    Multimorbidity prevalence and patterns across socioeconomic determinants: a cross-sectional survey

    <p>Abstract</p> <p>Background</p> <p>Studies on the prevalence of multimorbidity, defined as having two or more chronic conditions, have predominantly focused on the elderly. We estimated the prevalence and specific patterns of multimorbidity across different adult age groups. Furthermore, we examined the associations of multimorbidity with socio-demographic factors.</p> <p>Methods</p> <p>Using data from the Health Quality Council of Alberta (HQCA) 2010 Patient Experience Survey, the prevalence of self reported multimorbidity was assessed by telephone interview among a sample of 5010 adults (18 years and over) from the general population. Logistic regression analyses were performed to determine the association between a range of socio-demographic factors and multimorbidity.</p> <p>Results</p> <p>The overall age- and sex-standardized prevalence of multimorbidity was 19.0% in the surveyed general population. Of those with multimorbidity, 70.2% were aged less than 65 years. The most common pairing of chronic conditions was chronic pain and arthritis. Age, sex, income and family structure were independently associated with multimorbidity.</p> <p>Conclusions</p> <p>Multimorbidity is a common occurrence in the general adult population, and is not limited to the elderly. Future prevention programs and practice guidelines should take into account the common patterns of multimorbidity.</p

    SARS-CoV-2 infection, clinical features and outcome of COVID-19 in United Kingdom nursing homes.

    OBJECTIVES: To understand SARS-Co-V-2 infection and transmission in UK nursing homes in order to develop preventive strategies for protecting the frail elderly residents. METHODS: An outbreak investigation involving 394 residents and 70 staff, was carried out in 4 nursing homes affected by COVID-19 outbreaks in central London. Two point-prevalence surveys were performed one week apart where residents underwent SARS-CoV-2 testing and had relevant symptoms documented. Asymptomatic staff from three of the four homes were also offered SARS-CoV-2 testing. RESULTS: Overall, 26% (95% CI 22-31) of residents died over the two-month period. All-cause mortality increased by 203% (95% CI 70-336) compared with previous years. Systematic testing identified 40% (95% CI 35-46) of residents as positive for SARS-CoV-2, and of these 43% (95% CI 34-52) were asymptomatic and 18% (95% CI 11-24) had only atypical symptoms; 4% (95% CI -1 to 9) of asymptomatic staff also tested positive. CONCLUSIONS: The SARS-CoV-2 outbreak in four UK nursing homes was associated with very high infection and mortality rates. Many residents developed either atypical or had no discernible symptoms. A number of asymptomatic staff members also tested positive, suggesting a role for regular screening of both residents and staff in mitigating future outbreaks

    Syndecan-1 and FGF-2, but Not FGF Receptor-1, Share a Common Transport Route and Co-Localize with Heparanase in the Nuclei of Mesenchymal Tumor Cells

    Syndecan-1 forms complexes with growth factors and their cognate receptors in the cell membrane. We have previously reported a tubulin-mediated translocation of syndecan-1 to the nucleus. The transport route and functional significance of nuclear syndecan-1 is still incompletely understood. Here we investigate the sub-cellular distribution of syndecan-1, FGF-2, FGFR-1 and heparanase in malignant mesenchymal tumor cells, and explore the possibility of their coordinated translocation to the nucleus. To elucidate a structural requirement for this nuclear transport, we have transfected cells with a syndecan-1/EGFP construct or with a short truncated version containing only the tubulin binding RMKKK sequence. The sub-cellular distribution of the EGFP fusion proteins was monitored by fluorescence microscopy. Our data indicate that syndecan-1, FGF-2 and heparanase co-localize in the nucleus, whereas FGFR-1 is enriched mainly in the perinuclear area. Overexpression of syndecan-1 results in increased nuclear accumulation of FGF-2, demonstrating the functional importance of syndecan-1 for this nuclear transport. Interestingly, exogenously added FGF-2 does not follow the route taken by endogenous FGF-2. Furthermore, we prove that the RMKKK sequence of syndecan-1 is necessary and sufficient for nuclear translocation, acting as a nuclear localization signal, and the Arginine residue is vital for this localization. We conclude that syndecan-1 and FGF-2, but not FGFR-1 share a common transport route and co-localize with heparanase in the nucleus, and this transport is mediated by the RMKKK motif in syndecan-1. Our study opens a new perspective in the proteoglycan field and provides more evidence of nuclear interactions of syndecan-1

    Heterozygous Yeast Deletion Collection Screens Reveal Essential Targets of Hsp90

    Hsp90 is an essential eukaryotic chaperone with a role in folding specific “client” proteins such as kinases and hormone receptors. Previously performed homozygous diploid yeast deletion collection screens uncovered broad requirements for Hsp90 in cellular transport and cell cycle progression. These screens also revealed that the requisite cellular functions of Hsp90 change with growth temperature. We present here for the first time the results of heterozygous deletion collection screens conducted at the hypothermic stress temperature of 15°C. Extensive bioinformatic analyses were performed on the resulting data in combination with data from homozygous and heterozygous screens previously conducted at normal (30°C) and hyperthermic stress (37°C) growth temperatures. Our resulting meta-analysis uncovered extensive connections between Hsp90 and (1) general transcription, (2) ribosome biogenesis and (3) GTP binding proteins. Predictions from bioinformatic analyses were tested experimentally, supporting a role for Hsp90 in ribosome stability. Importantly, the integrated analysis of the 15°C heterozygous deletion pool screen with previously conducted 30°C and 37°C screens allows for essential genetic targets of Hsp90 to emerge. Altogether, these novel contributions enable a more complete picture of essential Hsp90 functions

    Internetifieringen i vårt samhälle går rasande fort och vi blir mer och mer uppkopplade.Detta projekt handlar om att upplysa betraktaren om mötet med internet och skapa enpersonlig relation till det fenomen som internet är. Genom ett synliggörande med aktiv interaktionligger projektet i gränslandet mellan kritisk design, interaction design och industridesign med fokuspå 'Design För Alla' frågor för alla.’Design För Alla’ är en innovationsmetod som innebär att frågeställningar ställs på sin spets ochatt man utgår från och testar sina idéer tillsammans med dem som ställer högst krav och harstörst behov. ’Design För Alla’ gör att man kan ifrågasätta gammal praxis och existeradereferensramar för standardlösningar och istället angripa en frågeställning utifrån nyaassociationsbanor och inspirationsingångar som gör researchen mer tillåtande och spännande.Ett av projektets viktigaste moment var en workshop med en grupp kognitivt nedsatta ungdomarfrån Höghammarskolan. Att blanda in personer med kognitiv funktionsnedsättning lägger fokuspå andra frågor än en “normalstörd” studiegrupp skulle ha gjort. Dessa ungdomar lever under en”disorienterad norm” där de med sin funktionsnedsättning lärt sig hitta vägar runt sinaproblem och ofta går problemlösning till mötes mer ”rakt på” och intuitivt. ’Design För Alla’ har berikatmin process genom att jag försökt anamma deras ”hands on” och lekfulla sätt för att förberedamig för workshopens laborativa moment kring färg, ljud, form, material, rörelse ochsemiotiska funktioner kring framför allt på och avstängning. Det har gjort att jag sökt efter merintuitiva och basala kroppsliga uttryck för interaktion och kommunikation med mitt internetkoncept.För hur lever och upplever vi något som är osynligt? Resultatet är Labyrinternet; ett homage tillinternet i form av en el&amp;internet-station som också agerar plattform för debatten kring frågan”Vilken plats tar internet i vår vardag och i våra liv?
