Fatores de risco para doenças crônica degenerativas não transmissíveis em pacientes com esquistossomose crônica
A esquistossomose é uma doença parasitária presente em regiões de climas tropicais e
subtropicais. Aproximadamente 200 milhões de pessoas ao redor do mundo estão
infectadas pela doença. A fase crônica é caracterizada por comprometimento de órgãos
e tecidos principalmente fígado e baço. A forma hepatoesplênica (HS) é o estágio mais
avançado, no qual se observam maiores danos no parênquima hepático, hipertensão
portal e congestão hepática. Em muitos pacientes na forma HS se faz necessária cirurgia
de esplenectomia (pacientes HSS). Na forma HS, ocorrem grandes alterações no
metabolismo de lipídios. Alterações lipídicas são consideradas importantes fatores de
risco para Doenças Crônica Degenerativas Não Transmissíveis (DCDNT) tais como
diabetes mellitus, resistência insulínica (RI) e doenças cardiovasculares (DCV). Além
disso, fatores genéticos também podem influenciar no padrão e grau de alteração
lipídicas. O polimorfismo do gene da Apolipoproteína E (Apo E) tem sido reportado
como um importante fator contributivo para alterações lipídicas. O objetivo deste estudo
foi investigar em pacientes esquistossomóticos a ocorrência de fatores de risco para
DCDNT tais como RI e dislipidemias bem como a influência do polimorfismo do gene
da Apo E nas alterações lipídicas. Para tanto foram obtidas amostras de plasma
sangüíneo de indivíduos HS e HSS bem como de indivíduos saudáveis. Foram
determinadas as concentrações plasmáticas de insulina e glicose de jejum, triglicerídios,
HDL-c, VLDL-c, LDL-c, colesterol total, LDL-oxidada, Albumina, transaminase
glutâmico oxalacética (TGO), transaminase glutâmico pirúvica (TGP), fosfatase alcalina
e gama glutaril transferase (GGT). Além de terem sido determinados os valores de
HOMA-IR e da razão triglicerídios por HDL-c para acessar a resistência e os índices de
Castelli para acessar o risco de desenvolver DCV, além de extrair DNA leucocitário
para determinar o polimorfismo do gene da Apo E. Os indivíduos HS e HSS
apresentaram níveis mais elevados de LDL-oxidada, das enzimas hepáticas, bem como
da glicemia e insulina de jejum e HOMA-IR, além de níveis mais baixos de Colesterol,
triglicerídios e albumina, quando comparado ao grupo controle. Não houve diferença
significativa da razão triglicerídios/HDL-c, bem como dos índices de Castelli entre os
grupos. O polimorfismo do gene da Apo E também influenciou na maneira pela qual a
esquistossomose modula o metabolismo de lipídios. Desta maneira, os resultados
demonstraram que os indivíduos portadores da esquistossomose crônica apresentaram
alterações no metabolismo lipídico, elevadas concentrações de LDL-oxidada e um
quadro de resistência insulínica. Estes, por sua vez, representam importantes fatores de
risco para o desenvolvimento de DCDNTs
Impacto da esquistossomose mansônica no perfil lipídico e nas concentrações plasmáticas das apolipoproteínas A-I e B
A esquistossomose é uma doença parasitária intravascular presente em regiões de climas tropical e subtropical que acomete, aproximadamente, 200 milhões de pessoas no mundo. Estudos utilizando animais infectados têm demonstrado alterações no metabolismo de lipídios. Entretanto, estudos têm reportado diferenças entre as alterações observadas nos animais e humanos. Desta forma, o presente trabalho teve como objetivo avaliar alterações no perfil lipídico e das apolipoproteínas A-I e B em pacientes portadores da esquistossomose crônica. Amostras de sangue foram coletadas em 136 indivíduos infectados, encaminhados pelo serviço de ultrassonografia do setor gastroenterologia do Hospital das Clínicas da Universidade Federal de Pernambuco e em 90 indivíduos saudáveis. Os parâmetros lipídicos foram obtidos por métodos enzimáticos colorimétricos, exceto a VLDL-c e o LDL-c que foram calculados pela equação de Friedwald. Os níveis plasmáticos de Apo A-I e Apo B foram determinados por imunoturbidimetria. Os resultados mostraram que os indivíduos infectados apresentaram níveis diminuídos de Colesterol total, LDL-c, HDL-c e Apo A-I, e níveis elevados de Triglicerídeos, VLDL-c e Apo B, quando comparados com o grupo controle. A relação Apo B/Apo A-I também foi significativamente (p<0,0001) mais elevada nos indivíduos infectados em comparação ao grupo controle. Os resultados também sugerem que a esquistossomose mansônica crônica influi diretamente para elevação da relação Apo B/Apo A-I (O. R. 5,3/p=0,0477). Desta maneira, observou-se que a esquistossomose mansônica crônica provoca alterações no metabolismo lipídico e das apolipoproteínas A-I e B semelhantes aquelas apresentadas por indivíduos portadores de doenças crônicas degenerativas não transmissíveis como, por exemplo, a diabetes mellitus, obesidade e ateroscleros
Human plasma lipid modulation in schistosomiasis mansoni depends on apolipoprotein E polymorphism.
