    Referral Patterns, Procedures, and Outcomes of a Large Community- Based Urology Group: a Retrospective Chart Review

    In 2012 The United States Preventive Services Task Force (USPSTF) recommended against prostate cancer (Pca) screening. The purpose of our study is to assess the effects of the USPSTF grade D recommendation for PSA based screening on new PCa diagnoses within a large-community based private practice setting. We completed a retrospective chart review of men with an elevated PSA seen in our clinic between May 2009–May 2015 who had undergone a prostate biopsy and were diagnosed with PCa. We observed that following the 2012 USPSTF recommendations, there was a significant increase in patients with clinically significant prostate cancer GG ≄ 2, with the majority being 55-65 years of age. In addition we observed a decrease in the diagnosis of very low and low risk PCa with an increased incidence of intermediate and high risk PCa

    ORAL CANDIDA ALBICANS COLONIZATION RATE IN FIXED ORTHODONTICS PATIENTS

    Objectives:  The objective of this study was to assess the oral Candida albicans colonization ( OCAC)  in a cluster of teenagers and young adults while being treated with a fixed orthodontic appliance (FOA). Subjects and methods:  The investigational group was selected from orthodontic patients whom were examined clinically as soon as to get baseline information before active treatment.  The cluster included 210 patients; 45 males, 165 females (mean age 21.6 ± 4.5 years).  Clinical, demographic data and risk factors were collected in standard questionnaire then each individual was directed to carry out oral wash by a phosphate-buffered saline solution, which was expectorated and processed intended for the isolation of Candida species on Sabouraud’s dextrose agar. The isolated Candida species were identifying by culturing on chromogenic Candida agar and notice species-specific colony natures. Results:  The predominant Candida species isolated was C. albicans with OCAC rate equal to 13.8% extensively enhanced after the insertion of a FOA, as revealed by the oral rinse (P < 0.05) techniques. The results also revealed an increase of OCAC in male patients (24.4%) than female patients (10.9%),   21-25 years patients (17.1%), and regular smoking and Qat chewing were significant associated risk factors (OR=28.6, OR=10.7 respectively , P < 0.0001).  There was no significant association between C. albicans  colonization with oral hygiene in fixed Orthodontic patients. Conclusion:  As a whole, the current data suggest that the introduction of FOA is likely to promote OCAC. Moreover, it becomes visible that the routine oral hygiene procedures performed by these patients may not necessarily reduce OCAC while smoking and chewing Qat habits significantly increased OCAC in FOA. Also smoking and Qat chewing during FOA treatment should be banned if potential harmful effects are to be prevented.     Objective: Biofilm formation by uropathogens on the surface of indwelling medical devices can inflict obstinate or recurring infection, thought-provoking antimicrobial therapy.  This study included 227 patients with indwelling urinary catheters and suffering from CAUTI. They were analyzed for biofilm formation and antibiogram susceptibility, 59.4% were males and 40.6% were females. Methods: Ensuing phenotypic identification of isolated bacteria, antibiotic sensitivity test was performed by modified Kirby–Bauer disc diffusion method following   the Clinical and Laboratory Standards Institute (CLSI 2015) guidelines; Biofilm-forming uropathogens were detected by the tissue culture plate (TCA) method. Results: The predominant uropathogen in catheter-associated UTIs (CAUTIs) was Escherichia coli 46.3%, followed by K. pneumoniae18.5%, P. aeruginosa 11.9%,7%, S. coagulase negative 5.7%, S. aureus 4.8%, Enterobacter spp. 4.4%, E. faecalis 1.3%.  The total rate of biofilm producer bacteria was 49.3% (21.1% high producers, 28.2% moderate producers). Prime biofilm producers were E. coli 60% with OR=8.6 (p=0.002), followed by K. pneumoniae 57.1% with OR=10.1 (p=0.002), and P. aeruginosa 37% with OR=6.6 (p=0.02). Gram-negative biofilm producers found 100%, 100%, 88.6%, 82.9%, 81.9%, 80.9%, and 72.4%, 40%, 33% resistant to ampicillin , amoxyclave, cotrimoxazole,  ceftraxone, nalidixic acid, ciprofloxacin, cefotaxime, nitrofurantoin and amikacin respectively. Conclusion: In conclusion, a high antimicrobial resistance was observed in biofilm producers than non-biofilm producers. Of recommended antimicrobial therapies for CAUTIs, ampicillin and amoxicillin-clavulanate were the least active antibiotics, whereas imipenem and amikacin were found as the most effectual for gram-negative biofilm producer.     Objective: Hepatitis C virus infection is a constant worldwide public health concern. The prevalence of HCV infection is higher in patients on chronic haemodialysis (HD) than in the general population. Despite the control of blood products, hepatitis C virus transmission is still being observed among patients undergoing dialysis. Detection systems for serum HCV antibodies are insensitive in the acute phase because of the long serological window. Direct detection of HCV depends on PCR test but this test is not suitable for routine screening.  The objective of this study was to determine prevalence of HCV, genotyping and  if  HCV core antigen test could be a better alternative to NAT techniques for the diagnosis of HCV infection during the window period and whether the sensitivity for antibody detection is preserved. Methods: Current study includes screening of 159 patients on long-term dialysis by HVC antibodies test, PCR HCV-RNA and HCV core antigen test by commercial tests. Results: The prevalence of HCV was 10.7% (17 patients) and genotype 4 was the most common one (64.7%). The sensitivity of HCV core antigen test was 94.1%, the specificity 100%, the positive predictive power 100%, and the negative predictive power 97.9%. In conclusions; patients on maintenance HD in Yemen have a high prevalence of HCV infection comparing with general population; and genotype 4 is predominant.     TETANUS IMMUNIZATION AMONG PREGNANT WOMEN: COVERAGE RATE AND RATE OF PROTECTION AT TIME OF DELIVERY

