    Going above and beyond for implementation: the development and validity testing of the Implementation Citizenship Behavior Scale (ICBS).

    BackgroundIn line with recent research on the role of the inner context of organizations in implementation effectiveness, this study extends research on organizational citizenship behavior (OCB) to the domain of evidence-based practice (EBP) implementation. OCB encompasses those behaviors that go beyond what is required for a given job that contribute to greater organizational effectiveness. The goal of this study was to develop and test a measure of implementation citizenship behavior (ICB) or those behaviors that employees perform that go above and beyond what is required in order to support EBP implementation.MethodsThe primary participants were 68 supervisors from ten mental health agencies throughout California. Items measuring ICB were developed based on past research on OCB and in consultation with experts on EBP implementation in mental health settings. Supervisors rated 357 of their subordinates on ICB and implementation success. In addition, 292 of the subordinates provided data on self-rated performance, attitudes towards EBPs, work experience, and full-time status. The supervisor sample was randomly split, with half used for exploratory factor analyses and the other half for confirmatory factor analyses. The entire sample of supervisors and subordinates was utilized for analyses assessing the reliability and construct validity of the measure.ResultsExploratory factor analyses supported the proposed two-factor structure of the Implementation Citizenship Behavior Scale (ICBS): (1) Helping Others and (2) Keeping Informed. Confirmatory factor analyses with the other half of the sample supported the factor structure. Additional analyses supported the reliability and construct validity for the ICBS.ConclusionsThe ICBS is a pragmatic brief measure (six items) that captures critical behaviors employees perform to go above and beyond the call of duty to support EBP implementation, including helping their fellow employees on implementation-related activities and keeping informed about issues related to EBP and implementation efforts. The ICBS can be used by researchers to better understand the outcomes of improved organizational support for implementation (i.e., implementation climate) and the proximal predictors of implementation effectiveness. The ICBS can also provide insight for organizations, practitioners, and managers by focusing on key employee behaviors that should increase the probability of implementation success

    BackgroundAlthough the importance of the organizational environment for implementing evidence-based practices (EBP) has been widely recognized, there are limited options for measuring implementation climate in public sector health settings. The goal of this research was to develop and test a measure of EBP implementation climate that would both capture a broad range of issues important for effective EBP implementation and be of practical use to researchers and managers seeking to understand and improve the implementation of EBPs.MethodsParticipants were 630 clinicians working in 128 work groups in 32 US-based mental health agencies. Items to measure climate for EBP implementation were developed based on past literature on implementation climate and other strategic climates and in consultation with experts on the implementation of EBPs in mental health settings. The sample was randomly split at the work group level of analysis; half of the sample was used for exploratory factor analysis (EFA), and the other half was used for confirmatory factor analysis (CFA). The entire sample was utilized for additional analyses assessing the reliability, support for level of aggregation, and construct-based evidence of validity.ResultsThe EFA resulted in a final factor structure of six dimensions for the Implementation Climate Scale (ICS): 1) focus on EBP, 2) educational support for EBP, 3) recognition for EBP, 4) rewards for EBP, 5) selection for EBP, and 6) selection for openness. This structure was supported in the other half of the sample using CFA. Additional analyses supported the reliability and construct-based evidence of validity for the ICS, as well as the aggregation of the measure to the work group level.ConclusionsThe ICS is a very brief (18 item) and pragmatic measure of a strategic climate for EBP implementation. It captures six dimensions of the organizational context that indicate to employees the extent to which their organization prioritizes and values the successful implementation of EBPs. The ICS can be used by researchers to better understand the role of the organizational context on implementation outcomes and by organizations to evaluate their current climate as they consider how to improve the likelihood of implementation success

    Superoxide Dismutase (SOD) Enzyme Activity Assay in Fasciola spp. Parasites and Liver Tissue Extract

