    Parallel and overlapping Human Immunodeficiency Virus, Hepatitis B and C virus Infections among pregnant women in the Federal Capital Territory, Abuja, Nigeria

    Background: Risk factors that are associated with HIV infection are also associated with HBV and HCV infections in sub-Saharan Africa. The HIV-infected pregnant cohort represents a unique population and infection with the hepatitis virus is considered a public health problem worldwide. Objective: The purpose of this study was to evaluate the prevalence of Human Immunodeficiency Virus, Hepatitis B and C virus parallel and overlapping infections among pregnant women attending antenatal clinics in Federal Capital Territory (FCT), Abuja. Method: Five hundred (500) blood samples were collected from three district hospitals in the FCT and tested at Wuse General Hospital, Abuja for the presence of antibodies to HIV and Hepatitis C virus, and HBsAg by ELISA technique in accordance with the manufacturer’s instructions. HIV seropositive sera were confirmed by Western blot. Result: Of the 500 pregnant women, those detected with HIV antibodies, HBsAg and anti-HCV antibodies were 42 (8.4%), 19 (3.8%) and 8 (1.6%) respectively. The overall seroprevalence of HIV and HBV or HCV co-infection was 9.5% while 7.1% and 2.4% HIV positive pregnant women were specifically co-infected with HBV and HCV respectively. Those within the age bracket of 15-20 years had the highest prevalence of HIV (13.4%), HBV (5.1%) and HCV (1.9%) infections. Among the occupation characteristics of the women, those of them involved in trading recorded the highest prevalence of HIV (60.6%), HBV (30.3) and HCV (6.1%). HIV was higher among the married women than the singles ((8.6% vs 6.5%); with HBV infection the reverse was the case (3.0% vs 9.8%) while HCV was same for both groups. History of blood transfusion did not reflect a higher rate of HIV and HBV (1.4% vs 9.6%; 2.8% vs 4.0% respectively) unlike HCV infection with 0.5% recorded only among those that had transfusion experience. Conclusion: When monitoring the risk of hepatotoxicity to antiretroviral drugs among these group of patients caution should be maintained. Moreover, evidence of parallel and overlapping HIV, HBsAg and HCV infections among this cohort should motivate inclusion of HBV and HCV among the diseases of surveillance in the national sentinel survey in order to ascertain the bigger picture of these infections in Nigeria

    The Impact of Immunization Control Activities on Measles Outbreaks in Akwa Ibom State, South-South, Nigeria

    Background: The increase of vaccination rates means that fewer children will be vulnerable to vaccine preventable diseases such as measles, which will invariably result in a drop in the infant mortality and morbidity rates. Objective: To assess the impact of the implementation of measles reduction strategies from 2006 to 2008 using the quarterly national program for immunization (NPI) in Akwa Ibom state, Nigeria. Method: Following informed consent, individuals presenting with febrile rash illnesses were routinely bled and tested for measles specific IgM using commercially available ELISA kit-MV-ELISA (Enzygnost; Behring Diagnostics, Marburg, Germany) in accordance with the manufacturer’s instructions. Results: A total of four hundred and four individuals comprising of 216 vaccinated and 188 unvaccinated, presenting with febrile rash illness were screened for measles specific IgM antibodies as indication of active infection between January 2006 and December 2008 out of which 122 (30.2%) had detectable levels of measles antibodies. Among the vaccinated and unvaccinated groups, 32 (14.8%) and 90 (47.9%) respectively were detected with measles IgM antibodies. The highest and lowest antibody levels were detected in 2006 (vaccinated: 54.7%; unvaccinated: 78.4%) and 2008 (vaccinated: 1.2%; unvaccinated: 12%) respectively. The distribution of measles burden by year show an overall decline in prevalence from 70% in 2006, 8.9% in 2007 to 3.7% in 2008. While, children under the age of 5 similarly had a decline in measles incidence of 73.3%, 10.7% and 3.3% respectively. Sex distribution of infection within the 3-year period shows that more females (37.4%) than males (21.2%) expressed measles IgM antibodies, and active infection was detected more in the rural (31.4%) than urban area (27.7%). However, findings indicate a tremendous decline in active infection in the rural areas from 67% in 2006 to 0% in 2008, and in the urban areas from 78% in 2006 to 9.3% in 2008 among both vaccinated and unvaccinated groups. A highly significant reduction in measles infection was observed more among males than females (P=0.009). Infection distribution by location did not show any significant difference (P=0.65) even though more individuals in the rural areas were noted with active infection. Conclusion: The study shows a highly significant reduction in measles burden among vaccinated individuals (P=0.0001) and invariably increases protective coverage of measles vaccination most especially among children under 5 years of age (P=0.0066) in the state. Findings justify effort by government and WHO in carrying immunization campaigns in children, 5-years and below irrespective of vaccination status and experience

