    Headspace for parents: Using a mindfulness app to manage parenting stress

    Parenting stress can influence childrenā€™s psychological adjustment. Some evidence suggests parenting stress is associated with, and ameliorated by, parental mindfulness. This thesis aimed to determine the magnitude and meaning of associations between parenting stress, parental mindfulness, and childrenā€™s adjustment outcomes. It is proposed child outcomes may improve following improvements in parental wellbeing by practising mindfulness. To test this, the effectiveness of a self-directed mindfulness app (Headspaceā€”the collaborative partner) was evaluated with parents of children aged 2-5. Three projects addressed these aims. Project 1 scoped research investigating mindfulness and parenting, identifying gaps for digital, self-directed mindfulness interventions for parents of typically developing children. Project 2 aimed to address these gaps using a mixed methods approach to investigate the initial effectiveness of Headspace, aiming to understand the appā€™s acceptability and feasibility in parentsā€™ daily lives. The qualitative results of Project 2 were preliminarily published in BJPsych Open, and are expanded on in Chapter 4. Due to confounding by COVID-19 (see review, Appendix O), the quantitative results of this project were not published but are described here in Chapter 5. Project 3 was intended to determine the feasibility of a more robust study testing Headspace, using an internal pilot randomised controlled trial design (see Chapter 6). Overall, this thesis suggests parents generally found Headspace feasible and acceptable, and some improvements were noted for wellbeing in both Projects 2 and 3. Qualitative reports indicated improvements in parental sleep and ability to manage stress, andā€”with cautionā€”some quantitative support was found for improvements in sleep post-intervention in Project 3. Limited evidence for improvements in childrenā€™s adjustment were found, however, this thesis demonstrates the potential of Headspace for parent wellbeing. Future research might benefit from investigating relationships between parental mindfulness and sleep, particularly its potential to improve parentsā€™ stress perceptions, in more diverse samples

    Description of a pharmacy COVID champion scheme in South East London to reduce vaccine hesitancy

    In the United Kingdom, COVID vaccinations were rolled out from December 2020. In July 2021 in South East London there were areas of high COVID prevalence and low vaccination uptake. Therefore, a COVID champion service was launched in community pharmacies enabling pharmacy teams to have conversations with patients regarding the vaccination programme and their concerns and signposting as needed. The aim of the project was to evaluate the impact of the service on COVID vaccination uptake and perceptions of pharmacy teams. Quantitative data was received from pharmacy interaction records plus a pharmacy survey. In addition, qualitative data was received through interviews with 12 pharmacists involved in the service, which was analysed using content analysis, along with interviews with commissioner representatives. Between July and October 2021, 8539 conversations took place. From these 6094 patients agreed to vaccination, with 2019 initially hesitant patients converted. Lack of understanding, risk of blood clots and cultural concerns were the largest areas of hesitance. Pharmacy teams were happy to support conversation and local working with knowledge and confidence. Engagement within the team was the biggest enabler, with pressure to deliver other services being the biggest barrier. These results show the value of community pharmacy teams, having conversations with the public, addressing concerns where applicable, and signposting to the appropriate sites so patients are supported to achieve maximum health outcomes

    Headspace for parents: qualitative report investigating the use of a mindfulness-based app for managing parents' stress during COVID-19

    Background Stress can compromise parental well-being and may contribute to harsh and critical parenting styles, which are in turn associated with children's conduct problems. Coronavirus disease 2019 (COVID-19)-related restrictions are likely to have exacerbated parental stress as, for many, UK-based family life was altered considerably. Mindfulness has been demonstrated to improve stress management and emotion regulation when delivered to parents in person, however, more accessible online interventions are under-researched. Aims To provide preliminary data on family well-being and parentā€“child relationships as well as the acceptability and usability of the Headspace app ā€“ a self-delivered mindfulness-based intervention ā€“ for parents in low-risk families during the early days of the COVID-19 pandemic. Method We provided 12 parents with access to Headspace, and collected qualitative data (semi-structured interviews and 5 minute speech samples) immediately following the initial COVID-19 lockdown in the UK. The resulting transcripts were thematically analysed. Results Most parents reported Headspace to be acceptable and useful ā€“ improvements in parentsā€™ own sleep were particularly noted ā€“ and there was high adherence to the intervention. However, difficulties related to family well-being and parentā€“child relationships following the lockdown were also reported. Conclusions As a result of the confounding impact of COVID-19 restrictions, and varied access to app content, we were unable to determine any outcomes to be a result of practising mindfulness specifically. However, COVID-19 has had a profound impact on many UK-based families, including those previously at low risk, and our results demonstrate that Headspace may have beneficial effects for parents. There is a need to more rigorously test this tool with a broader range of families

    Smoking and infertility: multivariable regression and Mendelian randomization analyses in the Norwegian Mother, Father and Child Cohort Study

