[์˜๋ฌธ] The role of neuromuscular system which is responsible for stimulating masticatory region has always managed to bring much attention of many reseanhers in the field of orthodontics, and this has caused to explain relationship between form and function from diverse point of views. It has been conventionally accepted that many types of malocclusion resulted from abnormal function of neuromuscular system, and even the best orthodontic treatment result can not be maintained if the balance between the patient's muscle function and occlusion is disrupted. Presently, however, many orthdontists lean toward the theory that orthodontic treatment, which eliminates occlusal imbalance and stabilizes mandibular position, brings morphologic changes of dentition and jaws, and thereby, induces muscular adaptative changes. EMG has been used as a tool to evaluate muscle activities, but still, study on muscle function of skeletal Class โ…ข malocclusion patients in need for orthognatic surgery is lacking. Thus, based on the theory that muscle function depends on the form of jaw bones and dentition EMG and cephalometric measurements were taken of 20 subjects with normal occlusion and 18 surgical-orthodontic patients with skeletal class โ…ข malocclusion. Biopak(Bie-Research version 3.0) was used for EMG measurement of anterior temporalis, masseter, stemocleidomastoid, and anterior digastric muscles. The following result were obtained: 1. When the cephalometric measurements were compared, there were statistical differences in antero-posterior and vertical measurements between pre-surgical and normal groups; however, there was no statistical difference in most of measurements when normal and post-surgical groups were compared. Also, decrease in both vertical and horizontal dimensions after orthognatic surgery indicates that facial profile is improved by surgical-orthodontic treatment.(p<0.05) 2. Resting EMG was the greatest in pre-surgical group(p<0.05). In clenching, pre-surgical group showed reduced activities of anterior temporalis and masseter, and the masseter activity increased after surgery. Also, in swallowing pre-surgical group showed reduced activities of masseter, diagastric, and SCM compared to the normal group; there was increase in muscle activities after the surgery.(p<0.05) 3. In all three groups, masseter showed highest activity in clenching and digastric muscle showed highest activity in swallowing with statistical significance.(p<0.05) 4. When cephalometric and EMG measurements were correlated, there was negative correlation between vertical facial component and activites of temporalis, masseter, and SCM in clenching of normal group. In pre-surgical group, negative correlations were shown between cephalometric measurements that indicate vertical growth and mandibular prognathism and activities of most muscles.
