    Gastric luminal epidermal growth factor is affected by diet

    Objective. Diet is an area of major interest to those investigating the causes of cancer of the oesophagus in  the Transkei. This study looked at the associations between intragastric epidermal growth factor level, diet  and intragastric pH.Setting and subjects. A dietary survey was co-ordinated with studies of gastric luminal epidermal growth  factor and gastric fluid pH in 120 rural Transkeians.Results. Gastric fluid epidermal growth factor was associated with low dietary intake of animal products (p = 0.002) and vegetables (p = 0.026). There was no association with pH.Conclusion. A dietary subgroup has been identified in the Transkei population with high levels of epidermal  growth factor in the upper gastrointestinal lumen. This adds to previously demonstrated diet-related changes in the upper gastrointestinal tract in Transkei. These changes may affect the disease pattern of the population

    Investigation of Possible Antidotal Effects of Activated Charcoal, Sodium Bicarbonate, Hydrogen Peroxide and Potassium Permanganate in Zinc Phosphide Poisoning

    Zinc phosphide, a commonly used rat poison in Kenya was mixed with maize flour in a concentration of 0.15 % w/w and fed to a group of 60 experimental mice for 3 hours. The mice were then randomly divided into 5 equal groups, A, B, C, D and E. To group A, B, C and D was administered 1 ml of activated charcoal (3 % w/v), sodium bicarbonate (10 % w/v), hydrogen peroxide (0.5 % v/v) and potassium permanganate (1:5000), respectively. Group E was given 1 ml distilled water and used as control. All five groups were observed for symptoms of toxicity, often culminating in death. The observations were continued over a period of 36 hours. Results of the experiment showed that all 4 test substances minimized the lethal effect of zinc phosphide. Although no attempt was made to quantify the antidotal effect of the 4 substances, activated charcoal appeared to have a higher effect than the others, while potassium permanganate had a low rating.     Perception of Nurses about Palliative Care: Experience from South-West Nigeria

    Background: Nurses play a major role all over the world in the palliative care team.Aim: The aim of this study was to investigate the knowledge and attitude of nurses toward palliative care in a tertiary level hospital in Nigeria. Subjects and Methods: Setting: This cross-sectional questionnaire-based study was carried out among nurses at a tertiary health care facility inAdo-Ekiti, South-West Nigeria. Design: A cross-sectional questionnaire-based study was carried out. The questionnaire sought information about the sociodemographic profile of respondents, their knowledge of definition and philosophy of palliative care among other things. Descriptive statistics was used to obtain the general characteristics of the study participants, while Chi-square was used to determine the association between categorical variables. A two-sided P < 0.05 was considered as significant.Results: A total of 100 questionnaires were returned with a female preponderance among the respondents with F: M ratio of 9:1. Regarding the definition of palliative care, 71.8% (48/66) of the respondents  understood palliative care to be about pain medicine, 55% (33/60) thought it to be geriatric medicine, while 90.2% (83/92) felt palliative care is about the active care of the dying. Exactly 80.5% (66/82) respondents agreedthat palliative care recognizes dying as a normal process while 84.1% (74/88) respondents were of the opinion that all dying patients would require palliative care. The use of morphine would improve the quality of life of patients according to 68.9% (42/61) of respondents.Conclusion: There are gaps in the knowledge of healthcare workers in the area of palliative care and this call for a review of the current nursing curriculum and practice guidelines in Nigeria.Keywords: End of life, Nursesf attitude, Pain management, Palliative car

    A 16-nm SoC for Noise-Robust Speech and NLP Edge AI Inference With Bayesian Sound Source Separation and Attention-Based DNNs

    The proliferation of personal artificial intelligence (AI) -assistant technologies with speech-based conversational AI interfaces is driving the exponential growth in the consumer Internet of Things (IoT) market. As these technologies are being applied to keyword spotting (KWS), automatic speech recognition (ASR), natural language processing (NLP), and text-to-speech (TTS) applications, it is of paramount importance that they provide uncompromising performance for context learning in long sequences, which is a key benefit of the attention mechanism, and that they work seamlessly in polyphonic environments. In this work, we present a 25-mm 2^2 system-on-chip (SoC) in 16-nm FinFET technology, codenamed SM6, which executes end-to-end speech-enhancing attention-based ASR and NLP workloads. The SoC includes: 1) FlexASR, a highly reconfigurable NLP inference processor optimized for whole-model acceleration of bidirectional attention-based sequence-to-sequence (seq2seq) deep neural networks (DNNs); 2) a Markov random field source separation engine (MSSE), a probabilistic graphical model accelerator for unsupervised inference via Gibbs sampling, used for sound source separation; 3) a dual-core Arm Cortex A53 CPU cluster, which provides on-demand single Instruction/multiple data (SIMD) fast fourier transform (FFT) processing and performs various application logic (e.g., expectation–maximization (EM) algorithm and 8-bit floating-point (FP8) quantization); and 4) an always-on M0 subsystem for audio detection and power management. Measurement results demonstrate the efficiency ranges of 2.6–7.8 TFLOPs/W and 4.33–17.6 Gsamples/s/W for FlexASR and MSSE, respectively; MSSE denoising performance allowing 6 ×\times smaller ASR model to be stored on-chip with negligible accuracy loss; and 2.24-mJ energy consumption while achieving real-time throughput, end-to-end, and per-frame ASR latencies of 18 ms