    Influence of intercrops on pests’ populations in upland rice (Oriza sativa L.)

    We conducted a 3 x 4 factorial experiment fitted into a randomized complete block design, using three intercrops: cowpea (Vigna unguiculata L. Walp) at 0, 50,000,  100,000, 150,000 plants/ha; groundnut (Arachis hypogea L.) at 100,000,  200,000, 300,000 plants/ha; and egusi-melon (Colocynthis vulgaris L.) at 10,000, 20,000, and 30,000 plants/ha to evaluate their influence on incidence of stem  borer (Chilo zacconius) (Blesz) and the green stink bug (Nezara viridula (L.)) in upland rice (Oryza sativa L.) .The experiment was conducted during the wet and dry seasons of 2004. Rice/cowpea recorded the highest incidence of N. viridula (3.1, 3.3%) compared to rice/groundnut (1.3, 1.3%) and rice/egusi, (1.8, 1.8%) during the wet and dry seasons respectively. Intercrop with groundnut significantly (p   0.05) reduced stem borer (C. zacconius) incidence to 7.4 and 13.2% respectively for wet and dry season cultivation compared with the control (12.0 and 18.0%). Rice and groundnut (100,000 - 200,000 plants/ha) intercrop is recommended for  reduced incidence of C. zacconius and N. viridula. This result demonstrates that a careful selection of crop combination and plant population could lead to reduced  insect pests’ incidence in upland rice.Key words: intercrops, plant populations, pest incidence, Oryza sativa, Chilo  zacconius, Nezara viridul

    Assessing Participation in Secondary Education Quality Enhancement: Teachers, Parents and Communities in Cross River State

    The study examined some factors responsible for low participation of teachers, parents and communities in quality enhancement of secondary education in Cross River State, Nigeria. Teachers and parents of students in public secondary schools were used. A total of 8,590 subjects constituted the population of the study. However, only 840 teachers and 100 parents i.e. 940 subjects formed the sample from where data were obtained through questionnaires and interview methods respectively. Twenty (20) indicators of reasons for low participation of stakeholders in school affairs were built into the questionnaire. A survey design was adopted and multistage sampling technique was used. Teststatistics used for data analysis were weighted mean (X) and standard deviation (SD), and a mean score of 2.5 and above formed the level of significance/acceptance. The findings indicated poor motivation of teachers, inadequate school facilities, unrealistic pupils assessment, poor knowledge of subject matter and inadequate supervision as teachers factors, while large family size, state and national ailing economy, low moral values and philosophy of the end justifies the means were reasons for low parents and communities involvement in secondary education-quality improvement. It was recommended that the quality of instruction in teachers training institutions be improved, as well as intense public enlightenment programme were among the recommendations made.Keywords: Participation, Quality, Enhancement, Parents-teachers Association and Communitie

    Beliefs And Practice Concerning Pregnancy Delivery And Puerperium In Rural Akwa Ibom State

