Soliton pair dynamics in patterned ferromagnetic ellipses
Confinement alters the energy landscape of nanoscale magnets, leading to the
appearance of unusual magnetic states, such as vortices, for example. Many
basic questions concerning dynamical and interaction effects remain unanswered,
and nanomagnets are convenient model systems for studying these fundamental
physical phenomena. A single vortex in restricted geometry, also known as a
non-localized soliton, possesses a characteristic translational excitation mode
that corresponds to spiral-like motion of the vortex core around its
equilibrium position. Here, we investigate, by a microwave reflection
technique, the dynamics of magnetic soliton pairs confined in lithographically
defined, ferromagnetic Permalloy ellipses. Through a comparison with
micromagnetic simulations, the observed strong resonances in the subgigahertz
frequency range can be assigned to the translational modes of vortex pairs with
parallel or antiparallel core polarizations. Vortex polarizations play a
negligible role in the static interaction between two vortices, but their
effect dominates the dynamics.Comment: supplemental movies on

Exploring the similarities and differences between medical assessments of competence and criminal responsibility
The medical assessments of criminal responsibility and competence to consent to treatment are performed, developed and debated in distinct domains. In this paper I try to connect these domains by exploring the similarities and differences between both assessments. In my view, in both assessments a decision-making process is evaluated in relation to the possible influence of a mental disorder on this process. I will argue that, in spite of the relevance of the differences, both practices could benefit from the recognition of this similarity. For cooperative research could be developed directed at elucidating exactly how various mental disturbances can affect decision-making processes
Inhibition of IL-34 Unveils Tissue-Selectivity and Is Sufficient to Reduce Microglial Proliferation in a Model of Chronic Neurodegeneration
The proliferation and activation of microglia, the resident macrophages in the brain,
is a hallmark of many neurodegenerative diseases such as Alzheimer’s disease (AD)
and prion disease. Colony stimulating factor 1 receptor (CSF1R) is critically involved
in regulating microglial proliferation, and CSF1R blocking strategies have been recently
used to modulate microglia in neurodegenerative diseases. However, CSF1R is broadly
expressed by many cell types and the impact of its inhibition on the innate immune
system is still unclear. CSF1R can be activated by two independent ligands, CSF-1 and
interleukin 34 (IL-34). Recently, it has been reported that microglia development and
maintenance depend on IL-34 signaling. In this study, we evaluate the inhibition of IL-34
as a novel strategy to reduce microglial proliferation in the ME7 model of prion disease.
Selective inhibition of IL-34 showed no effects on peripheral macrophage populations in
healthy mice, avoiding the side effects observed after CSF1R inhibition on the systemic
compartment. However, we observed a reduction in microglial proliferation after IL-34
inhibition in prion-diseased mice, indicating that microglia could be more specifically
targeted by reducing IL-34. Overall, our results highlight the challenges of targeting the
CSF1R/IL34 axis in the systemic and central compartments, important for framing any
therapeutic effort to tackle microglia/macrophage numbers during brain disease
Possible Race and Gender Divergence in Association of Genetic Variations with Plasma von Willebrand Factor: A Study of ARIC and 1000 Genome Cohorts
The synthesis, secretion and clearance of von Willebrand factor (VWF) are regulated by genetic variations in coding and promoter regions of the VWF gene. We have previously identified 19 single nucleotide polymorphisms (SNPs), primarily in introns that are associated with VWF antigen levels in subjects of European descent. In this study, we conducted race by gender analyses to compare the association of VWF SNPs with VWF antigen among 10,434 healthy Americans of European (EA) or African (AA) descent from the Atherosclerosis Risk in Communities (ARIC) study. Among 75 SNPs analyzed, 13 and 10 SNPs were associated with VWF antigen levels in EA male and EA female subjects, respectively. However, only one SNP (RS1063857) was significantly associated with VWF antigen in AA females and none was in AA males. Haplotype analysis of the ARIC samples and studying racial diversities in the VWF gene from the 1000 genomes database suggest a greater degree of variations in the VWF gene in AA subjects as compared to EA subjects. Together, these data suggest potential race and gender divergence in regulating VWF expression by genetic variations
Protocol for implementation of family health history collection and decision support into primary care using a computerized family health history system
<p>Abstract</p> <p>Background</p> <p>The CDC's Family History Public Health Initiative encourages adoption and increase awareness of family health history. To meet these goals and develop a personalized medicine implementation science research agenda, the Genomedical Connection is using an implementation research (T3 research) framework to develop and integrate a self-administered computerized family history system with built-in decision support into 2 primary care clinics in North Carolina.</p> <p>Methods/Design</p> <p>The family health history system collects a three generation family history on 48 conditions and provides decision support (pedigree and tabular family history, provider recommendation report and patient summary report) for 4 pilot conditions: breast cancer, ovarian cancer, colon cancer, and thrombosis. All adult English-speaking, non-adopted, patients scheduled for well-visits are invited to complete the family health system prior to their appointment. Decision support documents are entered into the medical record and available to provider's prior to the appointment. In order to optimize integration, components were piloted by stakeholders prior to and during implementation. Primary outcomes are change in appropriate testing for hereditary thrombophilia and screening for breast cancer, colon cancer, and ovarian cancer one year after study enrollment. Secondary outcomes include implementation measures related to the benefits and burdens of the family health system and its impact on clinic workflow, patients' risk perception, and intention to change health related behaviors. Outcomes are assessed through chart review, patient surveys at baseline and follow-up, and provider surveys. Clinical validity of the decision support is calculated by comparing its recommendations to those made by a genetic counselor reviewing the same pedigree; and clinical utility is demonstrated through reclassification rates and changes in appropriate screening (the primary outcome).</p> <p>Discussion</p> <p>This study integrates a computerized family health history system within the context of a routine well-visit appointment to overcome many of the existing barriers to collection and use of family history information by primary care providers. Results of the implementation process, its acceptability to patients and providers, modifications necessary to optimize the system, and impact on clinical care can serve to guide future implementation projects for both family history and other tools of personalized medicine, such as health risk assessments.</p
Exploring factors that influence the spread and sustainability of a dysphagia innovation: an instrumental case study
Background: Swallowing difficulties challenge patient safety due to the increased risk of malnutrition, dehydration
and aspiration pneumonia. A theoretically driven study was undertaken to examine the spread and sustainability of
a locally developed innovation that involved using the Inter-Professional Dysphagia Framework to structure
education for the workforce. A conceptual framework with 3 spread strategies (hierarchical control, participatory
adaptation and facilitated evolution) was blended with a processual approach to sustaining organisational change.
