    Cytochrome P450 2C19 Polymorphism in Iranian Patients with Coronary Artery Disease

    <p><strong>BACKGROUND:</strong> The polymorphisms of cytochrome P450 2C19 (CYP2C19) gene are major prognostic factors for the response to clopidogrel therapy in patients with coronary artery diseases (CAD). The CYP2C19*2 is the most important allele responsible for resistance to clopidogrel therapy. This study examined CYP2C19 gene polymorphism (CYP2C19*1 and *2) in Iranian patients.</p> <p>&nbsp;</p> <p><strong>METHODS:</strong> This cross-sectional study was performed on 43 Iranian patients with CAD who underwent percutaneous coronary intervention (PCI) and received drug-eluted stents (DES). CYP2C19 polymorphisms were assessed using real time PCR and frequency of CYP2C19*1 and CYP2C19*2 were determined, and then homo- or heterozygous state of genes was detected by Melt Curve Analysis method.</p> <p>&nbsp;</p> <p><strong>RESULTS:</strong> Forty three patients (mean age = 58.8 &plusmn; 10.0 years, 79.1% male) participated in this study. CYP2C19*1/CYP2C19*1 genotype was observed in 31 (72.1%) of participates, CYP2C19*1/CYP2C19*2 genotype in 10 (23.3%), and CYP2C19*2/CYP2C19*2 genotype in 2 patients (4.7%). The frequency of CYP2C19*2 allele in the sample was 27.9%.</p> <p>&nbsp;</p> <p><strong>CONCLUSION:</strong> This study demonstrated a high prevalence of CYP2C19*2 gene polymorphism in Iranian patients.     Serodiagnosis of fasciolosis by fast protein liquid chromatography-fractionated excretory/secretory antigens

    In several studies, different antigenic preparations and diverse immunological tests were applied for serodiagnosis of Fasciola hepatica infections. Most of these preparations showed cross-reactivity with proteins of other parasites. Application of purified antigens might reduce these cross-reactivities. Here, we used fast protein liquid chromatography (FPLC)-fractionated extracts of F. hepatica excretory/secretory antigens (E/S Ags) for serodiagnosis of human and sheep fasciolosis. To develop an improved diagnostic method, we fractionated F. hepatica E/S Ags by anion exchange chromatography on a Sepharose CL-6B column and then tested the serodiagnostic values of the fractions. We used sera from F. hepatica-infected human and sheep as positive controls. Sera from patients with hydatidosis and strongyloidiasis were used for cross-reactivity studies. Enzyme-linked immunosorbent assays (ELISA) of the second FPLC peak, containing 20, 25, and 70 kDa proteins, discriminated between F. hepatica-infected and uninfected human and sheep samples. Fractionation of F. hepatica E/S Ags by FPLC is a fast and reproducible way of obtaining antigens useful for serodiagnosis of human and sheep fasciolosis with acceptable sensitivity and specificity

    Morphological Description, Phylogenetic and Molecular Analysis of Dirofilaria immitis Isolated from Dogs in the Northwest of Iran

    Background: Dirofilariasis is a globally distributed arthropod-borne parasitic disease of mainly canids and felids. We evaluated to extend the knowledge of morpho-molecular characteristics and outer ultrastructure of Dirofilaria immitis isolated from Northwest of Iran. Methods: Overall, 67 filarial worms including 41 females and 26 males parasites were collected from the cardiovascular system of the 43 stray dogs in Meshkinshar, Ardebil Province, Northwest of Iran in 2017, and subjected to light and scanning electron microscopy (SEM) as well as carmine alum staining for morpho-molecular and identification. Molecular methods were used for confirmation of morphological findings by sequencing of Cyto-chrome c oxidase subunit I (cox1) gene. Results: The partial DNA sequencing of cox1 gene of adult parasites showed considerable homology and close proximity to the previously isolated from Kerman and Meshkinshahr, Iran. The lowest genetic variation and the highest intra-species variability was found in D. immitis and Dirofilaria repens, respectively. No similarity was identified between D. immitis nucleotide sequence and Wolbachia species as its endosymbiont bacteria.     Depression Symptoms and Sleep Quality during COVID-19 Outbreak in Iran: A National Cross-Sectional Study

    Background: Coronavirus 2019 is a new infectious disease that has been declared by the World Health Organization as a global pandemic. This pandemic may have psychological effects on populations. Objectives: This study aimed to investigate the quality of sleep and depression in the Iranian community during COVID-19 pandemic. Methods: This nationwide cross-sectional web-based study was conducted from April 19 to May 12, 2020 in Iran. The data collection tools were the Pittsburgh Sleep Quality Index and Depression Center for Epidemiologic Studies' Depression Scale questionnaires. Each province in Iran was assumed as a cluster, and generalized estimating equations models were used to determine the demographic variables that were related to depression symptoms and sleep quality during COVID-19 outbreak. Results: In this study, 73.5% and 41.4% of the participants had poor sleep quality and had depression symptoms, respectively. Perceived income, hours spent outdoors, the number of people with COVID-19 that were acquainted with, and a history of depression significantly were related to sleep quality. Also, gender, age, having a child, education level, economic status, and a history of depression were significantly associated with having depression symptoms. Conclusions: Most participants had poor sleep quality, and less than half had depressionsymptomsduring the COVID-19 pandemic. These problems can disturb daily routines, cut social ties, and increase worries and fears of the future. Interventions are necessary to reduce psychological problems in Iran

    A Systematic Review on the Climate and Ecosystem Change Associated With the COVID-19 Epidemic: Global Challenges

