    Isolation of bacteria that are able to digest keratin as an alternative to harmful chemicals used in tanning industries

    Abstract:Macroccocus is a bacteria that was isolated from a soil sample containing decaying hair and It was found to contain keratinase enzyme due to its high ability to digest keratin protein. The isolated bacteria by this study can be used as an alternative to harmful chemicals tanning processes represents a safe and, clean technology that achieves economic feasibility in the process of skin soaking and hair removal. Background: Leather tanning is considered one of the ancient professions that Hebron has been known for hundreds of years which is the only source of raw materials for Palestinian factories thatdepends on this commodity. In the past, the industry relied on environmentally friendly natural materials in tanning, but today they have become a source of pollution of soil and groundwater, especially after the entry of chemicals in this craft, such as chromium and some acids that infect humans and destroy the Palestinian environment because of the toxic gases that result from the process of soaking and hair removal. Objectives: 1. To Isolate keratinolytic bacteria from soil sample 2. To test the bacterial effectiveness for hair degradation 3. To identify and characterize of bacteria by sequencing tool. Methods: Soil sample containing decaying hair was collected from the industrial area in Hebron city and processed in the lab, bacteria were isolated by using spreading and streaking plate technique, their ability for hair degradation was tested by incubating bacterial colony, hair and water for several days, after the bacterial ability for hair degradation appeared bacterial isolate was identified by sequencing tool and the type of bacteria was determined by bioinformatics tool. Results: Isolated a bacterial strain that is able to digest keratin and was tested on several samples and proved to be effective in degrading hair. It has been welcomed by the Modern Leather Tanning Company (MLTC-Hebron), which has cooperated with us in this project. The strain was genotyped and appeared to belong to macroccocus, this is the first report to find an industrial value for such bacteria which is also safe for human and the environment. Conclusions: Among microbial isolate screened for keratin digestion by using soil sample, a newly isolated Macroccocusequipercicuswas found with a greater ability for hair degradation, the use of such bacteria as an alternative to harmful chemicals tanning processes represents a safe and, clean technology that achieves economic feasibility in the process of skin soaking and hair removal. but in the same time, the full sequence of these strains of bacteria is unknown and not significantly studied. Therefore, additional researches have to be done for characterization of this type of bacteri

    Mortality from gastrointestinal congenital anomalies at 264 hospitals in 74 low-income, middle-income, and high-income countries: a multicentre, international, prospective cohort study

    Background: Congenital anomalies are the fifth leading cause of mortality in children younger than 5 years globally. Many gastrointestinal congenital anomalies are fatal without timely access to neonatal surgical care, but few studies have been done on these conditions in low-income and middle-income countries (LMICs). We compared outcomes of the seven most common gastrointestinal congenital anomalies in low-income, middle-income, and high-income countries globally, and identified factors associated with mortality. // Methods: We did a multicentre, international prospective cohort study of patients younger than 16 years, presenting to hospital for the first time with oesophageal atresia, congenital diaphragmatic hernia, intestinal atresia, gastroschisis, exomphalos, anorectal malformation, and Hirschsprung's disease. Recruitment was of consecutive patients for a minimum of 1 month between October, 2018, and April, 2019. We collected data on patient demographics, clinical status, interventions, and outcomes using the REDCap platform. Patients were followed up for 30 days after primary intervention, or 30 days after admission if they did not receive an intervention. The primary outcome was all-cause, in-hospital mortality for all conditions combined and each condition individually, stratified by country income status. We did a complete case analysis. // Findings: We included 3849 patients with 3975 study conditions (560 with oesophageal atresia, 448 with congenital diaphragmatic hernia, 681 with intestinal atresia, 453 with gastroschisis, 325 with exomphalos, 991 with anorectal malformation, and 517 with Hirschsprung's disease) from 264 hospitals (89 in high-income countries, 166 in middle-income countries, and nine in low-income countries) in 74 countries. Of the 3849 patients, 2231 (58·0%) were male. Median gestational age at birth was 38 weeks (IQR 36–39) and median bodyweight at presentation was 2·8 kg (2·3–3·3). Mortality among all patients was 37 (39·8%) of 93 in low-income countries, 583 (20·4%) of 2860 in middle-income countries, and 50 (5·6%) of 896 in high-income countries (p<0·0001 between all country income groups). Gastroschisis had the greatest difference in mortality between country income strata (nine [90·0%] of ten in low-income countries, 97 [31·9%] of 304 in middle-income countries, and two [1·4%] of 139 in high-income countries; p≤0·0001 between all country income groups). Factors significantly associated with higher mortality for all patients combined included country income status (low-income vs high-income countries, risk ratio 2·78 [95% CI 1·88–4·11], p<0·0001; middle-income vs high-income countries, 2·11 [1·59–2·79], p<0·0001), sepsis at presentation (1·20 [1·04–1·40], p=0·016), higher American Society of Anesthesiologists (ASA) score at primary intervention (ASA 4–5 vs ASA 1–2, 1·82 [1·40–2·35], p<0·0001; ASA 3 vs ASA 1–2, 1·58, [1·30–1·92], p<0·0001]), surgical safety checklist not used (1·39 [1·02–1·90], p=0·035), and ventilation or parenteral nutrition unavailable when needed (ventilation 1·96, [1·41–2·71], p=0·0001; parenteral nutrition 1·35, [1·05–1·74], p=0·018). Administration of parenteral nutrition (0·61, [0·47–0·79], p=0·0002) and use of a peripherally inserted central catheter (0·65 [0·50–0·86], p=0·0024) or percutaneous central line (0·69 [0·48–1·00], p=0·049) were associated with lower mortality. // Interpretation: Unacceptable differences in mortality exist for gastrointestinal congenital anomalies between low-income, middle-income, and high-income countries. Improving access to quality neonatal surgical care in LMICs will be vital to achieve Sustainable Development Goal 3.2 of ending preventable deaths in neonates and children younger than 5 years by 2030

