    IgG4 related disease (IgG4 RD) is a newly recognized entity that includes a group of immune-mediated fibro-inflammatory conditions. It can affect a wide range of organs, producing a broad spectrum of presentations. The selection of imaging modality is based on patient symptoms and the organs under evaluation. The diagnosis is based on a combination of radiological findings, serum IgG4 elevation levels, histopathological features, association with other IgG4-related diseases and response to steroids. We report a rare case of an IgG4-related sclerosing mesenteritis in a man who presented with an incidental right lower quadrant mesenteric mass and was unsuccessfully treated with corticoids.     Mercapturate Pathway in the Tubulocentric Perspective of Diabetic Kidney Disease

    BACKGROUND: The recent growing evidence that the proximal tubule underlies the early pathogenesis of diabetic kidney disease (DKD) is unveiling novel and promising perspectives. This pathophysiological concept links tubulointerstitial oxidative stress, inflammation, hypoxia, and fibrosis with the progression of DKD. In this new angle for DKD, the prevailing molecular mechanisms on proximal tubular cells emerge as an innovative opportunity for prevention and management of DKD as well as to improve diabetic dysmetabolism. SUMMARY: The mercapturate pathway (MAP) is a classical metabolic detoxification route for xenobiotics that is emerging as an integrative circuitry detrimental to resolve tubular inflammation caused by endogenous electrophilic species. Herein we review why and how it might underlie DKD. Key Messages: MAP is a hallmark of proximal tubular cell function, and cysteine-S-conjugates might represent targets for early intervention in DKD.     Microstructure characterisation and mechanical properties of a functionally-graded NbC/high chromium white cast iron composite

    A functionally graded NbC – high chromium white cast iron composite was successfully fabricated by centrifugal casting for high wear applications. The migration of primary NbC particles to the outer periphery, as predicted by Stokes’ law, resulted in an extremely hard outer layer at the surface of a softer bulk material. Microstructural characterisation showed that both the volume fraction and the morphology of the carbides varies along the thickness of the specimen. At the outer surface, the NbC was present as primary carbides while Chinese-script NbC was found in the bulk of the material. Hardness, sliding wear and three-point bending tests confirmed that the outer layer was harder and more wear resistant than the rest of the material, which was correspondingly tougher. The study of the fracture surface of the specimens showed that there is a strong bond between NbC and the matrix such that delamination between the reinforcements and the matrix did not occur during fracture. If Chinese-script NbC can be reduced or prevented in the bulk material, further improvements to toughness could be achieved.ARC LP130100111, ARC DE150101703, FEDER POCI-01-0145-FEDER-00763

    In vitro effects of selected medicinal plants shortlisted for clinical use in the Brazilian public health system in CYP3A4 mRNA gene expression, glutathione levels and P-glycoprotein activity and their implications for herb-drug interactions

    The Brazilian Unified Public Health System (SUS) shortlisted various plant species of interest (RENISUS) for future clinical use. However, very little is known about their effects on metabolic and transporter proteins, which could potentially lead to herb-drug interactions (HDI). To evaluate this, we conducted in vitro preclinical studies on twenty-four plant extracts to disclose their effects on CYP3A4 mRNA gene expression, intracellular glutathione (GSH) levels, inhibition of γ-glutamyl transferase (GGT) in HepG2 cells and P- glycoprotein (P-gp) activity in vincristine resistant Caco-2 (Caco-2 VCR) cells. We also investigated whether four Brazilian native species were able to activate the human pregnane X receptor (hPXR) in transiently co-transfected HeLa cells. This preclinical research showed that all but two plant extracts were able to modulate at least one of the selected targets. CYP3A4 mRNA gene expression in HepG2 cells was significantly affected by half of the extracts. The antagonistic effect of Solanum paniculatum L. on hPXR could explain its ability to inhibit CYP3A4. GSH levels were affected by 80% of the extracts. There was depletion of intracellular GSH levels by Cordia verbenacea A. DC., Costus spicatus (Jacq.) Sw., Persea americana Mill., Salix alba L., Schinus terebinthifolia Raddi and Syzygium jambolanum (Lam.) DC. accompanied because of the inhibition of GGT activity. P-gp activity was modulated in a significant manner by 17% of the extracts. The approaches used for the conduction of in vitro preclinical studies in herbal medicines revealed a series of challenges faced especially by academics in order to anticipate cases of HDI. Clinicians have also to consider the presence of intrinsic factors such as genetic polymorphisms in each patient. The possible presence of undesirable interactions between RENISUS herbal medicines and essential drugs in SUS need eventually be clinically confirmed to attest our observed in vitro effects

