    Community knowledge, attitude and practice on rabies, incidence in humans and animals and risk factors to rabies in selected districts of Tigray Region, Ethiopia

    Community awareness and assessing the trend of suspected rabies cases play a significant role in preventing its fatality. Therefore, a cross-sectional study design was employed (October 2016 - April 2017) to assess community knowledge, attitude and practice (KAP), and Incidence and risk factors to rabies (human and animal) in the study area. A semi-structured questionnaire was employed to collect required information from 1440 study participants. Retrospective data of five-year (2012-2016) from hospitals and health centers (human cases), and veterinary clinics (animal cases) was used. Majority of the study participants (64.3%) were rural residents, 95.2% have heard about rabies and 50.1% were found dog owners. Among the study participants, 72.2%, 66.0%, and 62.4% have a good level of knowledge, attitude, and practices about rabies, respectively. A strong association between knowledge, attitude and practice with sex; educational level; occupation, dog ownership and rural/urban dwellers (p<0.05) was recorded. Furthermore, a total dog bite cases of 398 domestic animals and 4617 humans were found registered on casebooks of both veterinary and human health service centers of the study districts during the five years study period among which the highest percentage (36.4%) was recorded from canines. The highest anti-rabies vaccine coverage recorded was 36.0% in the year 2016, and higher human dog bite cases recorded was 50.1% on individuals aged between 5-15 years (both male and female). Hence, the current findings suggest that there is a need for coordinated and integrated effort of government, professionals (medical and veterinarians), community and other stake holders towards rabies control and prevention.Keywords: Animal, Human, Tigray, Rabies, Statu

    Male involvement in prevention of mother-to-child transmission of HIV in the context of partner testing in Goba town, Ethiopia: A facility-based cross-sectional study

    Background. Antenatal care (ANC) is an entry point for the prevention of mother-to-child transmission (PMTCT), particularly when a man accompanies his spouse for voluntary counselling and testing (VCT), even though this seldom happens in Ethiopia.Objective. To study the role of male partners in improving PMTCT/ANC, which is essential to prevent HIV/AIDS transmission in this country. Our study focused on identifying barriers for the low involvement in PMTCT/ANC among male partners whose spouses received ANC.Methods. A total of 422 male partners in Goba town, Oromia regional state, Ethiopia, were recruited in September - October 2014 and enrolled by a systematic sampling method. A facility-based cross-sectional study and two focus group discussions (FGDs) were conducted. Binary logistic regression and odds ratios were calculated to ascertain sociodemographic and other important information compared with the outcome variable and PMTCT/ANC, while the findings of the FGDs were grouped according to the emerging themes and analysed manually by means of a thematic approach.Results. The percentage of male partners (22.7%) accompanying their spouses for HIV testing and counselling at antenatal clinics improved three-fold when an invitation letter was introduced as a new initiative to increase male participation. Individuals in the age group 15 - 29 years (adjusted odds ratio (AOR) 5.4, 95% confidence interval (CI) 1.66 - 17.85), and whose duration of marriage was ˂5 years (AOR 5.6, 95% CI 1.83 - 17.30), were more likely to be tested than their referent groups. Men without a higher education (AOR 0.1, 95% CI 0.02 - 0.50), who disagreed with legal enforcement (AOR 0.1, 95% CI 0.07 - 0.32), were less likely to be tested, while those who did not notify their partners (AOR 8.4, 95% CI 1.92 - 37.12) were more likely to use the service. Being busy, proxy testing, fears of testing HIV-positive, neglecting the importance of VCT, and inadequate knowledge about the PMTCT/ANC programme were other barriers that came to the fore in the FGDs.Conclusion. Identified barriers have to be addressed, and helpful practices, such as using invitation letters to increase the uptake of the service by male partners, have to be instituted

    Study on knowledge, attitude and dog ownership patterns related to rabies prevention and control in Addis Ababa, Ethiopia