    Background: The purpose of this comparative study was to detect superoxide dismutase (SOD) activities in Fasciola hepatica, F. gigantica parasites, infected and healthy liver tissues in order to determine of species effects and liver infection on SODs activity level.Methods: Fasciola spp. parasites and sheep liver tissues (healthy and infected liver tissues), 10 samples for each, were collected, homogenized and investigated for protein measurement, protein detection and SOD enzyme activity assay. Protein concentration was measured by Bradford method and SODs band protein was detected on SDS-PAGE. SODs activity was determined by iodonitrotetrazolium chloride, INT, and xanthine substrates. Independent samples t-test was conducted for analysis of SODs activities difference.Results: Protein concentration means were detected for F. hepatica 1.3 mg/ ml, F. gigantica 2.9 mg/ml, healthy liver tissue 5.5 mg/ml and infected liver tissue 1.6 mg/ml (with similar weight sample mass). Specific enzyme activities in the samples were obtained 0.58, 0.57, 0.51, 1.43 U/mg for F. hepatica, F. gigantica, healthy liver and infected liver respectively. Gel electrophoresis of Fasciola spp. and sheep liver tissue extracts revealed a band protein with MW of 60 kDa. The statistical analysis revealed significant difference between SOD activities of Fasciola species and also between SOD activity of liver tissues (P<.05).Conclusion: Fasciola species and liver infection are effective causes on SOD enzyme activity level

    Infection of some species of fresh water fishes to parasitic worms in Khouzestan Province, Iran

    During a survey on parasitic worms infection of local freshwater fishes in years 1994-95 in Khouzestan province wetlands, 206 individuals of fishes (62 individuals of Barbus luteus Heckel, 1843; 30 individuals of Cyprinus carpio L.; 6 individuals of Silirus triostegus Heckel, 1843; 92 individuals of Liza abu Heckel, 1843 and 15 individuals of Aspius vorax Heckel, 1843 of Hoor-al-azim and Hoor-shadegan were catched and they were examined. One of 62 B. luteus and two of 3 C. carpio were infected to Bothriocephalus opsariichthys Yamaguti, 1943 and two of 6 S. triostegus were infected to proteocephalus sp. Weinland, 1858. In this study, frequency of infection in B. luteus and C. carpio to B. opsariichthys was 1.6% and 6.6% respectively, and in S. triostegus was 33.3% to proteocephalus sp. One of 15 A. vorax and 51 of 93 L. abu were infected to Neoechinorhynchus tylosuri. In this study female and male parasites were collected from fish intestine. Frequency of infection in A. vorax and L abu were 6.6% and 54.8% respectively. This is the first record of N. tylosuri infection in A. vorax in Iran

    "nBackground: Fascioliasis is a chronic hepatic disease and may be resulted from mechani­cal/molecular parasite adhesion to host liver tissue. The aim of this study was to detect surface car­bohydrate and lectin, carbohydrate-binding protein isolation that might be responsible of this molecular binding."nMethods: The present experimental work was conducted in the Department of Medical Parasitol­ogy and Mycology, School of Public Health, Tehran University of Medical Sciences, Te­hran, Iran.  Fasciola hepatica parasites were collected from abattoir (Saman, Tehran, Iran) and surface mannose-carbohydrate was detected by fluorescein isothiocyanate (FITC) conju­gated lectin (Lentil). Lectin of tegumental tissue from F. hepatica was isolated by affinity chroma­tography and detected by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE)."nResults: Mannose carbohydrate was observed on the surface of tegumental tissue from para­site under fluorescence microscope.     Haemolymph Components of Infected & None Infected Lymnaea snails with Xiphidiocercariae

