(A) study for the military medical support system against bioterrorism

병원행정학과/석사[한글] 미국의 9.11테러사건과 북한의 지속적인 생화학무기연구 실태 보고 등에 따라 타 무기에 비해 저렴한 비용과 파급효과가 큰 생물무기에 대한 세계의 관심이 고조되고 있는 가운데 미국의 탄저균에 의한 테러를 계기로 각 나라별로 생물테러가능성에 대비한 대책을 마련하고 있다.우리나라에서도 ‘01년 11월 6일 발효된 테러대비 정부종합대책에 의하여 보건복지부가 생물테러 주관부처로 임명되었고 2003년 12월 질병관리본부에 생물테러대응과를 신설하여 생물테러대비 및 대응 업무를 수행하고 있다. 한편, 군은 생물테러 발생시 민ㆍ관으로부터 지원요청을 받고 합참 지휘통제에 따라 화생방방호사령부에서 화생방테러에 대한 임무를 수행하게 되며, 생물무기에 대해 별도로 대처하기보다는 화생방 방호대책으로 포괄하여 대처하고 있으며, 화생방방호사령부 주도로 민·관·군 합동 훈련을 주도하고 있다. 그러나, 훈련 과정에서 의무지원 부분은 배제된 상태의 평시 물자보급 및 훈련이 이루어지고 있으며, 이는 생물무기에 대한 대처에 많은 문제점을 가져오고 생물학전시 의무지원에도 제한을 가져오게 된다.이 연구는 생물무기에 대한 군 의무지원능력을 재판단해보고, 평시 생물테러에 대비한 군 의무지원체계 확립을 통해 생물학전에도 원활한 의무지원을 할 수 있는 체계를 확립하는데 있으며, 연구의 목적은첫째, 생물무기에 대한 위협이 점점 증가하는 현실과 이에 대한 민·관·군 준비체계 및 현황을 파악해보고둘째, 생물무기사용추세와 비교하여 현 대응시스템 및 대비책상의 문제점을 분석함으로써세째, 민·관·군 공조하에 효과적으로 대응하기 위한 군 의무지원 체계를 수립하는데 있다.이 연구는 생물테러에 대비한 민간 활동의 현 시스템과 활동지원을 위한 장비 및 물자현황 등을 파악하고 민·관·군이 연계하여 조치한 사례인 ‘백색가루’와 ‘사스''등의 사례분석을 통해 체계운용상의 점검결과 및 의무지원의 제한사항을 파악해 보았다. 그리고 외국의 생물테러대비 체계를 알아보기 위해 미국의 9.11 이후 생물테러대비 방안을 주한미군의 준비현황과 함께 분석해 봄으로써 현재 우리군의 생물테러에 대한 준비실태와 특히 의무분야에서의 병원과 연구소 기능을 진단해보고 앞으로의 대응방향을 염출하였다.결론적으로 생물테러에 대비한 원활한 의무지원체계를 구축하기 위해 군병원을 생물테러대비 전문지정병원으로 운영하고, 군 의학연구소의 연구기능을 민간과 차별화하여 확대운영하며, 생물무기 대처에 대해 의무병과의 비중을 높이기 위해 의무에 탐지기구 및 보호장비를 보급하는 등 의무병과를 중심으로한 대응체계를 마련할 수 있도록 한다. 또한 응급실 증후군 관리체계의 일원화와 연계된 교육프로그램을 운용함으로써 군이 보유하고 있는 유용한 인적·물적 자원으로 상시 조직화된 체계를 갖추어, 민·관·군이 유기적 공조체제를 공고히 함으로써 국민의 생명과 재산을 보호함과 동시에 국민의 군대로서 임무를 더욱 확실히 할수 있게 될 것이다. [영문]With the increasing worldwide concerns, due to 9.11 and the report of the ongoing biochemical weapon research in N. Korea, about biological weapons that are cheaper and having far-reaching effects than any other weapons, and being definite the biological terror possibility because of terrorism like the anthrax terror occurred in USA, each countries are starting to consider counter plans against bioterrorism.Korea also determined the Department of Health and Welfare Services as the managing department against the biological terrorism in 2001 by 11.6. Government Comprehensive Countermeasure against terrorism. In addition to that, we established the countermovement section against biological terrorism in KCDC and conducted the duty of prevention & reaction for biological terrorism. The duty of the military is to restrict application of manpower when biological terrorism is occurred. So, when they are needed to support, CBR Defense Command will conduct the duty against biological terrorism. In military, instead of dealing with each biological weapons, managing and training against chemical weapons with NBC soldiers are accomplished as the other part of NBC managing tactics under the management of CBR Defense Command. Training without the support of medical part limits the ability of medical support in real war situations and also causes many problems regarding appropriate management of biological weapons casualitiesTherefore, the goal of this research is to understand the ability of military medical support against biological weapons while accomplishing the system for effective medical support during biological war by establishing military medical support system against biological terrorism during non-war situations. Thus, the objects of the research are:First, determine the current situation, and civilian and military preparationprocesses regarding the increasing threat of biological weapons and,Second, by analyzing problems of the current counter acting system and other processes compared to the current tendency to use biological weapons,third, to establish effective counter acting military medical support system through the cooperation of the military and civilian sectorThis research will understand the supplies and equipments that are needed for the civilian systems that will be used against biological terrorism, and it will also go through limitations of medical support and the check result of system management with the case studies, such as ''SARS'' and ''White Powder''. Also, to gain knowledge regarding other countries'' counter terrorism tactics, I have analyzed American biological terrorism counter tactics after 9.11 with present preperation condition of US Armed Forces in Korea, and provided our future counter terrorism tactics while analyzing our preparation level against biological terrorism focusing on the functions of hospital and laboratory in medical fields. This is to achieve mutual assistance and support system through analyzing current military and civilian biological terrorism system.Finally, to establish the effective medical supporting system against biological terrorism, we need to operate the military hospital as counter biological terrorism specialty hospitals, expand the functions of military research facilities in a separate way to civilian ones, and establish medical centered counter terrorism tactics by supplying detection and protection equipments to change the tactics from chemical to medical ways against biological weapons. Also, through centralization of management system of ER symptoms and usages of connected educational programs, military possessed resources, such as personnels, or material resources, can be utilized in more systematic ways. The mutual assistance between civilian, government, and military will make the military to be able to protect the properties and lives of its citizens and lead it to perform in more proper ways to become the nation''s guard.prohibitio
