    Factors associated with poor outcomes among hospitalized patients with COVID-19: Experience from a MERS-CoV referral hospital

    BACKGROUND: Coronavirus disease 2019 (COVID-19) has resulted in millions of deaths, including more than 6000 deaths in the Kingdom of Saudi Arabia (KSA). Identifying key predictors of intensive care unit (ICU) admission and mortality among infected cases would help in identifying individuals at risk to optimize their care. We aimed to determine factors of poor outcomes in hospitalized patients with COVID-19 in a large academic hospital in Riyadh, KSA that serves as a Middle East Respiratory Syndrome coronavirus (MERS-CoV) referral center. METHODS: This is a single-center retrospective cohort study of hospitalized patients between March 15 and August 31, 2020. The study was conducted at King Saud University Medical City (KSUMC). COVID-19 infection was confirmed using real-time reverse transcriptase polymerase chain reaction (RT-PCR) for SARS-COV-2. Demographic data, clinical characteristics, laboratory, radiological features, and length of hospital stay were obtained. Poor outcomes were, admission to ICU, need for invasive mechanical ventilation (IMV), and in-hospital all-cause mortality. RESULTS: Out of 16,947 individuals tested in KSUMC, 3480 (20.5%) tested positive for SARS-CoV-2 and of those 743 patients (21%) were hospitalized. There were 62% males, 77% were younger than 65 years. Of all cases, 204 patients (28%) required ICU admission, 104 (14%) required IMV, and 117 (16%) died in hospital. In bivariate analysis, multiple factors were associated with mortality among COVID-19 patients. Further multivariate analysis revealed the following factors were associated with mortality: respiratory rate more than 24/min and systolic blood pressure 37 units/L in the first 48 h of presentation, while a RT-PCR cycle threshold (Ct) value ≤24 was a predictor for IMV. CONCLUSION: Variable factors were identified as predictors of different outcomes among COVID-19 patients. The only predictor of IMV was a low initial Ct values of SARS-CoV-2 PCR. The presence of tachypnea, hypotension, lymphopenia, and elevated AST in the first 48h of presentation were independently associated with mortality. This study provides possible independent predictors of mortality and invasive mechanical ventilation. The data may be helpful in the early identification of high-risk COVID-19 patients in areas endemic with MERS-CoV

References.By the end of the second quarter of 2016, Facebook had approximately 1.7 billion active users, thereby offering marketers an effective platform for developing creative campaigns. Considering the significance of creativity in social media campaigns, and given the paucity of studies on this topic, the need for qualitative research to fill this gap was clear. Therefore, this thesis was aimed at providing a conceptual basis for understanding how advertising agencies produce creative social media campaigns. The research adopted a grounded theory involving in-depth interviews with social media professionals from 20 Australian advertising agencies. Nine themes emerged from the findings with regard to increasing creativity in developing social media campaigns: the agency-client relationship, trust, risk attitude, openness, target audience, budget, time, creativity components, and the creative process. These themes were grouped into three main factors: client, agency, and creativity. The findings point to conflicts between social media agencies and their clients which are also typical of the conflicts observed between advertising agencies and their clients. Respondents reported that most of these conflicts can decrease creativity, and the clients should be more open to the agency resolving these conflicts.     Pyoderma Gangrenosum Induced by BNT162b2 COVID-19 Vaccine in a Healthy Adult

    (1) Background: Pyoderma gangrenosum (PG) is a rare neutrophilic dermatosis of unknown etiology. Coronavirus disease 2019 (COVID-19) vaccines can cause a variety of adverse cutaneous manifestations. PG associated with mRNA vaccines has not previously been described. This case study reports on the first patient to develop PG after receiving BNT162b2. (2) Case Presentation: An otherwise-healthy 27-year-old man developed multiple skin lesions 24 h after receiving the first dose of the messenger RNA-based Pfizer/BioNTech BNT162b2 COVID-19 vaccine. When in hospital, he developed a new painful ulcerative lesion on his right hand. Skin ulcer edge biopsy showed severe epidermal neutrophilic infiltrate with epidermal and dermal edema, underlying superficial dermal necrosis, and characteristic undermining with extensive mixed inflammatory infiltration of the dermis and abscess formation consistent with an ulcer with mixed dermal inflammation compatible with pyoderma gangrenosum. The lesion showed rapid improvement after the initiation of immunosuppressive therapy. (3) Conclusions: PG may be a rare adverse event related to the BNT162b2 vaccine, which could be more frequently encountered with the wide-scale use of mRNA vaccines.     Flood Susceptibility Mapping Using Watershed Geomorphic Data in the Onkaparinga Basin, South Australia

