Low prevalence of collateral cerebral circulation in the circle of Willis in patients with severe carotid artery stenosis and recent ischemic stroke
Introduction: The circle of Willis is thought to play a key role in development of collateral flow in patients with internal carotid artery stenosis (ICAS). Aim: To assess flow in the circle of Willis in patients with recent ischemic stroke (IS). Material and methods: The study included 371 patients, 102 symptomatic with severe ICAS and recent IS (within the last 3 months) (group I) and 269 asymptomatic with severe ICAS (group II). Flow in the middle (MCA), anterior (ACA) and posterior (PCA) cerebral arteries and pattern of the cross-flow through anterior (ACoA) and posterior (PCoA) communicating arteries were assessed with transcranial color-coded Doppler ultrasonography (TCCD). Results: The ACoA or PCoA was less prevalent in group I than in group II (54% vs. 78%, p < 0.001 and 20% vs. 42%, p < 0.001, respectively), resulting in lower peak-systolic velocity (PSV) in the MCA in group I vs. group II (p = 0.015). Any collateral pathway was present in 67% of patients in group I, compared to 86% in group II (p < 0.001). Both PSV and end-diastolic (EDV) flow velocity in the ACA were lower in patients with recent IS, compared to asymptomatic subjects (71 ±24 cm/s vs. 86 ±34 cm/s, p < 0.001 and 32 ±12 cm/s vs. 37 ±17 cm/s, p = 0.038, respectively). Presence of ACoA or PCoA and higher PSV in the MCA and ACA were associated with significant risk reduction of IS (RR = 0.28 (95% CI = 0.16–0.49, p < 0.001), RR = 0.28 (95% CI = 0.15–0.52, p < 0.001), RR = 0.97 (95% CI = 0.96–0.99, p < 0.001), RR = 0.99 (95% CI = 0.98–0.99, p < 0.032), respectively). However, ROC curves failed to show reliable MCA or ACA PSV cut-offs for IS risk assessment. Conclusions: The ACoA and PCoA seem to play a key role in the evaluation of IS risk in subjects with severe ICAS
Evaluation of effectiveness of a toothpaste containing tea tree oil and ethanolic extract of propolis on the improvement of oral health in patients using removable partial dentures
The aimof this studywas to evaluate the effect of toothpaste containing natural tea tree essential
oil (TTO) and ethanolic extract of propolis (EEP), onmicroflora and selected indicators of oral health in
patients using removable acrylic partial dentures. Fifty patients with varying conditions of hygiene were
divided into two groups. The study group received the toothpaste with TTO and EEP, while the control
group received the same toothpaste but without TTO and EEP. At the first visit, oral hygiene and hygiene
of the prostheses were carried out. Control visits took place 7 and 28 days later and compared to baseline.
Indexes like API (Approximal Plaque Index),mSBI (modified Sulcus Bleeding Index), OHI-s (simplified
Oral Hygiene Index), and DPI (Denture Plaque Index) were assessed in three subsequent stages, and
swabs were collected fromfloor of themouth area to assess themicrobiota. After 7 and 28 days of using
the toothpaste with TTO and EEP, a statistically significant decrease of the examined indicator values were
observed in the study group as compared to the values upon the initial visit. The number of isolated
strains ofmicroorganisms in the study group was decreased ormaintained at the same level, whereas in
the control group an increase in the number of isolated strains was observed. The observed stabilization of
oralmicrobiota in patients fromthe study group confirms the beneficial activity of toothpaste containing
EEP and TTO compared to the control group
Costs of the use of carbetocin in the prevention of uterine atony following delivery of the infant by Caesarean section — retrospective multicenter study
Objectives: The aim of this study was to compare the costs of using carbetocin in the prevention of uterine atony following delivery of the infant by Cesarean section (C-section) under epidural or spinal anesthesia with standard methods of prevention (SMP).
Material and methods: This retrospective multicenter study was based on data from three medical centers. A questionnaire was developed to gather patient records on consumption and costs of resources related to C-section, prevention of uterine atony and postpartum hemorrhage (PPH) treatment. Six subpopulations were considered, depending on patient characteristics. The analysis covered two perspectives: that of the hospital and of the public payer.
