    Enhanced Obfuscation for Software Protection in Autonomous Vehicular Cloud Computing Platforms

    Nowadays, sensors, communications connections, and more powerful computing capabilities are added to automobiles, making them more intelligent. The primary goal was to eliminate the need for human control, making them Autonomous Vehicles (AVs). Consequently, researchers thought to put all that newly added computational power to use for other endeavors. Hence, Autonomous Vehicular Cloud Computing (AVCC) models were introduced. Nevertheless, this goal is not an easy undertaking, the dynamic nature of autonomous vehicles introduces a critical challenge in the development of such a distributed computing platform. Furthermore, it presents far complicated issues as far as security and protection of services associated with this framework. In this paper, we center around securing programs running on AVCC. Here, we focus on timing side-channel attacks which aim to leak information about running code, which can be utilized to reverse engineer the program itself. We propose to mitigate these attacks via obfuscated compilation. In particular, we change the control flow of an input program at the compiler level, thereby changing the program’s apparent behavior and accompanying physical manifestations to hinder these attacks. We improve our previous ARM-based implementation to address its limitations and provide more comprehensive coverage for different programs. Our solution is software-based and generically portable - fitting different hardware platforms and numerous input program languages at the source level. Our findings prove a considerable improvement over our previous technique, which may provide more defense against timing side-channels

    Thoracoscopic excision of asymptomatic posterior mediastinal ganglioneuroma. A case series study

    Background Posterior mediastinal ganglioneuroma is the most frequent type of neurogenic tumors, and usually it is asymptomatic. The surgical approach to excise it may differ from center to center based on the surgeon's experience as well as the size of tumor. We aim to present our center's experience and short-term outcomes in surgical excision of posterior mediastinal ganglioneuroma. Methods This retrospective case series study, from June 2019 till December 2022, included 9 asymptomatic patients with mean age of 31± 5 years who expressed lesion in the posterior mediastinum. The mean size of the tumor was   5.3±3.6 cm in maximal dimensions.  Results There were 7 males and 2 females with mean age was 31± 5 years,  the excision by video-assisted thoracoscopy (VATS)  was  employed in 8 patients and conventional thoracotomy was carried out in 1 case.  The mean operative time was 57±14 minutes with mean operative blood loss 50±10 ml. The mean postoperative chest tube drainage was 120±80 ml and no complications related to the procedures were observed. The average time of hospital stay after VATS treatment was 2 days, while after thoracotomy it was 5 days and patient suffered from ptosis that eventually recovered after 1 month with supportive treatment. Histopathological examination confirmed benign nature of ganglioneuroma in all cases. Conclusions Thoracoscopic excision of asymptomatic posterior mediastinal neurogenic tumor is safe, feasible and reduces postoperative pain, however, open technique might be considered for large-sized tumor to minimize the possible complications

    Role of IL-28B polymorphisms in virologic response to combined pegylated interferon and ribavirin therapy in genotype 4 chronic HCV infected patients with and without cirrhosis

    Background: Chronic hepatitis C virus (HCV) represents one of the common causes of chronic liver disease worldwide with Egypt having the highest prevalence, namely genotype 4. The rs12979860 CC genotype of the interleukin 28B (IL28B) polymorphisms is associated with high rates of sustained virological response to pegylated interferon and ribavirin in HCV genotype-1 patients. Data on other genotypes are more limited.Objective: We aim to evaluate the predictive power of the rs12979860 IL28B single nucleotide  polymorphisms for treatment response at 3 and 6 months in chronic HCV genotype 4 Egyptian patients in relation to other predictors.Patients and methods: The study included 60 chronic HCV Egyptian patients receiving pegylated interferon and ribavirin therapy. Patients were classified into 2 groups; 30 patients with compensated cirrhosis, and 30 patients without cirrhosis. We analyzed selected pretreatment factors such as age, sex, HCV viral load, anti-schistosomal antibodies, insulin resistance, alpha fetoprotein, low and high density lipoproteins and single nucleotide polymorphisms of IL28B and tried to find out which of them influence sustained virological response.Results: In univariate analysis, CC genotype showed a significant association with sustained virological response at 6 months among the cirrhotic patients (81.8% responders had the CC genotype, 58.3% had the CT/TT genotype) (p= 0.009).     Can a foreign body migrate against natural body barriers?

    Pericardial foreign bodies (FBs) are a rare cause of chest pain in children. They can reach the pericardium through several routes including direct or iatrogenic implantation, transbronchial or transesophageal migration of inhaled or swallowed FBs. We reported a case of a 4-year-old girl presenting with persistent chest pain for 1 month. The child described the pain as ‘stitching’ in nature localized on the left side of the sternum. The child presented with increased pain intensity and a new onset of fever and cough. No history of chocking or swallowing of FB and no signs of trauma or child abuse were noted. Chest radiography revealed a needle in the left side of the chest. Computed tomography scan and echocardiography were used to precisely localize the needle and exclude intracardiac extension. ECG showed elevated ST segment and cardiac enzymes were normal. Removal of the needle was carried out surgically under fluoroscopic guidance. A small portion of the needle was found intrapericardially complicated by localized purulent pericarditis.
