Klinische Evaluation eines Ăberdruckventils fĂŒr Kindertuben mit Cuff
Zusammenfassung: Ziel: Zum sicheren Einsatz von Tuben mit Cuff bei Kindern ist eine Cuffdruckbegrenzung bei 20-25cmH2O von entscheidender Bedeutung. Ziel der Studie war es, die ZuverlĂ€ssigkeit und den Nutzen eines neuen CuffĂŒberdruckventils (Ăffnungsdruck 20cmH2O) in Kombination mit Tuben mit Cuff in der klinischen Anwendung bei Kindern zu evaluieren. Methodik: In einer prospektiven Studie wurde der Cuffdruck bei Kindern und Adoleszenten, die mithilfe von Tuben mit Cuff intubiert waren und eine Sevofluran-Lachgas-AnĂ€sthesie erhielten, protokolliert. Das Cuffdruckentlastungsventil wurde zwischen Cuffmanometer und Pilotballon geschaltet. Bei 25Patienten wurde die Messung vom initialen Ăffnungsdruck (GruppeA) aus und bei weiteren 25Patienten vom minimalen Dichtungsdruck (GruppeB) aus gestartet. Ergebnisse: FĂŒnfzig Patienten im Alter von 0,4 bis 17,8Jahren (Median 7,4Jahre) wurden in die Studie aufgenommen. Der Ăffnungsdruck (GruppeA) lag bei 19,7±0,8cmH2O und der minimale Dichtungscuffdruck (GruppeB) bei 11,7±2,9cmH2O (Variationsbreite 6-16cmH2O). Die mittleren Cuffdruckwerte in GruppeA stabilisierten sich bei 20,4±1,6cmH2O, wĂ€hrend sich die Cuffdruckwerte in GruppeB bei 16,5±3,3cmH2O einpendelten. Bei einem Patienten der GruppeA kam es zu einem Ăberschreiten der Cuffdruckgrenze von 25cmH2O; dies hatte eine manuelle Entlastung gemÀà Protokoll zur Folge. Insgesamt waren wĂ€hrend 103,1Messstunden 24NachfĂŒllmanöver (14 in GruppeA und 10 in GruppeB) wegen Druckabfalls mit hörbarer Luftleckage notwendig. Die meisten davon waren durch Druckerhöhungen bei der Umlagerung des Patienten bzw. nachfolgender Entlastung durch das Ăberdruckventil verursacht. "Low-dose"-Droperidol-Gabe bei Kindern: "Rescue"-Therapie gegen persistierende postoperative Ăbelkeit und Erbrechen
Zusammenfassung: Hintergrund: Persistierende postoperative Ăbelkeit und Erbrechen ("postoperative nausea and vomiting", PONV) trotz Prophylaxe ist in der KinderanĂ€sthesie ein wichtiges Problem. In der vorgestellten Studie wurde untersucht, ob die "Low-dose"-Droperidol-Gabe (10”g/kgKG) als Rescue-Therapie bei persistierender PONV effektiv ist. Patienten und Methoden: Im elektronischen Archiv der AnĂ€sthesieabteilung der UniversitĂ€ts-Kinderkliniken ZĂŒrich wurden von 2004 bis 2009 alle Patienten herausgefiltert, die im Aufwachraum Droperidol erhalten hatten. Es wurde ĂŒberprĂŒft, ob Low-dose-Droperidol gegen persistierende PONV wirksam ist und ob unerwĂŒnschte Nebenwirkungen aufgetreten sind. Ergebnisse: Daten von 144Patienten im Alter von 12,3Jahren [Interquartilabstand (IQR) 9,5 bis 15,2Jahre] konnten fĂŒr die Untersuchung verwendet werden. Bei 128Patienten (89%) war die Rescue-Therapie mit Low-dose-Droperidol effektiv. Bei 16Patienten (11%) erfolgte keine Besserung. Sedation war die einzige beobachtete Nebenwirkung bei 39Patienten (27%). Schlussfolgerung: Die Gabe von Low-dose-Droperidol erwies sich als effektive Rescue-Therapie bei Kindern, bei denen trotz prophylaktischer antiemetischer Therapie PONV persistiert
Intravenöse Midazolam-Ketamin-AnÀsthesie zur geschlossenen Reposition der Vorderarmfraktur bei Kindern: Bringt eine zusÀtzliche axillÀre Plexusblockade Vorteile?
