    Peutz Jeghers Syndrome Presented as intermittent gastric outlet obstruction

    Peutz Jeghers Syndrome (PJS), which was first described in 1921 by Peutz, followed by Jeghers etal in 1949, is an uncommon but not a rare disorder characterized by mucocutaneous melanin pigmentation, gastrointestinal hamartomatous polyps and increased risk of gastrointestinal and otherorgans cancer. The polyps vary in size from few millimeters to several centimeters, with lobulated surface, and could be pedunculated as in large polyps, or sessile as in the small ones. The clinical symptoms of the disease are recurrent abdominal pain, intestinal intussusception and obstruction, gastrointestinal bleeding with symptoms of iron deficiency anaemia. The symptoms usually take place in the second and third decade of life. The complications of PJS which brings the patient to the doctor is severe anemia due to blood loss from GIT, acute abdomen, intestinal obstruction due to tumor-mass obstruction or intussusception, or rarely due to gastric outlet obstruction as in this presented case. The treatment of polyposis is by endoscopic polypectomy, laprotomy and resection which may lead to complications as short bowel syndrome. The other difficulty which the doctor comes across while managing these cases is to follow-up the occurrence of malignant disease in these patients, malignant changes of hamartomatus polyps and other organs cancers have been reported in patients with PJS.Keywords: hamartomatus polyps, polypectomy, mucocutaneous

    Expression of TGFβ1 by Pulp Tissue of Human Permanent and Primary Teeth Capped by BiodentineTM

    BiodentineTM,represented a novel bioactive tricalcium silicate cement ,that is introduced into dentistry. It was suggested to be biocompatible, having optimal working and setting time, excellent workability with superior adhesion to tooth structure. BiodentineTM was proved to maintain pulp as a vital tissue and enhance dentinal bridging, as alternative to Mineral trioxide aggregate that can be provided with a better handling characteristics and shorter working time. Moreover, dental pulp cells have the potential to differentiate into odontoblast-like cells and enhanced reparative dentine.In the present study "BiodentineTM" was directly applied on the dental pulp of cultured tooth of (36) human maxillary first premolars and (36) human maxillary first primary molar tooth . After various culture periods (2,14 and 28) days, the interaction of the material with dental pulp tissue was analyzed on tissue cultures. The effect of this material on TGFB1 expression on pulp tissue was studied by immunohistochemical investigation. The results illustrate that BiodentineTM" induced mineralized foci formation early after its application as direct pulp capping material in permanent and primary teeth. The mineralization appeared beneath the reparative dentine. BiodentineTM shows a highl significant increment of TGF- β1 expression by pulp cells (P< 0.01) in both permanent and primary teeth.Conclusions: When "BiodentineTM" was applied as direct pulp capping material for permanent and primary teeth, it induced an early form of reparative dentine synthesis, probably due to a modulation of pulp cell for expression TGF-β1.     The Relationship between P-Wave Morphology and Atrial Fibrillation

    The objective of this paper is to develop an efficient P-wave detection algorithm based on the morphology characteristics of arrhythmias using time domain analysis. ECG from normal subjects, and patients with atrial fibrillation were studied. After baseline wander cancellation, power line interference filtration, the step of QRS detection using the pan- Tompkins algorithm is utilized to calculate R peak which represent the reference point to detect P peak. The algorithm was tested with experiments using MIT-BIH arrhythmia database which included Paroxysmal Atrial Fibrillation PAF prediction challenge, Massachusetts Institute of Technology MIT-BIH normal sinus rhythm, long term Atrial Fibrillation AF and MIT-BIH atrial fibrillation where every P-wave was extracted. The results reveal that the algorithm is accurate and efficient to detect and classify arrhythmias resulted from atrial fibrillation

    Non-Dispersive Near Infrared Gas Flow Cell Design for Oxygenator-Exhaust Capnometry

    Non-dispersive near-infrared technique is widely used nowadays for the detection of gases, especially in harsh environments. In this study, an optical gas cell was designed for oxygenator exhaust capnometry. A computer-based simulation was used for the analysis of air flows for model selection. ANSYS Discovery 2020 R2 was used for model simulation. The gas flow cells were tested using a custom-made gas rig to measure the fraction absorbance of carbon dioxide gas at the detector. Two gases were used, nitrogen gas as a reference gas (0%) and 9% carbon dioxide. Three gas cells with the following optical path lengths were tested: 31mm, 36mm, and 40mm. The results showed that all gas flow cells produced laminar flow and small pressure drop across the inlet and outlet of the cell (11~12 Pa). Further, the minimum velocity is obtained in the 40mm gas flow sensor and it is located at the gas outlet path away from the effective optical gas path. The simulation and experimental results indicate that the gas flow cell of 40mm optical path length is more suitable for the intended application as it offers a maximum effective absorption path compared to the stagnation areas, and as a result, it provides the maximum fraction absorbance

We design an expansion trajectory that yields a final state identical to the initial state up to an irrelevant phase factor.
