    Sinais clínicos associados a um caso de envenenamento iatrogênico de um equino com três dias de idade por um produto comercial baseado em terebintina

    O objetivo deste estudo é relatar um caso de intoxicação iatrogênica por um produto comercial a base de terebentina em um potro de três días de idade e o sucesso do tratamento dessa condição. Um potro de três dias de idade foi apresentou um colapso cardiorrespiratório agudo, choque anafilático, desconforto respiratório grave, dispnéia e edema pulmonar. A história incluiu a administração prévia de um produto veterinário comercial (UNITIPAN). Ao exame clínico: temperatura: (39,9 °C), pulso: 135 bpm, respiração 51 bpm, membranas mucosas, congestionamento úmido, tempo de enchimento capilar: três segundos ou menos. O potro foi tratado com hidratação de emergência, duphalyte, anti-inflamatório esteroidal, sulfato de atropina e antibiótico. Após seis horas de tratamento, o potro foi estabilizado e os sinais vitais foram normalizados depois de oito horas do inicio do tratamento. O presente trabalho relata um caso de intoxicação iatrogênica por um produto à base de terebintina comercial em um potro de três dias de idade com rápido desenvolvimento de sinais de toxicidade aguda e uma resposta muito favorável ao tratamento sintomático.The aim of this study is to report a case of iatrogenic poisoning by a turpentine-based commercial product in a 3-day-old foal and the successful treatment of this condition. A 3-day-old male foal presented with acute cardio-respiratory collapse, anaphylactic shock, severe respiratory distress, dyspnea and pulmonary edema. The animal’s history included previous administration of a commercial veterinary product (UNITIPAN). Clinical examination revealed: temperature: (39.9 °C), pulse: 135 bpm, respiration: 51 bpm, mucous membranes: moist and congested, capillary refill time: 3 sec or less. The foal was treated with emergency hydration, duphalyte, steroidal anti-inflammatory, atropine sulphate and antibiotic. After 6 h of treatment, the foal was stabilized and vital signs were normalized 8 h after treatment start.El objetivo de este estudio es informar un caso de envenenamiento iatrogénico por un producto comercial basado en la trementina en un potro de 3 días y el tratamiento exitoso de esta afección. Se presentó un potro de 3 días de edad que mostraba colapso cardiorrespiratorio agudo, shock anafiláctico, dificultad respiratoria grave, disnea y edema pulmonar. La historia incluía la administración previa de un producto veterinario comercial (UNITIPAN). En el examen clínico: temperatura: (39.9 ° C), pulso: 135 lpm, respiración 51 lpm, membranas mucosas, congestión húmeda, tiempo de llenado capilar: tres segundos o menos. El potro fue tratado con hidratación de emergencia, duphalito, antiinflamatorio esteroideo, sulfato de atropina y antibiótico.     Paper and electronic versions of HM-PRO, a novel patient-reported outcome measure for hematology: an equivalence study.

    © 2019 Goswami, Oliva, Ionova et al.Aim:To determine measurement equivalence of paper and electronic application of the hematologi-cal malignancy-patient-reported outcome (HM-PRO), a specific measure for the evaluation of patient-reported outcomes in HMs.Patients & methods:Following International Society of Pharmacoeconomicsand Outcomes Research ePRO Good Research Practice Task Force guidelines, a total of 193 adult patientswith different HMs were recruited into a multicenter prospective study. The paper and the electronic ver-sion of the instrument were completed in the outpatient clinics in a randomized crossover design with a30-min time interval to minimize the learning effect. Those who completed the paper version first, com-pleted the electronic version after 30 min and vice versa. Instrument version and order effects were testedon total score of the two parts of the HM-PRO (Part A: quality of life and Part B: signs & symptoms) in atwo-way ANOVA with patients as random effects. Intraclass correlation coefficients (95% CI) and Spear-man’s rank correlation coefficients were used to evaluate test–retest reliability and reproducibility. Theeffects of instrument version and order were tested on total score of the two parts of HM-PRO.Results:The questionnaire version and administration order effects were not significant at the 5% level. Therewere no interactions found between these two factors for HM-PRO (Part A [quality of life]; p=0.95); and(part B [signs and symptoms]; p=0.72]. Spearman’s rank correlation coefficients were greater than 0.9, andintraclass correlation coefficients ranged from 0.94 to 0.98; furthermore, the scores were not statisticallydifferent between the two versions, showing acceptable reliability indexes. Noteworthy, the differencebetween the completion time for both paper (mean=6:38 min) and electronic version (mean=7:29 min)was not statistically significant (n=100; p=0.11). Patients did not report any difficulty in completing theelectronic version during cognitive interviews and were able to understand and respond spontaneously.Conclusion:Measurement equivalence has been demonstrated for the paper and electronic applicationof the HM-PRO.Peer reviewe