    Effects of Supersymmetric Threshold Corrections on High-Scale Flavor Textures

    Integration of superpartners out of the spectrum induces potentially large contributions to Yukawa couplings. These corrections, the supersymmetric threshold corrections, therefore influence the CKM matrix prediction in a non-trivial way. We study effects of threshold corrections on high-scale flavor structures specified at the gauge coupling unification scale in supersymmetry. In our analysis, we first consider high-scale Yukawa textures which qualify phenomenologically viable at tree level, and find that they get completely disqualified after incorporating the threshold corrections. Next, we consider Yukawa couplings, such as those with five texture zeroes, which are incapable of explaining flavor-changing proceses. Incorporation of threshold corrections, however, makes them phenomenologically viable textures. Therefore, supersymmetric threshold corrections are found to leave observable impact on Yukawa couplings of quarks, and any confrontation of high-scale textures with experiments at the weak scale must take into account such corrections.Comment: 25 pages, submitted to JHE

    The use of weight loss dietary supplements is prevalent in the United States, and over the past decade, there has been tremendous growth of the use of these products. It is well documented that ephedra-based products are associated with various cardiovascular adverse effects. With new restrictions placed on such products, companies are now manufacturing caffeine-based ephedra-free herbal supplements. We present the case of 36-year old, previously healthy female who developed malignant hypertension and aortic dissection while taking various caffeine-based dietary supplements. Given the lack of research studies in regards to their safety and efficacy, judicious care should be taken with the use of dietary supplements, including those designated as ephedra-free

    Mindfulness-based interventions in epilepsy: a systematic review

    Mindfulness based interventions (MBIs) are increasingly used to help patients cope with physical and mental long-term conditions (LTCs). Epilepsy is associated with a range of mental and physical comorbidities that have a detrimental effect on quality of life (QOL), but it is not clear whether MBIs can help. We systematically reviewed the literature to determine the effectiveness of MBIs in people with epilepsy. Medline, Cochrane Central Register of Controlled Trials, EMBASE, CINAHL, Allied and Complimentary Medicine Database, and PsychInfo were searched in March 2016. These databases were searched using a combination of subject headings where available and keywords in the title and abstracts. We also searched the reference lists of related reviews. Study quality was assessed using the Cochrane Collaboration risk of bias tool. Three randomised controlled trials (RCTs) with a total of 231 participants were included. The interventions were tested in the USA (n = 171) and China (Hong Kong) (n = 60). Significant improvements were reported in depression symptoms, quality of life, anxiety, and depression knowledge and skills. Two of the included studies were assessed as being at unclear/high risk of bias - with randomisation and allocation procedures, as well as adverse events and reasons for drop-outs poorly reported. There was no reporting on intervention costs/benefits or how they affected health service utilisation. This systematic review found limited evidence for the effectiveness of MBIs in epilepsy, however preliminary evidence suggests it may lead to some improvement in anxiety, depression and quality of life. Further trials with larger sample sizes, active control groups and longer follow-ups are needed before the evidence for MBIs in epilepsy can be conclusively determined

    Severe sepsis caused by Aeromonas hydrophila in a patient using tocilizumab: a case report

    <p>Abstract</p> <p>Introduction</p> <p><it>Aeromonas </it>species do not commonly cause disease in humans. However, when disease is seen, it often occurs in patients with underlying immunosuppression or malignancy and has a high fatality rate.</p> <p>Case presentation</p> <p>A 72-year-old Japanese woman with rheumatoid arthritis treated with tocilizumab (which has an immunosuppressive effect) presented with severe epigastric pain. She had a fever with chills, hypotension and jaundice. She was diagnosed with acute suppurative cholangitis and treated with cefoperazone-sulbactam and an endoscopic drainage was performed. Jaundice was slightly improved, but the shock state and inflammatory reactions were prolonged as typical of septic shock. On the second day after admission, an electrocardiogram showed ST segment elevation and echocardiography showed ventricular wall dysfunction. Coronary arteries were patent in coronary angiography and she was diagnosed with stress-induced cardiomyopathy. Blood cultures showed <it>Aeromonas hydrophila</it>. A stool culture was negative for <it>A. hydrophila</it>. On day six, her white blood cell count and neutrophils were normalized and cefoperazone-sulbactam treatment was halted. Left ventricular function normalized on day twelve and a laparoscopic cholecystectomy for cholelithiasis was performed on the 16th day of hospitalization. A culture from the bile showed <it>A. hydrophila</it>. Eighteen days after surgery, tocilizumab treatment was restarted and there were no complications. Two months after restarting tocilizumab, our patient is stable without any serious events.</p> <p>Conclusion</p> <p>We present a rare case of <it>A. hydrophila </it>sepsis and acute suppurative cholangitis in an elderly patient with gallstones and rheumatoid arthritis using tocilizumab. This clinical course may suggest that preemptive treatment for cholelithiasis prior to using molecular-targeting agents might be feasible in elderly patients.</p
