    Trabajo de investigaciónBasados en la experiencia en la ejecución de contratos de mantenimiento de la infraestructura Vial del Instituto de Desarrollo Urbano (IDU) en la ciudad de Bogotá y en las constantes problemáticas que surgen en las etapas constructivas de los proyectos de mantenimiento. Es necesario buscar alternativas desde el punto de vista de dirección de proyectos para mitigar las circunstancias que no permiten cumplir las expectativas tanto técnicas como económicas de estos proyectos.     THE ISLAMIC DECORATIONS ON SAFAVID COPPER COINS BETWEEN THE UNITY AND DIVERSITY IN ITS ILLUSTRATED DECORATIOS ''SELECTED MODELS FROM FUJAIRAH MUSEUM''

    The Museum of Fujairah in the United Arab Emirates contains a collection which includes of 13 copper coins dating to the period of weakness and crisis at the end of Safavid Dynasty, during the reigns of Shah Suleiman "Safi II" (1077-1105 H./1666-1694 C.) and Shah Sultan Hussein I (1105-1135 A.H/1694-1722). The Study aims to discusses the illustrated drawings on these coins throughout the Iranian cities, the theme of the study, and other cities during the Safavid period in general; and this historical period especially which was the beginning of the end of Safavid Daynasty; with the origins of these drawings of the Safavid copper coins "Fulus" through the early Islamic Coins, and through its inclusion on the applied artifacts, such as pottery, textile, metal, wood, carpets, stone and glass during the Islamic Age. The Study shows that the Safavid artists included the decorative elements which represent Sasanian traditions, because the Safavid Dynasty attempted to revive Persian nationalism and regain its previous glory. The Study ends with the results which shows that the decorative elements on theses coins are considered an expansion of the decorative elements which adorned different applied artifacts with drawings on pottery; or carving on wood, stones or marble; or enameling on glass; or printing on textile, that occurred in all Muslim countries since the first century A.H, either in Andalusia, Egypt, Iraq, Levant, and Turkey assuring the unity of decorative elements on coins and different applied artifacts in the frame of Islamic art with the diversity in the decoration methods.&nbsp

    Comparative study between minimally invasive and open esophagectomy for the treatment of esophageal cancer

    Background: Complete resection of the esophageal tumor is the gold standard therapy. The optimal surgical approach for esophagectomy is still controversial. This study compared the short-term outcomes of minimally invasive (MIE) and open esophagectomy for treating esophageal cancer. Methods: A prospective study was conducted on 70 consecutive patients who received esophagectomy for esophageal carcinoma between 2017 and 2019 at Henan Cancer Hospital, Zhengzhou, China. Fifty patients received MIE (Group A), and 20 received open esophagectomy (Group B). Among Group B, 17 patients had left thoracotomy, and three had three incision esophagectomy. Results: The mean age in Group A was 61.48 ± 8.06 years, and 61.3 ± 7.52 years in Group B (p= 0.932). In Group A, most of the tumors were located in the middle thoracic area (56%), while in the open esophagectomy group, most of the tumors were located in the lower thoracic-esophagogastric junction area (50%) (p<0.001). The most common stage in Group A was (T3N0M0) and (T3N0M0) in Group B (p= 0.044). Neoadjuvant therapy was used in 48% of patients in Group A and 15% in Group B (p=0.08). The mean number of resected lymph nodes in Group A was 28.8 ± 7.8 lymph nodes versus 22.4 ± 7.7 in the open esophagectomy group (p=0.003). The mean operative bleeding amount was 80±34.6 ml and 185± 46.2 ml for groups A and B, respectively (p=0.001). The mean ICU stay for Group A was 0.5± 0.7 days versus 0.4± 0.6 days for Group B (p=0.4). The mean postoperative hospital stay for Group A was 8± 3 days, while in Group B, the mean postoperative hospital stay was 14± 3 days (p=0.001). Postoperative complications occurred in 2 patients (4%) in Group A and seven in Group B (p= 0.001). No tumor recurrence was detected radiologically among the two groups in the three months follow-up period. Conclusion: Minimally invasive esophagectomy may be a feasible and safe procedure for patients with early-stage esophageal cancer or locally advanced neoplasms who have received neoadjuvant therapy

    The Yolk Sac Abnormalities, Maternal Serum Level of Cancer Antigen 125 (CA-125) and Beta Human Chorionic Gonadotropin (B-HCG) as an Early Predictors of First Trimester Pregnancy Loss in Patients with Threatened Miscarriage

    Background: Pregnancy loss before 20 weeks is considered a miscarriage, as is the loss of a fetus weighing less than 500 grams before viability. A medical emergency, threatened miscarriage affects 15–25% of pregnancies. Aim and objectives: The goal of this study was to assess the predictive value of maternal blood levels of Cancer Antigen 125 (CA-125) and beta-human chorionic gonadotropin (B- HCG) in individuals at risk of miscarriage during the first trimester. Subjects and methods: This study was a prospective cohort study. This study included 120 pregnant women with threatened abortion between (6-11 weeks) and followed up till end of 14th week. Results: 36(30%) of pregnant women aborted, while 84(70%) of women continued till 14th weeks of pregnancy. At a cut-off value of 45 U/ml, the CA125 test was shown to have a sensitivity of 88.9% and a specificity of 77.5%, respectively, while also having a positive predictive value of 79.8% and a negative predictive value of 87.5%. At a cut-off value of 18.501 mlIU/ml, the B-HCG test's sensitivity and specificity were determined to be 96.3 and 88.9, respectively, with a positive predictive value of 89.7% and a negative predictive value of 96%. Conclusion: Even before fetal morphology can be investigated sonographically, abnormalities in the size of the yolk sac can be utilized as a good prognostic sign of early pregnancy loss. Pregnancy viability can be estimated from first trimester serum CA 125 and Beta HCG measurements

