    CT Diagnosis of an Abortion-Related Retroperitoneal Space Abscess

    A female patient with a history of recent abortion was transferred in critically ill condition in our hospital. CT imaging on emergency basis depicted an abscess in the right retroperitoneal space that included the head of fetus, along with rupture of the right lateral wall of uterus. Spiral CT enabled prompt diagnosis and therapy of this rare abortion-related complication

    The effects of integrated food and bioenergy cropping systems on crop yields, soil health, and biomass quality: The EU and Brazilian experience

    Integrated food and bioenergy production is a promising way to ensure regional/national food and energy security, efficient use of soil resources, and enhanced biodiversity, while contributing to the abatement of CO2 emissions. The objective of this study was to assess alternative crop rotation schemes as the basis for integrating and enhancing the sustainable biomass production within the food-energy agricultural context. Sunn hemp (Crotalaria spp.) in rotation with wheat (Triticum spp.) in the EU and with sugarcane (Saccharum spp.) in Brazil were evaluated. Sunn hemp did not negatively affect crop's productivity and soil fertility; wheat grain yields were maintained around the mean regional production levels (6, 7, 3 and Mg ha(-1) in Greece, Italy, and Spain, respectively), and the cumulative biomass in the extended rotation (wheat straw+sunn hemp) was between 1.5 and 2.0 times higher than in the conventional rotation. In Brazil, sugarcane stalks yield in clay soils increased by around 15 Mg ha(-1) year(-1) under sunn hemp rotation in comparison with bare fallow. Moreover, sunn hemp in the EU rotations did not have negative effects on soil available macronutrients, organic matter, pH, and cation exchange capacity, neither on C and N stocks in Brazil. The qualitative characteristics (mineral, ash, and hemicelluloses contents) of the cumulated biomass were somehow higher (in average +26%, +35%, and +3.4%, respectively) than in the conventional system. In summary, in temperate and tropical climates the integration of dedicated biomass legume crops within conventional systems could lead to enhanced biomass availability, crop diversification, and efficient use (in space and time) of the land resources

    Quality standards for managing children and adolescents with bronchiectasis: an international consensus

    The global burden of bronchiectasis in children and adolescents is being recognised increasingly. However, marked inequity exists between, and within, settings and countries for resources and standards of care afforded to children and adolescents with bronchiectasis compared with those with other chronic lung diseases. The European Respiratory Society (ERS) clinical practice guideline for the management of bronchiectasis in children and adolescents was published recently. Here we present an international consensus of quality standards of care for children and adolescents with bronchiectasis based upon this guideline. The panel used a standardised approach that included a Delphi process with 201 respondents from the parents and patients’ survey, and 299 physicians (across 54 countries) who care for children and adolescents with bronchiectasis. The seven quality standards of care statements developed by the panel address the current absence of quality standards for clinical care related to paediatric bronchiectasis. These internationally derived, clinician-, parent-and patient-informed, consensus-based quality standards statements can be used by parents and patients to access and advocate for quality care for their children and themselves, respectively. They can also be used by healthcare professionals to advocate for their patients, and by health services as a monitoring tool, to help optimise health outcomes.</p

    Pennycress (Thlaspi arvense) a new non-food crop for oil-based biofuel production in Europe and USA

    The development of alternative feedstocks for producing oil-based biofuels needs to meet the majority of the following criteria: low cost, high oil content, low agricultural inputs, favorable fatty acid (FA) composition, compatibilitywith existing farm equipment and infrastructure, production in off-season from conventional commodity crops, adaptability to marginal/idle lands, and viable markets for co-products such as seed meal. Recently a “potential weed”, pennycress (Thlaspi arvense L.) has become one of the most attractive new non-food oil crops. Pennycress is highly tolerant to low temperatures, tolerating temperatures below-15°C after reaching a 4-to-6-leaves rosette stage. The seed contains up to 37% oil (DM) with the major fatty acid as erucic (36%). The fatty acid composition in pennycress has been shown to have physical properties suitable for biofuels like biodiesel and hydro-treated renewable jet fuel (HRJ). In the last decade, pennycress has attracted increasing interest as a potential oilseed for biofuel production in the USA, either biodiesel and/or jet fuel, in Europe very few studies have specifically focused on pennycress. In the present study, we compared the productivity of pennycress in response to environment in two European countries, Italy (Bologna) and Greece (Aliartos), and in two USA states, Illinois (Peoria) and Minnesota (Morris). Seed yield, seed oil content and oil compositions were evaluated in response to growing environment.     Long-Term Yields of Switchgrass, Giant Reed, and Miscanthus in the Mediterranean Basin

