    Chronic myeloid leukemia (CML) is characterized by formation of the BCR-ABL fusion gene, usually as a consequence of the Philadelphia (Ph) translocation between chromosomes 9 and 22. Large deletions on the derivative chromosome 9 have recently been reported, but it was unclear whether deletions arose during disease progression or at the time of the Ph translocation. Fluorescence in situ hybridization (FISH) analysis was used to assess the deletion status of 253 patients with CML. The strength of deletion status as a prognostic indicator was then compared to the Sokal and Hasford scoring systems. The frequency of deletions was similar at diagnosis and after disease progression but was significantly increased in patients with variant Ph translocations. In patients with a deletion, all Ph+ metaphases carried the deletion. The median survival of patients with and without deletions was 38 months and 88 months, respectively (P = .0001). By contrast the survival difference between Sokal or Hasford high-risk and non-high-risk patients was of only borderline significance (P = .057 and P = .034). The results indicate that deletions occur at the time of the Ph translocation. An apparently simple reciprocal translocation may therefore result in considerable genetic heterogeneity ab initio, a concept that is likely to apply to other malignancies associated with translocations. Deletion status is also a powerful and independent prognostic factor for patients with CML. The prognostic significance of deletion status should now be studied prospectively and, if confirmed, should be incorporated into management decisions and the analysis of clinical trials.     Teeing up for success: The effects of rational and irrational self-talk on the putting performance of amateur golfers

    © 2018 Elsevier Ltd The investigation of rational emotive behaviour therapy (REBT) in sport settings is growing, but controlled experimental field studies are sparse. In the limited extant literature, researchers have found that irrational (extreme, rigid, and illogical) self-talk leads to disrupted motor skill performance, compared to rational (non-extreme, flexible, and logical) self-talk. However, methodological limitations of past research and the absence of sport-relevant tasks limit the application of findings to athletic settings. Therefore, the current study examined the effects of rational and irrational self-talk on the pressured putting performance of amateur golfers, by adopting a controlled and field-based experimental study design. A two-way repeated-measures ANCOVA, controlling for baseline putting ability, revealed that golfers' putting accuracy was significantly better when using rational self-talk than when using irrational self-talk. In addition, one-samples and paired-samples t-tests showed that golfers reported that rational self-talk was more usable than the irrational self-talk, and perceived the rational self-talk to be more facilitative than the irrational self-talk for their putting performance. The results are discussed with reference to potential explanatory mechanisms, study limitations, and future research needs

    Developing performance using rational emotive behavior therapy (REBT): A case study with an elite archer

    © 2017 Human Kinetics, Inc. Rational emotive behavior therapy (REBT; Ellis, 1957) is a psychotherapeutic approach receiving increasing interest within sport. REBT is focused on identifying, disputing, and replacing irrational beliefs (IBs) with rational beliefs (RBs) to promote emotional well-being and goal achievement. This study provides a detailed case outlining the application and effect of seven one-to-one REBT sessions with an elite level archer who was experiencing performance-related anxiety, before and during competition. The case also offers an insight into common misconceptions, challenges, and guidance for those who may consider applying REBT within their practice. Data revealed meaningful short and long-term (6-months) reductions in IBs and improvements in RBs, self-efficacy, perception of control and archery performance. The case supports the effective application of REBT as an intervention with athletic performers, promoting lasting changes in an athlete's ability to manage their cognitions, emotions and behaviors in the pursuit of performance excellence

    On sparse variational methods and the Kullback-Leibler divergence between stochastic processes

    The variational framework for learning inducing variables (Titsias, 2009a) has had a large impact on the Gaussian process literature. The framework may be interpreted as minimizing a rigorously defined Kullback-Leibler divergence between the approximating and posterior processes. To our knowledge this connection has thus far gone unremarked in the literature. In this paper we give a substantial generalization of the literature on this topic. We give a new proof of the result for infinite index sets which allows inducing points that are not data points and likelihoods that depend on all function values. We then discuss augmented index sets and show that, contrary to previous works, marginal consistency of augmentation is not enough to guarantee consistency of variational inference with the original model. We then characterize an extra condition where such a guarantee is obtainable. Finally we show how our framework sheds light on interdomain sparse approximations and sparse approximations for Cox processes

    Investigating the effects of irrational and rational self-statements on motor-skill and hazard-perception performance

Rational emotive behavior therapy (REBT) is a psychotherapeutic approach based on the premise that when individuals are faced with adversity, irrational beliefs determine unhealthy negative emotions and maladaptive behaviors, whereas rational beliefs lead to healthy and adaptive alternatives. The detrimental effects of irrational beliefs on psychological health are established; however, less is known about their deleterious effects on human behavior and performance. In the present study, we examined the effects of irrational and rational self-statements on motor-skill performance (Experiment 1), performance effectiveness, and efficiency during a modified hazard-perception task, and on persistence during a breath-holding task (Experiment 2). Using a repeatedmeasures counterbalanced design, we recruited 2 cohorts of 35 undergraduate university students for Experiments 1 and 2, each participating in no self-statement, irrational, and rational self-statement conditions. Data indicated no differences in motor-skill and task performance, performance efficiency, task persistence, mental effort, and preperformance anxiety between irrational and rational self-statement conditions. In contrast to previous research, our findings provide insight into a juxtaposition that irrational beliefs hinder psychological health yet may help performance, highlighting important distinctions in factual and practical rationality that have been overlooked within the extant literature. The findings have important practical implications for practitioners who may look to REBT to enhance the psychological health and performance for individuals who operate in high-performance contexts. Further, the short-and longterm effects of irrational and rational beliefs on performance and psychological health warrants greater investigation

