    A review of salt transport in porous media : assessment methods and salt reduction treatments

    It is an unpalatable fact that while objects can deteriorate through lack of care and attention, they can also deteriorate as a result of inappropriate and misguided interventions. This is particularly the case with regard to salt-related deterioration problems. A successful treatment outcome using aqueous salt reduction methods demands an understanding of the transport processes involved as well as detailed information regarding the characteristics and specific situation of the individual object. The use of poultice materials to reduce the salt content of salt deteriorated objects is a long established technique in conservation. However, due to the complex nature of salt problems within historic structures the result of such interventions can be variable and unpredictable. The amount and depth to which salts are mobilised, and where they are transported to, is dependent on the inter-relationship between the poultice and the substrate, the drying conditions and also the initial salt distribution. This paper examines the current scientific understanding of salt and moisture transport processes, and the extent to which this knowledge can feed back into the practical arena, to aid the conservator. Moreover, areas where further research is required are identified. In particular, the importance of pre- and post treatment investigations is highlighted, showing how, in combination with knowledge of salt and moisture transport mechanisms, these can give useful indications regarding treatment options. The role of selective salt extraction and the post treatment behaviour of residual salts are discussed

    Reducing inappropriate antibiotics prescribing: The role of online commentary on physical examination findings

    Objective: This study investigates the relationship of ‘online commentary’(contemporaneous physician comments about physical examination [PE] findings) with (i) parent questioning of the treatment recommendation and (ii) inappropriate antibiotic prescribing. Methods: A nested cross-sectional study of 522 encounters motivated by upper respiratory symptoms in 27 California pediatric practices (38 pediatricians). Physicians completed a post-visit survey regarding physical examination findings, diagnosis, treatment, and whether they perceived the parent as expecting an antibiotic. Taped encounters were coded for ‘problem’ online commentary (PE findings discussed as significant or clearly abnormal) and ‘no problem’ online commentary (PE findings discussed reassuringly as normal or insignificant). Results: Online commentary during the PE occurred in 73% of visits with viral diagnoses (n = 261). Compared to similar cases with ‘no problem’ online commentary, ‘problem’ comments were associated with a 13% greater probability of parents uestioning a non-antibiotic treatment plan (95% CI 0-26%, p = .05,) and a 27% (95% CI: 2-52%, p < .05) greater probability of an inappropriate antibiotic prescription. Conclusion: With viral illnesses, problematic online comments are associated with more pediatrician-parent conflict over non-antibiotic treatment recommendations. This may increase inappropriate antibiotic prescribing. Practice implications: In viral cases, physicians should consider avoiding the use of problematic online commentary

    Conservation science: Reflections and future perspectives

    The ICCROM Forum 2013 on Conservation Science resulted in a series of recommendations for improving the relevance and impact of science within cultural heritage conservation. These recommendations are outlined in this paper. Central to the Forum recommendations is the responsibility of conservation science to provide benefit through research and innovation. This relies on shared strategic vision and good governance, to identify priority needs and align efforts accordingly. To enhance the effectiveness of conservation science research, it is imperative to adopt an approach based on needs assessment, collaboration, and sharing. However, to establish whether desired goals are being met, systematic assessment of what is delivered and how it is used is required. Evaluation tools provide a structured way to identify needs and to measure results, offering a basis for learning and improvement. A new initiative is outlined, launched by ICCROM in follow-up to the Forum, to develop a common framework for needs and outcome assessment for heritage conservation science. To achieve this will require participation and support at multiple levels, and collaboration is called for to continue and sustain this effort