Zusammenfassung: Hintergrund: Das Ziel dieser Studie war es zu vergleichen, ob der intravenöse Ketaminbedarf bei Midazolam-Ketamin-AnĂ€sthesie durch die Kombination mit einer axillĂ€ren Plexusblockade zur geschlossenen Reposition einer Vorderarmfraktur bei Kindern reduziert werden kann. Methoden: Mit dem EinverstĂ€ndnis der Ethikkommission wurde eine retrospektive Gruppenanalyse bei Kindern durchgefĂŒhrt, die in den Jahren 2000-2001 eine Midazolam-Ketamin-AnĂ€sthesie (GruppeA) oder in den Jahren 2002-2004 eine Midazolam-Ketamin-AnĂ€sthesie in Kombination mit einer axillĂ€ren Plexusblockade (GruppeB) zur geschlossenen Reposition einer Vorderarmfraktur erhielten. Der Bedarf an Ketamin und postoperativen Analgetika wurde erfasst. Die Daten der Gruppen wurden mit dem Mann-Whitney-U-Test (nichtnormalverteilte Daten) oder dem T-Test (normalverteilte Daten) und dem Ï2-Test verglichen (p<0,05). Ergebnisse: Insgesamt wurden 455Kinder (GruppeA: 225, GruppeB: 230) in die Studie aufgenommen. Der Bedarf an intravenösem Ketamin differierte statistisch nicht signifikant zwischen den beiden Gruppen (p=0,154). Der Ketaminbedarf in GruppeB wurde jedoch signifikant geringer, wenn das Zeitintervall zwischen dem Beginn der PlexusanĂ€sthesie und dem Beginn der Intervention mehr als 15min betrug (p<0,05). Patienten der GruppeB benötigten weniger Analgetika in der postoperativen Phase (p<0,01). Schlussfolgerung: Durch die Kombination der Midazolam-Ketamin-AnĂ€sthesie mit der axillĂ€ren Plexusblockade zur geschlossenen Reposition einer Vorderarmfraktur bei Kindern lieĂ sich der Bedarf an Ketamin in der klinischen Routine einer Notfallstation nicht reduziere
Intrathekale Opioidmedikation zur perioperativen Analgesie bei schwer behinderten Kindern mit WirbelsÀulenoperationen
Zusammenfassung: Hintergrund: An gesunden Kindern konnte gezeigt werden, dass intrathekal verabreichte Opioide eine sichere und effektive perioperative Analgesie bei wirbelsĂ€ulenchirurgischen Eingriffen ermöglichen. Das Ziel der vorliegenden Untersuchung ist es, ihre Anwendbarkeit bei schwer- und schwerstbehinderten Kindern fĂŒr wirbelsĂ€ulenchirurgische Eingriffe zu ĂŒberprĂŒfen. Methode: Mit Zustimmung der lokalen Ethikkommission wurden Patienten vom Status3 und 4 der Klassifikation der American Society of Anesthesiologists (ASA), die sich einem wirbelsĂ€ulenchirurgischen Eingriff unterziehen mussten, retrospektiv untersucht. ZusĂ€tzlich zur AllgemeinanĂ€sthesie mit Sevofluran oder i.v.-verabreichtem Propofol erhielten die Patienten vor der Operation 20”g/kgKG Morphin und 1,5”g/kgKG Sufentanil intrathekal. Nach Beendigung des Eingriffs wurde eine Nalbuphindauerinfusion gestartet. Der Bedarf an zusĂ€tzlichen intra- und postoperativen Analgetika, der Zeitpunkt der Extubation, die postoperativen Schmerz-Scores und die arteriellen Kohlendioxidpartialdruck- (paCO2)-Werte sowie das Auftreten unerwĂŒnschter Nebenwirkungen wurden untersucht. Ergebnisse: Es wurden 28Patienten im Alter von 2,8 bis 18,5Jahren (Median 11,6Jahre) untersucht. Unmittelbar im OP konnten 17Patienten extubiert werden; bei 11Patienten entschied man sich zu einer verzögerten Extubation. Bis auf einen Patienten mit postoperativer Massivtransfusion konnten alle innerhalb von 24h extubiert werden. Eine adĂ€quate postoperative Analgesie mit Schmerz-Scores â€3 wurde mithilfe der Kombination von intrathekal verabreichten Opioiden mit Nalbuphin postoperativ bei 26 von 28Patienten (93%) erreicht. Bei 2Patienten war der Wechsel auf eine i.v.-Morphin-Gabe erforderlich. Postoperative Ăbelkeit und Erbrechen ("postoperative nausea and vomiting", PONV), Pruritus und leichte Hypoventilation mit paCO2-Werten zwischen 5,2 bis 9,7kPa (Median 6,3kPa) waren die beobachteten Nebenwirkungen. Schlussfolgerung: Der Einsatz intrathekal verabreichter Opioide, ergĂ€nzt durch eine postoperative Nalbuphininfusion, erlaubt in den allermeisten FĂ€llen eine frĂŒhzeitige Extubation ohne persistierende Atemdepression und ist eine praktikable sowie effektive Methode zur postoperativen Analgesie bei schwer behinderten Kindern nach wirbelsĂ€ulenchirurgischen Eingriffe
Gastric emptying after overnight fasting and clear fluid intake: a prospective investigation using serial magnetic resonance imaging in healthy childrenâ