Results: The subpopulations were homogenous, which was a premise for pooling the data. The use of carbetocin in the prevention of uterine atony following Cesarean section generates savings for hospital in comparison with SMP (oxytocin) in 5 of 6 subpopulations. The biggest savings were observed amongst patients who experienced severe PPH and reached 2.6–6.2 thousand PLN per patient. Costs of services related to C-section borne by the hospitals were higher than the refund received from a public payer. The greatest underestimation reached 12.1 thousand PLN per patient. Nevertheless, loss generated by this underfunding was lower in carbetocin versus oxytocin group.
Conclusions: The use of carbetocin instead of SMP gives hospitals an opportunity to make savings as well as to reduce losses resulting from the underfunding of the services provided by the National Health Fund
Oral Health of Patients Treated with Acrylic Partial Dentures Using a Toothpaste Containing Bee Product
This study was carried out to investigate the influence of a propolis and tee tree oil-containing hygienic agent on selected oral health parameters, oral microflora, and the condition of periodontal health. Thirty-seven patients who underwent oral rehabilitation with a removable acrylic denture were selected and randomly assigned into two groups: study group (A) which received a newly formulated propolis and tee tree oil-containing toothpaste or a control group (C) without an active ingredient. API, S-OHI, and mSBI were assessed in three subsequent stages. During each examination swabs were employed for microbiological inoculation: in the study group after 4 weeks use of the active toothpaste showed a decrease in the number of isolated microorganisms. In the control group, after 4 weeks use of the toothpaste without active ingredients resulted in increase in the number of the isolated microorganisms. Improvements in hygiene and the condition of periodontium were observed in patients using active toothpastes. In the study group the oral flora diversity was reduced by the decrease in the number of cultured microorganism species, while in the control group an increase in the number of cultured microorganisms and their species was observed
Evolution of thematic areas of cross-border cooperation under EU programs in selected countries of Central and Eastern Europe
Współpraca transgraniczna polskich regionów z sąsiadującymi regionami w Czechach, Słowacji i Ukrainie wspierana była za pośrednictwem programów wspólnotowych już w latach 90. XX wieku. Na zdecydowanie większą skalę rozwinęła się jednak za pośrednictwem programów transgranicznych wdrażanych po akcesji Polski, Słowacji i Czech do Unii Europejskiej. Celem artykułu jest przedstawienie zmian celów i tematycznych obszarów wsparcia podejmowanych w programach współpracy transgranicznej: Republika Czeska-Polska, Republika Słowacka-Polska oraz Polska-Białoruś-Ukraina w dwóch unijnych perspektywach finansowych: 2004-2006 oraz 2014-2020. Artykuł jest próbą odpowiedzi na następujące pytania badawcze: Czy, a jeśli tak, to w jaki sposób zmieniły się cele współpracy transgranicznej? Czy istnieją różnice w zakresie tematycznych obszarów współpracy transgranicznej? Czy pomiędzy tematycznymi obszarami współpracy transgranicznej na pograniczu czesko-polskim i słowacko-polskim oraz polsko-białorusko-ukraińskim istnieją różnice? Analizie poddano dane zastane wykorzystując przede wszystkim metodę badania dokumentów programowych trzech programów współpracy transgranicznej realizowanych w dwóch okresach unijnego programowania. Badanie wykazało, iż cele współpracy transgranicznej w pierwszej perspektywie finansowej we wszystkich programach przedstawiały się w bardzo podobny sposób i generalnie dotyczyły poprawy standardu życia oraz integracji społeczno-gospodarczej sąsiadujących regionów. W ostatniej perspektywie cele te były zdecydowanie bardziej zróżnicowane i korespondowały z ówczesnymi potrzebami regionów objętych wsparciem. W przypadku pogranicza polsko-czeskiego i polsko-słowackiego wspólnym celem było inwestowanie w kształcenie, szkolenie oraz szkolenie zawodowe na rzecz zdobywania umiejętności i uczenia się przez całe życie. Natomiast w przypadku pogranicza polsko-słowackiego i polsko-białorusko-ukraińskiego wspólne okazały się następujące cele: zachowanie i ochrona środowiska naturalnego (w tym kulturowego) oraz poprawa dostępności regionów i rozwój transportu oraz sieci i systemów komunikacyjnych. Tematyczne obszary współpracy odpowiadały zdefiniowanym celom w ramach poszczególnych programów. W perspektywie 2004-2006 współpraca transgraniczna dotyczyła przede wszystkim rozwoju i modernizacji infrastruktury oraz szeroko rozumianego