    Ecohydraulic modelling of anabranching rivers

    In this paper we provide the first quantitative evidence of the spatial complexity of habitat diversity across the flow regime for locally anabranching channels, and their potential increased biodiversity value in comparison to managed single-thread rivers. Ecohydraulic modelling is used to provide evidence for the potential ecological value of anabranching channels. Hydraulic habitat (biotopes) of an anabranched reach of the River Wear at Wolsingham, UK is compared with an adjacent artificially straightened single-thread reach downstream. 2D hydraulic modelling was undertaken across the flow regime. Simulated depth and velocity data were used to calculate Froude number (Fr) index; known to be closely associated with biotope type, allowing biotope maps to be produced for each flow simulation using published Fr limits. The gross morphology of the anabranched reach appears to be controlling flow hydraulics, creating a complex and diverse biotope distribution at low and intermediate flows. This contrasts markedly with the near uniform biotope pattern modelled for the heavily modified single-thread reach. As discharge increases the pattern of biotopes altered to reflect a generally higher energy system, interestingly, however, a number of low energy biotopes were activated through the anabranched reach as new sub-channels became inundated and this process is creating valuable refugia for macroinvertebrates and fish, during times of flood. In contrast, these low energy areas were not seen in the straightened single thread reach. Model results suggest that anabranched channels have a vital role to play in regulating flood energy on river systems and in creating and maintaining hydraulic habitat diversity

    Stage dependent variability in tractive force distribution through a riffle–pool sequence

    High resolution data on spatial and temporal variability in flow hydraulics and sediment transport within riffle–pool sequences are required to improve understanding of how fluvial processes maintain these meso-scale bedforms. This paper addresses this issue by providing Ž 3 y 1 . velocity and boundary shear stress data over a range of discharges from base flow 0.07 m s Ž 3 y 1 . to just over bankfull 8.52 m s , from a sequence of four pools and three riffles in the River w Rede, Northumberland. The data supports the reversal hypothesis of Keller Geol. Soc. Am. Bull. Ž. x 87 1971 753. as the primary explanation for the maintenance of the riffle–pool sequence, although they also indicate that spatial variability in tractive force is highly stage dependent and complex. Section-averaged velocity data indicate reversal to be evident at four out of six riffle–pool units. An equalisation in velocity was found for the other two riffle–pool units close to bankfull stage. The spatial patterns of tractive force exhibited in the study reach as a result of increased discharge demonstrate that riffle–pool units operate independently of one another. Shear stress reversals were observed in individual riffle–pool units at different river stages during a flood hydrograph, and in some instances, two occurred in the same riffle–pool unit during a single flow event. Pools were characterised by coarser bed sediments and narrower channel widths in comparison to riffles, increasing the likelihood of tractive force reversal in the River Rede. Areas of predicted bed sediment entrainment obtained from t y t , matched observed channel changes oc in the upper part of the study reach, but over-estimated change in the middle portion of the reach

    Displays and claims of understanding in conversation by people with aphasia

    Background: There is scope for additional research into the specific linguistic and sequential structures used in speech and language therapist (SLT)-led therapeutic conversations with people with aphasia (PWA). Whilst there is some evidence that SLTs use different conversational strategies than the partners of PWA, research to date has focussed mainly on measuring the effects of conversation-based therapies—not on analysing therapeutic conversations taking place between SLTs and PWA. Aims: This paper presents an analysis of the use of oh-prefacing by some PWA during therapeutic supported conversations with SLTs. Methods & Procedures: Normally occurring therapeutic conversations between SLTs and PWA after stroke were qualitatively analysed using Conversation Analysis. Interactions with five PWA were video-recorded, involving three different specialist stroke SLTs. Outcomes & Results: The analysis revealed a difference in the way some PWA use turns that display understanding (e.g., oh right) versus those that continue the conversation, merely claiming understanding (e.g., right). This use of oh-prefacing is similar to that described in the literature on typical conversations. In our data, SLTs are shown to treat oh-prefaced turns differently from non-oh-prefaced turns, by pursuing the topic in the latter, and progressing on to a new topic in the former. Conclusions: At least some PWA use oh-prefacing in the same way as non-language-impaired adults to display understanding of information versus merely claiming to understand. The SLTs in our data are shown to treat non-oh-prefaced turns as mere claims of understanding by providing the PWA with additional information, using supported conversation techniques, and pursuing additional same-topic talk, whereas oh-prefaced turns are treated as displays of understanding by being confirmed, and leading to changes of topic. This study is a first step in providing SLTs with a clearer understanding of the ways in which they are assessing the understanding of PWA, which may in turn help them better support non-therapy staff