Background Current guidelines recommend preoperative fasting of 2 h for clear fluids, which is often exceeded in routine clinical practice. Existing data on residual gastric volumes in children do not consider fluid intake within <2 h and rely on the aspiration of gastric contents via a gastric tube. This study evaluated the emptying of clear fluids from the stomach using magnetic resonance imaging (MRI). Methods Healthy volunteers aged 6-14 years were asked to fast overnight. MRI scans to assess gastric volumes were obtained before and immediately after drinking 7 ml kgâ1 of diluted raspberry syrup and then every 30 min up to 120 min. Volumes were determined by a blinded investigator and indexed gastric fluid/air volumes (GFVw/GAVw) and half-life (t1/2) of GFVw course after clear fluid intake were calculated. Results Sixteen children, median age 9.2 (range 6.4-12.8) years, were investigated. Median (range) GFVw was 0.62 (0.15-0.97) ml kgâ1 before and 6.68 (4.77-7.78) ml kgâ1 immediately after fluid intake, and 2.92 (0.43-5.04), 1.27 (0.28-3.62), 0.42 (0.07-2.49), and 0.32 (0.04-1.13) ml kgâ1 30, 60, 90, and 120 min thereafter. Median GFVw declined exponentially (t1/2=26.1 min). Median individual t1/2 was 23.6 (range 17.9-47.8) min. GAVw showed considerable intra- and inter-individual variation. Clinical evaluation of cuff and tube tip position in a newly designed paediatric preformed oral cuffed tracheal tube
Background. To assess the adequacy of the position of the tracheal tube cuff and tracheal tube tip in the recently introduced preformed oral Microcuff paediatric endotracheal tube (PET) using the manufacturers recommendations for Microcuff tracheal tube size selection. Methods. With Hospital Ethics Committee approval and informed parental consent, the tracheas of children from birth to adolescence were orally intubated with the preformed oral Microcuff PET. First, the position of the tracheal tube's intubation depth mark in relation to the vocal cords was assessed. Second, the distance âtracheal tube tip-to-carina' was endoscopically measured with the patient supine and the head in a neutral position and the tube placed with the centre mark at the lower incisors or alveolar ridge. Results. A total of 166 children aged from 0.1 to 16.4 yr (median 5.9 yr) were studied. In five patients the intubation depth mark was above (5 mm each), in 22 patients at the level of and in the remaining 139 patients below the vocal cords. No endobronchial intubation occurred. In four patients the distance âtracheal tube tip-to-carina' was smaller than the safety margin to prevent endobronchial intubation during head-neck flexion. Conclusion. The new oral preformed cuffed tracheal tubes allow safe placement in almost all patients when inserted according to the tube bend. The critically low tube tip and the high cuff positions in a few tubes when placed according to the tube bend requires clinical alertnes
Effect of different quantities of a sugared clear fluid on gastric emptying and residual volume in children: a crossover study using magnetic resonance imaging
Background Gastric emptying in the first 2 h after 7 ml kgâ1 of sugared clear fluid has recently been investigated in healthy children using magnetic resonance imaging (MRI). This study aims to compare gastric volume and emptying half-life during 1 h after 3 or 7 ml kgâ1 sugared clear fluid intake. Methods Fourteen healthy volunteer children aged 11.1 (8.2-12.5) yr were investigated prospectively after administration of 3 and 7 ml kgâ1 diluted raspberry syrup in a randomized order, after overnight fasting (baseline). Gastric content volume (GCVw) was assessed with a 1.5 Tesla MRI scanner in a blinded fashion. Data are presented as median (range) and compared using the Wilcoxon test. Results Baseline GCVw was 0.39 (0.04-1.00) and 0.34 (0.07-0.75) before intake of 3 and 7 ml kgâ1 syrup, respectively (P=0.93). GCVw was 0.45 (0.04-1.55)/1.33 (0.30-2.60) ml kgâ1 60 min after ingestion of 3/7 ml kgâ1 syrup (P=0.002). Thus GCVw had declined to baseline after 3 ml kgâ1 (P=0.39) but not after 7 ml kgâ1 (P=0.001) within 60 min. T1/2 was 20 (10-62)/27 (13-43) min (P=0.73) after 3/7 ml kgâ1. Conclusion In healthy volunteer children, residual GCVw 1 h after intake of 3 ml kgâ1 syrup is significantly smaller than that after 7 ml kgâ1 and within the range of baselin
Cardiac output measurement in children: comparison of AesculonÂź cardiac output monitor and thermodilution
