    Effect of Noise Cancelling Devices on Dental Handpieces Using Ear Model

    This study intended to evaluate noise level of dental handpieces and the effects of noise cancelling devices. An ear model was designed to measure the level of the noise delivered to the inner ear during dental handpiece operation. The level of noise was measured in various conditions using the ear model and the portable noise meter. Noise level was measured again after applying of 4 different noise cancelling devices to the model ear. The noise level of dental handpieces was 82.5 - 84.4 dB. When 4 types of noise canceling devices were applied, the noise level reduced to 67.4 - 73.8 dB. All 4 devices had statistically significant effect in reducing the noise of the handpiece. Considering the intensity and exposure time, noise in dental clinics can cause hearing damage to dentists. For protection from the risk of occupational hearing damages, noise canceling devices can be recommended.ope

    Analysis of Primary Molar Relationship using Digital Photograph

    The aim of this study was to assess the occlusal relationships of primary dentition of Korean preschool children in Hwaseong city. Total 444 children of age group between 2 and 5 years were selected. One pediatric dentist took intraoral photos of children. Evaluation and categorization of the primary molar relationships were done by single examiner. Statistical analysis was performed with Pearson chi-squared test. Total 389 samples, which is consisted of 185 boys, 204 girls were satisfied with inclusion criteria. Bilateral mesial step was seen in 61.7%. Bilateral flush terminal plane was seen in 15.4%. When the primary molar relations of the right and left sides were not the same, the combination of the mesial step and flush terminal plane was 14.7%. The present study provides insight into occlusal relationships in the primary dentition of 2 - 5 years old of Korean children in Hwaseong city. Pediatric dentists should understand occlusal relationships of primary dentition for establishment of appropriate treatment strategies and occlusal guidance.ope

    The Piston Elastic: A Novel Device for Treating Entrapped Ectopic Permanent Molars

    Ectopic eruption of the permanent molar may absorb the distal root of the primary second molar and may result in a decreased arch length or delayed eruption of the permanent tooth, requiring timely treatment. Therefore, we devised an effective and convenient method to unlock the entrapped tooth using a novel device called a โ€œpiston-elasticโ€. This case report aims to explain the design and clinical application of this piston-elastic and to describe successful cases. Three patients (aged 6, 13, and 16 years) with ectopically erupted maxillary and mandibular molars, respectively, were treated with a piston-elastic. It was bound to the locked molar to improve the eruption path. After a certain time period, the repulsive force pushed the surface of the adjacent tooth, improving the eruption path of the entrapped tooth. The piston-elastic is a novel device that simply and effectively changes the direction of eruption of ectopically entrapped molars. As it can be manufactured and attached to the chair side, impression acquisition on a model cast and laboratory procedures are unnecessary. Compared to existing methods, the piston-elastic can be easily produced and delivered, causes little irritation, and is inexpensive.ope

    Frequency of Buccal Pits and Defective Buccal Pits in Mandibular Molars of Children and Adolescents

    A buccal pit is a prominent point-like depression that appears at the cervical end of the mandibular molar developmental grooves. A defective buccal pit can be defined as a buccal pit in which the continuity of the dentinoenamel junction is broken and the pit extends to the dentinal level. This study aimed to determine the frequency of buccal pits and defective buccal pits in un-erupted mandibular first and second molars using cone-beam computed tomography (CBCT). The analysis was performed on CBCT images taken from 417 Korean children and adolescents who visited the Department of Pediatric Dentistry, Yonsei University Dental Hospital between 2004 and 2020. Based on cross-sectional views of CBCT images, buccal pits were categorized into 4 classes according to the depth of the pits. The expression rate of the buccal pits was 29.1%. The prevalence of defective buccal pits was 7.9%. The buccal pits tended to develop bilaterally. To date, this is the most comprehensive study on the frequency of buccal pits with the largest sample size. This was the first attempt worldwide to analyze the depth of the buccal pit using CBCT images and to define a defective buccal pit worldwide.ope

