    Effect of proper oral rehabilitation on general health of mandibulectomy patients

    Introduction Reduced oral aperture and mandibular opening are relatively common problems, which have a wide variety of causes [1]. Limited access to the oral cavity can be problematic for patients and dental professionals [2]. According to Glossary of Prosthodontics terms GPT [3], reduced oral aperture is defined as microstomia, which is a term used to describe any congenital or acquired reduction in the size of the oral opening that is severe enough to compromise esthetic, deglutition, nutrition, and quality of life [4]. The mandible is the key bone involved in face esthetic, mastication, and speech. Surgical resection of the mandible (known as a mandibulectomy) is often performed for tumors of the head and neck area, which should be followed by oral rehabilitation (i.e., occlusal adjustments and replacement of missing teeth and/or soft tissues, if involved) [5]. The treatment of oral tumors such as squamous cell carcinomas may require mandibular resection to secure adequate margins [6]. Segmental resection of the mandible leads to significant patient illness if not properly managed. Mandibulectomy can lead to loss of mandibular support to the teeth, inadequate mastication, impaired speech and disfigurement of the face [7]

    Introduction :  Mots-clés : Maladie de Kimura, parotide, chirurgie.La maladie de Kimura ou lymphogranulome éosinophile est une pathologie inflammatoire chronique très rare, d’étiologie inconnue. Nous rapportons un cas de maladie de Kimura à localisation parotidienne et à travers une revue de la littérature, rappelons les principales caractéristiques cliniques, paracliniques, thérapeutiques et évolutives de cette pathologie. Matériel et méthodes : Patient de 17 ans qui a consulté devant l’apparition d’une tuméfaction de la région parotidienne gauche évoluant depuis un an et sans paralysie faciale. Résultats : L’échographie cervico-parotidienne a révélé une parotide gauche hypertrophiée siège de multiples nodules hypoéchogènes. La tomodensitométrie cervico-faciale a mis en évidence une glande parotide gauche augmentée de taille et de structure hétérogène nodulaire sans adénopathies cervicales. La cytopontion était non concluante. Le patient a bénéficié d’une parotidectomie exofaciale gauche et l’étude anatomopathologique de la pièce opératoire revenue en faveur de la maladie de Kimura. Les suites opératoires étaient simples. Le recul est d’un an sans récidive. Discussion : La Maladie de Kimura se caractérise cliniquement par des nodules sous cutanés de localisation cervicofaciale, une augmentation du volume des glandes salivaires et des adénopathies satellites. Son diagnostic est histologique reposant sur l’identification d’une hyperplasie follicullaire avec des abcès à polynucléaires éosinophiles. Le traitement est chirurgical le plus souvent mais dans certains cas la corticothérapie est proposée. Ce diagnostic doit cependant rester à l’esprit devant toute masse cervico-faciale

    A ring-connected dual active bridge based DC-DC multiport converter for EV fast-charging stations

    This paper proposes a multiport DC-DC converter for EV fast-charging stations. The proposed converter is comprised of Ring-Connected Dual Active Bridge (RCDAB) DC-DC converters, where the connection point between every two adjacent DABs provides a DC port. Bypass switches are added to each DAB to eliminate unnecessary power processing stages in the event of idle ports (no EVs) (open circuit ports). The nature of the ring connection of the RCDAB theoretically allows infinite internal power flow solutions within the ring to satisfy a certain power flow scenario at the DC ports, hence, the optimal power flow solution can be selected to minimize total RMS current and losses. Single-phase shift control is applied to this optimization problem to make it simple. A novel closed-loop control scheme using Bisection optimization is developed to minimize the total RMS current. A control-hardware-in-the-loop (CHiL) validation is carried out for a 5-port network of the proposed topology to investigate the converter efficiency and fault tolerance/availability characteristics. Also, an experimental hardware validation is implemented for a 3-port network where different scenarios for power flow and faults are performed. Finally, a comparative discussion between the proposed topology and other multiport topologies in literature is presented revealing the superior performance of the RCDAB topology

    Discrete Model of Ideological Struggle Accounting for Migration

    A discrete in time model of ideological competition is formulated taking into account population migration. The model is based on interactions between global populations of non-believers and followers of different ideologies. The complex dynamics of the attracting manifolds is investigated. Conversion from one ideology to another by means of (i) mass media influence and (ii) interpersonal relations is considered. Moreover a different birth rate is assumed for different ideologies, the rate being assumed to be positive for the reference population, made of initially non-believers. Ideological competition can happen in one or several regions in space. In the latter case, migration of non-believers and adepts is allowed; this leads to an enrichment of the ideological dynamics. Finally, the current ideological situation in the Arab countries and China is commented upon from the point of view of the presently developed mathematical model.     Copeptin as a Biomarker of Atherosclerosis in Type 1 Diabetic Patients

