    Analysis of Public Health Care Management in Dealing with Covid 19 Situations in India

    The COVID-19 pandemic will continue to have an impact on foundation projects worldwide in all sectors, as disruptions to supply chains, a decline in customer demand, and changes to health regulations force a reevaluation of how business is conducted. Public-private partnerships (PPPs) communicate foundation initiatives, which are more difficult to govern due to the long-term authoritative obligations imposed between counterparties, such as outside loan specialists and financial supporters. In addition to determining how the targeted population generally feels about the quality of services they receive, the study aims to gain a deeper understanding of Ayushmann Bharat, a program that falls under the public-private partnership model in the healthcare sector and is necessary to support public-private partnership. The study aims to present recommendations to the government that explicitly encourage the expansion of public-private partnerships and the involvement of private and public firms in the development of healthcare. Nevertheless, there are few state-level studies that address the COVID-19 scenario. The studies on public opinion of the PPP model in the health sector have a gap that the researcher has identified. Therefore, this study aims to investigate practitioners\u27 opinions regarding the implementation of the PPP model in the health sector as well as the public\u27s perspective of the model\u27s many aspects

    Role of diagnostic hysterolaparoscopy in evaluation of female infertility

    Background: Infertility affects 10-15% of reproductive age couples. Routine pelvic examination and usual diagnostic procedures can miss majority of the pelvic pathologies. Hysterolaparoscopy is an effective tool for diagnosing these pathologies and additionally therapeutic procedures, like polypectomy, myomectomy, septal resection and adhesiolysis, can be done in the same sitting. Objective of present study was to determine the role of diagnostic hysterolaparoscopy for evaluating the causes of infertility.Methods: It was a prospective study conducted at the Department of OBGY from September 2015 to August 2016. Patients aged 20-40 years with infertility were included in the study. The prevalence of different lesions was analysed.Results: Out of the 30 patients studied, most had primary infertility. Laparoscopic abnormalities (56.6%) were more common than hysteroscopic abnormalities (36.6%). Adnexal adhesions (26.6%) was the most common abnormality detected on laparoscopy, while the common intra-uterine pathologies were uterine septum and synechiae.Conclusions: Hysterolaparoscopy may be recommended as the procedure of choice for evaluation of female infertility, which are usually missed by imaging modalities

    Comparative study of maternal and foetal outcome in instrumental vaginal deliveries

    Background: The art of instrumental vaginal delivery is fading as more and more obstetricians resort to caesarean sections. Instrumental deliveries are deliveries accomplished with the use of vacuum device or forceps. Once either is applied to the foetal head, outward traction generates forces that augment maternal pushing to deliver the foetus vaginally. Objective of present study was to compare the maternal and foetal outcome of forceps versus ventouse delivery.Methods: It was a prospective observational study conducted in Sassoon Hospital between January 2014 to January 2015. A total of 120 cases, 60 each of forceps and ventouse, admitted in labour room were included in the study. Maternal and neonatal morbidity were compared in terms of perineal lacerations, episiotomy extension, post-partum haemorrhage, Apgar score, instrumental injuries. Chi square test and z test of proportion was used to analyze the data.Results: Maternal morbidity, namely episiotomy extension as well as perineal tears were significant in the forceps group. With regards to neonatal morbidity, no statistically significant difference was noted.Conclusions: Vacuum and forceps should remain appropriate tools in the armamentarium of the modern obstetrician. However, ventouse may be chosen first (if there is no foetal distress) as it is significantly less likely to injure the mother

    Active management of third stage of labour with special reference to misoprostol

    Active management of third stage of labour is an effective method of preventing postpartum hemorrhage. It includes administration of uterotonic immediately after delivery of the baby, delaying cord clamping for at least 1-3 minutes to reduce rates of infant anaemia, performing controlled cord traction for removing the placenta and postpartum vigilance, ie, assess the uterine tone to ensure a contracted uterus; and continue to check every 15 minutes for 2 hours. If there is uterine atony, fundal massage should be performed and patient should be monitored more frequently. Though oxytocin is the best drug for routine prophylaxis, misoprostol is a relatively newer drug which is now included in the various guidelines for prevention and treatment of postpartum hemorrhage. It can be used as an effective and safe drug in areas with poor access to skilled healthcare providers and facilities

    Search for spectroscopic binaries using rotational velocities in five open clusters observed by ESO