rozwoju społeczno-gospodarczego. W ostatniej perspektywie finansowej widoczna była konkretyzacja i specjalizacja obszarów wsparcia. W przypadku pogranicza polsko-białorusko-ukraińskiego szczególną uwagę zwrócono na zarzadzanie granicami, mobilnością i migracjami oraz bezpieczeństwem na granicach.Cross-border cooperation of Polish regions with neighboring regions in the Czech Republic, Slovakia, and Ukraine was supported through community programmes as early as the 1990s. However, it has developed on a much larger scale through cross-border programmes implemented after the accession of Poland, Slovakia, and the Czech Republic to the European Union. The article aims to present the changes in the objectives and thematic areas of support undertaken in the cross-border cooperation programmes: the Czech Republic-Poland, the Slovak Republic-Poland, and Poland-Belarus-Ukraine in two EU financial perspectives: 2004-2006 and 2014-2020. The article is an attempt to answer the following research questions: Have the goals of cross-border cooperation changed and if so, how? Are there differences in the thematic areas of cross-border cooperation? Are there any differences between the thematic areas of cross-border cooperation on the Czech-Polish, Slovak-Polish, and Polish-Belarusian-Ukrainian borderlands? The existing data was analyzed, using mainly the method of examining programme documents of three cross-border cooperation programmes implemented in two periods of EU programming. The study showed that the goals of cross-border cooperation in the first financial perspective in all the analyzed programmes were very similar and generally concerned with the improvement of the standard of living and the socio-economic integration of the neighboring regions. In the last perspective, these objectives were much more diversified and corresponded to the needs of the supported regions at that time. In the case of the Polish-Czech and Polish-Slovak borderlands, the common goal was to invest in education, training, and vocational training for skills and lifelong learning. On the other hand, in the case of the Polish-Slovak and Polish-Belarusian-Ukrainian borderlands, the following objectives turned out to be common: preservation and protection of the natural environment (including the cultural one) as well as improvement of the accessibility of regions and development of transport as well as communication networks and systems. Thematic areas of cooperation corresponded to the defined goals within individual programmes. In the 2004-2006 perspective, the cross-border cooperation concerned mainly the development and modernization of infrastructure and the broadly understood socio-economic development. In the last financial perspective, the specification and specialization of the areas of support were visible. In the case of the Polish-Belarusian-Ukrainian borderland, special attention was paid to border management, mobility and migration, and border security
Non-governmental organizations – as potential employers. Spatial diversity of the non-governmental labor market
W Polsce działa 75 tys. organizacji pozarządowych, z tego 4,6 tys. (6,2%) w województwie podkarpackim. Sektor pozarządowy należy więc również postrzegać jako część rynku pracy. Organizacje pozarządowe są miejscem, gdzie wiele osób spełnia się nie tylko w roli działaczy społecznych, ale także zawodowo, kiedy praca w tych organizacjach stanowi ich główne źródło utrzymania. Wysoko wykwalifikowana kadra sprzyja ich efektywności, umożliwiając dostosowa-nie się do zmiennych warunków funkcjonowania trzeciego sektora.
Sektor pozarządowy w Polsce, jak i w województwie podkarpackim ma jednak w dalszym ciągu marginalny udział w kreowaniu rynku pracy. Jedynie 14% organizacji pozarządowych za-trudnia pracowników (w woj. podkarpackim 10% organizacji). W 2010 r. zatrudnienie w sektorze pozarządowym znalazło 85,4 tys. osób (8 tys. w woj. podkarpackim). W trzecim sektorze stwo-rzono blisko 72 tys. etatów (3 tys. w woj. podkarpackim), podczas gdy pod koniec minionej deka-dy zatrudnienie w sektorze pozarządowym w państwach UE wynosiło 9 mln osób (Niemcy – ponad 1,8 mln, Wielka Brytania – ponad 1,6 mln, Francja – 1,2 mln).In Poland there are 75 thousand active non-governmental organizations, of which 4.6 thou-sand operate within the Podkarpackie province (that represents 6.2% of the total). Therefore, the non-governmental sector must also be seen as a part of the labor market. Non-governmental organ-izations are a right place both for social activists and those to whom work in such organizations is a major source of income. Highly qualified staff makes it possible to adapt to the changing condi-tions of the sector functioning.