    Assessment of Risk Factors for Developmental Defects of the Enamel in Preterm

    This study aimed to determine the criteria for quantifying developmental defects of enamel in primary teeth in premature babies and to investigate the severity of developmental defects according to the gestational age, birth weight, systemic complications, and treatments received after preterm birth. Birth information, a history of complications, the duration of parenteral nutrition, and endotracheal intubation were investigated by retrospectively reviewing the admission and discharge records of premature babies in the neonatal intensive care unit. The Preterm Developmental Defects of Enamel (PDDE) index was designed by modifying the existing developmental defects of enamel index. Based on PDDE index, the evaluator scored developmental defects of enamel by classifying them as enamel hypomineralization and hypoplasia. The PDDE scores in the extremely preterm and extremely low birth weight groups were significantly higher than those in other groups. Furthermore, PDDE scores of premature babies with bronchopulmonary dysplasia, rickets, intraventricular hemorrhage, or necrotizing colitis were significantly higher than those in the control group. In addition, more than 50 days of endotracheal intubation and more than 20 days of parenteral nutrition were associated with significantly higher PDDE scores than those in the control group and were risk factors for developmental defects of enamel. This study provides basic information for identifying risk factors for developmental defects of enamel in premature babies.ope

    Usefulness of Mouth Guard when the Endotracheal Intubation Is Indicated for Treacher Collins Syndrome Patient

    Treacher Collins syndrome (TCS) is autosomal dominant disorder that occurs approximately 1 in 25,000 to 50,000 live births. The main signs of syndrome are hypoplasia of facial bone and microtia. One in third of them is associated with cleft palate and often shows dental hypoplasia. TCS patients need several number of surgery with general anesthesia throughout their life time for recovery of function and esthetic. Endotracheal intubation of TCS patient is very difficult due to microstomia, retrognathia, choanal stenosis, and decreased oropharyngeal airway. Therefore, general anesthesia of adolescent TCS patient with immature incisor roots has high risk of causing dental trauma. This case is regarding TCS patient who was referred to the Department of Pediatric Dentistry, Yonsei University for avulsed upper left central incisor during endotracheal intubation. The purpose of this report is to emphasize the usefulness of mouth guard to prevent dental trauma when endotracheal intubation is needed for TCS patient.ope

    The purpose of this study was to compare the validity of Cariviewยฎ, a new colorimetric caries activity test, with CRTยฎ bacteria, a conventional bacterial culture method. In addition, this study assesses the correlation between the dental caries experience and activity between mothers and their children. 34 pairs of mothers and their children under the age of 6 years participated in this study with informed consent. After filling out a questionnaire and oral examination, the two caries activity tests above were performed on each subject. In the results, Cariviewยฎ scores were statistically significant with childrenโ€™s caries experience (r = 0.598, p 0.05). Cariviewยฎ colorimetric test will be clinically useful for predicting future caries risk and establishing a preventative strategy in pediatric dentistry.ope

    Behavioral problems could be easily observed in patients with various mental disorders, and may be inevitable. Self-injurious behavior(SIB) can be defined as a deliberate or subconscious alteration or destruction of one's body without conscious suicidal intent. SIB frequently involves oral tissues and could vary from a trifling injury to damages that could lead to further disabilities. The cases presented in this report discuss oral SIB due to Tic disorder and ADHD and their treatments. Clinicians should be well aware of the possibility of oral SIB in various mental disorders as well as the diverse depths of such behaviors accordingly. Moreover, different treatment modalities should be prioritized according to the causative mental disorders.ope

The treatment change was very fast and wearing was quite easy, compared to extraoral appliances. The purpose of this study was to verify the duration of the treatment on class โ…ข malocclusion using intraoral removable appliances, which designed to accelerate vertical facial growth. 56 patients were selected with the complaint of the protruded mandible and class โ…ข malocclusion (overjet : -3 - 0 mm, overbite : 0 - 4 mm). Information like; age at start, duration of the treatment events, type of the treatment, overjet, overbite etc. was collected and calculated. The average age of the patients delivering the initial brace was 8.75 ยฑ 1.10 year. Most of the anterior crossbite was resolved within 6 months. The total treatment period was 21.79 ยฑ 10.73 months with the additional procedures like the alignment of anterior teeth and torque control using additional removable and fixed orthodontic appliances. The correlation study showed that patientโ€™s cooperation (p = 0.000) and the use of fixed appliance (p = 0.032) were significantly influenced on treatment duration.ope

    VALPROATE-INDUCED GINGIVAL OVERGROWTH : A CASE REPORT

    Drug-induced gingival overgrowth is an abnormal increase of gingival tissues caused as a side effect of systemic medication. This report presents a severe case of valproate-induced gingival overgrowth combined with ulcerative and hemorrhagic lesions in a patient with Lennox-Gastaut syndrome. Considering the patientโ€™s limited cooperative ability, gingivectomy and excisional biopsy under general anesthesia were performed. The lesions were successfully treated without recurrence. When gingival enlargement does not subside with nonsurgical treatments, surgical procedure and excisional biopsy are to be performed. Postoperative management of oral hygiene is critical to prevent recurrence.restrictio
