Elaboration of Organizational Arrangements on the Improvement of Functioning of the System of Pharmaceutical Support of Sudan Population
In the article is considered the problem of support of Sudan population with available pharmaceutical production. The aim of research is an analysis of the price and assortment policy realized by the different pharmaceutical structures, determination of the level of their functions on guaranteeing the availability of medicaments, elaboration on the base of received results the arrangements on optimization of the pharmaceutical support system. It was determined that not all subjects of Sudan pharmaceutical market responsible for the pharmaceutical support cope with such problems at the proper level. Thus in the private drugstores, in the net drugstores of the Ministry of defense «Alia» and in drugstores of the health insurance system the prices of medicament exceed the recommended ones in more than 2 times, an assortment also does not corresponds to both internal and International requirements. The received results indicate the unreasonable use of budget means by the separate state structures, the negative tendencies in the system of guaranteeing the availability of pharmaceutical help. It was established that the biggest suppliers of the medicaments in Sudan, excluding the state Centre of medicament support CMS cannot ensure the system of health protection with medicaments of the proper quality, assortment and the price rate. There were elaborated the organizational-economic approaches to optimization of the structure of medical support of Sudan that include the change of the flows of medicaments distribution. There was offered the structure that unites all inflows in the aforesaid CMS and then distributes resources according to the needs of organizations and regions. Introduction of the offered structures will allow improve the system of purchase and distribution of medicaments, make it transparent. The result of change will be the rise of physical and economic availability of medicaments, widening of assortment, decrease of the price of preparations, significant economy of budget resources
Modeling and analysis of the space shuttle nose-gear tire with semianalytic finite elements
A computational procedure is presented for the geometrically nonlinear analysis of aircraft tires. The Space Shuttle Orbiter nose gear tire was modeled by using a two-dimensional laminated anisotropic shell theory with the effects of variation in material and geometric parameters included. The four key elements of the procedure are: (1) semianalytic finite elements in which the shell variables are represented by Fourier series in the circumferential direction and piecewise polynominals in the meridional direction; (2) a mixed formulation with the fundamental unknowns consisting of strain parameters, stress-resultant parameters, and generalized displacements; (3) multilevel operator splitting to effect successive simplifications, and to uncouple the equations associated with different Fourier harmonics; and (4) multilevel iterative procedures and reduction techniques to generate the response of the shell. Numerical results of the Space Shuttle Orbiter nose gear tire model are compared with experimental measurements of the tire subjected to inflation loading
Spatial Modelling and Analysis of an Electrical Distribution System
The distribution of electrical energy to end-users in Nigerian communities is faced with diverse spatial problems leading to low voltage, overload on equipment, difficulties in fault tracing and delay in fault clearing. The traditional management system is not only manual but also has flaws such as difficulties in searching and updating previous records as well as no real-time information on their distribution assets. In this study, a geospatial technique was employed for effective management of the electricity distribution system in Malete, Kwara State, Nigeria. Spatial information of distribution asset was collected from the field and used in the structural modelling of the distribution network in the ArcMap software. Analysis of the model and the available information showed that 80 % of the transformers were not properly located at the centroid of the load. The span of the network fairly conformed to standard as only 45.73 % were within 45-50 m. The result also showed that 40 % of the transformers were overloaded with an unbalanced load, hence the need for restructuring of the network
Identification of novel therapeutic targets in the PI3K/AKT/mTOR pathway in hepatocellular carcinoma using targeted next generation sequencing.
Understanding genetic aberrations in cancer leads to discovery of new targets for cancer therapies. The genomic landscape of hepatocellular carcinoma (HCC) has not been fully described. Therefore, patients with refractory advanced/metastatic HCC referred for experimental therapies, who had adequate tumor tissue available, had targeted next generation sequencing (NGS) of their tumor samples using the Illumina HiSeq 2000 platform (Foundation One, Foundation Medicine, MA) and their treatment outcomes were analyzed. In total, NGS was obtained for 14 patients (median number of prior therapies, 1) with advanced/metastatic HCC. Of these 14 patients, 10 (71%) were men, 4 (29%) women, 6 (43%) had hepatitis B or C-related HCC. NGS revealed at least 1 molecular abnormality in 12 patients (range 0-8, median 2). Detected molecular aberrations led to putative activation of the PI3K/AKT/mTOR pathway (n=3 [mTOR, PIK3CA, NF1]), Wnt pathway (n=6 [CTNNA1, CTNNB1]), MAPK pathway (n=2 [MAP2K1, NRAS]), and aberrant DNA repair mechanisms, cell cycle control and apoptosis (n=18 [ATM, ATR, BAP1, CCND1, CDKN2A, CDK4, FGF3, FGF4, FGF19, MCL1, MDM2, RB1, TP53]). Of the 3 patients with molecular aberrations putatively activating the PI3K/AKT/mTOR pathway, 2 received therapies including a mTOR inhibitor and all demonstrated therapeutic benefit ranging from a partial response to minor shrinkage per RECIST (-30%, -15%; respectively). In conclusion, genomic alterations are common in advanced HCC. Refractory patients with alterations putatively activating the PI3K/AKT/mTOR pathway demonstrated early signals of clinical activity when treated with therapies targeting mTOR
First-order intertwining operators with position dependent mass and - weak-psuedo-Hermiticity generators
A Hermitian and an anti-Hermitian first-order intertwining operators are
introduced and a class of -weak-pseudo-Hermitian position-dependent mass
(PDM) Hamiltonians are constructed. A corresponding reference-target
-weak-pseudo-Hermitian PDM -- Hamiltonians' map is suggested. Some
-weak-pseudo-Hermitian PT -symmetric Scarf II and periodic-type models
are used as illustrative examples. Energy-levels crossing and flown-away states
phenomena are reported for the resulting Scarf II spectrum. Some of the
corresponding -weak-pseudo-Hermitian Scarf II- and
periodic-type-isospectral models (PT -symmetric and non-PT -symmetric) are
