Evaluation of the accuracy of left ventricular ejection fraction using gated Tc-99m sestamibi myocardial SPECT

의학과/석사[한글] 심질환 특히 관상동맥 질환에서 좌심실구혈률은 좌심실 기능의 진단과 장기적 예후에 있어 중요한 역할을 하는 것으로 알려져 왔다 Cedars Sinai Gated SPECT Software를 사용하면 심근 관류와 심벽 운동을 동시에 평가할 수 있을 뿐 아니라 좌심실구혈률도 측정할 수 있는 장점이 있다. 이에 저자는 Gated Tc-99m-sestamibi myocardial SPECT(이하 Gated SPECT)의 자동화 프로토콜을 이용하여 측정한 좌심실구혈률을 심초음파를 이용하여 얻은 좌심실구혈률과 비교하여 Gated SPECT를 이용한 좌심실구혈률의 정확도를 평가하고, 그 유용성을 밝히고자 하였다 심근경색증 혹은 협심증이 의심된 200명의 환자(32-74세, 평균 58.3세, 남124명, 여 76명)를 대상으로 Gated SPECT와 심초음파를 7일 이내에 시행하였다. Gated SPECT는 Cedars Sinai 자동화 프로토콜을 사용하여 시행되었다. 감마카메라에는 저에너지 고분해능 조준기률 장착하였고, 에너지는 140keV±20%를 사용하였다. 획득행렬수는 64 × 64개였고 심박수당 8프레임 모드에서 자료를 획득하여 총 60프레임을 얻었다. 심초음파는 경흉부 2차원 심초음파와 M-mode 심초음파를 시행하였고, Quinones 방법에 의해 좌심실구혈률을 측정하였다. Gated SPECT로 얻은 좌심실구혈률은 심초음파로 얻은 좌심실구혈률에 대하여 높은 상관관계를 가졌다(r=0.82, p<0.0001, y=0.85×+16.98). Gated SPECT 상 관류 결손이 있거나 비정상적으로 낮은 좌심실구혈률을 갖는 군에서는 더 강한 양의 상관관계를 보였으나 Gated SPECT상 관류결손이 없거나 정상범위의 좌심실구혈률을 갖는 군의 경우, 상관관계는 낮았다. 전반적으로 Gated SPECT상 좌심실구혈률은 심초음파에 의한 측정값보다 낮게 측 정되는 경향이 있었으며, a 위험은 0.459로 높았으나 보다 심각한 β 위험은 0.029로 낮았다. 그러므로 Gated SPECT는 관류결손여부와 함께 좌심실구혈률을 측정하는데 유용하게 이용될 수 있다고 판단된다. [영문] Left ventricular ejection fraction(LVEF) plays an important role in the evaluation of left ventricular function and long term prognosis of coronary arterial disease. Newly developed Cedars Sinai Gated SPECT automation protocol makes LVEF measurable additionally to the evaluation of myocardial perfusion, and wall motion. The purpose of this study is to evaluate the accuracy of the LVEF using Cated SPECT compared with that obtained by echocardiography. Two hundred patients suspected to have coronary arterial disease (age=32-74, mean 58.3± 7.7, male 124, female 76) had received Gated SPECT and echocardiography within the 7-day interval. Gated SPECT was performed using Cedars Sinai automated quantitative imaging protocol, with a dual head gamma camera equipped with low energy high resolution collimators. Energy window was 140keV±20%, and matrix size was 64×64. Sixty projections with 8 frame/cycle were obtained. Echocardiography was performed using routine 2-dimensional and M-mode transthoracic echocardiography. LVEF was measured by Quinones method. The LVEF measured by Gated SPECT shows linear regression of y=0.85×+16.98, with correlation coefficient 0.82 and p<0.0001, to the echocardiographic LVEF. The better correlation was noted in the groups having perfusion defect or abnormally low LVEF in the Gated SPECT, but in the group having normal LVEF or no perfusion defect, correlation was poorer. LVEF from the Gated SPECT were generally underestimated, showing 0.459 of a risk and 0.029 of β risk .The group showing perfusion defect or 31-50% of LVEF on the Gated SPECT required additional echocardiogaphic evaluation due to the high a risk. However, Gated SPECT shows good efficiency in this study.restrictio

관광현상을 분석하는 인류학적 시선에 대한 일고: 『동아시아 관광의 상호시선』을 중심으로

인류학에서 문화접변이라는 현상은 인류학의 학문적 특성을 부각시 키는데 중요한 부분을 차지한다. 인류학은 이질적인 문화 간의 접촉으로 발생하는 여러 현상을 연구하는 것을 주 연구대상으로 가지고 있을 뿐만 아니라, 인류학자 자신도 대부분 자신에게 익숙하지 않은 이질적인 문화 속에 거의 던져진 채로 연구를 하면서 연구자 스스로 문화접변으로 야기 되는 다양한 변수들로부터 영향을 받지 않을 수 없는 상황에 처하게 된다. 처음부터 제국주의 지배자들과 식민지의 문화가 만나면서 발생하는 다양 한 현상을 중립적인 의미의 문화접변이라는 용어로 해석한 인류학은 지 금도 여전히 이질적인 문화 간의 접촉현상을 연구대상으로 삼는 것을 자 주 목격한다. 문화접변 현상이 인류학의 주요 연구대상이라면, 그것은 접변 이후 해 당 문화와 그 구성원들의 삶의 방식과 의미 또는 가치의 변화 등과 주로 관련된 것이었다. 즉 권력관계, 경제적 차이, 문화적인 영향력, 문명과 기 술의 도입 등이 어떻게 개입되어 각각의 문화에 영향을 미치는 지가 주로 연구된다. 인류학자들은 주로 영향을 더 많이 받을 수밖에 없는 소수민, 원주민, 피지배민, 약소국, 저개발국 등의 사회에 미치는 영향에 더 많은 관심을 기울여 왔다

Sodium polyacrylate를 이용한 하이드로젤 입자의 제조 및 전기장 민감성에 대한 연구

학위논문(석사) - 한국과학기술원 : 생명화학공학과, 2003.2, [ viii, 58 p. ]The electric field responsive behaviors of hydrogel micro- and nanoparticles of sodium polyacrylate, prepared by an inverse emulsion polymerization, were investigated, which follows the swelling transition under a DC electric field. Sodium polyacrylate was polymerized with N,N’-methylenebisacrylamide using a radical initiator in heptane containing emulsifiers and microparticles were prepared with different concentrations of crosslinking agents. While, the hydrogel nanoparticles were obtained by controlling emulsion droplet sizes using two emulsifiers, Span80 and Tween80. The micro and nanoparticles were characterized with various techniques such as fourier transform infrared spectroscopy, dynamic light scattering measurement, particle size analyzer and scanning electron microscopy. The electric field responsive behavior of the prepared hydrogel particles with different crosslinker concentration were measured with particles inserted in ITO electrodes cell by in-situ video monitoring. A significant reduction of electric field responsiveness of hydrogel particles was observed at a high crosslinker concentration. In fact, the electro-responsive swelling ratio of hydrogel particles could be reduced when crosslinking density increases.한국과학기술원 : 생명화학공학과
