    AbstractWe report the case of a child with a delayed presentation of penile strangulation with a coil of hair that resulted in a complete transection of the urethra. Hair coil strangulation of the penis is uncommon. It is also known as penile Tourniquet syndrome. It has been reported with circumcised and uncircumcised penises and it can lead to serious complications like the amputation of the penis. Prompt diagnosis and treatment are necessary to prevent complications

    Effect of the relative shift between the electron density and temperature pedestal position on the pedestal stability in JET-ILW and comparison with JET-C

    The electron temperature and density pedestals tend to vary in their relative radial positions, as observed in DIII-D (Beurskens et al 2011 Phys. Plasmas 18 056120) and ASDEX Upgrade (Dunne et al 2017 Plasma Phys. Control. Fusion 59 14017). This so-called relative shift has an impact on the pedestal magnetohydrodynamic (MHD) stability and hence on the pedestal height (Osborne et al 2015 Nucl. Fusion 55 063018). The present work studies the effect of the relative shift on pedestal stability of JET ITER-like wall (JET-ILW) baseline low triangularity (\u3b4) unseeded plasmas, and similar JET-C discharges. As shown in this paper, the increase of the pedestal relative shift is correlated with the reduction of the normalized pressure gradient, therefore playing a strong role in pedestal stability. Furthermore, JET-ILW tends to have a larger relative shift compared to JET carbon wall (JET-C), suggesting a possible role of the plasma facing materials in affecting the density profile location. Experimental results are then compared with stability analysis performed in terms of the peeling-ballooning model and with pedestal predictive model EUROPED (Saarelma et al 2017 Plasma Phys. Control. Fusion). Stability analysis is consistent with the experimental findings, showing an improvement of the pedestal stability, when the relative shift is reduced. This has been ascribed mainly to the increase of the edge bootstrap current, and to minor effects related to the increase of the pedestal pressure gradient and narrowing of the pedestal pressure width. Pedestal predictive model EUROPED shows a qualitative agreement with experiment, especially for low values of the relative shift

    Fibroepithelial polyps of the urethra (FEPU) are rare entity in infancy. They are congenital benign tumors. Most patients present with acute retention, hematuria or intermittent bladder outlet obstruction. The treatment of choice is transurethral endoscopic resection. Histological study confirms the diagnosis and the prognosis is excellent. We retrospectively reviewed the medical records of three male patients with fibroepithelial polyps of the urethra that were diagnosed and treated in the department of pediatric surgery of Monastir. The diagnosis was based on voiding cystourethrogram and cystourethroscopy. The treatment was endoscopic resection and histology confirmed the diagnosis in all cases

    Abstract Background Metanephric adenoma is a rare benign renal tumor of the kidney, uncommonly observed in children. It is often misdiagnosed preoperatively as a malignant neoplasm, leading to an unnecessary nephrectomy. The challenge is to make the right diagnosis preoperatively and therefore manage it with conservative surgery. We report a case of a child with metanephric adenoma who underwent nephron-sparing surgery. Case presentation A renal tumor was discovered fortuitously in an 18-month-old Caucasian girl with several congenital malformations. Investigations showed a 28 × 27 × 27 mm left renal mass centrally located, well defined, nonvascularized, with no calcifications and which compressed the adjacent renal tissue. Furthermore, there were no signs of metastasis. The decision of a multidisciplinary meeting was to perform a computed tomography (CT)-scan-guided biopsy. Histologic examination concluded it was a metanephric adenoma. We performed a left open partial nephrectomy via a flank retroperitoneal incision. The final histopathological examination confirmed the diagnosis. The postoperative course was uneventful. Conclusion Preoperative diagnosis of metanephric adenoma is challenging. Because of the high probability of unnecessary radical nephrectomy, preoperative biopsy can be safe and determining to guide a more conservative approach so nephron-sparing surgery can be performed

    Kyste hydatique du rein chez l’enfant: à propos de 8 cas

    Objectif: Le but de l’étude est d’ analyser les caractéristiques épidémiologiques, cliniques, para cliniques, les modalités évolutives et les procédures thérapeutiques de la prise en charge du kyste hydatique du rein chez l’enfant. Patients et méthodes: Il s’agit d’une étude rétrospective de 8 cas d’enfants opérés pour kyste hydatique du rein au service de chirurgie pédiatrique de Monastir de 1995 à 2011. Résultats: L’âge des patients variait de 3 à 14 ans avec un âge moyen de 8 ans et ½. Il s’agit de 3 garçons et 5 filles soit un sex ratio de 0,6. La symptomatologie clinique révélatrice était dominée par la douleur abdominale (4 cas/8), le diagnostic positif s’est basé sur l’imagerie et la sérologie hydatique. La chirurgie était à ciel ouvert dans 4 cas et vidéo-assistée dans 4 cas dont 3 laparoscopies et une rétropéritonéoscopie. L’évolution clinique a été favorable et aucun patient ne présente de gêne fonctionnelle. Conclusion: Le traitement du kyste hydatique du rein chez l’enfant est essentiellement chirurgical et en règle conservateur. Il peut être mené par chirurgie à ciel ouvert ou par chirurgie vidéo-assistée qui a l’avantage d’être mini-invasive. Le pronostic est fréquemment favorable