    Can falls risk prediction tools correctly identify fall-prone elderly rehabilitation inpatients? A systematic review and meta-analysis

    BACKGROUND: Falls of elderly people may cause permanent disability or death. Particularly susceptible are elderly patients in rehabilitation hospitals. We systematically reviewed the literature to identify falls prediction tools available for assessing elderly inpatients in rehabilitation hospitals. METHODS AND FINDINGS: We searched six electronic databases using comprehensive search strategies developed for each database. Estimates of sensitivity and specificity were plotted in ROC space graphs and pooled across studies. Our search identified three studies which assessed the prediction properties of falls prediction tools in a total of 754 elderly inpatients in rehabilitation hospitals. Only the STRATIFY tool was assessed in all three studies; the other identified tools (PJC-FRAT and DOWNTON) were assessed by a single study. For a STRATIFY cut-score of two, pooled sensitivity was 73% (95%CI 63 to 81%) and pooled specificity was 42% (95%CI 34 to 51%). An indirect comparison of the tools across studies indicated that the DOWNTON tool has the highest sensitivity (92%), while the PJC-FRAT offers the best balance between sensitivity and specificity (73% and 75%, respectively). All studies presented major methodological limitations. CONCLUSIONS: We did not identify any tool which had an optimal balance between sensitivity and specificity, or which were clearly better than a simple clinical judgment of risk of falling. The limited number of identified studies with major methodological limitations impairs sound conclusions on the usefulness of falls risk prediction tools in geriatric rehabilitation hospitals

    The examination of human chromosomes has been a part of the physician’s laboratory armamentarium since the correct diploid number of human chromosomes was established and a method was developed for the in vitro growth of peripheral blood leukocytes to yield metaphase chromosomes. The discovery that on ultraviolet microscopy (UV), metaphase chromosomes stained with fluorochrome dyes displayed a characteristic pattern of bright and dull bands unique for a given pair of homologous chromosomes, was a major technological breakthrough in human cytogenetics; for the first time, every chromosome in the karyotype could be unequivocally identified. Although the short storage life of fluorochrome-stained chromosomes and the costs of UV microscopy have limited the usability of fluorescence banding, the introduction of one discriminating procedure quickly led to the development of an array of similar banding techniques for conventional microscopy that yield comparable information. Some of these technical procedures depend on enzyme and/or heat denaturation of the chromosomes, resulting in the characteristic banding patterns seen by the trypsin-Giemsa method, the 5M urea method, and the acid-saline-Giemsa technique. A typical human karyotype prepared from metaphase chromosomes treated with trypsin, stained with Giemsa, and photographed with brightfield photomicrographic techniques is shown in Figure 1. Careful examination of this karyotype reveals that each chromosome in the homologous pair has an array of dark and light bands identical with those of its homolog and that each homologous pair, autosomes number 1 to number 22, has a characteristic, easily identifiable banding pattern

    Economic sustainability of heritage conservation in Hong Kong: The impact of heritage buildings on adjacent property prices

Using the hedonic price model, the research examines the economic impact of 50 publicly owned versus 50 privately owned heritage buildings on adjacent residential property prices with a sample size of over 43,240 property transaction records spanning a time period of 10 years. The research supports that heritage conservation can promote economic sustainability aside from cultural sustainability and social engagement. This research benefits government policymakers, urban planners, architects, and heritage conservationists by contributing new knowledge to the studies on sustainable urban development, heritage conservation, and cultural economics
