    Econometric and Machine Learning Methods to Identify Pedestrian Crash Patterns

    Walking plays an important role in overcoming many challenges nowadays, and governments and local authorities are encouraging healthy and environmentally sustainable lifestyles. Nevertheless, pedestrians are the most vulnerable road users and crashes with pedestrian involvement are a serious concern. Thus, the identification of pedestrian crash patterns is crucial to identify appropriate safety countermeasures. The aims of the study are (1) to identify the road infrastructure, environmental, vehicle, and driver-related patterns that are associated with an overrepresentation of pedestrian crashes, and (2) to identify safety countermeasures to mitigate the detected pedestrian crash patterns. The analysis carried out an econometric model, namely the mixed logit model, and the association rules and the classification tree algorithm, as machine learning tools, to analyse the patterns contributing to the overrepresentation of pedestrian crashes in Italy. The dataset consists of 874,847 crashes—including 101,032 pedestrian crashes—that occurred in Italy from 2014 to 2018. The methodological approach adopted in the study was effective in uncovering relations among road infrastructure, environmental, vehicle, and driver-related patterns, and the overrepresentation of pedestrian crashes. The mixed logit provided a clue on the impact of each pattern on the pedestrian crash occurrence, whereas the association rules and the classification tree detected the associations among the patterns with insights on how the co-occurrence of more factors could be detrimental to pedestrian safety. Drivers’ behaviour and psychophysical state turned out to be crucial patterns related to pedestrian crashes’ overrepresentation.     Study Design. A paleopathological case of posterior arch defect of the atlas associated to the absence of costal element of the foramen transversarium. Objective. In living patients as well as in postmortem analysis it should be difficult to distinguish between a congenital and an acquired anomaly. Any anomaly in the anatomy of atlas should be taken into consideration by clinicians, surgeons, radiologists, and anatomists in order to avoid misinterpretations and clinical complications. Summary of Background Data. Posterior arch defect has a current occurrence of approximately 4%. Posterior arch schisis is attributed to the defective or absent development of the cartilaginous preformation of the arch rather than a disturbance of the ossification. The absence of costal element of the foramen transversarium has an incidence of ranging from 2% to 10% and is attributed to a developmental defect or to variations in the course of the vertebral artery. Methods. The skeleton of a man aged 20–30 years, brought to light in the plague cemetery of 16th century Alghero (Sardinia), showed anomalies of the atlas, consisting in failure of the midline fusion of the 2 hemiarches with a small gap and an open anterior foramen trasversarium on the left side. A macroscopic, radiological, and stereomicroscopic study was carried out. Results. The study allowed to rule out a traumatic origin of the defects and to diagnose an association of 2 congenital anomalies. Conclusion. Osteoarchaeological cases provides with a valuable opportunity to examine and describe variations in the anatomy of the atlas

    High-Definition Ultrasound Characterization of Squamous Carcinoma of the Tongue: A Descriptive Observational Study

    High-definition ultrasonography is a diagnostic tool that uses sound echoes to produce images of tissues and organs. In the head and neck region, ultrasounds have been used to diagnose different types of lesions. The intraoral approach was shown to be a real-time, non-invasive way to characterize oral lesions. The tongue is the most often examined region because of its accessibility. This observational study aimed to describe the qualitative characteristics of tongue squamous cell carcinoma images obtained with high-definition intraoral ultrasound by comparing them with the corresponding histopathological sample. Twenty patients were enrolled in this study. The scans of the lesions were carried out with an 18 MHz linear ultrasound probe following the long axis of the lesion. For each lesion, five frames were selected, on which descriptive analysis was performed. A histological sample was taken and then compared to the ultrasonographic acquisition. The so-nographic appearance of the tissue layers has a good correlation between ultrasound and histolog-ical morphology, and it was easy to distinguish the tumor from the homogenous composition of the tongue tissues. Furthermore, a correlation between the structure by section and pattern of tumor margin features by ultrasound was obtained. Intraoral ultrasonography appears to be a promising technique in the non-invasive characterization of tongue squamous cell carcinoma. Further studies will be needed to improve the technique in terms of ergonomics and repeatability

    Dynamic Behaviour of the Patented Kobold Tidal Current Turbine: Numerical and Experimental Aspects

    This paper provides a summary of the work done at DPA on numerical and experimental investigations of a novel patented vertical axis and variable pitching blades hydro turbine designed to harness energy from marine tidal currents. Ponte di Archimede S.p.A. Company, located in Messina, Italy, owns the patented KOBOLD turbine that is moored in the Messina Strait, between the mainland and Sicily. The turbine has a rotor with a diameter of 6 meters, three vertical blades of 5 meters span with a 0.4 m chord ad hoc designed curved airfoil, producing high lift with no cavitation. The rated power is 160 kW with 3.5 m/s current speed, which means 25% global system efficiency. The VAWT and VAWT_DYN computer codes, based on Double Multiple Steamtube, have been developed to predict the steady and dynamic performances of a cycloturbine with fixed or self-acting variable pitch straight-blades. A theoretical analysis and a numerical prediction of the turbine performances as well as experimental test results on both a model and the real scale turbine will be presented and discussed.

