    Medical students’ attitudes towards patient-centered care, Fayoum Medical School, Egypt

    Background: Patient-centered care has been considered the foundation of healthcare quality and the core competency of the doctor-patient relationship.Aims: To assess the attitudes of medical students toward a patient-centered approach and identify the predictors of patient-centeredness scores.Methods: A cross-sectional study was carried out in Fayoum Medical School/Egypt using a validated Arabic version of the Patient-Practitioner Orientation Scale (PPOS).Results: The total PPOS score showed variable score as the grades move up. The total PPOS ranged from 0.05 to 4.39 with an average score of (2.71 ± 0.66) for the entire sample. The sharing and caring subscale score ranged from 0.56 to 4.44 and 0.44 to 5.33 with an average score of (2.33 ± 0.49) and (3.09 ± 0.92) respectively.Conclusion: Medical students had low patient-centered attitude. Medical curricula should be redesigned, and medical students may also benefit from engagement in patient healthcare service

    Correlation between Uterine Hemodynamics, Sex Steroid Hormone Concentrations, and Enzymatic Antioxidant Levels in Postpartum Buffaloes

    The present investigation aimed to evaluate uterine hemodynamics in six multiparous postpartum buffaloes and their relationship with sex steroid hormone concentrations and enzymatic antioxidant levels. The buffaloes were examined by transrectal Doppler ultrasonography to record the vascular perfusion in uterine arteries of both ipsilateral and contralateral ones. All Doppler indices such as peak (PV) endpoints (EV) of velocity, peak systolic velocity (PSV), blood flow volume (BFV), resistance (RI), and pulsatility index (PI) were recorded from 1st to 6th postpartum. The blood samples were collected starting from 1st-week post-calving and every week thereafter following each ultrasound Doppler examination for assay of steroid hormones (progesterone and estradiol) and antioxidant (superoxide dismutase (SOD), glutathione peroxidase (GPX), and catalase, CAT) were measured. PV of the ipsilateral previously gravid arteries showed a linear pattern of significant (P=0.001) decline from the 1st week after parturition till the 6th week. This decline was also associated with a linear decrease in EV from the 1st week till the 6th week after birth. While contralateral PV and EV are not significantly changed throughout weeks after parturition. The levels of SOD and CAT are significantly elevated at 1st week postpartum compared to the 6th week after parturition. In contrast, the GPx levels did not reveal any significant differences during the puerperal period. Estradiol and progesterone declined from 1st to 5th week after parturition. PV of the ipsilateral uterine artery had a significant (P≤0.05) positive correlation with BFV (r=0.49), estradiol 17- α(r=0.98) and progesterone (r=0.85). The same parameter showed a statically (P≤ 0.001) positive correlation with SOD (r=0.87) and CAT (r=0.92). While, Ipsilateral uterine RI showed a significant (P≤0.05) negative correlation with PV(r=-0.85), BFR ((r=-0.62), estradiol 17- α(r=-0.52), and progesterone (r=-0.88), in addition, RI also correlated negatively with both SOD (r=-0.57) and CAT (r=-0.63). Progesterone and estrogen levels are strongly correlated with SOD and CAT. The uterine hemodynamics in buffaloes is affected by the day of the postpartum period. SOD and CAT antioxidants recorded herein, except GPx, increase in the 1st weeks of calving and are affected by the day of the postpartum period

    Modulatory role of chelating agents in diet-induced hypercholesterolemia in rats

    AbstractIntroductionHypercholesterolemia is a major risk factor for the development of atherosclerosis and endothelial dysfunction. Chelating agents may play a modulatory role in atherosclerosis by removal of calcium from atherosclerotic plaques.AimThe present study aimed to explore the effects of calcium disodium ethylenediaminetetraacetic acid (CaNa2EDTA) and meso-2,3-dimercaptosuccinic acid (DMSA) on diet-induced hypercholesterolemia in rats using simvastatin as a reference standard.MethodsHypercholesterolemia was induced by feeding rats with cholesterol-rich diet for six weeks. Rats were divided into five groups (n=8): normal control, hypercholesterolemic control, simvastatin (20mg/kg; p.o.), CaNa2EDTA (100mg/kg; i.p.) and DMSA (100mg/kg; i.p.). Treatments continued daily for the six weeks of diet feeding.ResultsDiet-induced hypercholesterolemia resulted in alterations in the lipid profile markers and a state of oxidative stress coupled by compensatory increase in serum nitric oxide (NO) level and decreased aortic endothelial nitric oxide synthase (eNOS) activity parallel to increased inducible nitric oxide synthase (iNOS) activity, aortic calcium content and aortic wall thickness. Treatment with simvastatin, CaNa2EDTA and DMSA improved lipid profile and oxidative stress markers. In addition, they attenuated hypercholesterolemia-induced changes in serum NO level, aortic eNOS and iNOS activities, calcium content and aortic wall thickness.ConclusionPretreatment of hypercholesterolemic rats with simvastatin, CaNa2EDTA or DMSA attenuated most of the changes induced by feeding rats with cholesterol-rich diet owing to their observed anti-hyperlipidemic and antioxidant properties