    In the near future, natural disasters and associated risks are expected to increase, mainly because of the impact of climate change. Australia is considered one of the most vulnerable areas for natural disasters, including flooding. Therefore, an evaluation of the morphometric characteristics of the Onkaparinga basin in South Australia was undertaken using the integration of remote sensing and geospatial techniques to identify its impact on flash floods. The Shuttle Radar Topography Mission (SRTM) and Landsat images with other available geologic, topographic, and secondary data were analysed in geographic information system (GIS) to outline the drainage basins, estimate the morphometric parameters, and rank the parameters to demarcate the flash flood susceptibility zones of the basin. The main goal was to develop a flash flood susceptibility map showing the different hazard zones within the study areas. The results showed that 10.87%, 24.27%, and 64.85% are classified as low, moderate, and highly susceptible for flooding, respectively. These findings were then verified against secondary data relating to the historic flood events of the area. About 30.77% of the historical floods are found located within the high to extremely susceptible zones. Moreover, a significant correlation has been found between the high precipitation concentration index (PCI) and the irregular rainfall and high potential for flooding. Finally, the social and economic vulnerability was applied to determine the impact of the flood hazards. The result indicates a widespread threat to the economy, environment, and community in the study area. This study can be utilized to support and assist decision makers with planning and the devotion of alleviation measures to reducing and avoiding catastrophic flooding events, especially in highly susceptible areas in the world, such as South Australian basins

    Awareness of undergraduate dental students, interns, and freshly graduated dentists about oral manifestation associated with COVID-19

    Abstract Objective There is growing evidence supporting the presence of oral manifestations associated with COVID-19. The study investigates the knowledge of dental undergraduates and recently graduated dentists concerning oral presentations related to COVID-19. Methods A cross-sectional descriptive study in Saudi Arabia comprised 305 individuals, including undergraduate dental students, interns, and freshly graduated dentists. Data were collected using a questionnaire disseminated to approximately 500 subjects via WhatsApp groups and other applications. The questionnaire was tested in a pilot study for validity, edited, and validated by 2 supervisors at the College of Dentistry, Qassim University, Buraydah, Saudi Arabia. The questionnaire consisted of questions regarding sociodemographic attributes, the level of expertise of dental practitioners, and their knowledge and perspectives regarding COVID-19 and the implementation of oral lesions interrelated to it. The data was subjected to analysis through the utilization of descriptive statistics and chi-square tests, employing the statistical software SPSS (version 24). Results About 43.9% of subjects stated that COVID-19 causes oral symptoms. Respondents most frequently reported COVID-19-related dry mouth. Oral ulcers, Candida infection, Hyperpigmentation, tongue coating, atrophy, Petechiae, Herpes, white lesions, Gingivitis, and Periodontitis are further symptoms. COVID-19’s oral manifestation was unknown to 41.0% of subjects, and 37.7% of respondents lacked knowledge regarding the most impacted locations of oral manifestations. Oral signs and COVID-19 symptoms are debated and significantly associated with higher educational levels. Conclusion The dental students and freshly graduated dentists in this study have proper knowledge of COVID-19 and its symptoms. Also, most dental students and newly graduated dentists recognize the potential correlation between COVID-19 and oral manifestations with an average to excellent knowledge of the types and sites commonly affected.     Evolution of tigecycline resistance in <i>Klebsiella pneumoniae</i> in a single patient

    Carbapenemase-producing Klebsiella pneumoniae infections carry serious clinical and infection-control implications. Isolates possessing such hydrolyzing enzymes have been described in the United States and around the world. Besides being resistant to carbapenems, they usually confer resistance to fluoroquinolones, piperacillin-tazobactam, and extended-spectrum cephalosporins. Tigecycline demonstrates in vitro activity against these organisms, but reported resistance raises concern about tigecycline use for these infections. We describe a carbapenemase-producing K pneumoniae evolving resistance to tigecycline in a 75-year-old male after a prolonged stay in a critical care unit