Background We compared cardiac output (CO) measurements by the non-invasive electrical velocimetry (AesculonÂź) monitor with the pulmonary artery catheter (PAC) thermodilution method in children. Methods CO values using the AesculonÂź monitor and PAC thermodilution were simultaneously recorded during cardiac catheterization in children. Measurements were performed under general anaesthesia. To compare, three consecutive measurements for each patient within 3 min were obtained. The means of the three values were compared using simple regression and Bland-Altman analysis. Data were presented as mean (sd). A mean percentage of <30% was defined to indicate clinical useful reliability of the AesculonÂź monitor. Results A total of 50 patients with a median (range) age of 7.5 (0.5-16.5) yr were enrolled in the study. Mean CO values were 3.7 (1.5) litre minâ1 (PAC thermodilution) and 3.1 (1.7) litre minâ1 (AesculonÂź monitor). Analysis for CO measurement showed a good correlation between the two methods (r=0.894; P<0.0001). The bias between the two methods was 0.66 litre minâ1 with a precision of 1.49 litre minâ1. The mean percentage error for CO measurements was 48.9% for the AesculonÂź monitor when compared with PAC thermodilution. Conclusions Electrical velocimetry using the AesculonÂź monitor did not provide reliable CO values when compared with PAC thermodilution. Whether the AesculonÂź monitor can be used as a CO trend monitor has to be assessed by further investigations in patients with changing haemodynamic
Protective CD8+ T lymphocytes in Primates Immunized with Malaria Sporozoites
Live attenuated malaria vaccines are more potent than the recombinant protein, bacterial or viral platform vaccines that have been tested, and an attenuated sporozoite vaccine against falciparum malaria is being developed for humans. In mice, attenuated malaria sporozoite vaccines induce CD8+ T cells that kill parasites developing in the liver. We were curious to know if CD8+ T cells were also important in protecting primates against malaria. We immunized 9 rhesus monkeys with radiation attenuated Plasmodium knowlesi sporozoites, and found that 5 did not develop blood stage infections after challenge with live sporozoites. We then injected 4 of these protected monkeys with cM-T807, a monoclonal antibody to the CD8 molecule which depletes T cells. The fifth monkey received equivalent doses of normal IgG. In 3 of the 4 monkeys receiving cM-T807 circulating CD8+ T cells were profoundly depleted. When re-challenged with live sporozoites all 3 of these depleted animals developed blood stage malaria. The fourth monkey receiving cM-T807 retained many circulating CD8+ T cells. This monkey, and the vaccinated monkey receiving normal IgG, did not develop blood stage malaria at re-challenge with live sporozoites. Animals were treated with antimalarial drugs and rested for 4 months. During this interval CD8+ T cells re-appeared in the circulation of the depleted monkeys. When all vaccinated animals received a third challenge with live sporozoites, all 5 monkeys were once again protected and did not develop blood stage malaria infections. These data indicate that CD8+ T cells are important effector cells protecting monkeys against malaria sporozoite infection. We believe that malaria vaccines which induce effector CD8+ T cells in humans will have the best chance of protecting against malaria
Gastric pH and residual volume after 1 and 2 h fasting time for clear fluids in childrenâ
Introduction Current guidelines suggest a fasting time of 2 h for clear fluids, which is often exceeded in clinical practice, leading to discomfort, dehydration and stressful anaesthesia induction to patients, especially in the paediatric population. Shorter fluid fasting might be a strategy to improve patient comfort but has not been investigated yet. This prospective clinical trial compares gastric pH and residual volume after 1 vs 2 h of preoperative clear fluid fasting. Methods Children (1-16 yr, ASA I or II) undergoing elective procedures in general anaesthesia requiring tracheal intubation were randomized into group A with 60 min or B with 120 min preoperative clear fluid fasting. To determine gastric pH and residual volume, the gastric content was sampled in supine, left and right lateral patient position using an oro-gastric tube after intubation. Data are median (interquartile range) for group A or B (P<0.05). Results In total, 131 children aged 1.01-16.23 yr were included; gastric pH was determined in 120 cases. Patient characteristic data were similar between the two groups, except for gender (46/33 males in group A/B; P=0.02). Despite significantly shorter fasting times for clear fluids in group A compared with group B (76/136 min; P<0.001), no significant difference was observed regarding gastric pH [1.43 (1.30-1.56)/1.44 (1.29-1.68), P=0.66] or residual volume [0.43 (0.21-0.84)/0.46 (0.19-0.78) ml kgâ1, P=0.47]. Conclusion One hour clear fluid fasting does not alter gastric pH or residual volume significantly compared with 2 h fasting.