    Background: In this study the haemolymph components of infected and none infected Lymnaea gedrosiana with xiphidiocercaria larvae was compared.Methods: Five hundred Fifty Lymnaea snails were collected from Ilam and Mazandaran prov­inces, Iran, during 2008-2009. The snails were transported to the lab at Tehran University of Medi­cal Sciences and their cercarial sheddings were studied. Haemolmyphs of snails were ex­tracted and cells were counted using haemocytometer and cell-surface carbohydrate were recog­nized by conjugated lectin (Lentil). Haemolymph protein concentrations were measured by Brad­ford protein assay method and soluble protein compositions were determined on sodium dodecyl sulphate polyacrilamide gel electrophoresis (SDS-PAGE).Result: From the 550 examined Lymnaea snails for cercariae, 27 snails were infected with xiphidiocer­cariae. Mean of haemolymph cells (haemocyte) number were obtained 93480±2.43 (cells/ml) for none infected snails (25 snail) and 124560±2800 (cells/ml) for infected snails (25 snail). Mannose carbohydrate was recognized on haemocyte of none infected and infected snails. Mean of protein concentration of haemolymph plasma was obtained as 1354 ± 160 μg/ml (1.4 mg/ml) for none infected snails (25 snails) and 1802±138 μg/ml (1.8 mg/ml) for infected snail (25 snails). Comparing to none infected snails, the SDS-PAGE results of haemolymph plasma of infected snails, showed an extra protein band (70 kDa). The results showed a significant differ­ence between the amounts and the kinds of proteins in haemolymph of infected and none infected snails.Conclusion: This information might be useful to understand of parasite detection, adhesion, engulf­ment and antigen agglutination by snail

    The implementation leadership scale (ILS): development of a brief measure of unit level implementation leadership

    BACKGROUND: In healthcare and allied healthcare settings, leadership that supports effective implementation of evidenced-based practices (EBPs) is a critical concern. However, there are no empirically validated measures to assess implementation leadership. This paper describes the development, factor structure, and initial reliability and convergent and discriminant validity of a very brief measure of implementation leadership: the Implementation Leadership Scale (ILS). METHODS: Participants were 459 mental health clinicians working in 93 different outpatient mental health programs in Southern California, USA. Initial item development was supported as part of a two United States National Institutes of Health (NIH) studies focused on developing implementation leadership training and implementation measure development. Clinician work group/team-level data were randomly assigned to be utilized for an exploratory factor analysis (n = 229; k = 46 teams) or for a confirmatory factor analysis (n = 230; k = 47 teams). The confirmatory factor analysis controlled for the multilevel, nested data structure. Reliability and validity analyses were then conducted with the full sample. RESULTS: The exploratory factor analysis resulted in a 12-item scale with four subscales representing proactive leadership, knowledgeable leadership, supportive leadership, and perseverant leadership. Confirmatory factor analysis supported an a priori higher order factor structure with subscales contributing to a single higher order implementation leadership factor. The scale demonstrated excellent internal consistency reliability as well as convergent and discriminant validity. CONCLUSIONS: The ILS is a brief and efficient measure of unit level leadership for EBP implementation. The availability of the ILS will allow researchers to assess strategic leadership for implementation in order to advance understanding of leadership as a predictor of organizational context for implementation. The ILS also holds promise as a tool for leader and organizational development to improve EBP implementation

    The Role of Leadership in Creating a Strategic Climate for Evidence-Based Practice Implementation and Sustainment in Systems and Organizations

    There is a growing impetus to effectively implement evidence-based practices (EBPs) in health and allied health settings in order to improve the public health impact of such practices. To support implementation and sustainment of EBPs, it is important to consider that health care is delivered within the outer context of public health systems and the inner context of health care organizations and work groups (3). This article identifies two relevant types of leadership for implementation and recommends steps that leaders can take in developing a strategic climate for EBP implementation and sustainment within the outer and inner contexts of health and allied health care systems and organizations

    Fish Anisakidae Helminthes in KHuzestan Province, South West of Iran

    Fish including; Barbus spp, Cyprinus carpio, Liza abu and Aspius vorax have very important role in the economic condition of the rural areas of Khuzestan province. These fish have been consumed as fried or roasted. Inadequately cooked fish, could be served as a source of infection in these communities. For this reasons, 701 fish were trapped from 4 lagoons (Atash, Sobhanieh, Al-hai, Houfel) and transported alive to Ahwaz Health Research Center .Their skin, gills, eyes, muscles, intestine and body cavity were examined carefully. In 54 (7.7%) of fish,.6 cases of Contracaecum sp. (0.85%) and 48 Anisakis sp. (6.8%) were identified belong to helminth family of anisakidae. These results suggested that human anisakiasis could be health hazard in these areas