    Exogenously applied salicylic acid boosts Morpho-Physiological traits, yield, and water productivity of lowland rice under normal and deficit irrigation

    The main constraint on rice cultivation in the Mediterranean area is the limited irrigation and its large water consumption. In addition, rice is very sensitive to drought conditions because of drought stress on morpho-physiological traits and yield reduction. The application of salicylic acid (SA) has been noticed to be very effective in alleviating the adverse effects of drought stress on rice. The current investigation was conducted as a split-split arrangement under a randomized complete block design with two lowland rice cultivars (Giza177 and Giza179) and SA as a foliar application at four concentrations (0, 400, 700, and 1000 µM) under normal and drought conditions. The results showed that plant growth, leaf photosynthetic pigments, yields, and the most studied traits were significantly affected by irrigation (I), cultivar (C), and SA concentration (p ≤ 0.05 or 0.01). The interaction effect of I × C × SA was only significant on the carotenoids content (p ≤ 0.05). The reduction in grain yield and most studied traits was more pronounced under drought conditions. The Giza179 proved to be a drought-tolerant cultivar under all SA concentrations under drought conditions, while Giza177 was a drought-sensitive cultivar. The application of 700 µM SA gave the best grain yield in both rice cultivars under drought conditions compared to other SA concentrations. Grain yield for normal irrigation (Yp) and drought stress (Ys) conditions were highly positively correlated with indices of the mean productivity (MP), geometric mean productivity (GMP), stress tolerance index (STI), yield index (YI), yield stability index (YSI), drought resistance index (DI), harmonic mean (HM), and golden mean (GOL). While they are highly negatively correlated with the indices of the stress susceptibility index (SSI), tolerance index (TOL), yield reduction ratio (YR), stress susceptibility percentage index (SSPI), and abiotic tolerance index (ATI). It could be concluded that SA, as a growth regulator, could be used to alleviate the harmful effect of inadequate water availability in soil on rice cultivars as well as to improve the growth, water productivity, and grain yield

    SULPHATED POLYSACCHARIDES (SPS) FROM THE GREEN ALGA ULVA FASCIATA EXTRACT MODULATES LIVER AND KIDNEY FUNCTION IN HIGH FAT DIET-INDUCED HYPERCHOLESTEROLEMIC RATS

    Objective: Hypercholesterolemia (HC) was frequently associated with oxidative stress, and release of inflammatory cytokines is to determine the hypolipidemic effects of sulphated polysaccharides from seaweed Ulva fasciata algal extracts through measuring the activities of some parameters related to liver and kidney functions in the serum of hypercholesterolemic rats as compared to normal one.Methods: Different groups of rats were administered a high cholesterol diet. Liver and kidney functions, inflammatory cytokines (TNF-α, CRP, MPO and IL-10), oxidative stress (GSH, MDA and NO), in addition to cell adhesion molecules (ICAM-1 and VCAM-1) were assessed before and after treatment with the algal polysaccharides. In addition, histological examination of liver and kidney were performed to confirm the biochemical findings.Results: The obtained results showed that oxidative stress and inflammatory markers associated with hypercholesterolemia were significantly increased in HC-rats. The histopathological examination of liver and kidney demonstrated severe degeneration with diffuse vacuolar degeneration, necrosis and the presence of fatty droplets. In addition; nephron-histological examination revealed, mild glomerular injury with mild vascular and inflammatory changes. Treatment with the algal sulphated polysaccharides effectively improved these disorders and diminished the formation of fatty liver, as well as renal dysfunction more than the reference drug; fluvastatin. Conclusion: It could be concluded that the consumption of UFP (Ulva fasciata polysaccharides), may be associated with attenuation of inflammatory markers, amelioration of fatty liver and improvement of renal dysfunction, that in turn lead to counteract hypercholesterolemia and its related disorders; such as obesity, and heart disease.Keywords: Non-alcoholic fatty liver disease, Seaweed, Ulva fasciata, Hypercholesterolemia, Hypolipidemic activity, Sulphated polysaccharides (SPs

