    Myomectomy as a pregnancy-preserving option in the carefully selected patient

    Objectives: To present the indications for myomectomy during pregnancy and to discuss complications possibly related and unrelated to the procedure. Method and Results: A 33-year-old patient at 18 weeks of gestation underwent removal of a 1,570-gram symptomatic fundic myoma. Histologically the patient had a leiomyomatous neoplasm of uncertain malignant potential. The pregnancy was continued under sequential observation with magnetic resonance imaging and ultrasound. At 36 weeks of gestation a healthy girl with an upper extremity limb defect was born via cesarean section. Follow-up of the mother and the child was uneventful. Conclusions: Certain known risk factors in pregnant women with myomas can predispose to complications during pregnancy. Women with such risk factors or women who have failed medical therapy should be offered the option of undergoing myomectomy as a pregnancy-preserving procedure. Copyright (C) 2002 S.     Endothelin 1 levels in relation to clinical presentation and outcome of Henoch Schonlein purpura

    <p>Abstract</p> <p>Background</p> <p>Henoch Schonlein purpura (HSP) is a common vasculitis of small vessels whereas endothelin-1 (ET-1) is usually reported elevated in vasculities and systematic inflammation. The aim of the present study was to investigate whether ET-1 levels are correlated with the clinical presentation and the outcome of HSP.</p> <p>Methods</p> <p>The study sample consisted of thirty consecutive patients with HSP. An equal number of healthy patients of similar age and the same gender were served as controls. The patients' age range was 2–12.6 years with a mean ± SD = 6.3 ± 3 years. All patients had a physical examination with a renal, and an overall clinical score. Blood and urinary biochemistry, immunology investigation, a skin biopsy and ET-1 measurements in blood and urine samples were made at presentation, 1 month later and 1 year after the appearance of HSP. The controls underwent the same investigation with the exception of skin biopsy.</p> <p>Results</p> <p>ET-1 levels in plasma and urine did not differ between patients and controls at three distinct time points. Furthermore the ET-1 were not correlated with the clinical score and renal involvement was independent from the ET-1 measurements. However, the urinary ET-1 levels were a significant predictor of the duration of the acute phase of HSP (HR = 0.98, p = 0.032, CI0.96–0.99). The ET-1 levels did not correlate with the duration of renal involvement.</p> <p>Conclusion</p> <p>Urinary ET-1 levels are a useful marker for the duration of the acute phase of HSP but not for the length of renal involvement.</p

    Guideline Assessment Project II: statistical calibration informed the development of an AGREE II extension for surgical guidelines

    OBJECTIVE: To inform the development of an AGREE II extension specifically tailored for surgical guidelines. AGREE II was designed to inform the development, reporting, and appraisal of clinical practice guidelines. Previous research has suggested substantial room for improvement of the quality of surgical guidelines. METHODS: A previously published search in MEDLINE for clinical practice guidelines published by surgical scientific organizations with an international scope between 2008 and 2017, resulted in a total of 67 guidelines. The quality of these guidelines was assessed using AGREE II. We performed a series of statistical analyses (reliability, correlation and Factor Analysis, Item Response Theory) with the objective to calibrate AGREE II for use specifically in surgical guidelines. RESULTS: Reliability/correlation/factor analysis and Item Response Theory produced similar results and suggested that a structure of 5 domains, instead of 6 domains of the original instrument, might be more appropriate. Furthermore, exclusion and re-arrangement of items to other domains was found to increase the reliability of AGREE II when applied in surgical guidelines. CONCLUSIONS: The findings of this study suggest that statistical calibration of AGREE II might improve the development, reporting, and appraisal of surgical guidelines

    Taking as a starting point one of Maurice Gross’s work on the construction be Prep X, we present the classes we have established for the Greek language. We have retained the classes established by M. Gross for French. For the Greek study, our point of departure is a selection of 2200 frozen expressions. But we have gone beyond this to study not only frozen expression but also productive phrases. The aim of this analysis was to establish the similarities or differences which exist between Greek and French. By studying the elements of these syntactic strings in both languages, we observe that the area of greatest difference occurs in the selection of prepositions, We have also studied the relationship between the support verb to be in Modem Greek and French and its relations with other support verbs. This study will serve as a basis for future comparative studies which will ultimately serve to support automatic translation. © 1997 John Benjamins Publishing Company

    Un lexique-grammaire du football: Étude contrastive du grec moderne et du français

    Taking as a starting point one of P. Sabatier’s work on the Lexicon-Grammar of football, we present the phrases we have established and translated for the Greek language (Ph-foot). We have retained the classes of objects established by P. Sabatier for French. For the Greek study our point of departure is a selection of 600 sentences. We have established not only a lexicon-grammar but also a list of classes of objects and compound nouns of this vocabulary. The aim of this analysis was to describe morphosyntactic similarities as well as differences between simple sentence constructions and compound nouns in both languages. This study will serve as a basis for future comparative studies wich will ultimately serve to support automatic translation. © 2006 John Benjamins Publishing Company

    Etiology and clinical features of viral bronchiolitis in infancy

    Background: Bronchiolitis is a common lower respiratory tract infection in infancy. The aim of this review is to present the clinical profile of viral bronchiolitis, the different culprit viruses and the disease severity in relation to the viral etiology. Data sources: Databases including PubMed and Google Scholar were searched for articles about the clinical features of bronchiolitis and its viral etiology. The most relevant articles to the scope of this review were analyzed. Results: Currently there are two main definitions for bronchiolitis which are not identical, the European definition and the American one. The most common viral pathogen that causes bronchiolitis is respiratory syncytial virus which was identified in 1955; now many other viruses have been implicated in the etiology of bronchiolitis such as rhinovirus, adenovirus, metapneumovirus, and bocavirus. Several studies have attempted to investigate the correlation of bronchiolitis severity with the type of detected virus or viruses. However, the results were not consitent. Conclusions: For the time being, the diagnosis of bronchiolitis remains clinical. The isolation of the responsible respiratory pathogens does not seem to confer to the prognosis of the disease severity. © 2017, Children&apos;s Hospital, Zhejiang University School of Medicine and Springer-Verlag Berlin Heidelberg