    The most serious current challenge in the world is COVID-19 disease incidence. With the spread of COVID-19, in addition to widespread human and economic damages, concerns have increased about the world’s climate and ecosystem change. This change alters the genetic structure of viruses, leading to newer strains. This study addressed global challenges regarding the COVID-19 epidemic effects and possible two-way changes in climate and ecosystems. Studies conducted from 2019 to 2022 were reviewed in this systematic review. Articles on climate and ecosystem change related to the COVID-19 epidemic were searched in the Scopus, Web of Science, and PubMed databases in accordance with the MeSH search strategy using keywords such as "Climate Change" and "Ecosystem" or "COVID-19". In this research, the coding method based on the PRISMA chart was used, and 13 related articles were included in the study after qualitative evaluation. The COVID-19 epidemic is likely to have significant implications for progress in climate and ecosystem change. The phenomenon of climate change and its interaction with the COVID-19 epidemic is not limited to natural issues. One of the most important consequences is its impact on the social and economic issues of human societies, the most important of which are air pollution and environmental degradation. An increase in normal and special wastes, water consumption and wastewater production, air pollution after the start of the global economy, damage to forests and animals, and tendencies to use fossil fuels are only a part of the direct and indirect negative effects of COVID-19 on climate and ecosystem change. Epidemics directly threaten people and the health system, while climate and ecosystem change more broadly weakens natural and human systems. The COVID-19 crisis requires solutions within weeks and months, whereas responses to the climate and ecosystem change crisis seem less acute. However, the effects of climate and ecosystem change worsen with further procrastination. Thus, such crises with overlapping conditions and interactions require more attention and immediate public mobilization. Thus, the necessary planning should be implemented to moderate and reduce its effects

    Fatty acid and retinol-binding protein: A novel antigen for immunodiagnosis of human strongyloidiasis

    The tenacious human parasitic helminth Strongyloides stercoralis is a significant health problem worldwide. The current lack of a definitive diagnostic laboratory test to rule out this infection necessitates designing more specific diagnostic methods. Fatty acid and retinol-binding protein (FAR) plays a crucial role in the development and reproduction of nematodes. We generated a recombinant form of this protein and determined its applicability for immunodiagnosis of S. stercoralis. The L3 form of S. stercoralis was harvested and used for RNA extraction and cDNA synthesis. The coding sequence of S. stercoralis FAR (SsFAR) was cloned into pET28a(+) vector, expressed in E. coli BL21 and purified. ELISA and immunoblotting were employed to determine the specificity and sensitivity of rSsFAR using a set of defined sera. In addition, we analyzed the phylogenetic relationship of SsFAR with different FAR sequences from other nematodes. The cloned SsFAR had an open reading frame of 447 bp encoding 147 amino acids, with a deduced molecular mass of 19 kD. The SsFAR amino acid sequence was 93% identical to FAR of S. ratti. For differential immunodiagnosis of strongyloidiasis, rSsFAR exhibited 100% sensitivity and 97% specificity. However, cross-reactivity with FAR proteins of other parasites, namely Toxocara canis and Echinococcus granulosus, was noted. Our results provide a novel approach for immunodiagnosis of S. stercoralis infections using rSsFAR with reliable sensitivity and specificity

    Detection of Echinococcus multilocularis in Carnivores in Razavi Khorasan Province, Iran Using Mitochondrial DNA

    Echinococcus multilocularis causes alveolar echinococcosis, a serious zoonotic disease present in many areas of the world. The parasite is maintained in nature through a life cycle in which adult worms in the intestine of carnivores transmit infection to small mammals, predominantly rodents, via eggs in the feces. Humans may accidentally ingest eggs of E. multilocularis through contact with the definitive host or by direct ingestion of contaminated water or foods, causing development of a multivesicular cyst in the viscera, especially liver and lung. We found adult E. multilocularis in the intestine and/or eggs in feces of all wild carnivores examined and in some stray and domestic dogs in villages of Chenaran region, northeastern Iran. The life cycle of E. multilocularis is being maintained in this area by wild carnivores, and the local population and visitors are at risk of infection with alveolar echinococcosis. Intensive health initiatives for control of the parasite and diagnosis of this potentially fatal disease in humans, in this area of Iran, are needed

    Spatio-Temporal Evolution of the Thermo-Hygrometric Index (THI) during Cold Seasons: A Trend Analysis Study in Iran

    Global warming can cause deep and extensive changes in the Earth&rsquo;s climate and changes in the time and place of climatic phenomena. The present trend analysis study assesses cold stress using the thermo-hygrometric index (THI) in the two seasons of autumn and winter in outdoor environments in Iran. The data related to the average of the two variables of daily air temperature and relative humidity from 60 synoptic meteorological stations for a statistical period of 30 years were obtained from the Iranian Meteorological Organization. The THI index was calculated for autumn and winter, and the level of thermal discomfort was determined for each station. The Mann&ndash;Kendall statistical test with the help of Minitab ver17.1.0 software was also used to investigate the changes in air temperature, relative humidity and THI index. The THI for autumn increased in 68% of the stations, and this increasing trend is statistically significant in 51% of these stations. The THI for winter increased in 83% of the stations, and this increasing trend is statistically significant in 51% of these stations. In autumn, 53% of the stations were in the range of thermal discomfort, and in winter only 5% were in the range of thermal comfort. The decreasing trend in THI in some climatic types, along with the increasing trend in air temperature, can indicate the decrease in relative humidity in the monitored stations during the studied years. It is recommended to know the temporal and spatial distribution and the probability of occurrence of cold stress through the use of THI in order to adopt preventive measures and policies in the outdoors in Iran