The non-governmental sector in Poland as well as in the Subcarpathian voivodeship, however, performs a marginal part in the creation of the labor market. Merely 14% of the non-governmental organizations employ workers (in Podkarpackie 10%). In 2010, 85.4 thousand people were employed in the non-governmental sector (8 thousand in Podkarpackie). Thus, the third sector created almost 72 thousand working places (3 thousand in Podkarpackie), while at the end of the decade, the employment in the non-governmental sector in the EU countries amounted to 9 million people (Germany – over 1.8 million, United Kingdom – over 1.6 million, France – 1.2 million)
Priority effect and coexistence of species in urban environment: Apodemus’ model
Środowisko miejskie w obecnej formie powstało stosunkowo niedawno i różni się pod wieloma względami od terenów pozamiejskich. Miasta cechują się przekształconymi
warunkami takimi jak m.in. hałas, zanieczyszczenia gleby, wody i powietrza, łagodniejszy
klimat, silna fragmentacja terenów zielonych oraz stała, często intensywna obecność ludzi.
Odmienność warunków prowadzi do wielu zmian w populacjach miejskich takich jak zmiany
fizjologiczne, behawioralne, ekologiczne i genetyczne umożliwiające przystosowanie się
i życie w silnie przekształconym środowisku miejskim, co stwierdzono u różnych grup
zwierząt. Wielu autorów wskazuje, że zmiany adaptacyjne zachodzące w populacjach
gatunków synurbijnych na drodze mikroewolucji, będą tym wyraźniejsze i trwalsze im
dłuższy czas upłynął od zasiedlenia miasta przez dany gatunek.
Efekt pierwszeństwa jest koncepcją ekologiczną związaną z teorią wielu stanów
stabilnych. Zachodzi wtedy, gdy poszczególny gatunek może wywierać wpływ na
kształtowanie struktury zgrupowań wielogatunkowych ze względu na wcześniejsze
zasiedlenie danego obszaru. Charakter tego oddziaływania może być udogadniający bądź też
hamujący, zależnie od tego czy gatunek, który osiedlił się jako pierwszy ułatwia czy też
utrudnia osiedlenie się innym gatunkom. Zgodnie z efektem pierwszeństwa gatunek, który
osiedla się jako pierwszy może zostać dominantem w zgrupowaniu, jednak kluczowy jest
czas, który upłynął od zasiedlenia – musi być wystarczający do wytworzenia stabilnej
populacji i dostosowanie się do lokalnych warunków środowiska. Na siłę efektu
pierwszeństwa wpływają takie czynniki jak: czas zasiedlenia, konkurencyjność gatunku,
kolejność zasiedlenia, produktywność i wielkość płatu, a także specyfika warunków
środowiska, obecność drapieżników oraz liczba gatunków wchodząca w skład zgrupowania.
Moje badania poświęcone były dwóm gatunkom gryzoni z rodzaju Apodemus pospolicie
występującym na terenie Polski, różniącym się czasem procesu synurbizacji w Warszawie.
Myszarka polna (Apodemus agrarius Pallas, 1771) po raz pierwszy została stwierdzona na
terenach zielonych w Warszawie pod koniec XIX w., przy czym już na początku XX w.
uznano ją za gatunek powszechnie występujący w mieście, zaś w drugiej połowie XX wieku
stała się dominującym gatunkiem gryzonia na terenie parków, a obecnie można odnaleźć
wiele lokalnych populacji tego gatunku na terenie miasta. Myszarka leśna (Apodemus
flavicollis Melchior, 1834) po raz pierwszy została stwierdzona na terenach zielonych
w centrum Warszawy kilkanaście lat temu, a wcześniej gatunek ten był spotykany jedynie na
dalekich obrzeżach miasta. W środowisku pozamiejskim oba gatunki bardzo często
współwystępują, jednak ze względu na wielkość ciała oraz agresywność to myszarka leśna
jest gatunkiem dominującym, często prowadząc do ograniczenia liczebności myszarki polnej,
czego nie obserwuje się w populacjach tego gatunku wolnych od konkurenta. Obecnie na
niektórych terenach zielonych zlokalizowanych w centrum miasta można spotkać osobniki
obu badanych gatunków, gdzie wystawione są na działanie tych samych czynników, takich
jak m.in. hałas, zanieczyszczenia, czy stała obecność ludzi.