    AIM: To evaluate copeptin as an early marker of atherosclerosis in adolescent type 1 diabetics. METHODS: Sixty-two type 1 diabetic patients and 50 healthy volunteers were enrolled in the study. Serum copeptin, glycosylated haemoglobin (HbA1c), lipid profile, oxidised low-density lipoprotein (OxLDL), urinary albumin/creatinine ratio, carotid intimal medial thickness (cIMT), aortic intimal medial thickness (aIMT) and resistivity index were assessed for all participant in the study. RESULTS: HbA1, albumin/creatinine ratio, lipid profile, OxlDL, copeptin, cIMT and aIMT were significantly higher in diabetic patients. Copeptin was higher in patients with positive cIMT and aIMT. Copeptin correlated with cIMT and aIMT. Stepwise multiple regression analysis found that copeptin correlated with aIMT. ROC curve showed that copeptin had 100 % specificity with aIMT and cIMT and 95.2 and 60,7 sensitivity with aIMT and cIMT respectively. CONCLUSION: Copeptin can be used as a marker for early detection of atherosclerosis of type 1 diabetic patients

    The prevalence and the clinical characteristics of metabolic syndrome patients admitted to the cardiac care unit

    AbstractObjectivesMetabolic syndrome (MetS) is a group of multiple cardiovascular risk factors, including dysglycemia, central obesity, high cholesterol, and hypertension. Cardiovascular disease is one of the most common complications of MetS. Recent studies showed that prevalence of MetS among patients admitted with acute coronary syndrome was as high as 46%.DesignWe conducted a cross-sectional study of 203 patients at the two main hospitals in Ta’if, Saudi Arabia. Patients older than 18years who were admitted to the Cardiac Care Unit (CCU) between the months of August 2013 and June 2014 were asked to participate. MetS diagnosis was made based on the International Diabetes Federation definition.ResultsA total of 203 patients participated, with 59.1% male and 40.9% were female. The mean age was 60.9 years with a mean body mass index of 28.97kg/m2 and a mean waist circumference of 95.45cm. The prevalence of MetS was 47.8%, primarily among obese female patients who reported sedentary lifestyles. Additionally, MetS patients were more likely to be admitted with heart failure (p<0.05) and more likely to have moderate-to-severe left-ventricular hypertrophy (LVH; p<0.05) relative to non-MetS patients.ConclusionOf the patients admitted to the CCU, 47.8% had MetS, with those patients likely to be female and obese. Furthermore, MetS patients were more likely to be admitted with heart failure and suffer from moderate-to-severe LVH

    Seasonal histological effects on the Leydig and Sertoli cells in the testis of indigenous dogs (Canis Familiaris)

    This study was performed on twenty adult male dogs aged 2-3 years old. Samples of testis tissue were taken during the four seasons in 2016 in Iraq. Specimens were fixed in 10% formalin. Routine histological techniques were carried out for light microscopy. H&E and PAS stains were used. During active seasons (spring and autumn) the study revealed a significant increase in the number and diameter of Leydig and Sertoli cells compared to other inactive seasons (summer and winter). Cytological indications of mitotic events of Leydig cells were clearly observed, whereas, Sertoli cells never showed any mitotic divisions. The basement membrane of seminiferous tubules appears wavy with impacts of irregular gates of cellular penetrations. It was characterized by a dynamic, flexible structure, and can respond easily to the cellular mechanical pressure. The new generation of Leydig cells was derived either from the differentiation of interstitial fibroblast-like cells or from the mitosis of Leydig cells themselves; whereas the new generation of Sertoli cells was provided only by the differentiation of interstitial fibroblast-like cells. The study suggested that, after stimulation, the compensatory fibroblast-like cells can penetrate the basement membrane and then differentiate to Sertoli cells, as the similarity between the two cells were very large. It was concluded firstly that the proliferation of Sertoli cells within the seminiferous tubules was done only by the penetration and differentiation of the interstitial fibroblast-like cells and not by mitosis of Sertoli cells themselves

    Histopathological Changes of the Flaxseed Extract on Skin Wound Healing in Diabetic Rabbits