    Focus group discussions involving 100 rural women in five communities of Akwa Ibom State, Nigeria were carried out over a six-month period. The aim was to determine if the beliefs and practices of our rural dwellers are such that contribute to maternal morbidity. The discussions revealed that the participants had a shallow knowledge of the causes of complications in pregnancy. Complications were largely attributed to spirits and other harmless events. Reducing physically demanding activities in pregnancy was ascribed to laziness and sexual intercourse was largely to be avoided. There was no food taboos specific to pregnancy, but food restrictions were described. Antenatal and intrapartum care by spiritual and traditional midwives was considered superior to orthodox care. Practices by the midwives include turning a baby in an abnormal lie, massaging the vagina with oil in labour; conducting delivery without gloves, pulling on the cord, manually removing or squeezing out the placenta if delivery is not spontaneous. Management of complications involves divinations, use of herbs and other concoctions, anointing oil, prayers and fasting. Puerperal seclusion practiced to varying degrees and maybe beneficial. Most beliefs and practices of our rural women are potential contributors to maternal morbidity. A few are harmless or even beneficial. Public enlightenment and education of our rural dwellers while incorporating the harmless/beneficial practices into modern obstetric care services will help reduce maternal morbidity.     Demographic and living conditions of tuberculosis patients receiving directly observed therapy short course(DOTS) in Calabar, Nigeria

    Understanding the role of poverty in the continued epidemic of TB in developing countries will provide useful information for the improvement of TB control activities in these areas. A survey of the living conditions of 274 TB patients registered for DOTS at the National Tuberculosis Control Program Centre of the Lawrence Henshaw Memorial hospital Calabar and the Endemic Disease Clinic of the University of Calabar Teaching Hospital, Calabar was conducted to verify the view that poverty influences the incidence and outcome of TB. Using a structured pre-tested self- or interview- administered questionnaire, data were collected from a sample of 274 subjects: males (162/59%) and females (112/41%) within the economically viable age of 15-55 years. Results showed that the number of TB patients in the study decreased with increasing number of children of .3 in the households of subjects. The average income of the 244 (89%) income earners was N6,045.00 (46.50)amonth.Ofthisnumber,170(7046.50) a month. Of this number, 170 (70%) earned less than N5,000 (37.00) monthly. Thirty (11%) had no reliable means of income. Unemployed persons and students constituted the highest percentage of the subjects (18% and 17% respectively). A low socio-economic status and poor housing conditions typified by overcrowding and poor ventilation characterized the TB patients in this study. TB control programs anchored by government and non-governmental agencies need to address poverty reduction as part of their intervention strategies

    Phytochemical Screening and Antibacterial Activities of Aframomum melegueta (K. Schum) Seed Extracts on Salmonella typhi and Klebsiella pneumoniae

    The phytochemical screening and antibacterial effects of Aframomum melegueta (K. Schum)on Salmonella typhi and Klebsiella pneumoniae was carried out. The phytochemical screening revealed the presence of alkaloids, flavonoids, tannins, saponins, steroids, anthraquinones, terpenoids, glycosides and phenols in the seed extract. The susceptibility test showedzones of inhibition (ZOI) of S. typhi (11.0mm) and K. pneumonia (13.0mm) with methanolic seed extract (MSE), while the hot aqueous seed extract (HASE) recorded 3.0mmas ZOI for both test organisms. The Minimum inhibitory concentration (MIC) of 200mg/ml and 100mg/ml were recorded against S. typhi and K. pneumoniae respectively with MSE, while HASE indicated 400mg/ml and 200mg/ml MICs against S. typhi and K. pneumoniae respectively. The Minimum Bactericidal Concentration (MBC) obtained from MSE were 200 mg/ml and 25mg/ml for S. typhiand K. pneumoniae, while HASE had > 400mg/ml for test microbes. The effectiveness of different concentrations of the test plant extracts on the test organisms was significant (P<0.05).     Assessment of Biodiesel Fuel Potentials of Seed Crude Oil Extracts of Balanites aegyptiaaca (L.) Del

