    Graft Immune Cell Composition Associates with Clinical Outcome of Allogeneic Hematopoietic Stem Cell Transplantation in Patients with AML

    Complications of allogeneic hematopoietic stem cell transplantation (HSCT) have been attributed to immune cells transferred into the patient with the graft. However, a detailed immune cell composition of the graft is usually not evaluated. In the present study, we determined the level of variation in the composition of immune cells between clinical HSCT grafts and whether this variation is associated with clinical outcome. Sizes of major immune cell populations in 50 clinical grafts from a single HSCT Centre were analyzed using flow cytometry. A statistical comparison between cell levels and clinical outcomes of HSCT was performed. Overall survival, acute graft-versus-host disease (aGVHD), chronic graft-versus-host disease (cGVHD), and relapse were used as the primary endpoints. Individual HSCT grafts showed considerable variation in their numbers of immune cell populations, including CD123(+) dendritic cells and CD34(+) cells, which may play a role in GVHD. Acute myeloid leukemia (AML) patients who developed aGVHD were transplanted with higher levels of effector CD3(+) T, CD19(+) B, and CD123(+) dendritic cells than AML patients without aGVHD, whereas grafts with a high CD34(+) content protected against aGVHD. AML patients with cGVHD had received grafts with a lower level of monocytes and a higher level of CD34(+) cells than those without cGVHD. There is considerable variation in the levels of immune cell populations between HSCT grafts, and this variation is associated with outcomes of HSCT in AML patients. A detailed analysis of the immune cell content of the graft can be used in risk assessment of HSCT

    EBMT prospective observational study on allogeneic hematopoietic stem cell transplantation in T-prolymphocytic leukemia (T-PLL)

    Preliminary data suggest that allogeneic stem cell transplantation (allo-SCT) may be effective in T-prolymphocytic leukemia (T-PLL). The purpose of the present observational study was to assess the outcome of allo-SCT in patients aged 65 years or younger with a centrally confirmed diagnosis of T-PLL. Patients were consecutively registered with the EBMT at the time of transplantation and followed by routine EBMT monitoring but with an extended dataset. Between 2007 and 2012, 37 evaluable patients (median age 56 years) were accrued. Pre-treatment contained alemtuzumab in 95% of patients. Sixty-two percent were in complete remission (CR) at the time of allo-SCT. Conditioning contained total body irradiation with 6 Gy or more (TBI6) in 30% of patients. With a median follow-up of 50 months, the 4-year non-relapse mortality, relapse incidence, progression-free (PFS) and overall survival were 32, 38, 30 and 42%, respectively. By univariate analysis, TBI6 in the conditioning was the only significant predictor for a low relapse risk, and an interval between diagnosis and allo-SCT of more than 12 months was associated with a lower NRM. This study confirms for the first time prospectively that allo-SCT can provide long-term disease control in a sizable albeit limited proportion of patients with T-PLL.Peer reviewe