Silna specyfika środowiska miejskiego, różnice w sile konkurencyjnej obu gatunków oraz
znacząca różnica w czasie zasiedlenia terenów zurbanizowanych stwarzają doskonałą okazję
do sprawdzenia wystąpienia efektu pierwszeństwa, który jak dotąd nie był badany na ssakach.
Celem pracy było zbadanie, czy trwające już ponad 100 lat procesy adaptacji myszarki
polnej do silnie specyficznego środowiska miejskiego pozwolą temu potencjalnie słabszemu
konkurencyjnie gatunkowi sprostać silniejszemu, ale nieprzystosowanemu do życia w mieście
konkurentowi – myszarce leśnej.
Testowałam hipotezę, że różny czas trwania procesu synurbizacji dwóch badanych
gatunków znajdzie odzwierciedlenie w relacjach między nimi. Ze względu na dłuższy czas
trwania procesu synurbizacji, adaptację do warunków środowiska miejskiego oraz
wystąpienie efektu pierwszeństwa to myszarka polna będzie w uprzywilejowanej pozycji.
Założyłam, że wystąpienie efektu pierwszeństwa znajdzie odzwierciedlenie w rozmaitych
aspektach biologii i ekologii badanych gatunków dotyczących m.in.: reakcji na czynnik
stresowy, jakim w środowisku miejskim jest hałas, dynamice liczebności, strukturze populacji
i przebiegu procesów rozrodu, realizacji preferencji mikrosiedliskowych, wzorcu aktywności
okołodobowej oraz bezpośrednich interakcjach międzygatunkowych.
Aby zrealizować cel pracy badania prowadziłam z wykorzystaniem różnych metod takich
jak: test behawioralny w otwartym polu, oznaczenie poziomu kortykosteronu w odchodach
metodą immunoenzymatyczną, połowy w pułapki żywołowne metodą CMR, charakterystyka
mikrosiedlisk oraz analiza filmów uzyskanych z fotopułapek.
Uzyskane wyniki pozwoliły stwierdzić, że doszło do odwrócenia relacji konkurencyjnych
zachodzących pomiędzy myszarką polną i myszarką leśną w środowisku miejskim, ze
względu na różnice w czasie trwania procesu synurbizacji obu gatunków
i wystąpienie efektu pierwszeństwa. Znaczne różnice w czasie synurbizacji obu badanych
gatunków oraz ich przystosowaniu do funkcjonowania w warunkach miejskich silnie
zaznaczyły się w radzeniu sobie ze stresem, co wpłynęło na kondycję pojedynczych
osobników, ich rozrodczość, a także przeżywalność, i finalnie znalazło odzwierciedlenie
w funkcjonowaniu populacji tych gatunków na terenie miasta. W mieście, odmiennie niż poza
miastem, to populacja myszarki polnej była w lepszym stanie niż populacja myszarki leśnej,
co przejawiało się w wyższej liczebności, niższym tempie wymiany osobników i wyższym
udziale samic ciężarnych i karmiących. Z kolei populacja myszarki leśnej w mieście
wykazywała szereg zaburzeń polegających na zmianie wzorca przebiegu reakcji na stres,
zmianie wybiórczości mikrosiedliskowej, zmianie wzorca aktywności okołodobowej, a także
zmianie wzorca przebiegu dynamiki liczebności w porównaniu do wzorców
charakterystycznych dla tego gatunku w warunkach pozamiejskich. Kluczowe było również
odwrócenie kierunku bezpośrednich interakcji zachodzących między badanymi gatunkami.
Otrzymane wyniki wskazują, że specyficzne warunki środowiska miejskiego odgrywają
znaczącą rolę w kształtowaniu populacji miejskich o cechach odmiennych niż populacje środowisk naturalnych, jednak kluczowym czynnikiem decydującym o kształtowaniu
zgrupowań może być czas trwania procesu synurbizacji i wynikający z niego efekt
pierwszeństwa, mogący prowadzić do zmiany układów konkurencyjnych w miejskich
zgrupowaniach zwierząt.Urban habitat in its present form was created relatively recently and differs from natural,
non-urban habitats in numerous ways. Cities are characterized by transformed conditions:
high levels of noise, pollution of the soil, water and air, a milder climate, strong fragmentation
of green areas and permanent, often intense, human presence. Given the specific conditions of
urban habitats, the animal populations living in them generally develop different
characteristics than those found in non-urban habitats like physiological, behavioral,
ecological and genetic changes which enable adaptation and live in strongly specific urban
environment as confirmed in many animal groups. Many authors indicate that adaptive
changes in synurbic populations arising through microevolution become more marked and
enduring the longer the species has been synurbized.