    BACKGROUND: Wound healing includes phases such as cell migration, extracellular matrix deposition, remodeling, and angiogenesis. There are growing medicines that accelerate wound healing, significantly herbal medications that mainly safe and reliable. AIM: The aim of the study is to examine the histological changes induced by flaxseed oil during wound healing in diabetic animal model. METHODS: Forty-five male white New Zealand rabbits divided into two main groups diabetic and non-diabetic each group is divided into three groups (n = 9).Diabetic animal group include: (1) Study group (adding Flaxseed), (2) positive control group (adding fucidin 2% cream), and (3) negative control group (no treatment) same distribution of non-diabetic animals groups. Four linearshape,full-thickness wounds were made in both sides of the backbone skin in each animal. Tissue samples were obtained at days 4th, 7th, and 14th post wounding for microscopical analysis, histopathological parameters included inflammation, re-epithelialization, neovascularization, and surface closure rates. RESULTS: The study groups (using flax seed) in both diabetic and non-diabetic groups show significant reduction in inflammatory cells infiltration at day 14 (0.67 ± 0.753) (1.67 ± 0.516), and noticeable increase in wound closure rate (4.00+-0.000). Both diabetic and non-diabetic animal groups show high re-epithelialization potential at day 4 and day 7, respectively (2.00 ± 1.549; 4.00 ± 0.00). Diabetic animal study group using flaxseed shows high neovascularization rate at the day 14 (2.17 ± 0.753) compared with positive control diabetic animal group (2.00 ± 0.000). CONCLUSION: Topical application of flaxseed promoted healing process particularly in diabetic animal model by shortening inflammation phase, elevating surface closure rate, promoting re-epithelialization process, and enhancing neovascularization, flaxseed is suggested as a potential effective herbal base medication for facilitating wound healing

    Irisin in Adolescent Type 1 Diabetic Patients and its Relation to Diabetes Control and Atherosclerosis

    AIM: The objective of this was to determine the role of irisin in adolescent type 1 diabetes (T1D) patients. METHODS: This study was conducted on 65 patients with T1D and 50 healthy individuals as control group. Serum irisin, glycosylated hemoglobin (HbA1c), lipid profile, oxidized low-density lipoprotein (OxLDL), urinary albumin/creatinine ratio; carotid intimal medial thickness (cIMT), and aortic intimal medial thickness (aIMT) were evaluated for all participant. RESULTS: HbA1c, lipid profile, albumin/creatinine ratio, OxLDL, irisin, aIMT and cIMT were significantly higher in diabetic patients. Irisin had a positive correlation with age of diabetic patients, onset of diabetes, mid arm circumference, waist/height ratio, body mass index, HbA1c, and cIMT. CONCLUSION: Irisin is a marker for detection of diabetes control and early detection of subclinical atherosclerosis. Irisin had a relation with obesity

    The double heterozygocity point SC occupies the second place of sickle cell disease syndromes major. The diagnosis, often late, is usually asked on the occasion of a complication. The association "sickle cell anemia-pregnancy" causes the alteration of maternal prognosis on the slope and haematological fetal on the plan midwifery. Among the maternal complications related to this pathology, are found the crises vasoocclusives with the bilateral necrosis of femoral heads or heads of humerales, These crises are all the more serious in that they can be complicated by syndromes acute chest in report with a pulmonary embolism greasy or deadly syndrome vasculorenal. The support of such a haemoglobinopathy need a prophylactic transfusion or better an exchange transfusion of the 28th week of gestation to obtain a rate of HbA greater than 50%. In postpartum, the risk of complications is increased, the systematic support should include even in the early days, warming, oxygenation, hydration hydro-electrolytic and sy.La double hétérozygotie SC occupe la deuxième place des syndromes drépanocytaires majeures. Le diagnostic, souvent tardif, est habituellement posé à l’occasion d’une complication. L’association « drépanocytose-grossesse » entraîne l’altération du pronostic maternel sur le versant hématologique et fœtal sur le plan obstétrical. Parmi les complications maternelles liées à cette pathologie, sont retrouvées les crises vasoocclusives avec la nécrose bilatérale des têtes fémorales ou des têtes humérales, notamment en post partum, tel chez notre jeune patiente. Ces crises sont d’autant plus graves qu’elles peuvent se compliquer de syndromes thoraciques aigus en rapport avec une embolie pulmonaire graisseuse mortelle ou de syndrome vasculorénal. La prise en charge d’une telle hémoglobinopathie nécessite une transfusion prophylactique ou mieux un échange transfusionnel dès la 28ème semaine d’aménorrhée pour obtenir un taux d’ HbA supérieur à 50%. En post-partum, le risque de complications est accrue, la prise en charge systématique doit comprendre, dés les premiers jours, un réchauffement, une oxygénation, une hydratation hydro-éléctrolytique et une antibiothérapie systématique