    A New NO-Releasing Nanoformulation for the Treatment of Pulmonary Arterial Hypertension

The nitric oxide (NO) pathway has been implicated in the pathophysiology and progression of the disease. Its extremely short half-life and systemic effects have hampered the clinical use of NO in PAH. In an attempt to circumvent these major limitations, we have developed a new NO-nanomedicine formulation. The formulation was based on hydrogel-like polymeric composite NO-releasing nanoparticles (NO-RP). The kinetics of NO release from the NO-RP showed a peak at about 120 min followed by a sustained release for over 8 h. The NO-RP did not affect the viability or inflammation responses of endothelial cells. The NO-RP produced concentration-dependent relaxations of pulmonary arteries in mice with PAH induced by hypoxia. In conclusion, NO-RP drugs could considerably enhance the therapeutic potential of NO therapy for PAH.Peer reviewedFinal Published versio

    Performance, Bone Parameters and Phosphorus Excretion of Broilers Fed Low Phosphorus Diets Supplemented with Phytase from 23 to 40 Days of Age

    Abstract: An experiment was conducted using 300 growing Ross 308 broilers from 23 to 40 days of age. Six dietary treatments were formulated. A basal diet contained 1.48% Dicalcium Phosphate (DCP) was used as a control diet (diet 1). Diets 2 and 3 contained 50% and 25% of the DCP of diet 1 (0.74 and 0.37%, respectively), while diet 4 was formulated without DCP. Diets 3 and 4 were fed without or with supplemented phytase enzyme (500 U/kg). Every dietary treatment was fed to 5 replicates (10 chicks each). The results showed no significant differences between birds fed diets containing 1.48 % DCP or 0.74% DCP on BWG, FI and FCR. Reducing dietary DCP level to 0.37% slightly decreased BWG compared with birds fed 0.74% DCP with inferior value of FCR. When DCP was removed from the diet BWG significantly (p<0.01) decreased and FCR recorded worth value. Addition of 500 U phytase/kg to diet 3 of 0.37% DCP significantly enhanced BWG (p<0.01), feed intake and FCR (p<0.05). Addition of phytase to the diet of no DCP did improve neither BWG nor FCR. Decreasing dietary DCP did not significantly affect length, weights and width of tibia either with or without phytase supplementation. However, birds fed the highest level of dietary DCP showed the highest values of tibia weight and length among the different groups. Tibia breaking strength (kgf) significantly (p<0.001) decreased as dietary DCP level decreased. Addition of phytase significantly (p<0.001) improved tibia breaking strength and tibia ash %. Addition of phytase to diet of low DCP did increase tibia Ca and P to reach values comparable with those of the control diet. Decreasing dietary DCP showed significant (p<0.001) decrease in the excreted Ca and P. Addition of phytase to diets of low or no DCP also decreased (p<0.001) the excreted Ca and P. This means that phytase increased the utilization of dietary Ca and P. The excreted Ca and P decreased by 41.22% and 55.26%, respectively, when birds were fed diet of no DCP compared to those fed the control diet. Also, addition of phytase enzyme to diets of low or no DCP decreased the excreted percentage of Ca and P. It could be concluded that reducing dietary P level and using phytase enzyme could be used to limit quantity of P excreted from broilers. This reduce such impact in environmental pollution

    Beneficial Effects of Hesperidin against Cisplatin-Induced Nephrotoxicity and Oxidative Stress in Rats

    Abstract: Cisplatin has been frequently used for treatment of wide variety of tumors. The use of cisplatin is associated with severe cytotoxicity such as nephrotoxicity, hepatotoxicity and spermiotoxicity which radically limits its clinical use. The present study aimed to investigate the possible protective effects of multiple doses of hesperidin against cisplatin-induced nephrotoxicity induced by single i.p injection of cisplatin (7.5 mg/kg). Hesperidin was given to rats at two different doses (100 and 200 mg/kg p.o) for 7 days starting one day before cisplatin injection. Blood samples were collected for determination of serum creatinine and Blood Urea Nitrogen (BUN) levels. Kidneys were used for the determination of Malondialdehyde (MDA), Glutathione (GSH) and total nitrate and nitrite contents. Liver samples were also used for histopathological examination. Results showed that hesperidin significantly reduced cisplatin-induced elevations in serum creatinine and BUN levels. It also significantly reduced kidney MDA and NO content and elevated GSH content. In conclusion, hesperidin greatly protected kidney against cisplatin-induced toxicity in a dose-dependent manner