    Objective To investigate the association between smoking and infertility. Design Prospective study. Setting Nationwide cohort. Patients 28,606 women and 27,096 men with questionnaire and genotype information from the Norwegian Mother, Father, and Child Cohort Study. Intervention Self-reported information on smoking (having ever smoked [both sexes], age at initiation [women only], cessation [women only], and cigarettes/week in current smokers [both sexes]) was gathered. Genetically predetermined levels or likelihood of presenting these traits were estimated for Mendelian randomization. Main outcome measure Infertility (time-to-pregnancy ā‰„12 months). Results Having ever smoked was unrelated to infertility in women or men. Higher smoking intensity in women was associated with greater infertility odds (+1 standard deviation [SD, 48 cigarettes/week]: odds ratio [OR]crude, 1.19; 95% confidence interval [CI] 1.11ā€“1.28; ORadjusted 1.12; 95% CI, 1.03ā€“1.21), also after adjusting for the partnerā€™s tobacco use. Later smoking initiation (+1 SD [3.2 years]: ORcrude, 0.94; 95% CI, 0.88ā€“0.99; ORadjusted 0.89; 95% CI, 0.84ā€“0.95) and smoking cessation (vs. not quitting: ORcrude, 0.83; 95% CI, 0.75ā€“0.91; ORadjusted, 0.83; 95% CI, 0.75ā€“0.93) were linked to decreased infertility in women. Nevertheless, Mendelian randomization results were not directionally consistent for smoking intensity and cessation and were estimated imprecisely in the 2-sample approach. In men, greater smoking intensity was not robustly associated with infertility in multivariable regression and Mendelian randomization. Conclusions We did not find robust evidence of an effect of smoking on infertility. This may be due to a true lack of effect, weak genetic instruments, or other kinds of confounding.publishedVersio

    The Retinoid-related orphan receptor beta (RORĪ²) gene encodes a developmental transcription factor and has 2 predominant isoforms created through alternative first exon usage; one specific to the retina and another present more broadly in the central nervous system, particularly regions involved in sensory processing. RORĪ² belongs to the nuclear receptor family and plays important roles in cell fate specification in the retina and cortical layer formation. In mice, loss of RORĪ² causes disorganized retina layers, postnatal degeneration, and production of immature cone photoreceptors. Hyperflexion or high-stepping of rear limbs caused by reduced presynaptic inhibition by Rorb-expressing inhibitory interneurons of the spinal cord is evident in RORĪ²-deficient mice. RORĪ² variants in patients are associated with susceptibility to various neurodevelopmental conditions, primarily generalized epilepsies, but including intellectual disability, bipolar, and autism spectrum disorders. The mechanisms by which RORĪ² variants confer susceptibility to these neurodevelopmental disorders are unknown but may involve aberrant neural circuit formation and hyperexcitability during development. Here we report an allelic series in 5 strains of spontaneous Rorb mutant mice with a high-stepping gait phenotype. We show retinal abnormalities in a subset of these mutants and demonstrate significant differences in various behavioral phenotypes related to cognition. Gene expression analyses in all 5 mutants reveal a shared over-representation of the unfolded protein response and pathways related to endoplasmic reticulum stress, suggesting a possible mechanism of susceptibility relevant to patients

    A Spontaneous Mutation in Contactin 1 in the Mouse

    Mutations in the gene encoding the immunoglobulin-superfamily member cell adhesion molecule contactin1 (CNTN1) cause lethal congenital myopathy in human patients and neurodevelopmental phenotypes in knockout mice. Whether the mutant mice provide an accurate model of the human disease is unclear; resolving this will require additional functional tests of the neuromuscular system and examination of Cntn1 mutations on different genetic backgrounds that may influence the phenotype. Toward these ends, we have analyzed a new, spontaneous mutation in the mouse Cntn1 gene that arose in a BALB/c genetic background. The overt phenotype is very similar to the knockout of Cntn1, with affected animals having reduced body weight, a failure to thrive, locomotor abnormalities, and a lifespan of 2ā€“3 weeks. Mice homozygous for the new allele have CNTN1 protein undetectable by western blotting, suggesting that it is a null or very severe hypomorph. In an analysis of neuromuscular function, neuromuscular junctions had normal morphology, consistent with previous studies in knockout mice, and the muscles were able to generate appropriate force when normalized for their reduced size in late stage animals. Therefore, the Cntn1 mutant mice do not show evidence for a myopathy, but instead the phenotype is likely to be caused by dysfunction in the nervous system. Given the similarity of CNTN1 to other Ig-superfamily proteins such as DSCAMs, we also characterized the expression and localization of Cntn1 in the retinas of mutant mice for developmental defects. Despite widespread expression, no anomalies in retinal anatomy were detected histologically or using a battery of cell-type specific antibodies. We therefore conclude that the phenotype of the Cntn1 mice arises from dysfunction in the brain, spinal cord or peripheral nervous system, and is similar in either a BALB/c or B6;129;Black Swiss background, raising a possible discordance between the mouse and human phenotypes resulting from Cntn1 mutations

    Dual coding with STDP in a spiking recurrent neural network model of the hippocampus.

    The firing rate of single neurons in the mammalian hippocampus has been demonstrated to encode for a range of spatial and non-spatial stimuli. It has also been demonstrated that phase of firing, with respect to the theta oscillation that dominates the hippocampal EEG during stereotype learning behaviour, correlates with an animal's spatial location. These findings have led to the hypothesis that the hippocampus operates using a dual (rate and temporal) coding system. To investigate the phenomenon of dual coding in the hippocampus, we examine a spiking recurrent network model with theta coded neural dynamics and an STDP rule that mediates rate-coded Hebbian learning when pre- and post-synaptic firing is stochastic. We demonstrate that this plasticity rule can generate both symmetric and asymmetric connections between neurons that fire at concurrent or successive theta phase, respectively, and subsequently produce both pattern completion and sequence prediction from partial cues. This unifies previously disparate auto- and hetero-associative network models of hippocampal function and provides them with a firmer basis in modern neurobiology. Furthermore, the encoding and reactivation of activity in mutually exciting Hebbian cell assemblies demonstrated here is believed to represent a fundamental mechanism of cognitive processing in the brain