    Surgical management and outcome of intramedullary spinal cord tumour

    Abstract Objective Our aim is to assess the surgical management of intramedullary spinal cord tumours (IMSCTs) and evaluate factors associated with surgical outcomes in our hospitals. Patient and methods Between June 2013 and June 2016, a retrospective study was conducted on 16 consecutive cases of IMSCTs. All patients provided their signed consent, and MRI was performed. The patients were surgically treated and were evaluated pre- and post-operatively by the modified McCormick scale (MMS). Appropriate statistical analysis was conducted. Results The mean patient age was 50.4 years, and the median follow-up was 15 months. The most common histological origin was ependymoma (n = 9, 56.25%). A cervical tumour was detected in eight patients, and a dorsal tumour was detected in seven. Post-operatively, the score was clinically but not statistically improved in seven cervical (87.5%) and four dorsal (57.1%) tumours (p = 0.334). Ten patients underwent total resection. Post-operative MMS scores showed improvement in all cases of total resection (n = 10, 100%). This improvement was clinically and statistically significant on last follow-up (p = 0.008). Fewer than four segments were involved in 9 cases, and more than four segments were involved in 7 cases. Post-operatively, all 9 patients (100%) with fewer than four involved segments improved, while only three patients (42.9%) with more than 4 involved segments improved (p = 0.019). Low-grade tumours such as ependymomas were correlated with good surgical outcomes, while high-grade tumours such as astrocytomas were correlated with poor surgical outcomes (p = 0.022). Conclusions Total tumour resection coupled with good preoperative clinical condition for tumours localised in the cervical or conus region predicts good neurological outcomes. Tumour localisation in the dorsal region with multi-segmental extension and high-grade tumour pathology predicts poor neurological outcomes

    Background: Aromatase inhibitors (AI) can boost endogenous testosterone production without increasing circulating estrogen levels, as shown with estrogen receptor modulators, Aim and Objectives: The study's goal was to determine the efficacy of aromatase inhibitor medication in improving spermatogenesis in oligozoospemic and azoospermic males, as well as its relationship to sexuality. Subjects and methods: This study was done on eighty man subjects joining the outpatient clinic of Andrology, Kasr El Aini Hospital, Cairo University during the peroid from April 2018 – April 2020, and they were divided into (2) groups, 40 subjects in each group.  Results: There was a statistically significant difference in serum total testosterone measured before and after treatment in normal FSH patients with a p value ≤ 0.05. There was a notably not important change in sexual desire questionnaire measured before and after treatment in normal FSH patients with a p value > 0.05, Conclusion: ERs and aromatase share topographic sites in the brain with pheromones, indicating that estrogen influences both early sexual maturation and sexual behavior in adults. Estrogen can maintain libido while also influencing the number of serotonin receptors in the brain, which modulates mood, mental state, cognition, and emotion

    Application of Chitosan/Alginate Nanocomposite Incorporated with Phycosynthesized Iron Nanoparticles for Efficient Remediation of Chromium

    Biopolymers and nanomaterials are ideal candidates for environmental remediation and heavy metal removal. As hexavalent chromium (Cr6+) is a hazardous toxic pollutant of water, this study innovatively aimed to synthesize nanopolymer composites and load them with phycosynthesized Fe nanoparticles for the full Cr6+ removal from aqueous solutions. The extraction of chitosan (Cht) from prawn shells and alginate (Alg) from brown seaweed (Sargassum linifolium) was achieved with standard characteristics. The tow biopolymers were combined and cross-linked (via microemulsion protocol) to generate nanoparticles from their composites (Cht/Alg NPs), which had a mean diameter of 311.2 nm and were negatively charged (−23.2 mV). The phycosynthesis of iron nanoparticles (Fe-NPs) was additionally attained using S. linifolium extract (SE), and the Fe-NPs had semispherical shapes with a 21.4 nm mean diameter. The conjugation of Cht/Alg NPs with SE-phycosynthesized Fe-NPs resulted in homogenous distribution and stabilization of metal NPs within the polymer nanocomposites. Both nanocomposites exhibited high efficiency as adsorbents for Cr6+ at diverse conditions (e.g., pH, adsorbent dose, contact time and initial ion concentration) using batch adsorption evaluation; the most effectual conditions for adsorption were a pH value of 5.0, adsorbent dose of 4 g/L, contact time of 210 min and initial Cr6+ concentration of 75 ppm. These factors could result in full removal of Cr6+ from batch experiments. The composited nanopolymers (Cht/Alg NPs) incorporated with SE-phycosynthesized Fe-NPs are strongly recommended for complete removal of Cr6+ from aqueous environments
