    Knowledge and attitude towards mental illness among health and non-health university students in Riyadh

    Background: Mental illness can affect anyone regardless of age, gender, and residence. Studying the attitude and knowledge about mental illnesses among university students is important, because they are the future decision makers dealing with such problems. The aim of this study was to assess level of knowledge and attitude towards mental illnesses among health and non-health university students in Riyadh.Methods: This study was a cross-sectional study on students of the governmental universities in Riyadh with both health and non-health speciality. The total sample size was 587 students. The questionnaire was designed electronically, and the link was distributed through social media. It included socio-demographic questions, 17 questions to assess knowledge and 22 questions to assess the attitudes. The statistical tests used were chi square, independent sample t-test, spearman’s correlation and multiple linear regression tests.Results: More than half of the participants had a positive attitude toward mental illness (52%). Only 13.46% of university students had good knowledge about mental illness. Significant higher level of good knowledge and positive attitude were reported among health college students compared to non-health (24.7% versus 7.9% for good knowledge and 60.8% versus 48.3 for positive attitude respectively). Attitude towards mental illness can be successfully predicted by using the knowledge score about mental illness and the type of college.Conclusion: Low percentage of university students had good knowledge about mental illness and their attitude towards mental illness was generally positive. Health college students had better attitude and knowledge about mental illness than non-health

    Multi-Expression Programming (MEP): Water Quality Assessment Using Water Quality Indices

    Water contamination is indeed a worldwide problem that threatens public health, environmental protection, and agricultural productivity. The distinctive attributes of machine learning (ML)-based modelling can provide in-depth understanding into increasing water quality challenges. This study presents the development of a multi-expression programming (MEP) based predictive model for water quality parameters, i.e., electrical conductivity (EC) and total dissolved solids (TDS) in the upper Indus River at two different outlet locations using 360 readings collected on a monthly basis. The optimized MEP models were assessed using different statistical measurements i.e., coefficient-of-determination (R2), root-mean-square error (RMSE), mean-absolute error (MAE), root-mean-square-logarithmic error (RMSLE) and mean-absolute-percent error (MAPE). The results show that the R2 in the testing phase (subjected to unseen data) for EC-MEP and TDS-MEP models is above 0.90, i.e., 0.9674 and 0.9725, respectively, reflecting the higher accuracy and generalized performance. Also, the error measures are quite lower. In accordance with MAPE statistics, both the MEP models shows an “excellent” performance in all three stages. In comparison with traditional non-linear regression models (NLRMs), the developed machine learning models have good generalization capabilities. The sensitivity analysis of the developed MEP models with regard to the significance of each input on the forecasted water quality parameters suggests that Cl and HCO3 have substantial impacts on the predictions of MEP models (EC and TDS), with a sensitiveness index above 0.90, although the influence of the Na is the less prominent. The results of this research suggest that the development of intelligence models for EC and TDS are cost effective and viable for the evaluation and monitoring of the quality of river water

    Life satisfaction and risk of burnout among men and women working as physiotherapists

    Objectives: Recently in Poland as a result of the high rate of aging population and high rates of morbidity, a growing demand for the physiotherapist profession is observed. The results of this study can be used to formulate principles for better organization of physiotherapist's workplace in order to prevent occurrence of burnout. The aim of this study is to investigate the effect of gender on satisfaction with life and burnout among active physiotherapists. Material and Methods: The survey was anonymous and voluntary, and involved a group of 200 active physiotherapists working in health care units and educational centers in Poland. The study group was selected randomly and incidentally. Each respondent received a demographic data sheet and a set of self-rating questionnaires (Life Satisfaction Questionnaire, Burnout Scale Inventory). Results: Burnout among men decreased along with increasing satisfaction with one's work and occupation, friends, relatives and acquaintances, sexuality, and increased due to greater satisfaction with one's housing status. Burnout among women decreased along with increasing satisfaction with one's health, free time and friends, relatives and acquaintances, and increased due to work at a setting other than a health care unit or educational center. Statistical analysis failed to reveal any significant differences with regard to the BSI domains and with regard to the overall burnout index as well as with regard to the assessment of satisfaction with life between female and male physiotherapists. Conclusions: Satisfaction with children, marriage and partnership, with one's work and occupation, interactions with friends, relatives and acquaintances and sexuality may contribute to reduction of burnout among men. Women who are satisfied with their children, family, health, free time and contacts with friends, relatives and acquaintances are less prone to burnout. Weak financial situation among women and deficiency of free time among men can induce burnout. Improving staff happiness may contribute to decreasing burnout