    Study on assessmentof biodiesel fuel potentials of seed crude oil extracts of Balanites aegyptiaaca (L.) Del was carriedout. Standard methods of the Association of Official and Analytical Chemist (AOAC) were adopted to evaluate the proximate, physico-chemical properties and fatty acid compositions of crude seed oil extracts of the test plant. The proximate constituents of the crude seed oil extract gave crude protein (22.09%), crude fat (56.75%), moisturecontent (1.35%), ash (4.70%), crudefiber (12.75%) and carbohydrate (2.36%). The crude oil physicochemical properties included saponification value(216.439mgKOH/g), peroxide value(4.84meq/kg), acid value(2.18mgKOH/g), iodine value(77.08g/100g), viscosity value(150.3@30\ub0C) and cetane number(54.08), refractive index(1.487 @30\ub0C), relative density (0.949g/cm3) while calorific value was 39.03(MJ/kg). The fatty acids composition of crude kernel oil extract of B. aegyptiaca indicated the presence of four (4) fatty acids, with relative percentage abundance (RPA) in the order of 67.17% (9,12-Octadecanoic acid (C19H3402)) > 16.22% (Pentadecanoic acid (C17H3402)) > 11.8kg% (Heptacosanoic acid (C28H5602)) > 4.72% (Oleic acid(C18H3402)). These properties conferred relative prospects on the crude oil of the test plant as a suitable potential biodiesel substrate and consequently, large scale aforestation efforts be renewed, to guarantee ready availability of the raw materials

    Resistive Exercise for Arthritic Cartilage Health (REACH): A randomized double-blind, sham-exercise controlled trial

    <p>Abstract</p> <p>Background</p> <p>This article provides the rationale and methodology, of the first randomised controlled trial to our knowledge designed to assess the efficacy of progressive resistance training on cartilage morphology in women with knee osteoarthritis.</p> <p>Development and progression of osteoarthritis is multifactorial, with obesity, quadriceps weakness, joint malalignment, and abnormal mechanical joint forces particularly relevant to this study. Progressive resistance training has been reported to improve pain and disability in osteoarthritic cohorts. However, the disease-modifying potential of progressive resistance training for the articular cartilage degeneration characteristic of osteoarthritis is unknown. Our aim was to investigate the effect of high intensity progressive resistance training on articular cartilage degeneration in women with knee osteoarthritis.</p> <p>Methods</p> <p>Our cohort consisted of women over 40 years of age with primary knee osteoarthritis, according to the American College of Rheumatology clinical criteria. Primary outcome was blinded measurement of cartilage morphology via magnetic resonance imaging scan of the tibiofemoral joint. Secondary outcomes included walking endurance, balance, muscle strength, endurance, power, and velocity, body composition, pain, disability, depressive symptoms, and quality of life.</p> <p>Participants were randomized into a supervised progressive resistance training or sham-exercise group. The progressive resistance training group trained muscles around the hip and knee at 80% of their peak strength and progressed 3% per session, 3 days per week for 6 months. The sham-exercise group completed all exercises except hip adduction, but without added resistance or progression. Outcomes were repeated at 3 and 6 months, except for the magnetic resonance imaging scan, which was only repeated at 6 months.</p> <p>Discussion</p> <p>Our results will provide an evaluation of the disease-modifying potential of progressive resistance training for osteoarthritis.</p> <p>Trial Registration</p> <p>ANZCTR Reference No. 12605000116628</p

    Preparation and use of plant medicines for farmers' health in Southwest Nigeria: socio-cultural, magico-religious and economic aspects

    Agrarian rural dwellers in Nigeria produce about 95% of locally grown food commodities. The low accessibility to and affordability of orthodox medicine by rural dwellers and their need to keep healthy to be economically productive, have led to their dependence on traditional medicine. This paper posits an increasing acceptance of traditional medicine country-wide and advanced reasons for this trend. The fact that traditional medicine practitioners' concept of disease is on a wider plane vis-Ă -vis orthodox medicine practitioners' has culminated in some socio-cultural and magico-religious practices observed in preparation and use of plant medicines for farmers' health management. Possible scientific reasons were advanced for some of these practices to show the nexus between traditional medicine and orthodox medicine. The paper concludes that the psychological aspect of traditional medicine are reflected in its socio-cultural and magico-religious practices and suggests that government should fund research into traditional medicine to identify components of it that can be integrated into the national health system