Priority effect is an ecological concept connected with the many stable states theory.
Priority effect occurs when particular species can influence shaping community development
due to prior arrival at a site. The type of this impact can be inhibitory or facilitative,
depending on whether the species that arrived first at a site inhibits or facilitates other species
from settling. Due to priority effect the species which arrive first can become dominant in
community but the main factor is time of arrival, which should be sufficient to create a stable
population and adapt to specific local environmental conditions.
The power of priority effect depends on such factors as time of arrival, species
competitions, sequence of arrival, productivity, size and specific of microenvironment,
presence of predators and numbers of species creating the community.
I investigated two Apodemus species common in Poland, differing in time of Warsaw city
inhabitation. The striped field mouse (Apodemus agrarius Pallas, 1771) was first noted in
green areas in Warsaw in the late nineteenth century, by the early twentieth century it was
described as often found in the city, in the late twentieth was a dominant rodent species in the
parks and now there are many local populations of this species in Warsaw. The yellow-necked
mouse (Apodemus flavicollis Melchior, 1834) was first noted in Warsaw’s green areas several
years ago, prior to which the species was only found in distant suburbs. In non-urban areas
both species coexist very often but because of body size and temperament the yellow-necked
mouse is the dominant species, that often inhibits numbers of the striped field mouse. This
effect was not observed in areas free from competitor. In fact, in some green areas in Warsaw
both of these species occur together and are exposed to the same factors like noise, pollutions
and human presence.
The strong characteristics of urban environment, differences in competition abilities of
both species and very significant differentiation in synurbization process give a perfect
opportunity to check if the priority effect occurs. It is important to mention that priority effect
had not been investigated on mammals until now.
The aim of the study was to investigate if the adaptation process of the striped field mouse
to specific urban environment, that lasted over a hundred years, allows this weaker competitor
to deal with the yellow-necked mouse, the stronger but non-adapted competitor.
The hypothesis was tested that the difference in time of synurbization process of both
species occurs in relations between those species. I assumed that the striped field mouse is in
better position because of longer time of lasting synurbization process, adaptation to urban
environmental conditions and occurrence of priority effect. The latter effect is visible in
different aspects of biology and ecology of examined species connected especially with
reaction to the stress factor, population dynamics and structure, reproduction process,
microhabitat preferences, pattern of circadian activity and direct interspecific interactions.
In order to achieve the aim of the study, I conducted my research using different methods:
open field test, enzyme immunoassay test to measure fecal corticosterone level, CMR method,
microhabitat characteristics, analysis of videos from photo traps.
The obtained results allowed to conclude that there was a reversal of competitive
interactions between the striped field mouse and the yellow-necked mouse in the urban
environment, due to differences in the duration of the synurbization process of both species
and the occurrence of priority effect.
Significant differences in the duration of the synurbization process of both studied species
and their adaptation to functioning in urban conditions were strongly reflected in how those
species deal with stress, which affected the condition of individuals, their reproduction and
survival, and was reflected in the functioning of urban populations of these species.
In urban area, unlike outside the city, the striped field mouse population was in a better
condition than the yellow-necked mouse, which was reflected in a higher number, lower rate
of exchange of individuals and a higher percentage of pregnant and lactating females.
The urban population of the yellow-necked mouse showed a number of disorders
consisting of a change in the pattern of reaction to stress, change in microhabitat preferences,
change in the circadian activity pattern, and change in the population’s dynamics in
comparison with patterns characteristic for this species in non-urban areas. The reversal of the
direction of direct interactions between the studied species was also crucial.
My findings support the overall conclusion that the conditions present in urban habitats
act as a strong inducement causing populations to develop characteristics different from those
found in natural habitats. Crucially, this may be further enhanced by priority effect, resulting