    Virulence Markers, Genotypic versus Phenotypic Resistance and New Treatment Strategies in <em>Helicobacter pylori</em> Infection

    This chapter aims at studying the microbial virulence determinants and markers of Helicobacter pylori (Hp), the molecular diagnostic of Hp, the growing antibiotic resistance with the related problem of heteroresistance, the genotypic resistance to antimicrobials compared with the phenotypic methods and the new treatment strategies for Hp eradication also evaluating new antimicrobial agents (furazolidone, vonoprazan). The virulence markers cover an important area in Hp pathology due to the correlation between these and the different diseases. The Hp molecuar diagnosis is fast, accurate and reliable over the traditional methods that are expensive and time- consuming. Therapy regimens used over the past decade are declining in efficacy being the Hp treatment bedevilled by drug-resistant strains. New treatment strategies are under study worldwide. The determination of the genetic resistance to antibiotics is very useful when used directly on gastric biopsies for prediction of antibiotics ineffectiveness or for addressing changes in previous treatments

    Development of a Novel Hematological Malignancy Specific Patient-Reported Outcome Measure (HM-PRO) : Content Validity

There is a wide range of health-related quality of life (HRQoL) issues important to these patients. Any new instrument developed to measure HRQoL of such patients should be content valid, i.e., the items should be comprehensively relevant to the patients and their health condition. The aim of the present study was to examine content validity of a hematological malignancy specific patient reported outcome measure (HM-PRO) developed for use in routine clinical practice. Methods: Following literature review and semi-structured interviews, the generated themes and sub-themes were discussed to develop the prototype version of the HM-PRO. A 4-step approach was used for content validation: initial testing and cognitive interviewing; item rating; content validity panel meeting; final field testing and cognitive interviewing. Additional questions related to patients' perception of recall period and preferred sentence structure (i.e., question or statement) of the items were also asked during cognitive interviews. Results: The content analysis of 129 transcribed semi-structured interviews resulted in the prototype version of the instrument consisting of 58 items grouped into two parts: Part A (impact/HRQoL - 34 items) and Part B (signs and symptoms - 24 items). The initial testing showed intra-class correlation coefficient (ICC) of >0.8 for both Part A and Part B. Item rating for language clarity, completeness, relevance, and response scale by experts and patients showed content validity index for scales average >0.8 for both Part A and Part B, except 0.64 for relevance for Part A by the patient panel. The final testing of the revised version of the instrument showed the Cronbach's alpha value of 0.91 for Part A and 0.76 for Part B, suggesting high internal consistency, and ICC of 0.91 for Part A and 0.76 for Part B. The recall period of "today" for Part-A and "last 3 days" for Part-B were the patients' preferred "recall period." Furthermore, the patients expressed preference to the HM-PRO items as statements. Conclusion: The findings of this study confirm that the HM-PRO possesses a strong content validity, includes all the issues important to patients and is easy to read, understand and respond to spontaneously.Peer reviewedFinal Published versio

    Hermeneutic Realism and Veritative Hermeneutics

    Hermeneutics has a long tradition in the history of philosophy. It carries the task of Hermes to bring God’s message to humans and translate it without betraying it. This special issue of the journal Critical Hermeneutics proposes a double research track: veritative hermeneutics and hermeneutic realism. This double track testifies to the original purpose of hermeneutics to formulate fundamental philosophical questions in light of human historical experience. Opposed to relativism, veritative hermeneutics and hermeneutic realism focus on truth, objectivity, the structure of reality, normativity, and meaning. The issue publishes contributions of this track’s pioneers (Gaspare Mura – veritative hermeneutics, Günter Figal, and Anton Friedrich Koch – hermeneutic realism) as well as authors that explore this track from various perspectives: ethics, personalism, normativity, testimony, Biblical hermeneutics and Jewish thought, religious language, meaning.     Bioengineered ‘golden’ indica rice cultivars with β-carotene metabolism in the endosperm with hygromycin and mannose selection systems