    Objective: Even though attempts have been effectively applied to eradicate the neonatal tetanus through widespread childhood vaccination and improved conditions at delivery, it remains major cause of infant mortality and continues a problem of public health in developing countries including Yemen.  The aims of this study were to determine the tetanus immunization status, the association between the risk factors and failure of protection in pregnant women at time of delivery. Methods: This cross-sectional study included 476 women seeking care for delivery at Al Thawra Modern General Hospital and Al Sabain Hospital, women age ranged from 16-49 years old. Immunization information and factors affecting it were obtained through a standard questionnaire. Serum samples were collected and level of IgG antibody against Clostridium tetani was measured by ELISA technique. Protected women were defined as those with serum antibody levels >or=0.6 IU/ml. Results: The total vaccine covering rate of tetanus was 87%, and maternal vaccine rate was 33.6%, the protective rate at time of delivery was 68.5%. There were significant association between unvaccinated (OR=18.6), older ages (OR=1.7), rural residency (OR=34) and malaria infection during pregnancy (OR=2.9); with protection failure in pregnant women at time of delivery. Conclusion: It can be concluded that the total vaccine coverage rate and antenatal tetanus vaccine rate were insufficient. In addition, the protective rate at time of delivery was low and large numbers of neonate are susceptible to neonatal tetanus and death. Vaccinating every pregnant woman with at least one dose of TT would be an affordable and effective way to protect against neonatal tetanus, and would be a step toward eliminating the deaths that continue to occur due to this preventable disease in Yemen.                                           Peer Review History: Received 2 February 2018;   Revised 21 February; Accepted 1 March, Available online 15 March 2019 Academic Editor: Dr. Marwa A.     A Simple and Highly Structured Procaine Hydrochloride as Fluorescent Quenching Chemosensor for Trace Determination of Mercury Species in Water