    The study was conducted from May 2003 to August 2003 in Addis Ababa with the objective of understanding the distribution of stray and owned dogs, dog ownership patterns and attitudes of people towards rabies and its prevention and control methods. A total of 2390 households were selected from 6 Sub Cities of Addis Ababa using stratified random sampling and were interviewed using structured questionnaires. From the total households interviewed, 969 (40.5%) of them were known to own one or more dogs, and the total number of owned dogs was estimated to be 225,078. Male dogs make up 1042 (78.4%) of the total owned dogs. Almost half of all the dogs 463 (47.8%) were tied only for some time during the day whereas 320 (33.1%) of the dogs were not tied at all and freely move from place to place and contribute to high dog bites in human beings and for the widespread occurrence of canine rabies in Addis Ababa. Six hundred fifty five (67.6%) of the interviewed households who owned dogs had a well fenced house. Most of the owned dogs 856 (88.4%) get their food from their owners and only 321 (33.3%) of the owners reported that they let their dogs to be vaccinated while the majority of the owners 644 (66.5%) do not let their dogs vaccinated regularly. A significant proportion of the interviewed households 2,323 (97.2%), (P < 0.05) indicated that they have the knowledge that dogs, cats and other animals can transmit rabies to humans and 1,752 (73.4%) of the households replied that rabies can be transmitted through bite, scratch and lick to open wounds. Analysis of the list of 2400 post exposure human antirabies treatments recorded at the EHNRI Zoonoses Laboratory from February 2002 to October 2003 showed that the most common animals involved in biting people were dogs. A total of 2198 (91.6%) people were bitten by dogs during the same period and most of them, 2053 (85.5%) were bitten on their hands and legs.     Characterization of Ethiopian mega hydrogeological regimes using GRACE, TRMM and GLDAS Datasets

    Understanding the spatio-temporal characteristics of water storage changes is crucial for Ethiopia, a country that is facing a range of challenges in water management caused by anthropogenic impacts as well as climate variability. In addition to this, the scarcity of in situ measurements of soil moisture and groundwater, combined with intrinsic ‘‘scale limitations’’ of traditional methods used in hydrological characterization are further limiting the ability to assess water resource distribution in the region. The primary objective of this study is therefore to apply remotely sensed and model data over Ethiopia in order to (i) test the performance of models and remotely sensed data in modeling water resources distribution in un-gauged arid regions of Ethiopia, (ii) analyze the inter-annual and seasonal variability as well as changes in total water storage (TWS) over Ethiopia, (iii) understand the relationship between TWS changes, rainfall, and soil moisture anomalies over the study region, and (iv) identify the relationship between the characteristics of aquifers and TWS changes. The data used in this study includes; monthly gravity field data from the Gravity Recovery And Climate Experiment (GRACE) mission, rainfall data from the Tropical Rainfall Measuring Mission (TRMM), and soil moisture from the Global Land Data Assimilation System (GLDAS) model. Our investigation covers a period of 8 years from 2003 to 2011.The results of the study show that the western part and the north-eastern lowlands of Ethiopia experienced decrease in TWS water between 2003–2011, whereas all the other regions gained water during the study period. The impact of rainfall seasonality was also seen in the TWS changes. Applying the statistical method of Principal Component Analysis (PCA) to TWS, soil moisture and rainfall variations identified the dominant annual water variability in the western, north-western, northern, and central regions, and the dominant seasonal variability in the western, north-western, and the eastern regions. A correlation analysis between TWS and rainfall indicated a minimum time lag of zero to a maximum of six months, whereas no lag is noticeable between soil moisture anomalies and TWS changes. The delay response and correlation coefficient between rainfall and TWS appears to be related to recharge mechanisms, revealing that most regions of Ethiopia receive indirect recharge. Our results also show that the magnitude of TWS changes is higher in the western region and lower in the north-eastern region, and that the elevation influences soil moisture as well as TWS

    Unintended pregnancy among female sex workers in Mekelle city, northern Ethiopia: a cross-sectional study