    In the transportation industry there is a great competitive level between large transportation companies at national and international level with wide coverage, great benefits and direct access, however there are gaps in the system leaving aside a large market that needs these services, obtaining large delays, which will be presented in this writing, this market are those people who require deliveries, shipments or search for packages to and from abroad and even advice on how effective can be these shipments, mostly small businesses cannot acquire the services of transportation companies with large coverage. Based on this, the case study was applied in a microenterprise SA, a company specialized in logistics and transportation at national and international level, both export and import of goods, also has a complementary service of customs consultancy, where we could analyze in detail the step by step process that is performed in this entity noting certain factors where the process is being delayed. This allowed us to establish the processes to be followed.  This project focuses on the search for improvements in the delivery and shipment of packages nationally and internationally in order to provide the customer with a good experience in the service provided.  The respective analysis was carried out showing two major bottlenecks such as order taking and parcel collection, based on this we used tools such as: the application of time study, productivity calculation and critical analysis. Each of these processes were exposed in detail, but we limited the information only with the direct relation with the subject of the study and the time that is handled to work, so that a clear understanding of the processes to follow to achieve the established objective was obtained obtaining an improvement plan for the company.En la industria del transporte se evidencia una gran nivel competitivo entre grandes empresas de transporte a nivel nacional e internacional con amplias coberturas, grandes beneficios y accesos directos, sin embargo hay vacíos en el sistema dejando aparte un amplio mercado que necesita de estos servicios, obteniendo grandes retrasos, en los cuales se presentaran en este escrito, dicho mercado son aquellas personas que requieren realizar entregas, envíos o búsqueda de paquetes desde y hacia el exterior e incluso asesoramientos de que tan efectivos pueden ser estos envíos, en su gran mayoría los pequeños emprendimientos no pueden adquirir los servicios de empresas de transporte de gran cobertura. Basado a esto el caso de estudio fue aplicado en una microempresa de transporte, una compañía especializada en logística y transporte a nivel nacional e internacional, tanto de exportación como de importación de mercancías, también cuenta con servicio complementario de asesoría aduanera, donde pudimos analizar detalladamente el paso a paso del proceso que se realiza en esta entidad notando ciertos factores donde el proceso se está retrasando. Así se permitido establecer los procesos a seguir.  Este proyecto se centra en la búsqueda de mejoras de entrega y envíos de paquetes a nivel nacional e internacional para así brindarle al cliente una buena experiencia en el servicio prestado.  Se realizó el respectivo análisis demostrando dos grandes cuellos de botella como son toma de pedido y recogida de paquetería, basándonos a esto utilizamos las herramientas tales como: la aplicación de estudio de tiempo, cálculo de productividad y realización de análisis crítico. Cada uno de estos procesos fueron expuestos de manera detallada, pero limitamos la información únicamente con la relación directa con el tema del estudio y el tiempo que se maneja para trabajar, de forma que se obtuviera una clara comprensión de los procesos a seguir para alcanzar el objetivo establecido obteniendo un plan de mejora para la empresa &nbsp

    Clear cell chondrosarcoma of the head and neck

    Clear cell chondrosarcoma is a rare variant of chondrosarcoma that mostly involves the end of long bones. However, nine cases have been reported in the head and neck: four in larynx, two in nasal septum, two in maxilla and one in the skull. These cases form the basis of this review. Head and neck cases accounts for less than 5% of Clear cell chondrosarcomas in the whole body and the larynx is the most common place. The histological findings of head and neck cases are consistent with general features of this entity in the whole body and nearly all tumors in this case series had a component of conventional chondrosarcoma. Clear cell chondrosarcoma is an intracompartmental tumor and retains "Grenz zone" just beneath the epithelium. Therefore, the overlying mucosa remained intact in all laryngeal cases. Nasal tumor caused ballooning of the septum and the maxillary lesion did not involve the oral mucosa. This tumor presents various radiographic features in the head and neck area. Chondroblastoma, chondroma, osteoblastoma, osteosarcoma and metastatic renal cell carcinoma are included in the histologic differential diagnoses. Differentiation from chondroblastic osteosarcoma is important in the maxilla. A wide resection is adequate in most cases. However, some laryngeal cases show tendency to recur. Clear cell chondrosarcoma is a slow growing tumor and this necessitates a long time follow-up of patients. Due to the extreme rarity in the head and neck, diagnosis of Clear cell chondrosarcoma in this area, must be confirmed by histochemical and immunohistochemical studies

    Validation of the revised IPSS at transplant in patients with myelodysplastic syndrome/transformed acute myelogenous leukemia receiving allogeneic stem cell transplantation: a retrospective analysis of the EBMT chronic malignancies working party

    The International Prognostic Scoring System has been revised (IPSS-R) to predict prognosis of patients with myelodysplastic syndromes at diagnosis. To validate the use of the IPSS-R assessed before transplant rather than at diagnosis we performed a retrospective analysis of the EBMT database. A total of 579 patients had sufficient information available to calculate IPSS-R at transplant. Median overall survival (OS) from transplant was significantly different according to IPSS-R: very low 23.6 months, low 55.0 months, intermediate 19.7 months, high 13.5 months, very high 7.8 months (P < 0.001). In a multivariate Cox model the following parameters were significant risk factors for OS: IPSS-R, graft source, age and prior treatment. Median relapse free survival also showed significant differences according to IPSS-R: very low: 23.6 months, low: 24.8 months, intermediate 10.6 months, high 7.9 months, very high 5.5 months (P < 0.001). Multivariate risk factors for relapse-free survival (RFS) were: IPSS-R, reduced intensity conditioning, graft source and prior treatment. A trend for an increased relapse incidence was noted for very high risk IPSS-R. We conclude that the IPSS-R at transplant is a useful prognostic score for predicting OS and RFS after transplantation, capturing both disease evolution and response to prior treatment before transplant