    Uncertainty in predictions of long-term yields of perennial grasses makes business plans untenable in the short run. Long-term data across varied environments, including marginal lands, will help in preventing uncertainty while providing farmers and entrepreneurs with sound information to estimate reliable and affordable strategies on what, where, and how long to grow perennial grasses. In the present study, the long-term yields (11 to 22&nbsp;years) of switchgrass (Panicum virgatum L.), miscanthus (Miscanthus × giganteus Greef et Deuter), and giant reed (Arundo donax L.) grown in northern and southern Mediterranean environments are reported. Switchgrass was grown in Greece and northern Italy, giant reed in southern and northern Italy, and miscanthus in southern Italy. Furthermore, lowland and upland switchgrass ecotypes were compared in Greece. Despite similar biomass productions (9.8 and 10.0&nbsp;Mg DM ha−1 for uplands and lowlands, respectively), the upland ecotypes showed a significantly higher yield stability (CV of 24 and 32&nbsp;% for uplands and lowlands, respectively) over a 17-year period. Biomass yield varied considerably across years and locations; giant reed outperformed switchgrass under northern Italy environment (21.2 and 13.6&nbsp;Mg DM ha−1 for giant reed and switchgrass, respectively). Annual yield of switchgrass was 30&nbsp;% higher in the north than south Mediterranean; miscanthus showed intermediate production compared to giant reed and switchgrass (average of 22&nbsp;years) and a CV similar to switchgrass. In summary, these results evidence that multi-location, long-term trials are strongly needed to reduce uncertainties on crop yield variability and provide more accurate data from which optimized socio-economic and environmental predictions can be achieved

    Mechanical Harvesting of Castor Bean (<em>Ricinus communis</em> L.) with a Combine Harvester Equipped with Two Different Headers: A Comparison of Working Performance

    Castor bean (Ricinus communis L.) is a promising industrial crop suitable for cultivation in marginal conditions in the Mediterranean area, but the mechanical harvesting of the seeds is still usually performed manually. In this manuscript, the authors present a preliminary test to assess the effectiveness of equipping a combine harvester with a sunflower header to mechanically harvest castor beans. Machinery performance, seed loss from impact (ISL) and cleaning systems (CSL), and seed cleaning were evaluated and compared with the results obtained from the same combine harvester equipped with a cereal header. According to the results, no statistically significant difference in CSL was found. Values ranged from 162. 41 kg dry matter (DM) ha−1 in the cereal header to 145.56 kg DM ha−1 in the sunflower header, corresponding, respectively, to 8% w/w and 7% w/w of the potential seed yield (PSY). Using the sunflower header significantly lowered ISL (158.16 kg DM ha−1, i.e., 8% w/w of PSY) in comparison with the cereal header (282.02 kg DM ha−1, i.e., 14% w/w of PSY). This suggests more gentle cutting and conveying capability of the sunflower header to harvest the plants without losing capsules. On the other hand, the use of different headers did not significantly affect the cleaning of the seeds which averaged at 20% of the total seeds collected in both cases. In conclusion, the study highlights that a conventional combine harvester equipped with a sunflower header could be the first step towards the development of a fully mechanized harvest phase in castor beans which triggers lower seed loss and does not negatively affect the cleaning capacity of the combine harvester. Further studies are also encouraged to confirm these findings in other hybrids

    Imaging of Acute Complications of Community-Acquired Pneumonia in the Paediatric Population—From Chest Radiography to MRI

    The most common acute infection and leading cause of death in children worldwide is pneumonia. Clinical and laboratory tests essentially diagnose community-acquired pneumonia (CAP). CAP can be caused by bacteria, viruses, or atypical microorganisms. Imaging is usually reserved for children who do not respond to treatment, need hospitalisation, or have hospital-acquired pneumonia. This review discusses the imaging findings for acute CAP complications and the diagnostic role of each imaging modality. Pleural effusion, empyema, necrotizing pneumonia, abscess, pneumatocele, pleural fistulas, and paediatric acute respiratory distress syndrome (PARDS) are acute CAP complications. When evaluating complicated CAP patients, chest radiography, lung ultrasonography, computed tomography (CT), and magnetic resonance imaging (MRI) can be used, with each having their own pros and cons. Imaging is usually not needed for CAP diagnosis, but it is essential for complicated cases and follow-ups. Lung ultrasound can supplement chest radiography (CR), which starts the diagnostic algorithm. Contrast-enhanced computed tomography (CECT) is used for complex cases. Advances in MRI protocols make it a viable alternative for diagnosing CAP and its complications.</p