    Bewusstsein für und Einhaltung von Infektionskontrolle durch Studenten und Praktikanten der Zahnmedizin an der Riyadh Elm University

    Aim: Dental students have increasing patient contact during their education and clinical years, putting them at high risk of cross-infection; therefore, the purpose of this study was to determine the level of infection control practices among dental students and interns at Riyadh Elm University, Riyadh, Saudi Arabia. Methods: Total of 400 questionnaires were distributed among interns and clinical students at Riyadh Elm University. The questionnaire comprised 32 items assessing infection control practices, and the data were tabulated and analyzed by SPSS to produce descriptive statistics.Results: Three hundred nine questionnaires were answered (response rate 77%).The implementation of different infection control measures was good to excellent, except for hepatitis B vaccination and wearing eye protection: only 76% of males and 83% of females were vaccinated against HBV, and only 30% of males and 26% of females always wore protective glasses. Conclusion: Compared to previous studies, an increased awareness regarding infection control practices among dental students and interns was noticeable. However, greater emphasis on the importance of infection control, especially compliance with HBV vaccination and wearing protective eyewear, is necessary.Zielsetzung: Zahnmedizinstudenten und Praktikanten sind während ihrer klinischen Ausbildung Infektionsrisiken ausgesetzt. Daher sollte die Einhaltung der Schutzmaßnahmen der Auszubildenden an der Riyadh Elm University, Saudi Arabien, analysiert werden.Methode: Es wurden 400 Fragebögen mit 32 Items zur Einhaltung ausgewählter Präventionsmaßnahmen ausgegeben. Die Daten wurden mittels SPSS deskriptiv ausgewertet.Ergebnisse: 309 Fragebögen waren auswertbar (Response Rate 77%). Die Implementierung der Präventionsmaßnahmen war mit Ausnahme der HBV-Schutzimpfung und des Tragens einer Schutzbrille gut bis sehr gut. Geimpft waren jedoch nur 76% der männlichen and 83% der weiblichen Personen. Augenschutz trugen sogar nur 30% der männlichen and 26% der weiblichen Personen.Schlussfolgerung: Im Vergleich zu vorangegangen Analysen wurde ein gesteigertes Bewusstsein für Infektionskontrollpraktiken bei zahnmedizinischen Studenten und Praktikanten festgestellt. Es ist jedoch notwendig, dass alle Auszubildenden zur HBV-Schutzimpfung und zum Tragen von Schutzbrillen motiviert werden

Students spend their first university year in a combined extensive teaching program before they can be assigned to a particular health science specialty. It is thought that students enrolled in a highly competitive environment such as HSPP with a long list of potential stressors, including developmental, academic overload, language barriers and competition, are more disposed to stress and stress-related complications. This study aims to measure the level of academic stress and to determine its risk factors in students enrolled in HSPP-adapted local universities in Saudi Arabia. The study was conducted at two Saudi universities, King Saud University (KSU) and Imam Mohammad ibn Saud Islamic University (IMSU) with competition-based and non-competition-based HSPP learning models, respectively. Both universities adopt the HSPP system. The scale for assessing academic stress (SAAS) was used to assess students’ perceived stress. A total of 290 students successfully completed the questionnaire (N=290), with a mean age of 18.66 years. Mean SAAS scores for KSU and IMSU students were 8.37 (SD = 4.641) and 7.97 (SD = 5.104), P=0.480, respectively. Only “satisfaction” and “associated social and health problems” have shown statistically significant correlation with university (P=0.000 and P=0.049, respectively). This study has found mean SAAS score for two local universities with competition-based versus non-competition-based HSPP learning models. Academic stress correlation with age, gender and universities was discussed, and valuable future work guidance was recommended.     Orthodontic treatment and referral patterns: A survey of pediatric dentists, general practitioners, and orthodontists

