    PURPOSE:We hypothesized that the pattern of branching of the lenticulostriate arteries (LSAs) is involved in the variation of the distribution of the infarction within the LSA region. Our purpose was to evaluate the visibility of LSAs in 3D time-of-flight (TOF) MR angiography (MRA) with a 3.0 T scanner and to investigate the branching patterns of LSAs.MATERIALS AND METHODS:We performed 3D TOF MRA at 3.0 T for 100 healthy subjects. We assessed the number of LSAs and the number of branches arising from each LSA by evaluating MRA source images.RESULTS:In 200 hemispheres, 330 LSAs were visualized (mean = 1.65/hemisphere). In 3.5% of all hemispheres, no LSA was depicted; one LSA was depicted in 39%, two in 46.5%, and three in 11%. The maximum number of depicted LSA branches was five in 2% of all subjects, four in 7%, three in 26%, and two in 49% (mean = 2.3/subject). A large LSA trunk with three or more branches was found in 35% of subjects.CONCLUSION: Visualization of LSAs was possible in 96.5% of subjects by use of 3.0 T MRA.     A Case of Androgen Receptor-positive Triple Negative Breast Cancer with Good Response to Anti-androgen Therapy

    Anti-androgen therapy has been proposed to be effective in the treatment of androgen receptor (AR)-positive triple negative breast cancer (TNBC). Herein, we report on the case of a 91-year-old female patient with AR-positive TNBC who underwent anti-androgen therapy and had a good response. Because of dementia, the patient lives in an aged care facility. It was here that a staff member noticed a mass with a rash on the patient\u27s breast. Consequently, the patient was sent for further examination. Ultrasonography revealed an irregularly shaped, indistinct hypoechoic mass measuring 19×18×9mm located in the upper outer quadrant of the left breast. Core needle biopsy (CNB) was performed and the mass was diagnosed as apocrine carcinoma. Further immunohistochemical analysis showed that the mass was AR-positive TNBC. Anti-androgen therapy was determined to be the optimal treatment option for this patient. The Institutional Review Board approval the off-label use of an anti-androgen for the treatment of this patient, who was subsequently treated with the anti-androgen flutamide. After 8 months, the size of the tumor had reduced to 15×9×5mm

    Transition from convection rolls to large-scale cellular structures in turbulent Rayleigh-Benard convection in a liquid metal layer

    Turbulent Rayleigh-Benard convection was investigated within a liquid metal layer, Prandtl number Pr = 0.03, in a square vessel having a moderate aspect ratio, Gamma = 5. Laboratory experiments were performed at moderate Rayleigh numbers, 7.9 x 10(3) < Ra < 3.5 x 10(5). Ultrasonic velocity profiling was used to visualize the spatiotemporal flow structure in two horizontal planes, while temperature fluctuations were monitored simultaneously in the fluid layer. By using multiple ultrasonic sensors, a grid of orthogonal measurement lines was created. This configuration enabled the identification of coherent flow structures showing periodic oscillations. In particular, oscillatory roll-like structures were observed for Ra less than or similar to 6 x 10(4), while the transition to a new-found, fully three-dimensional cellular structure occurs around Ra = 7 x 10(4). The Fourier analysis of the temperature fluctuations indicates that the convection reaches the developed state of thermal turbulence at this Ra number. This cellular structure of the flow field is recognized as a representation of the large-scale circulation in thermal turbulence for the specific situation of confined convection in the rectangular vessel. The transition from laminar convection to thermal turbulence manifests itself in the occurrence of unstable intermediate regimes accompanied by a stepwise increment in the horizontal scale. We suggest scaling laws for the characteristic velocity and the dominating oscillation frequency and based on that for the horizontal length scale as a function of the Ra number. The comparison to corresponding values of characteristic length scales published for thermal convection in air in larger aspect ratios [Pr = 0.7, T. Hartlep et al., Phys. Rev. Lett. 91, 064501 (2003), A. Pandey et al., Nat. Commun. 9, 2118 (2018), and D. E. Fitzjarrald, J. Fluid Mech. 73, 693 (1976)] reveals a different Ra number dependence of the horizontal wave number

    Which symptoms negatively affect the oral health-related quality of life in patients with osteonecrosis of the jaw?

    Objectives. One of the treatment goals for osteonecrotic lesions of the jaw, such as medication-related osteonecrosis of the jaw (MRONJ) or osteoradionecrosis (ORN), is restoration of quality of life (QOL). This study aimed to identify symptoms that negatively affect QOL in patients with unhealed MRONJ or ORN. Study Design. This cross-sectional study included patients who were previously diagnosed with MRONJ or ORN and who underwent treatment at the Kobe University Hospital between June 2015 and February 2016. Patient QOL was measured by using the Oral Health Impact Profile (OHIP-14). The predictor variable was disease status (stage and healing). The outcome variable was OHIP-14. One-way analysis of variance and Tukey's test were performed. Results. The study included 74 patients (37 men and 37 women; mean age 70 years). Although there was no significant difference between the OHIP-14 scores of unhealed MRONJ and ORN (stages 1-3) and those of healed ones, the "worsened sense of taste" resulted in significant differences among stages in patients with unhealed MRONJ (P = .027) and the "painful mouth aching" in patients with unhealed ORN (P = .041). Conclusions. Worsened sense of taste and pain negatively affected QOL in patients with unhealed MRONJ and ORN

    Adjusted Anion Gap Is Associated with Glomerular Filtration Rate Decline in Chronic Kidney Disease

    Background: Metabolic acidosis is known to accelerate the progression of chronic kidney disease (CKD). However, whether undetermined anions as indicated by the adjusted anion gap (aAG) are associated with estimated glomerular filtration rate (eGFR) decline in patients with CKD is unclear. Methods: Data from 42 patients with CKD (baseline eGFR, 7.1-52.0 ml/min/ 1.73 m2) without massive proteinuria (urinary protein-creatinine ratio, UPCR Results: The mean baseline eGFR was 27.5 ± 11.1 ml/min/1.73 m2 and the mean %&#x0394;eGFR/6m was 13.8 ± 10.3. UPCR and aAG were 1.13 ± 0.93 and 9.48 ± 1.88, respectively. %&#x0394;eGFR/6m was associated with aAG (r = 0.438, p 3- concentration. Conclusion: These data suggest that aAG appears to be associated with the progression of CKD. aAG might be an independent predictor of CKD progression