Schistosomiasis mansoni is a parasitic liver disease, which causes several metabolic disturbances. Here, we evaluate the influence of Apolipoprotein E (APOE) gene polymorphism, a known modulator of lipid metabolism, on plasma lipid levels in patients with hepatosplenic schistosomiasis.Blood samples were used for APOE genotyping and to measure total cholesterol (TC), LDL-C, HDL-C and triglycerides. Schistosomiasis patients had reduced TC, LDL-C and triglycerides (25%, 38% and 32% lower, respectively; Pε3>ε4) was absent in patients (ε2 or ε4>ε3), and the increase in HDL-C of ε2 or ε4 patients compared to ε3 patients was not seen in the control groups.We confirm that human schistosomiasis causes dyslipidemia and report for the first time that certain changes in plasma lipid and lipoprotein levels depend on APOE gene polymorphism. Importantly, we also concluded that S. mansoni disrupts the expected regulation of plasma lipids by the different ApoE isoforms. This finding suggests ways to identify new metabolic pathways affected by schistosomiasis and also potential molecular targets to treat associated morbidities
Relationship between splenomegaly and hematologic findings in patients with hepatosplenic schistosomiasis
BACKGROUND Schistosomiasis is a tropical disease. Patients who develop hepatosplenic schistosomiasis have clinical findings including periportal fibrosis, portal hypertension, cytopenia, splenomegaly and gastrointestinal hemorrhage. OBJECTIVE The aim of this study was to analyze the hemostatic and hematologic findings of patients with schistosomiasis and correlate these to the size of the spleen. METHODS Fifty-five adults with hepatosplenic schistosomiasis and 30 healthy subjects were selected through a history of contact with contaminated water, physical examination and ultrasound characteristics such as periportal fibrosis and splenomegaly in the Gastroenterology Service of the Universidade Federal de Pernambuco. Blood samples were collected to determine liver function, blood counts, prothrombin (international normalized ratio), partial thromboplastin time and fibrinogen and D-Dimer levels using the Pentra 120 hematological analyzer (HORIBA/ABX), Density Plus (test photo-optical Trinity Biotech, Ireland) and COBAS analyzer 6000 (Roche). Furthermore, the longitudinal size of the spleen was measured by ultrasound (Acuson X analyzer 150, Siemens). The Student t-test, the Fisher test and Pearson's correlation were used to analyze the results with statistical significance being set for a p-value < 0.05. RESULTS The mean age was higher for the Study Group than for the Control Group (54 ± 13.9 vs. 38 ± 12.7 years). The average longitudinal diameter of the spleen was 16.9 cm (Range: 12.3-26.3 cm). Anemia is a common finding in patients with schistosomiasis (36.3%). The mean platelet and leukocyte counts of patients were lower than for the Control Group (p-value < 0.001). Moreover, the international normalized ratio (1.42 vs. 1.04), partial thromboplastin time (37.9 vs. 30.5 seconds) and D-Dimer concentration (393 vs. 86.5 ng/mL) were higher for the Study Group compared to the Control Group. CONCLUSION This study suggests that hematological and hemostatic abnormalities are associated with splenomegaly, hypersplenism and portal hypertension

This form is associated with portal hypertension and splenomegaly, and often episodes of gastrointestinal bleeding, even with liver function preserved. Splenectomy is a validated procedure to reduce portal hypertension following digestive bleeding. Here, we evaluate beneficial effects of splenectomy on blood coagulation factors and liver function tests in hepatosplenic schistosomiasis mansoni compared to non-operated patients
