5kg was classified as low birth weight (LBW).Results: A total of 9,853 symptom episodes were recorded of fever, cough, difficulty breathing, runny nose, sore throat, headache, chills, myalgias/lethargy in the enrolled pregnant women during the study. Out of 243 pregnant women whose newborns were weighed within 14days of birth, LBW proportion was 21% (n=53). On multivariate analysis, independent significant risk factors noted for delivering LBW babies were early pregnancy weight of \u3c 57.5kg [odds ratio adjusted (ORadj)=5.1, 95% CI: (1.3, 19.9)] and gestational age [ORadj=0.3, 95% CI (0.2, 0.7) for every one week increase in gestational age]. Among mothers with high socioeconomic status (SES), every 50-unit increase in the number of episodes of respiratory illness/100weeks of pregnancy had a trend of association with an increased risk of delivering LBW infants [ORadj=1.7, 95% CI: (1.0, 3.1)]. However, among mothers belonging to low SES, there was no association of the number of episodes of maternal respiratory illness during pregnancy with infants having LBW [ORadj=0.9, 95% CI: (0.5, 3.5)].CONCLUSIONS: While overall respiratory illnesses during pregnancy did not impact newborn weight in our study, we found this trend in the sub-group of mothers belonging to the higher SES. Whether this is because in mothers belonging to lower SES, the effects of respiratory illnesses were overshadowed by other risk factors associated with poverty need to be further studied

    The Potential Clinical Utility of Circulating Tumor DNA in Esophageal Adenocarcinoma: From Early Detection to Therapy

    Esophageal adenocarcinoma (EAC) is a lethal cancer requiring improved screening strategies and treatment options due to poor detection methods, aggressive progression, and therapeutic resistance. Emerging circulating tumor DNA (ctDNA) technologies may offer a unique non-invasive strategy to better characterize the highly heterogeneous cancer and more clearly establish the genetic modulations leading to disease progression. The presented review describes the potential advantages of ctDNA methodologies as compared to current clinical strategies to improve clinical detection, enhance disease surveillance, evaluate prognosis, and personalize treatment. Specifically, we describe the ctDNA-targetable genetic markers of prognostic significance to stratify patients into risk of progression from benign to malignant disease and potentially offer cost-effective screening of established cancer. We also describe the application of ctDNA to more effectively characterize the heterogeneity and particular mutagenic resistance mechanisms in real-time to improve prognosis and therapeutic monitoring strategies. Lastly, we discuss the inconsistent clinical responses to currently approved therapies for EAC and the role of ctDNA to explore the dynamic regulation of novel targeted and immunotherapies to personalize therapy and improve patient outcomes. Although there are clear limitations of ctDNA technologies for immediate clinical deployment, this review presents the prospective role of such applications to potentially overcome many of the notable hurdles to treating EAC patients. A deeper understanding of complex EAC tumor biology may result in the progress toward improved clinical outcomes

    Numerical investigation on flame propagation in vented gas explosion

    Explosion venting technology is one of the effective and widely used methods in protection measures against accidental internal gas explosions by relieving the pressure generated within the volume. Extensive studies have been carried out to investigate factors governing to the explosion development i.e. ignition position and vent burst pressure. However, the physical and dynamic process of explosion development during the venting to ambient air is yet not well understood. The primary motivation of this research was to gain improved understanding of turbulent flame propagation in vented gas explosion, with a view to develop improved models and methods for assessing explosion risks in the process industries. Computational Fluid Dynamic (CFD) analyses using FLUENT is adopted to study the phenomenology underlying vented gas explosions. Computations were run on deflagrating turbulent flames in small-scale combustion chambers with two different volumes (0.02 m3 and 0.0065 m3), with both closed at the rear end and open at the opposite face, in order to replicate the experimental work. All cases are initialised from stagnation. Only stoichiometric concentration of propane and methane-Air mixtures was considered with different ignition positions and vent static burst pressure, Pv. From the finding, end ignition gave higher reduced overpressure on both experimental and simulation results, compared to central ignition. The inclusion of vents in the enclosures provides significant reduction on the peak overpressures. However, it has been recognised on a tendency to a less effective reduction as the vent burst pressure, Pv was further increased. The competition between combustion rate and venting rate allows the explanation on both number and intensity of the overpressure peaks observed in propane-Air explosion
