The chronic otitis media with cholesteatoma (COMC) may evoluate to intracranial and extra cranial complications, including the labyrithine fístulae. In this study, we present the evolution of our patients with labyrinthine fístulae. STUDY DESIGN: Clinical prospective. MATERIAL AND METHOD: Ten out 82 patients with COMC had labyrinthine fístulae and underwent surgery from January/2001 to April/2002. They were assessed by clinical exam, computed tomography scans, and pre and postoperative audiogram. RESULTS: Hearing loss, otorrhea, tinnitus and dizziness were present in 100%, 90%,80%, and 40% of the cases. In one patient the fístulae was seen only in the coronal CT-scan, in another patient the fístulae was not seen neither in coronal nor axial images. Among the patients who had tinnitus, 66% referred improvement of this complaint after surgery. DISCUSSION: in the cases without invasion of the perilymphatic space, we noticed a tendency of improvement of the postoperative audiogram pattern and clinical outcome. In the extensive fístulae, on the other hand, there were no clinical changes. CONCLUSION: The CT-scan remains the best exam to assess the COMC with 90% of sensitivity for labyrinthine fístulae. In the stage II we had a good postoperative outcome.

Activator Protein-1 Transcriptional Activity Drives Soluble Micrograft-Mediated Cell Migration and Promotes the Matrix Remodeling Machinery

Impaired wound healing and tissue regeneration have severe consequences on the patient's quality of life. Micrograft therapies are emerging as promising and affordable alternatives to improve skin regeneration by enhancing the endogenous wound repair processes. However, the molecular mechanisms underpinning the beneficial effects of the micrograft treatments remain largely unknown. In this study, we identified the active protein-1 (AP-1) member Fos-related antigen-1 (Fra-1) to play a central role in the extracellular signal-regulated kinase- (ERK-) mediated enhanced cell migratory capacity of soluble micrograft-treated mouse adult fibroblasts and in the human keratinocyte cell model. Accordingly, we show that increased micrograft-dependent in vitro cell migration and matrix metalloprotease activity is abolished upon inhibition of AP-1. Furthermore, soluble micrograft treatment leads to increased expression and posttranslational phosphorylation of Fra-1 and c-Jun, resulting in the upregulation of wound healing-associated genes mainly involved in the regulation of cell migration. Collectively, our work provides insights into the molecular mechanisms behind the cell-free micrograft treatment, which might contribute to future advances in wound repair therapies

The objective of the study was to evaluate, by system simulations, the cost-benefit of FTAI use in three different scenarios in the Pantanal of Mato Grosso do Sul: Conventional Ranch (P1), Conventional Ranch with FTAI (P2), and Ranch with cultivated pasture and the use of FTAI (P3), all based on modal farms in the Pantanal of Mato Grosso do Sul. The study simulated the impact of increased productivity and herd quality in the economic turnover of extensive breeding farms in the region. A deterministic model was developed, by using spreadsheets to perform the costs and the turnover calculations. The economic margins were estimated by the difference between the costs and turnovers of the production systems studied. The variables number of male and female calves weaned and increased number of cows in production showed the highest impact in the compensation. The use of FTAI on farms with natural or cultivated pastures increased the pregnancy rate at the end of the breeding season (EM), leading to an increased number of calves available for sale and consequently increased turnover and decreased marginal cost of the calf produced on extensive breeding farms in the Pantanal of Mato Grosso do Sul.Na publicação: Juliana Corrêa Borges

Abstract: In this study, an index natural potential for livestock ranching (NPLR) at ranch level in the Pantanal wetland was obtained using landscape indicators combined with fuzzy inference process. Four landscape indicators related with livestock production were selected by an expertise group. The application of this approach was illustrated through a pilot ranch located in the Nhecolândia sub-region, Pantanal, MS. Remote Sensing (RS) and Geographic Information System (GIS) technologies were used to map the vegetation types and aquatic habitats (landscapes types) in this ranch. Landscape type?s composition metrics were obtained using the ArcGis 9 and were used to estimate the four indicators selected by expertise: forest cover proportion (FC); landscape productive value (LPV); diversity of aquatic habitats (DAH) and flooding degree (FD). Fuzzy inference process involving definitions of membership functions, fuzzy set operations and inference rule were run and validated with the participation of core stakeholders. Different scenarios also were simulated in batch and validated with the participation of stakeholders. Both procedures were performed by Webfuzzy software. The NPLR index value found in the pilot ranch was as expected by both expertise and stakeholders. Fuzzy logic combined with landscape metric seems to be suitable for the definition of the natural potential of ranches to produce livestock in the Pantanal.Geopantanal 2012
