    Monophasic synovial sarcoma presenting as a primary ileal mass: a case report and review of the literature

    <p>Abstract</p> <p>Introduction</p> <p>Synovial sarcoma is a rare malignant mesenchymal tumor mainly arising in the peri-articular tissue in young adults. There are few cases reported in other areas.</p> <p>Case presentation</p> <p>We report the case of a 29-year-old Saudi woman of Arabian ethnicity with synovial sarcoma arising primarily from the ileum who presented with abdominal pain, a palpable mass and incomplete intestinal obstruction. A literature review was performed to gather information on this rare gastrointestinal tract sarcoma.</p> <p>Conclusions</p> <p>Although it is a rare tumor of the pre-articular tissues, synovial sarcoma can present, in exceedingly rare cases, in unusual anatomical sites such as the gastrointestinal tract. We believe the reporting of all rare or unexpected presentations of sarcoma will eventually improve our understanding of this relatively unusual malignancy.</p

    Liver phospholipids fatty acids composition in response to different types of diets in rats of both sexes

    Background: Dietary intake influence changes in fatty acids (FA) profiles in liver which plays a central role in fatty acid metabolism, triacylglycerol synthesis and energy homeostasis. We investigated the effects of 4-weeks treatment with milk-and fish-based diet, on plasma biochemical parameters and FA composition of liver phospholipids (PL) in rats of both sexes. Methods: Adult, 4 months old, Wistar rats of both sexes, were fed with different types of diets: standard, milk-based and fish-based, during 4 weeks. Analytical characterization of different foods was done. Biochemical parameters in plasma were determined. Fatty acid composition was analyzed by gas-chromatography. Statistical significance of FA levels was tested with two-way analysis of variance (ANOVA) using the sex of animals and treatment (type of diet) as factors on logarithmic or trigonometric transformed data. Results: Our results showed that both, milk-and fish-based diet, changed the composition and ratio of rat liver phospholipids FA, in gender-specific manner. Initially present sex differences appear to be dietary modulated. Although, applied diets changed the ratio of total saturated fatty acids (SFA), monounsaturated fatty acids (MUFA) and polyunsaturated fatty acids (PUFA), and effects were gender specific. Milk-based diet lowered SFA and elevated MUFA in males and increased PUFA in females vs. standard diet. The same diet decreased n-3, increased n-6 and n-6/n-3 ratio in males. Fish-based diet increased n-3, decreased n-6 and n-6/n-3 ratio vs. standard and milk-based diet in females. However, the ratio of individual FA in liver PL was also dietary-influenced, but with gender specific manner. While in females fish-based diet decreased AA (arachidonic acid) increased level of EPA (eicosapentaenoic acid), DPA (docosapentaenoic acid) and DHA (docosahexaenoic acid), the same diet elevated only DHA levels in males. Conclusion: Gender related variations in FA composition of rat liver PL were observed, and results have shown that those initial differences could be significantly modulated by the type of diet. Furthermore, the modulatory effects of milk-and fish-based diets on liver phospholipids FA profiles appeared to be sex-specific

    A Case of Transient Local Anesthetic Induced Bilateral Vocal Cord Palsy

    We report a rare case of bilateral vocal cord palsy following total thyroidectomy with successful extubation within 12 hours. The patient is a 33-year-old lady who underwent uneventful total thyroidectomy for compressive symptoms. Thirty minutes after extubation, she developed stridor and the flexible laryngoscopy showed bilaterally adducted vocal cords. The patient, thus, was reintubated and after 12 hours she met the extubation parameters and so she was extubated successfully. The repeated flexible laryngoscopy showed normal vocal cords. A review of the literature revealed limited information on this transient condition

    Criterion Validity of the Newly Developed Occlusal Cant Index

    Objectives: To assess the criterion-related (concurrent) validity of the newly developed occlusal cant index (OCI). Materials and Methods: Four standardized posterior&ndash;anterior (PA) cephalometric radiographs of four patients were obtained at a 0&deg; occlusal cant (OC) and manipulated to create various degrees of OC from 1&deg; to 4&deg; on the right and left sides, with a total of 36 PA images. The angle between the actual horizontal line and the occlusal plane was manually drawn on each PA radiographic image. The set of radiographic images was displayed to 36 orthodontists, who were asked to measure the drawn angle and apply the OCI to each PA radiographic image. Results: The overall criterion-related validity of the OCI was statistically significant among all grades. Conclusion: The OCI is highly valid and recommended for clinical consideration

    Aplasia Cutis Congenita (ACC) is a condition characterized by congenital absence of skin, usually on the scalp. ACC can occur as an isolated condition or in the presence of other congenital anomalies. Here we describe a case of a 16-day-old baby girl with an isolated ACC of the scalp. Her elder two siblings have been diagnosed with ACC with concomitant cardiac or limb anomalies. The patient was managed conservatively until the defect has formed scar tissue 6 months later

    A New Model for Determining Factors Affecting Human Errors in Manual Assembly Processes Using Fuzzy Delphi and DEMATEL Methods

    Human errors (HEs) are common problems in manual assembly processes, impacting product quality and resulting in additional costs. Based on expert judgments, this study aims to identify the most significant factors affecting HEs in manual assembly processes and explore the cause-and-effect relationships among those factors. In order to achieve this objective, a proposed model is constructed using two types of Multi-Criteria Decision-Making (MCDM) techniques. Firstly, using two rounds of the fuzzy Delphi method (FDM), twenty-seven factors with an influence score of 0.7 or higher were found to have a major impact on HEs during manual assembly processes, with at least a 75% consensus among experts. After that, the twenty-seven factors affecting HEs were given to experts in a third round to analyze the cause-and-effect relationships among those factors using the fuzzy decision-making trial and evaluation laboratory (DEMATEL) method. In MCDM techniques, symmetry refers to an important property that can be used to find relationships between variables. It is based on the principle that the relative importance or preference between two variables should remain the same regardless of their positions or roles. Therefore, symmetry is a factor that MCDM approaches take into account to ensure that the relationships between variables are accurately represented, leading to more reliable decision-making outcomes. The reliability and normality of the surveying data were examined using the SPSS 22.0 software program. The study results revealed that training level, poor workplace layout, a lack of necessary tools, and experience were the major factors affecting HEs as root causes. Moreover, a failure to address the error-causing problem, unintentional unsafe acts, fatigue, and poor error visual perception were found to be effect (dependent) factors. The findings of this study can help organizations make better-informed decisions on how to reduce worker errors and interest in the factors that contribute to assembly errors and provide a good basis for reaching the quality of final assembled parts
