    Alexithymia in anorexia and bulimia: Ubiquitous and primary trait?

    Background: Alexithymia is associated to Eating Disorders (ED) and relevant for their prognosis but it is uncertain if it is ubiquitous, primary, and necessary for ED outburst. Methods: 124 ED outpatients and 80 healthy controls were compared with the Toronto Alexithymia Scale, personality, and psychopathology measures. Alexithymia and the other features are compared between anorexia nervosa (AN), bulimia nervosa (BN) and control groups. Alexithymia-based clusters were explored and compared with controls. Alexithymia traits are correlated with the other features. Results: Difficulty in identifying feelings was ubiquitous in ED subjects. A Non-Alexithymic Cluster (NAC) and an Alexithymic Cluster (ALC) were found with no difference in AN/BN distribution. ALC displays disordered personality and high psychopathology. Self-directiveness and interoceptive awareness were independently related to alexithymia and to depressive feelings. These two features along with depressive features completely accounted for alexithymia variance. Conclusion: Even though the difficulty in identifying feelings is ubiquitous in ED subjects, Alexithymia may not represent a primary trait but a complex dysfunction consequent to co-occurring character immaturity, altered interoceptive awareness, and depressive traits

    Design of electron beam bending magnet system for electron and photon therapy: A simulation approach

    The doubly achromatic electron beam bending magnet system using two sector magnets has been designed for the medical applications to treat the cancer. The aim of electron beam bending magnet system is to focus an electron beam having a spot size less than 3 mm × 3 mm, energy spread within 3% and divergence angle ≤ 3mrad at the target position. To achieve these parameters, the simulation has been carried out using Lorentz-3EM software. The beam spot, divergence angle and energy spread have been observed with respect to the variation in angles of sector magnets and drift distance. Based on the simulated results, it has been optimized that the first and second magnet has an angle 206° and 35° and the drift distance 80 mm. It is also observed that at the 1125, 1762, 2570, 3265 and 4155 Amp-turn, the optimized design produces 3369, 4972, 6384, 7584 and 9568 Gauss of magnetic field at median plane which require to bend 6, 9, 12, 15 and 18 MeV energy of electron, respectively, for the electron therapy application. The output beam parameters of the optimized design are energy spread ±3%, divergence angle ~3 mrad and spot size 2.6 mm. Moreover, for 6 MV and 15 MV photon therapy applications, an electron beam of energy 6.5 MeV and 15.5 MeV extracted from magnet system and focused on the bremsstrahlung target. Various materials have been studied for photon generation using Monte Carlo based Fluka code and Tungsten material has been optimized as bremsstrahlung target which produces continuous energy bremsstrahlung spectrum. For the photon therapy, the 1233 and 3327 amp-turn, in an optimized design produces 3616 and 7785 Gauss of magnetic field at median plane require to bend 6.5 and 15.5 MeV energy of electron, respectively, which further produces bremsstrahlung radiation from Tungsten target

    Decisional Needs of People From Minority Ethnic Groups Around Living Donor Kidney Transplantation: A UK Healthcare Professionals’ Perspective

    Despite improved patient and clinical outcomes, living donor kidney transplantation is underutilized in the United Kingdom, particularly among minority ethnic groups, compared to deceased donor kidney transplantation. This may in part be due to the way in which kidney services present information about treatment options. With a focus on ethnicity, semi structured interviews captured the views of 19 kidney healthcare professionals from two renal centres in West Yorkshire, about the decisional needs and context within which people with advanced kidney disease make transplant decisions. Data were analysed using thematic analysis. Themes were categorized into three groups: 1) Kidney healthcare professionals: language, cultural awareness, trusted personnel, and staff diversity, 2) Patient information resources: timing and setting of education and suitability of patient-facing information and, 3) People with advanced kidney disease: knowledge, risk perception, and cultural/religious beliefs. To our knowledge, this is the first study in the United Kingdom to investigate in depth, healthcare professionals’ views on living donor kidney transplantation decision making. Six recommendations for service improvement/delivery to support decision making around living donor kidney transplantation among minority ethnic groups are described

    Fault diagnosis of wind turbine gearboxes through on-site measurements and vibrational signal processing

    Condition monitoring of wind turbine gearboxes has attracted an impressive amount of attention in the wind energy literature. This happens for practical issues, as gearbox damages account for at least the 20% of wind turbines operational unavailability, and for scientific issues as well, because the condition monitoring of gear-based mechanical systems undergoing non-stationary operation is particularly challenging. The present work is devoted to the diagnosis of gearbox damages through a novel approach, designed exclusively for this study, based on on-site measurements and data post-processing. The main point of this method is the relatively easy repeatability, also for wind turbine practitioners, and its low impact on wind turbine operation: actually, the measuring site is not the gearbox itself, but the tower, further from the gearbox but in an easily accessible place. A real test case has been considered: a multi mega-watt wind turbine sited in Italy and owned by the Renvico company. The vibration measurements at the wind turbine suspected to be damaged and at a reference wind turbine are processed through a multivariate Novelty Detection algorithm in the feature space. The application of this algorithm is justified by univariate statistical tests on the time-domain features selected and by a visual inspection of the dataset via Principal Component Analysis. Finally, the novelty indices based on such time-domain features, computed from the accelerometric signals acquired inside the turbine tower, prove to be suitable to highlight a damaged condition in the wind-turbine gearbox, which can be then successfully monitored