    Os concelhos de Amadora e Sintra constituem um território de fortes características de identidade na Área Metropolitana de Lisboa que resultam, entre outros aspectos, da elevada densidade populacional (sobretudo Amadora), do marcado crescimento na última década (sobr etudo Sintra, onde a população residente aumentou cerca de 40% entre 1991 e 2001) e, principalmente, da sua diversidade social, cultural e étnica. D e facto, a proporção de imigrantes nestes concelhos atinge um dos v alores mais elevados do país. A equidade na pr estação de cuidados tem sido demonstrada como factor de r edução das disparidades na saúde que determina a morbilidade e a mortalidade decorrentes da assimetria das populações. Na Unidade de Saúde D (Hospital Fernando Fonseca e 9 Centr os de Saúde da Amadora e Sintra), não tinha ainda sido desenvolvida uma investigação científica estruturada sobre os níveis de saúde e o acesso e utilização dos serviços e que sustentem políticas ajustadas às vulnerabilidades deste grupo.     Retrospective study of canine cutaneous tumors submitted to a diagnostic pathology laboratory in Northern Portugal (2014-2020)

    Background Cutaneous neoplastic diseases are the most and second-most frequently reported tumors in male and female dogs, respectively. The aims of this study were to report the occurrence of canine cutaneous tumors in a pathology laboratory located in Northern Portugal between 2014 and 2020, and to characterize and categorize the anatomical locations, breed, age, and sex of the animals affected with different types of neoplasms. Results Throughout the 7-year study, 1,185 cases were diagnosed as cutaneous tumors, with 62.9% being classified as benign, and 37.1% as malignant. Mast cell tumors (22.7%) were the most frequently diagnosed tumor type, followed by benign soft tissue tumors (9.7%), sebaceous gland tumors (8.1%), vascular tumors (7.9%) and soft tissue sarcomas (7.6%). Cutaneous tumors commonly exhibited multicentric occurrence (14.6%) followed by single occurrence in hindlimb (12.1%), forelimb (8.6%), buttock (7.1%), abdominal (6.5%) and costal (5.2%) areas. The odds of developing cutaneous neoplasia were higher with increasing age (p < 0.001). Females had an increased odds of developing skin tumors compared to males (crude OR = 2.99, 95% (2.51, 3.55); adj OR = 2.93, 95% (2.46, 3.49). Purebred dogs, as a group, showed a reduced odds of developing cutaneous tumors when compared to mixed-breed dogs (crude OR = 0.63, 95% (0.53, 0.74); adj OR = 0.75, 95% (0.62, 0.89). Conclusions Mast cell tumors, benign soft tissue tumors and sebaceous tumors were the most common histotypes encountered. The epidemiological survey achieved with this study demonstrates the relative frequency of different types of tumors in this particular population. Furthermore, the results herein achieved can act as a basis or a beneficial reference for local veterinarians helping in the establishment of a preliminary and presumptive diagnosis of canine cutaneous tumors histotypes

    Abstrak: Dinas Kearsipan dan Perpustakaan (Disarpus) Kabupaten Cirebon memiliki salah satu tugas untuk mengelola arsip milik negara. Arsip sangat dibutuhkan dalam pelaksanaan kegiatan kedinasan yang dijadikan sebagai pusat ingatan untuk setiap hal didalam kantor. Arsip vital merupakan arsip dinamis yang memerlukan perlakuan khusus, baik dalam hal pengamanan maupun perlindungannya, karena informasi yang dimilikinya sangat terkait dengan keberadaan dan keberlangsungan dinas. Penanganan arsip sudah dapat dianggap cukup baik namun masih banyak menimbulkan resiko kehilangan data karena dokumen yang masih menggunakan kertas, dan tidak ada arsip cadangan serta memakan waktu untuk penemuan kembali. Perancangan sistem ini difokuskan pada arsip vital yang meliputi BPKB Kendaraan, Izin Mendirikan Bangunan (IMB), dan Izin Gangguan (HO). Penelitian ini bertujuan untuk merancang sebuah sistem informasi repositori yang mampu menyelesaikan permasalahan terkait penanganan arsip dengan menggunakan QR Code yang efektif, efisien, akurat dan dapat memproteksi arsip vital. Metode Rapid Application Development (RAD) dipilih dalam perancangan sistem ini. Rapid Application Development merupakan proses model perangkat lunak yang berkembang secara teratur dimana proses pemodelannya menekankan siklus pengembangan secara singkat. Metode RAD terdiri dari beberapa tahapan yaitu perencanaan kebutuhan, workshop desain dan implementasi. Archives are very much needed in the implementation of official activities which are used as memory centers for everything in the office. Vital archives are dynamic records that require special treatment, both in terms of security and protection, because the information they hold is closely related to the existence and sustainability of the service. The handling of archives can be considered quite well, but there are still many risks of losing data because documents still use paper, and there is no backup archive and it takes time to recover. The design of this system is focused on vital archives which include Vehicle BPKB, Building Construction Permit (IMB), and Disturbance Permit (HO). This research aims to design a repository information system that can solve problems related to archive handling using QRCode which is effective, efficient, accurate, and can protect vital archives. The method was Rapid Application Development (RAD) chosen in designing this system. Rapid Application Development is a software modeling process that develops regularly where the modeling process emphasizes a short development cycle. The RAD method consists of several stages, namely requirements planning, workshops design, and implementation.
