    Bacteriologie de l’otite moyenne suppuree chronique de l’enfant au mali

    Introduction: L’otite moyenne suppurée chronique est une affection fréquente chez l’enfant. Plusieurs facteurs peuvent favoriser l’installation d’un tel processus infectieux (rhinopharyngites, otites mal traitées …). L’identification du germe causal de l’otite est une étape essentielle pour mener un traitement adéquat. But : Déterminer l’écologie bactérienne des otites purulentes chroniques de l’enfant dans notre unité. Patients et méthodes : L’étude menée de novembre 2010 à octobre 2011 a concerné 76 jeunes patients (40 filles et 36 garçons âgés de 6 à 180 mois avec une moyenne d’âge de 52,73 mois) présentant une otite moyenne suppurée chronique. L’otorrhée purulente des 76 patients (80 oreilles) a été prélevée. Celle-ci était recueillie à l’oreille d’un écouvillon stérile après nettoyage du conduit auditif externe et conduit le même jour au laboratoire pour examen bactériologique. Résultats : 94,74% des prélèvements étaient positifs et 5,26% étaient stériles. Parmi les prélèvements positifs 75% étaient monomicrobiens et 25% plurimicrobiens. Staphylococcus aureus (41,31%), proteus mirabilis (34,79%) sont les principales espèces bactériennes responsables d’otite moyenne suppurée chronique chez l’enfant dans notre unité. Conclusion : Ainsi le rôle pathogène de staphylococcus aureus est prépondérant dans les otites moyennes suppurées chroniques de l’enfant dans notre unité. Mots-clés : bactériologie - otite moyenne suppurée chronique - enfant.

    Cholinergic and anticholinesterase activities of total protein extract of Morinda morindoĂŻdes on isolated rabbit duodenum

    Traditional herbal medicines such as Morinda morindoïdes are used for treatment of intestinal disorders including constipation in Ivory Coast. The aim of present study was to investigate the effect of total protein of Morinda morindoïdes extract (PT-Mm) on rabbit duodenum contractility and the involved possible mechanism(s). PT-Mm was extracted according to the saturation method of Dawson with ammonium sulfate. The cholinergic effect of the extract was determined by the in vitro organ bath method. The acetylcholinesterase (AChE) extracted from rabbit duodenum and it activity was determined by Ellman’s assay using acetylthiocholine (ASCh) as substrate. PT-Mm concentrations (40, 80, 120 and 200 μg/mL) showed dose-dependent effect a both tonicity and amplitude of the duodenum spontaneous contractions. The effective concentration which induces 50% effect of PT-Mm (EC50) was obtained with 68.57 ± 0.89 μg/mL. The antagonist tests carried out showed a considerable reduction (90%) in the amplitudes of duodenal contractions in the presence of atropine, but with nifedipine, the contractions were completely inhibited. PT-Mm also exerted non-competitive inhibition on AChE (Vmax = 5687 mM/min and KM = 578 μM). These results suggest that PT-Mm could stimulate duodenum smooth muscle contraction because it contains anti-AChE and cholinomimetic substances which, through muscarinic receptors, increase Ca2+ mobilization from extracellular. Therefore, PT-Mm could be used as a laxative, due to its stimulating effects on duodenal contractility.Keywords: Morinda morindoides, acetylcholinesterase, duodenum, contractio

    Therapeutic efficacy of sulphadoxine-pyrimethamine and chloroquine for the treatment of uncomplicated malaria in pregnancy in Burkina Faso

    BACKGROUND: A reduction in the therapeutic efficacy of chloroquine (CQ) and sulphadoxine-pyrimethamine (SP) has recently been observed in Burkina Faso. As these two drugs are used in pregnancy, their efficacy in pregnant women was studied to directly assess the level of drug resistance in this specific population, rather than to extrapolate results of studies conducted in children < 5 years of age. METHODS: During the malaria transmission season of 2003 in Ouagadougou, the clinical efficacy of SP and CQ, using the WHO 28-day protocol, was assessed in primigravidae and secundigravidae presenting with uncomplicated malaria. RESULTS: PCR-corrected results by day 28 showed that among 62 women treated with SP, eight (12.9%) experienced late parasitological failure, but no clinical failures. Among 60 women treated with CQ, the overall failure rate was 46.7% including 1.7% early treatment failures, 5% late clinical failures and 40% late parasitological failures. SP induced a haemoglobin gain of 0.3 g/dL by day 14 and 0.9 g/dL by day 28. Treatment responses were independent of gravidity, gestational age and prior antenatal care visits. CONCLUSION: While CQ should no longer be used, the efficacy of SP is still compatible with use for intermittent preventive treatment (IPT) in pregnancy. However, given the possible spread of resistance, the drug should be restricted in its use

    High malnutrition rate in Venezuelan Yanomami compared to Warao Amerindians and Creoles: significant associations with intestinal parasites and anemia

    Contains fulltext : 125614.pdf (publisher's version ) (Open Access)BACKGROUND: Children in rural areas experience the interrelated problems of poor growth, anemia and parasitic infections. We investigated the prevalence of and associations between intestinal helminth and protozoan infections, malnutrition and anemia in school-age Venezuelan children. METHODS: This cross-sectional study was conducted in 390 children aged 4-16 years from three rural areas of Venezuela: the Amazon Region, Orinoco Delta and Carabobo State. Stool samples were collected for direct parasitic examinations. Anthropometric indicators of chronic (height-for-age Z score) and acute (weight-for-height and Body Mass Index (BMI)-for-age Z score in respectively children under 5 years of age and children aged 5 years and above) malnutrition were calculated. Multivariate linear and logistic regression models were built to determine factors associated with nutritional status and polyparasitism. RESULTS: Hookworm and Strongyloides stercoralis prevalences were highest in children from the Amazon rainforest (respectively 72% and 18%) while children from the Orinoco Delta and Carabobo State showed higher rates of Ascaris lumbricoides (respectively 28% and 37%) and Trichuris trichiura (40% in both regions). The prevalence of Giardia lamblia infection was not significantly different between regions (average: 18%). Anemia prevalence was highest in the Amazon Region (24%). Hemoglobin levels were significantly decreased in children with a hookworm infection. Malnutrition was present in respectively 84%, 30% and 13% of children from the Amazon Region, Orinoco Delta and Carabobo State. In multivariate analysis including all regions, G. lamblia and helminth infections were significantly and negatively associated with respectively height-for-age and weight-for-height/BMI-for-age Z scores. Furthermore, hemoglobin levels were positively associated with the height-for-age Z score (0.11, 95% CI 0.02 - 0.20). CONCLUSIONS: In rural populations in Venezuela helminthiasis and giardiasis were associated with acute and chronic nutritional status respectively. These data highlight the need for an integrated approach to control transmission of parasites and improve the health status of rural Venezuelan children