    A candidate tolerance gene identified in a natural population of field voles (Microtus agrestis)

    The animal immune response has hitherto been viewed primarily in the context of resistance only. However, individuals, can also employ a tolerance strategy to maintain good health in the face of on-going infection. To shed light on the genetic and physiological basis of tolerance, we use a natural population of field voles, Microtus agrestis, to search for an association between the expression of the transcription factor Gata3, previously identified as a marker of tolerance in this system, and polymorphism in 84 immune and non-immune genes. Our results show clear evidence for an association between Gata3 expression and polymorphism in the Fcer1a gene, with the explanatory power of this polymorphism being comparable to that of other non-genetic variables previously identified as important predictors of Gata3 expression. We also uncover the possible mechanism behind this association using an existing protein-protein interaction network for the mouse model rodent, Mus musculus, which we validate using our own expression network for M. agrestis. Our results suggest that the polymorphism in question may be working at the transcriptional level, leading to changes in the expression of the Th2-related genes, Tyrosine-protein kinase BTK and Tyrosine-protein kinase TXK, and hence potentially altering the strength of the Th2 response, of which Gata3 is a mediator. We believe our work has implications for both treatment and control of infectious disease

    The potential role of cost-utility analysis in the decision to implement major system change in acute stroke services in metropolitan areas in England

    BACKGROUND: The economic implications of major system change are an important component of the decision to implement health service reconfigurations. Little is known about how best to report the results of economic evaluations of major system change to inform decision-makers. Reconfiguration of acute stroke care in two metropolitan areas in England, namely London and Greater Manchester (GM), was used to analyse the economic implications of two different implementation strategies for major system change. METHODS: A decision analytic model was used to calculate difference-in-differences in costs and outcomes before and after the implementation of two major system change strategies in stroke care in London and GM. Values in the model were based on patient level data from Hospital Episode Statistics, linked mortality data from the Office of National Statistics and data from two national stroke audits. Results were presented as net monetary benefit (NMB) and using Programme Budgeting and Marginal Analysis (PBMA) to assess the costs and benefits of a hypothetical typical region in England with approximately 4000 strokes a year. RESULTS: In London, after 90 days, there were nine fewer deaths per 1000 patients compared to the rest of England (95% CI -24 to 6) at an additional cost of £770,027 per 1000 stroke patients admitted. There were two additional deaths (95% CI -19 to 23) in GM, with a total costs saving of £156,118 per 1000 patients compared to the rest of England. At a £30,000 willingness to pay the NMB was higher in London and GM than the rest of England over the same time period. The results of the PBMA suggest that a GM style reconfiguration could result in a total greater health benefit to a region. Implementation costs were £136 per patient in London and £75 in GM. CONCLUSIONS: The implementation of major system change in acute stroke care may result in a net health benefit to a region, even one functioning within a fixed budget. The choice of what model of stroke reconfiguration to implement may depend on the relative importance of clinical versus cost outcomes

    The Development and Validation of the Thai-Translated Irrational Performance Beliefs Inventory (T-iPBI)

One of the most commonly employed cognitive-behavioural approaches to psychotherapy is rational-emotive behaviour therapy, but researchers have been troubled by some of the limitations of irrational beliefs psychometrics. As a result, Turner et al. (Eur J Psychol Assess 34:174–180, 2018a. https://doi.org/10.1027/1015-5759/a000314) developed the Irrational Performance Beliefs Inventory (iPBI), a novel measure of irrational beliefs for use within performance domains. However, the linguistic and cross-cultural adaptation of the iPBI into other languages is necessary for its multinational and multicultural use. The purpose of this paper is to develop the Thai-translated version of the iPBI (T-iPBI) and examine the validity and reliability of the T-iPBI. Data retrieved from 166 participants were analysed using SPSS and AMOS software packages. Thirty-three participants completed two follow-up T-iPBI measurements (1- and 3-week repeat assessment). After the linguistic and cross-cultural adaptation processes, the T-iPBI demonstrated excellent levels of reliability, with internal consistency and test–retest reliability, as well as construct, concurrent, and predictive validity. The current findings indicate that the 20-item T-iPBI can be used as a self-assessment instrument to evaluate individual’s irrational performance beliefs in a Thai population. We also highlight the implications of this study and suggest a variety of future research directions that stem from the results

    Chemotaxis When Bacteria Remember: Drift versus Diffusion

    {\sl Escherichia coli} ({\sl E. coli}) bacteria govern their trajectories by switching between running and tumbling modes as a function of the nutrient concentration they experienced in the past. At short time one observes a drift of the bacterial population, while at long time one observes accumulation in high-nutrient regions. Recent work has viewed chemotaxis as a compromise between drift toward favorable regions and accumulation in favorable regions. A number of earlier studies assume that a bacterium resets its memory at tumbles -- a fact not borne out by experiment -- and make use of approximate coarse-grained descriptions. Here, we revisit the problem of chemotaxis without resorting to any memory resets. We find that when bacteria respond to the environment in a non-adaptive manner, chemotaxis is generally dominated by diffusion, whereas when bacteria respond in an adaptive manner, chemotaxis is dominated by a bias in the motion. In the adaptive case, favorable drift occurs together with favorable accumulation. We derive our results from detailed simulations and a variety of analytical arguments. In particular, we introduce a new coarse-grained description of chemotaxis as biased diffusion, and we discuss the way it departs from older coarse-grained descriptions.Comment: Revised version, journal reference adde