    In this paper we detect double-lined spectroscopic binaries (SB2) in five open clusters: NGC 2243, NGC 2420, NGC 3532, NGC 6253 and NGC 6705 (M 11) using a method based on high values of the projected rotational velocity when they are fitted with single star spectral model. Observed spectra were obtained from ESO archive. The method was validated on sets of synthetic spectra for the single and binary stars. It is able to reliably select spectroscopic binaries without confusing them with single stars, if components in binary rotate slowly and radial velocity separation is sufficiently high. We found 60 SB2 candidates: two in NGC~2243, eight in NGC~2420 and NGC~3532, 17 in NGC 6253 and 25 in NGC~6705. Comparison with literature confirms 18 of them, thus we found 42 new SB2 candidates.Comment: accepted in MNRAS 07.12.202

    Using K-fold cross validation proposed models for SpikeProp learning enhancements

    Spiking Neural Network (SNN) uses individual spikes in time field to perform as well as to communicate computation in such a way as the actual neurons act. SNN was not studied earlier as it was considered too complicated and too hard to examine. Several limitations concerning the characteristics of SNN which were not researched earlier are now resolved since the introduction of SpikeProp in 2000 by Sander Bothe as a supervised SNN learning model. This paper defines the research developments of the enhancement Spikeprop learning using K-fold cross validation for datasets classification. Hence, this paper introduces acceleration factors of SpikeProp using Radius Initial Weight and Differential Evolution (DE) Initialization weights as proposed methods. In addition, training and testing using K-fold cross validation properties of the new proposed method were investigated using datasets obtained from Machine Learning Benchmark Repository as an improved Bohte's algorithm. A comparison of the performance was made between the proposed method and Backpropagation (BP) together with the Standard SpikeProp. The findings also reveal that the proposed method has better performance when compared to Standard SpikeProp as well as the BP for all datasets performed by K-fold cross validation for classification datasets

    Backpropagation Neural Network Based on Local Search Strategy and Enhanced Multi-objective Evolutionary Algorithm for Breast Cancer Diagnosis

    The role of intelligence techniques is becoming more significant in detecting and diagnosis of medical data. However, the performance of such methods is based on the algorithms or technique. In this paper, we develop an intelligent technique using multiobjective evolutionary method hybrid with a local search approach to enhance the backpropagation neural network. First, we enhance the famous multiobjective evolutionary algorithms, which is a non-dominated sorting genetic algorithm (NSGA-II). Then, we hybrid the enhanced algorithm with the local search strategy to ensures the acceleration of the convergence speed to the non-dominated front. In addition, such hybridization get the solutions achieved are well spread over it. As a result of using a local search method the quality of the Pareto optimal solutions are increased and all individuals in the population are enhanced. The key notion of the proposed algorithm was to  show a new technique to settle automaticly artificial neural network design problem. The empirical results generated by the proposed intelligent technique evaluated by applying to the breast cancer dataset and emphasize the capability of the proposed algorithm to improve the results. The network size and accuracy results of the proposed method are better than the previous methods. Therefore, the method is then capable of finding a proper number of hidden neurons and error rates of the BP algorithm

    Association between Body Mass Index and Bone Mineral Density in Patients Referred for Dual-Energy X-Ray Absorptiometry Scan in Ajman, UAE

    Body Mass Index (BMI) is a good indicator for measurements of Bone Mineral Density (BMD) which measures the density of minerals present in the bones using a special scan. This study was conducted to assess the association between BMI and status of BMD among 101 individuals who underwent Dual-Energy X-ray Absorptiometry (DEXA) scan. 39 subjects had normal and 62 had low bone mineral density. BMD was low in 82.4% of people with normal BMI, 78.1% among overweight, and 44.2% among obese. There was a statistically significant association between these two variables (P < .001). Low BMD was recorded in 59.1% of females and 76.9% of males. Association between advancing age and lower BMI is an important risk factor in the occurrence of low BMD

    Abstract Admission procalcitonin measurements and microbiology results were available for 1040 hospitalized adults with coronavirus disease 2019 (from 48 902 included in the International Severe Acute Respiratory and Emerging Infections Consortium World Health Organization Clinical Characterisation Protocol UK study). Although procalcitonin was higher in bacterial coinfection, this was neither clinically significant (median [IQR], 0.33 [0.11–1.70] ng/mL vs 0.24 [0.10–0.90] ng/mL) nor diagnostically useful (area under the receiver operating characteristic curve, 0.56 [95% confidence interval, .51–.60]).</jats:p