    Atlas occipitalisation associated with other anomalies in a 16th century skeleton from Sardinia (Italy)

    Archaeological excavations carried out in the plague cemetery of 16th century Alghero (Sardinia) brought to light the skeleton of a male aged 35–45 years, showing anomalies of the atlas. A macroscopic and radiological study has been carried out. The first cervical vertebra is fused with the skull base, resulting in an occipitalisation of the atlas. Absence of the costal element of the left foramen transversarium, resulting in an open anterior foramen transversarium, and posterior arch defect are also observed. The atlanto-occipital junction is a complex structure, susceptible to develop different patterns of congenital defects. These anatomical variations of atlas should be considered in modern clinical practice in order to formulate a correct diagnosis and to conceive an appropriate treatment. Osteoarchaeological cases are important as, beside to ascertain the presence of congenital defects in past populations, allow an in-depth study in dry bones, which can help modern medicine in interpreting anatomical variations.     Leptin as a metabolic link to multiple sclerosis.

    Clinical and experimental data, together with epidemiological studies, have suggested that the pathogenesis of multiple sclerosis (MS) might involve factors that link the immune system with metabolic status. Moreover, recent research has shown that leptin, the adipocyte-derived hormone that controls food intake and metabolism, can promote experimental autoimmune encephalomyelitis, an animal model of MS. In patients with MS, the association of leptin with disease activity has been dissected at the molecular level, providing new mechanistic explanations for the role of this hormone in MS. Here, we review the intricate relationship between leptin and other metabolic modulators within a framework that incorporates the latest advances linking the CNS, immune tolerance and metabolic status. We also consider the translational implications of these new findings for improved management of MS

    KISS1 and KISS1R expression in the human and rat carotid body and superior cervical ganglion

    KISS1 and its receptor, KISS1R, have both been found to be expressed in central nervous system, but few data are present in the literature about their distribution in peripheral nervous structures. Thus, the aim of the present study was to investigate, through immunohistochemistry, the expression and distribution of KISS1 and KISS1R in the rat and human carotid bodies and superior cervical ganglia, also with particular reference to the different cellular populations. Materials consisted of carotid bodies and superior cervical ganglia were obtained at autopsy from 10 adult subjects and sampled from 10 adult Sprague-Dawley rats. Immunohistochemistry revealed diffuse expression of KISS1 and KISS1R in type I cells of both human and rat carotid bodies, whereas type II cells were negative. In both human and rat superior cervical ganglia positive anti-KISS1 and -KISS1R immunostainings were also selectively found in ganglion cells, satellite cells being negative. Endothelial cells also showed moderate immunostaining for both KISS1 and KISS1R. The expression of both kisspeptins and kisspeptin receptors in glomic type I cells and sympathetic ganglion cells supports a modulatory role of KISS1 on peripheral chemoreception and sympathetic function. Moreover, local changes in blood flow have been considered to be involved in carotid body chemoreceptor discharge and kisspeptins and kisspeptin receptors have also been found in the endothelial cells. As a consequence, a possible role of kisspeptins in the regulation of carotid body blood flow and, indirectly, in chemoreceptor discharge may also be hypothesized

    Photooxidation of cathecol (1) is carried out in aqueous solution at k > 300 nm using different sensitizers: rose bengal (RB), 9,10-dicyanoanthracene (DCA), 2,4,6-triphenylpyrylium tetrafluoroborate (Pyryl). The highest degradation is observed in the UV/RB-sensitized reaction (66% after 15 h of irradiation), mineralization and formation of dimers are the final events. This procedure has been extended to tyrosol (2), caffeic acid (3), vanillic acid (4), 4-hydroxycinnamic acid (5) and 4-hydroxybenzoic acid (6) as well as to a mixture of all phenols. A reduced toxicity of the UV/RB-irradiated solutions of cathecol and tyrosol towards alga Ankistrodesmus braunii is also verified

    Tel-eVax: a genetic vaccine targeting telomerase for treatment of canine lymphoma

    Background: we have recently shown that Tel-eVax, a genetic vaccine targeting dog telomerase (dTERT) and based on Adenovirus (Ad)/DNA Electro-Gene-Transfer (DNA–EGT) technology can induce strong immune response and increase overall survival (OS) of dogs affected by multicentric Diffuse Large B cell Lymphoma (DLBCL) when combined to COP therapy in a double-arm study. Here, we have utilized a clinically validated device for veterinary electroporation called Vet-ePorator , based on Cliniporator technology currently utilized and approved in Europe for electro- chemotherapy applications and adapted to electrogenetransfer (EGT). Methods: 17 dogs affected by DLBCL were vaccinated using two Ad vector injections (Prime phase) followed by TM DNA–EGT (Boost phase) by means of a Vet-ePorator device and treated in the same time with a 27-week Madison Wisconsin CHOP protocol. The immune response was measured by ELISA assays using pool of peptides. Results: No significant adverse effects were observed. The OS of vaccine/CHOP animals was 64.5 weeks, in line with the previous study. Dogs developed antibodies against the immunizing antigen. Conclusions: Tel-eVax in combination with CHOP is safe and immunogenic in lymphoma canine patients. These data confirm the therapeutic efficacy of dTERT vaccine and hold promise for the treatment of dogs affected by other cancer types. More importantly, our findings may translate to human clinical trials and represent new strategies for cancer treatment