    An ultrasensitive, simple and highly selective spectrofluorometric strategy for quantifying traces of mercury(II) in environmental water has been established using the fluorescent probe procaine hydrochloride (PQ+.Cl−). The procedure was based upon the formation of the ternary ion associate complex [(PQ+)2.(HgI4)2−] between PQ+.Cl− and mercury(II) in iodide media at pH 9.0–10.0 with its subsequent extraction onto dichloromethane accompanied by a change in fluorescence intensity at λex/em = 268/333 nm. The developed strategy exhibited a linear range of 1–114 Όg L−1 with lower limit of detection (LOD) and quantification (LOQ) of mercury(II) 1.3 and 3.98 nM, respectively. Intra and inter-day laboratory accuracy and precision for trace analysis of mercury(II) in water were performed. Complexed mercury(II) in environmental water, chemical speciation and successful literature comparison was performed. The proposed system offered excellent selectivity towards mercury(II) ions examined in the presence of competent ions in excess, relevant to real water samples. The method was applied for analysis of mercury(II) in tap water samples. Statistical comparison (Student’s t and F tests) of the proposed method with the reference ICP-OES method revealed no significant differences in the accuracy and precision

    Acaricidal activity of Foeniculum vulgare against Rhipicephalus annulatus is mainly dependent on its constituent from trans-anethone

    Globally, the economic losses due to hard ticks infestation and the control of the associated diseases have been calculated at USD $13.9-18.7 billion per year. The economic impact is related to its direct damage to the skins, blood loss, anemia, severe immunological reactions and indirect losses that related to the effects of hemoparasites, cost of treatment for clinical cases and expenses incurred in the control of ticks. The current study evaluated the acaricidal activities of fennel Foeniculum vulgare essential oil and its main components; trans-anethole and fenchone; against R. annulatus. GC-MS analysis revealed that this oil contained 16 components representing 99.9% of the total identified compounds with E-anethole being the predominant component(64.29%), followed by fenchone (9.94%). The fennel oil and trans-anethole showed significant acaricidal activities. The LC50 of the fennel oil was attained at concentrations of 12.96% for adult ticks and 1.75% for tick larvae meanwhile the LC50 of trans-anethole was reached at concentrations of 2.36% for adult tick and 0.56% for tick larvae. On the contrary, fenchone showed no any significant adulticidal activities and its LC50 attained at a concentration of 9.11% for tick larvae. Regarding repellence activities, trans-anethole achieved 100% repellency at the concentration of 10% while fennel showed 86% repellency at the same concentration. Fenchone showed no repellency effect. Treatment of larvae with fennel, trans-anethole, and fenchone LC50 concentrations significantly inhibited the acetylcholinesterase activity. Meanwhile, glutathione s-transferase activity was significantly decreased in fennel treated larvae but no significant effect was found in the larvae of trans-anethole and fenchone groups. These results indicate that the acaricide effect of fennel oil may attributed to its high content of trans-anethole. This was supported by potent adulticidal, larvicidal, and repellency effects of trans-anethole against Rhipeciphalus annulatus tick and therefore it could be included in the list of acaricide of plant origin

    Malware detection issues, challenges, and future directions: A survey

    The evolution of recent malicious software with the rising use of digital services has increased the probability of corrupting data, stealing information, or other cybercrimes by malware attacks. Therefore, malicious software must be detected before it impacts a large number of computers. Recently, many malware detection solutions have been proposed by researchers. However, many challenges limit these solutions to effectively detecting several types of malware, especially zero-day attacks due to obfuscation and evasion techniques, as well as the diversity of malicious behavior caused by the rapid rate of new malware and malware variants being produced every day. Several review papers have explored the issues and challenges of malware detection from various viewpoints. However, there is a lack of a deep review article that associates each analysis and detection approach with the data type. Such an association is imperative for the research community as it helps to determine the suitable mitigation approach. In addition, the current survey articles stopped at a generic detection approach taxonomy. Moreover, some review papers presented the feature extraction methods as static, dynamic, and hybrid based on the utilized analysis approach and neglected the feature representation methods taxonomy, which is considered essential in developing the malware detection model. This survey bridges the gap by providing a comprehensive state-of-the-art review of malware detection model research. This survey introduces a feature representation taxonomy in addition to the deeper taxonomy of malware analysis and detection approaches and links each approach with the most commonly used data types. The feature extraction method is introduced according to the techniques used instead of the analysis approach. The survey ends with a discussion of the challenges and future research directions