    Objective: This study aims to assess the orthodontic diagnostic skills, referral patterns, and the perceptions of orthodontic benefits of pediatric and general dentists in comparison with orthodontists. Materials and methods: Two online surveys were e-mailed to pediatric dentists, general dentistry practitioners, and orthodontists registered as members of the Saudi Dental Society and the Saudi Orthodontic Society. The surveys included questions about the type of orthodontic treatment provided, referral trends, and timing; presumed benefits associated with successful orthodontic treatment; and diagnosis and treatment plans of seven cases representing different malocclusions. Results: In total, 25 orthodontists, 18 pediatric dentists, and 14 general practitioners completed the survey. Only 38.8% of pediatric dentists and 7.1% of general practitioners reported that they practiced orthodontics clinically. The perceptions of the three groups toward the benefits of orthodontic treatment were comparable in the psychosocial areas. However, the orthodontists perceived significantly lesser effects of orthodontic treatment on the amelioration of temporomandibular disorder (TMD) symptoms. Pediatric dentists tended to rate the need and urgency of treatment higher, while general practitioners tended to rate the need of treatment lower. The selected treatment plans for three early malocclusion cases showed the greatest discrepancies between the orthodontists and the other two groups. Conclusions: The orthodontists consistently and significantly downplayed the perceived benefit of orthodontic treatment to reduce TMD symptoms. Also, while there was a similarity in the diagnosis, there were notable differences in the proposed treatment approaches, perceived treatment need, and timing of intervention between the three groups of practitioners.     Ophthalmic features of PLA2G6-related paediatric neurodegeneration with brain iron accumulation.

    BACKGROUND: Neurodegeneration with brain iron accumulation (NBIA) refers to genetically heterogenous paediatric neurodegenerative disorders characterised by basal ganglia iron deposition. One major cause is recessive mutations in the PLA2G6 gene. While strabismus and optic nerve pallor have been reported for PLA2G6-related disease, the ophthalmic phenotype is not carefully defined. In this study we characterise the ophthalmic phenotype of PLA2G6-related NBIA. METHODS: Prospective cohort study. RESULTS: The eight patients were 4-26 years old when examined. All had progressive cognitive and motor regression first noted between 9 months and 6 years of age that typically first manifested as difficulty walking (ataxia). Ophthalmic examination was sometimes limited by cognitive ability. Four of eight had exotropia, 7/7 bilateral supraduction defect, 5/7 poor convergence, 6/8 saccadic pursuit, 4/8 saccadic intrusions that resembled square-wave jerks, and 8/8 bilateral optic nerve head pallor. All patients lacked Bell phenomenon. CONCLUSIONS: Upgaze palsy, although not a previously reported finding, was confirmed in all patients (except in one for whom assessment could not be performed) and thus can be considered part of the phenotype in children and young adults. Other frequent findings not previously highlighted were abnormal convergence, saccadic pursuit, and saccadic intrusions. Optic nerve head pallor and strabismus, previously reported findings in the disease, were found in 100% and 50% of our cohort, respectively, and the strabismus in our series was always exotropia. Taken together, these clinical findings may be helpful in distinguishing PLA2G6-related neurodegeneration from the other major cause of NBIA, recessive PANK2 mutations

    Multi-drug carbapenem-resistant Klebsiella pneumoniae infection carrying the OXA-48 gene and showing variations in outer membrane protein 36 causing an outbreak in a tertiary care hospital in Riyadh, Saudi Arabia