    Vitamin-A deficiency (VAD) is a major malnutrition problem in South Asia, where indica rice is the staple food. Indica-type rice varieties feed more than 2 billion people. Hence, we introduced a combination of transgenes using the biolistic system of transformation enabling biosynthesis of provitamin A in the endosperm of several indica rice cultivars adapted to diverse ecosystems of different countries. The rice seed-specific glutelin promoter (Gt-1 P) was used to drive the expression of phytoene synthase (psy), while lycopene β-cyclase (lcy) and phytoene desaturase (crtI), fused to the transit peptide sequence of the pea-Rubisco small subunit, were driven by the constitutive cauliflower mosaic virus promoter (CaMV35S P). Transgenic plants were recovered through selection with either CaMV35S P driven hph (hygromycin phosphotransferase) gene or cestrum yellow leaf curling virus promoter (CMP) driven pmi (phophomannose isomerase) gene. Molecular and biochemical analyses demonstrated stable integration and expression of the transgenes. The yellow colour of the polished rice grain evidenced the carotenoid accumulation in the endosperm. The colour intensity correlated with the estimated carotenoid content by spectrophotometric and HPLC analysis. Carotenoid level in cooked polished seeds was comparable (with minor loss of xanthophylls) to that in non-cooked seeds of the same transgenic line. The variable segregation pattern in T1 selfing generation indicated single to multiple loci insertion of the transgenes in the genome. This is the first report of using nonantibiotic pmi driven by a novel promoter in generating transgenic indica rice for possible future use in human nutrition

    Quality-of-life issues and symptoms reported by patients living with haematological malignancy: a qualitative study

    Background: Our aim was to identify health-related quality-of-life (HRQoL) issues and symptoms in patients with haematological malignancies (HMs) and develop a conceptual framework to reflect the inter-relation between them. / Methods: A total of 129 patients with HMs were interviewed in a UK multicentre qualitative study. All interviews were audio recorded, transcribed and analysed using NVivo-11. / Results: Overall, 34 issues were reported by patients and were grouped into two parts: quality of life (QoL) and symptoms. The most prevalent HRQoL issues were: eating and drinking habits; social life; physical activity; sleep; and psychological well-being. Furthermore, most prevalent disease-related symptoms were: tiredness; feeling unwell; breathlessness; lack of energy; and back pain. The most prevalent treatment side effects were: tiredness; feeling sick; disturbance in sense of taste; and breathlessness. / Conclusions: Both HMs and their treatments have a significant impact on patients’ HRQoL, in particular on issues such as job-role change, body image and impact on finances

    The phases of deuterium at extreme densities

    We consider deuterium compressed to higher than atomic, but lower than nuclear densities. At such densities deuterium is a superconducting quantum liquid. Generically, two superconducting phases compete, a "ferromagnetic" and a "nematic" one. We provide a power counting argument suggesting that the dominant interactions in the deuteron liquid are perturbative (but screened) Coulomb interactions. At very high densities the ground state is determined by very small nuclear interaction effects that probably favor the ferromagnetic phase. At lower densities the symmetry of the theory is effectively enhanced to SU(3), and the quantum liquid enters a novel phase, neither ferromagnetic nor nematic. Our results can serve as a starting point for investigations of the phase dynamics of deuteron liquids, as well as exploration of the stability and dynamics of the rich variety of topological objects that may occur in phases of the deuteron quantum liquid, which range from Alice strings to spin skyrmions to Z_2 vortices.Comment: 9 pages, 6 figures; v2: fixed typo

    A complete, multi-level conformational clustering of antibody complementarity-determining regions

    Classification of antibody complementarity-determining region (CDR) conformations is an important step that drives antibody modelling and engineering, prediction from sequence, directed mutagenesis and induced-fit studies, and allows inferences on sequence-to-structure relations. Most of the previous work performed conformational clustering on a reduced set of structures or after application of various structure pre-filtering criteria. In this study, it was judged that a clustering of every available CDR conformation would produce a complete and redundant repertoire, increase the number of sequence examples and allow better decisions on structure validity in the future. In order to cope with the potential increase in data noise, a first-level statistical clustering was performed using structure superposition Root-Mean-Square Deviation (RMSD) as a distance-criterion, coupled with second- and third-level clustering that employed Ramachandran regions for a deeper qualitative classification. The classification of a total of 12,712 CDR conformations is thus presented, along with rich annotation and cluster descriptions, and the results are compared to previous major studies. The present repertoire has procured an improved image of our current CDR Knowledge-Base, with a novel nesting of conformational sensitivity and specificity that can serve as a systematic framework for improved prediction from sequence as well as a number of future studies that would aid in knowledge-based antibody engineering such as humanisation