    Burnout among surgeons before and during the SARS-CoV-2 pandemic: an international survey

    Background: SARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice. Methods: This is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure. Results: Nine hundred fifty-four surgeons completed the survey. The median length of practice was 10&nbsp;years; 78.2% included were male with a median age of 37&nbsp;years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout. There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management. Conclusions: There was a significant burnout among trainees. Almost all aspects of clinical and research activities were affected with a significant reduction in the volume of research, outpatient clinic visits, surgical procedures, on-call hours, and emergency cases hindering the training.     Parental influences on ethnic identity development: Implications of parenting style, attachment, and ethnic socialization

    The development of ethnic identity (EI) during emerging adulthood was examined in relation to retrospective perceptions of earlier life experiences, with specific regard to parenting style (PS), security of attachment, and familial ethnic socialization (FES). The overall model consisted of determining whether knowledge of all three parenting variables (PS, attachment, and FES) would offer increased power in predicting outcomes with respect to EI exploration and commitment. The ethnically diverse sample consisted of 268 undergraduate and graduate students and recent alumni aged 18–30 years (M = 22.11, SD = 3.41). Measures included a demographics questionnaire, the Multigroup Ethnic Identity Measure-Revised (MEIM-R; Phinney & Ong, 2007), Familial Ethnic Socialization Measure (FESM; Umaña-Taylor, 2001), Parental Authority Questionnaire (PAQ, Buri, 1991) and Parental Attachment Questionnaire (PAttQ; Kenny, 1990). Regression analyses were conducted to determine the strength of the study variables, both individually and collectively, in predicting outcomes with respect to EI exploration and commitment. Results showed that perceived FES is the strongest predictor of EI exploration and commitment, suggesting that families who emphasize the importance of their ethnic heritage raise children who are more knowledgeable about and feel more strongly connected to their ethnicity, thereby strengthening their EI. The overall model accounted for the highest degree of variance in EI when compared to the predictive power of each variable separately, suggesting that early family experiences have significant implications for EI development in emerging adulthood. In addition, one-way analyses of variance (ANOVAs) revealed significant differences both across ethnic groups and across immigration generational status, suggesting that specific cultural characteristics, as well as the immigrant experience, are significant contributing factors in both parenting and EI development. Implications for clinical and school psychology practice are discussed

    Cranial Strain Patterns Associated With Concussions.

    CONTEXT: Concussions can cause cognitive impairment, somatic symptoms, and behavioral changes. Symptoms may vary in severity, depending on the degree of traumatic force. Due to the biomechanical nature of this trauma, cranial somatic dysfunctions may commonly be seen in patients with concussion. OBJECTIVE: To determine whether patients were more likely to have nonphysiologic cranial somatic dysfunctions than physiologic cranial somatic dysfunctions after sustaining a concussion. METHODS: College athletes who had a concussion based on the Immediate Post-Concussion Assessment and Cognitive test were evaluated by a physician within 1 week of the injury. Patients were evaluated for somatic dysfunctions of the cranium. Cranial somatic dysfunctions were documented; test scores and force vectors were compared with the type of strain pattern using SPSS, with P RESULTS: Sixteen patients were included in the study: 10 with nonphysiologic cranial strain somatic dysfunctions and 6 with physiologic dysfunctions. Compared with lateral forces, forces of impact with anterioposterior vectors were associated 1.5 times more often with nonphysiologic rather than physiologic cranial somatic dysfunctions (P=.697). An analysis of specific cranial strain patterns and impact force vectors showed no statistical significance (P=.096). CONCLUSION: There was no statistically significant association showing that concussion patients were more likely to have nonphysiologic cranial somatic dysfunctions compared with physiological cranial somatic dysfunctions. However, nonphysiologic cranial somatic dysfunctions did show a trend toward association with concussion. Further studies are needed to better understand the potential association between concussion and cranial somatic dysfunctions