    Objectives: To investigate the genes of antibiotic resistance among isolates from the first reported carbapenem-resistant Klebsiella pneumoniae (CRKP) outbreak in a tertiary care hospital, Riyadh, Saudi Arabia. Methods: Antimicrobial susceptibility testing was performed on bacterial isolates using the Microscan Walkaway system (Siemens, Germany) and was confirmed by Etest (AB Biodisk, Sweden). bla-CTX-M, -SHV, -TEM, -OXA-48, OXA-A,B,C,D, -KPC, -NDM, -VIM, -IMP, integron 1, and outer membrane proteins(Omp)-35 and Omp-36 were investigated by PCR amplification and direct sequencing of PCR products. Isolates were sequence-typed by multilocus sequence typing (MLST). Results: All isolates were resistant to cefotaxime, ceftazidime, cefepime, ciprofloxacin, and piperacillin–tazobactam, and 91% (21 out of 23) were resistant to amikacin and gentamicin. All isolates except two from a single patient were resistant to one of the carbapenems. CTX-M and SHV genes were detected in all isolates, CTX-M-15 and SHV-1 types being predominant among these extended-spectrum beta-lactamases (ESBLs). TEM-1 was found in all except one isolate (isolate 3). Significantly, the OXA-48 gene was also found in all isolates. OXA-D-gene was found in three out of 23 isolates. KPC, NDM, OXA-A, -B, -C, VIM, and IMP genes were absent in all isolates. Disruption of the Omp-36 gene due to insertion of transposon IS903 and/or IS4 was detected in four out of 23 isolates, and some unique variations were also observed in this gene, including an insertion of two amino acids in the L3 region of Omp-36 in one isolate (isolate 3) and a mutation resulting in a premature stop codon in another isolate (isolate 25). MLST revealed ST29 to be the predominant sequence type (17 out of 23 isolates, 74%). Three were ST709 and one each was ST37 and ST111; one isolate had an unknown ST. Conclusions: This is probably the first reported outbreak of multidrug/carbapenem-resistant Klebsiella infection involving the OXA-48 gene from Saudi Arabia. Although the presence of ESBLs such as OXA, CTX-M, TEM, and SHV are predictable reasons for resistance, variations in the Omp-36 gene might also have precipitated this phenomenon. Disruption of the Omp-36 sequence by large insertional elements, the insertion of two amino acids in a very crucial part of this protein, and the presence of a premature stop codon in one isolate might have rendered this protein incomplete and non-functional. The study also demonstrated that more than one type of clone was responsible for this reported apparent outbreak and that ST29, a clone not reported from this region before, was the major clone responsible

    Evolving sequence mutations in the Middle East Respiratory Syndrome Coronavirus (MERS-CoV)

    Background: Middle East respiratory syndrome coronavirus (MERS-CoV) has continued to cause sporadic outbreaks of severe respiratory tract infection over the last 8 years. Methods: Complete genome sequencing using next-generation sequencing was performed for MERS-CoV isolates from cases that occurred in Riyadh between 2015 and 2019. Phylogenetic analysis and molecular mutational analysis were carried out to investigate disease severity. Results: A total of eight MERS-CoV isolates were subjected to complete genome sequencing. Phylogenetic analysis resulted in the assembly of 7/8 sequences within lineage 3 and one sequence within lineage 4 showing complex genomic recombination. The isolates contained a variety of unique amino acid substitutions in ORF1ab (41), the N protein (10), the S protein (9) and ORF4b (5). Conclusion: Our study shows that MERS-CoV is evolving. The emergence of new variants carries the potential for increased virulence and could impose a challenge to the global health system. We recommend the sequencing every new MERS-CoV isolate to observe the changes in the virus and relate them to clinical outcomes

    A newly recognized autosomal recessive syndrome affecting neurologic function and vision

    Genetic factors represent an important etiologic group in the causation of intellectual disability. We describe a Saudi Arabian family with closley related parents in which four of six children were affected by a congenital cognitive disturbance. The four individuals (aged 18, 16, 13, and 2 years when last examined) had motor and cognitive delay with seizures in early childhood, and three of the four (sparing only the youngest child) had progressive, severe cognitive decline with spasticity. Two affected children had ocular malformations, and the three older children had progressive visual loss. The youngest had normal globes with good functional vision when last examined but exhibited the oculodigital sign, which may signify a subclinical visual deficit. A potentially deleterious nucleotide change (c.1A>G; p.Met1Val) in the C12orf57 gene was homozygous in all affected individuals, heterozygous in the parents, and absent in an unaffected sibling and >350 normal individuals. This gene has no known function. This family manifests a autosomal recessive syndrome with some phenotypic variability that includes abnormal development of brain and eyes, delayed cognitive and motor milestones, seizures, and a severe cognitive and visual decline that is associated with a homozygous variant in a newly identified gene