    BACKGROUND: Unintended pregnancy is a significant public health concern in the world. Particularly, female sex workers are exposed to the risk of unintended pregnancy, abortion and their consequences. The aim of this study was, therefore, to assess unintended pregnancy and associated factors among female sex workers in Mekelle city, northern Ethiopia. METHODS: A community based cross-sectional study was conducted among 346 female sex workers from five Kebelles (smallest administrative units in Ethiopia) of Mekelle city from March-April, 2014. Sex workers were selected with simple random sampling technique using sampling frame obtained from urban health extension program. Epi-data version 3.1 was used to enter data and analysis was done using SPSS version 20. Bivariate and multivariate logistic regressions were performed to identify factors associated with unintended pregnancy using odds ratio and 95% confidence interval with P-value of 0.05. RESULTS: The magnitude of unintended pregnancy among female sex workers in the past two years was 28.6%. During this period, 59 women had abortion which represents three-fifths, (59.6%), of those who had unintended pregnancies, and 17.1% of all female sex workers. Female sex workers who gave birth and had history of abortion formerly had 3.1 (AOR = 3.07, 95% CI: [1.54, 6.09]) and 15.6 (AOR = 15.64 95% CI: [8.03, 30.47]) times higher odds of unintended pregnancy compared to their counterparts, respectively. Sex workers who had steady partners had 2.9 (AOR = 2.87, 95% CI: [1.47, 5.61]) times higher odds of have unintended pregnancy than those who hadn't. Drug users had 2.7 (AOR = 2.68, 95% CI: [1.30, 5.52]) times higher odds of unintended pregnancy than those who hadn't use. Sex workers who had 60-96 months of duration in sex work were 67% less likely to have unintended pregnancy than those with <12 months (AOR = 0.33, 95% CI: [0.11, 0.95]). CONCLUSIONS: High level of unintended pregnancy and a range of associated factors were identified among sex workers.     Wheat rust epidemics damage Ethiopian wheat production: A decade of field disease surveillance reveals national-scale trends in past outbreaks.

    Wheat rusts are the key biological constraint to wheat production in Ethiopia-one of Africa's largest wheat producing countries. The fungal diseases cause economic losses and threaten livelihoods of smallholder farmers. While it is known that wheat rust epidemics have occurred in Ethiopia, to date no systematic long-term analysis of past outbreaks has been available. We present results from one of the most comprehensive surveillance campaigns of wheat rusts in Africa. More than 13,000 fields have been surveyed during the last 13 years. Using a combination of spatial data-analysis and visualization, statistical tools, and empirical modelling, we identify trends in the distribution of wheat stem rust (Sr), stripe rust (Yr) and leaf rust (Lr). Results show very high infection levels (mean incidence for Yr: 44%; Sr: 34%; Lr: 18%). These recurrent rust outbreaks lead to substantial economic losses, which we estimate to be of the order of 10s of millions of US-D annually. On the widely adopted wheat variety, Digalu, there is a marked increase in disease prevalence following the incursion of new rust races into Ethiopia, which indicates a pronounced boom-and-bust cycle of major gene resistance. Using spatial analyses, we identify hotspots of disease risk for all three rusts, show a linear correlation between altitude and disease prevalence, and find a pronounced north-south trend in stem rust prevalence. Temporal analyses show a sigmoidal increase in disease levels during the wheat season and strong inter-annual variations. While a simple logistic curve performs satisfactorily in predicting stem rust in some years, it cannot account for the complex outbreak patterns in other years and fails to predict the occurrence of stripe and leaf rust. The empirical insights into wheat rust epidemiology in Ethiopia presented here provide a basis for improving future surveillance and to inform the development of mechanistic models to predict disease spread

    Opportunities for increasing livestock water productivity in the Nile River Basin

    Livestock keeping in the Nile Basin contributes greatly to human security, income, culture and agricultural gross domestic product (GDP). Inappropriate livestock management uses excessive water and causes water and land degradation. Livestock-water interactions are complex, not well understood, and often ignored in agricultural water development. This results in lost opportunities to achieve sustainable and higher investment returns. Typically, livestock management also ignores important livestock-water issues. This lack of integration creates knowledge gaps resulting in inefficient and inequitable use of water resources. This paper summarizes selected research findings on livestock-water productivity (LWP) in the Ethiopian Blue Nile Highlands, Uganda‟s Cattle Corridor, and the Central Belt of Sudan. It suggests selected intervention options to increase LWP, improve livelihoods and reduce land and water degradation. Overall, LWP compares favorably with crop-water productivity. Yet, huge opportunities remain to further increase LWP potentially enabling more agricultural production and support for ecosystem services without depleting additional water. Four strategies to increase LWP are selection of animal feeds derived from plant materials with high crop water productivity, improved water conservation through better management of watering sites, vegetation and soil on grazing, crop and riparian lands, adoption of technologies to improve animal health, genetics, nutrition and husbandry, and strategic allocation of watering sites to adjust grazing pressure to sustainably match the spatial distributions of pasture and drinking water availability. Implications and opportunities for benefit sharing, IWRM, and poverty reduction in the Nile Basin are discussed in the context of the Ethiopian, Sudanese and Ugandan case studies