The aim was to understand the processes, mechanism and outcomes associated with the spread and sustainability
of this safety initiative.
Methods: An instrumental case study, prospectively tracked a dysphagia innovation for 34 months (April 2011
to January 2014) in a large health care organisation in England. A train-the-trainer intervention (as participatory
adaptation) was deployed on care pathways for stroke and fractured neck of femur. Data were collected at the
organisational and clinical level through interviews (n = 30) and document review. The coding frame combined
the processual approach with the spread mechanisms. Pre-determined outcomes included the number of staff
trained about dysphagia and impact related to changes in practice.
Results: The features and processes associated with hierarchical control and participatory adaptation were
identified. Leadership, critical junctures, temporality and making the innovation routine were aspects of hierarchical
control. Participatory adaptation was evident on the care pathways through stakeholder responses, workload and
resource pressures. Six of the 25 ward based trainers cascaded the dysphagia training. The expected outcomes
were achieved when the top-down mandate (hierarchical control) was supplemented by local engagement and
support (participatory adaptation).
Conclusions: Frameworks for spread and sustainability were combined to create a ‘small theory’ that described
the interventions, the processes and desired outcomes a priori. This novel methodological approach confirmed
what is known about spread and sustainability, highlighted the particularity of change and offered new insights
into the factors associated with hierarchical control and participatory adaptation. The findings illustrate the dualities
of organisational change as universal and context specific; as particular and amendable to theoretical generalisation.
Appreciating these dualities may contribute to understanding why many innovations fail to become routine
An Ethical Façade? Medical Students' Miscomprehensions of Substituted Judgment
Background: We studied how well first-year medical students understand and apply the concept of substituted judgment, following a course on clinical ethics. Method: Students submitted essays on one of three ethically controversial scenarios presented in class. One scenario involved a patient who had lost decisional capacity. Through an iterative process of textual analysis, the essays were studied and coded for patterns in the ways students misunderstood or misapplied the principle of substituted judgment. Results: Students correctly articulated course principles regarding patient autonomy, substituted judgment, and nonimposition of physician values. However, students showed misunderstanding by giving doctors the responsibility of balancing the interests of the patient against the interests of the family, by stating doctors and surrogates should be guided primarily by a best-interest standard, and by suggesting that patient autonomy becomes the guiding principle only when patients can no longer express their wishes. Conclusion: Students did not appear to internalize or correctly apply the substituted judgment standard, even though they could describe it accurately. This suggests the substituted judgment standard may run counter to students ’ moral intuitions
Elevated atmospheric CO2 and humidity delay leaf fall in Betula pendula, but not in Alnus glutinosa or Populus tremula × tremuloides
Context: Anthropogenic activity has increased the level of atmospheric CO2, which is driving an increase of global temperatures and associated changes in precipitation patterns. At Northern latitudes, one of the likely consequences of global warming is increased precipitation and air humidity.
Aims: In this work, the effects of both elevated atmospheric CO2 and increased air humidity on trees commonly growing in northern European forests were assessed. Methods: The work was carried out under field conditions by using Free Air Carbon dioxide Enrichment (FACE) and Free Air Humidity Manipulation (FAHM) systems. Leaf litter fall was measured over 4 years (FACE) or 5 years (FAHM) to determine the effects of FACE and FAHM on leaf phenology. Results: Increasing air humidity delayed leaf litter fall in Betula pendula, but not in Populus tremula × tremuloides. Similarly, under elevated atmospheric CO2, leaf litter fall was delayed in Betula pendula, but not in Alnus glutinosa. Increased CO2 appeared to interact with periods of low precipitation in summer and high ozone levels during these periods to effect leaf fall.
Conclusions: This work shows that increased CO2 and humidity delay leaf fall, but this effect is species specific
