    Propofol and Kearns-Sayre Syndrome: An idiographic approach

    With the focus on an idiographic approach whereby the observations incorporated the various dimensions of individual functioning ‘top-down’ to ‘bottom-up’, this case report describes the successful management of a 14-year-old girl with Kearns-Sayre syndrome and Dyggve-Melchior-Clausen disease requiring a transvenous permanent pacemaker implantation for complete heart block. The patient presented to a tertiary care centre in Muscat, Oman, in 2023 seeking consultation. The current idiographic approach appears to have a heuristic value for 2 interrelated reasons. Firstly, it is unlikely that even tertiary care units can accrue such rare presentations and scrutinise them under nomothetic approach. Secondly, by employing the idiographic approach that is capable of examining each case in-depth, the aspiration for good health and well-being may come to the forefront. To the best of the authors’ knowledge this is the first published idiographic report in anaesthesia care.     Modeling the Advancement of Sanad Services Implementation in the Sultanate of Oman

    One of the widely most used service providers across the Sultanate are Sanad Service Centers. They provide number of services for various purposes to facilitate people. Some of the services are related to Government offices such as Ministry of Labor and Royal Oman Police [3]. The Sanad Service Centers are licensed offices [3] and few of their services are intermediary in nature between the applicants and Government bodies. The Sanad Offices are operating physically and processing the request of different types of applications for individuals and companies such as visa processing, visa renewal and many others. To avail the offered services, physical handling of documents and visits are needed which can be transformed based on the idea of this paper is presenting. The advancements in this sector will have a major impact in a bigger scope to facilitate the large number of end-users with convenience and simplicity. The paper highlights the transformation and implementation of Online Sanad Services and will discuss the advancements of digital transformation to mobilize the services accessible to end-user in better, fast, timely, remotely, and secure manners. The transformation of Sanad Services from brick-and-mortar to click-and-mortar scenario will be an advantage and beneficial for all the stakeholders. It will reduce the chances of documents misplacement and ensure the availability of the contents for reuse as some of the services are required to complete and repeat on periodical basis due course of time. The study aims to design an industry specific model that can serve in the implementation and development of Online Sanad Services to meet the challenges of modern world requirements. The basic elements discussed and represented in this paper are the solution architecture comprises the system architecture, process modeling, data flow modeling, entity relationship modeling, and system requirements of the proposed system

    A relatively scarce number of morphological features available for delimiting closely related species and an increasingly worrisome scenario on Global Climate Change causing the rapid dispersion of invasive alien species can lead to the rapid spread of reports of a given species around the world. Craspedacusta sowerbii Lankester, 1880 is considered the most widespread freshwater jellyfish species and has been reported in numerous locations on all continents except Antarctica. Recently, a few medusae attributed to C. sowerbii were collected from a water reservoir (Bin El Ouidan) in Morocco, this being the first confirmed record of the species from North Africa. The morphology of these newly collected specimens agrees well with previous descriptions, but mitochondrial (Cox1 and 16S) and nuclear ITS (ITS1–5,8S–ITS2) molecular data lead to a discussion of a more complex general view concerning the number of species, synonyms and nomenclatural problems hidden behind the reports of Craspedacusta sowerbii

    Saudi mothers' preferences about breaking bad news concerning newborns: a structured verbal questionnaire

    <p>Abstract</p> <p>Background</p> <p>Breaking bad news (BBN) to parents whose newborn has a major disease is an ethical dilemma. In Saudi Arabia, BBN about newborns is performed according to the parental preferences that have been reported from non-Arabic/non-Islamic countries. Saudi mothers' preferences about BBN have not yet been studied. Therefore, we aimed to elicit the preferences of Saudi mothers about BBN concerning newborns.</p> <p>Methods</p> <p>We selected a convenience sample of 402 Saudi mothers, aged 18-50 years, who had no previous experience with BBN. We selected them via a simple number-randomization scheme from the premises of a level III Saudi hospital between October of 2009 and January of 2011. We used a hypothetical situation (BBN about trisomy 21) to elicit their preferences about BBN concerning newborns via a structured verbal questionnaire composed of 12 multiple-choice questions. We expressed their preferences as percentages (95% confidence interval), and we used the Kendall's W test (W) to assess the degree of agreement in preferences.</p> <p>Results</p> <p>The Saudi mothers preferred that BBN be conducted with both parents together (64% [60-69]), albeit with weak levels of agreement (W = 0.29). They showed moderate agreement in their preferences that BBN should be conducted early (79% [75-83], W = 0.48), in detail (81% [77-85], W = 0.52), in person (88% [85-91], W = 0.58), and in a quiet setting (86% [83-90], W = 0.53). With extremely weak agreement, they preferred to have a known person present for support during BBN (56% [51-61], W = 0.01), to have close bodily contact with their babies (66% [61-70], W = 0.10), and to have no another patients present (64% [59-68], W = 0.08). They showed moderate levels of agreement in their desires to detail, in advance, their preferences about process of BBN by giving a reversible, written informed consent that could be utilized for guidance, if needed (80% [76-84], W = 0.36).</p> <p>Conclusions</p> <p>In our experience, Saudi mothers' preferences about BBN concerning newborns are varied, suggesting that a "one-size-fits-all" approach is inappropriate. A reversible, written informed consent detailing their preferences about BBN that would be kept in their medical records and utilized for guidance, if needed, may be the best solution, given this level of diversity. These findings merit further study.</p
