    Teaching Practice of Technology Education to Learn Basic Knowledge and Skills of “Technology of Energy Conversion” : Planning and Teaching using Teaching Material “TECHMIRAI”

    In technology education, it is important to learn basic knowledge and skills regarding the “technology of energy conversion.” From this viewpoint, we considered a nine-hour technology class that is planned and taught using the teaching material, “TECH mirai.” As a result of this practice and the examination of 269 students (eighth grade), it was found that the usefulness of technology education and student interest in new technology and consumer electronics improved. The students self-evaluated their technological literacy, such as problem-solving ability, cooperative attitude, and recognition of technology.     Intracavernous injection of vasoactive drugs for treating erectile impotence

    Intracavernous injection of various vasoactive drugs was performed in six erectile failure patients. The effects of each drug were evaluated in four grades: complete functional erection, incomplete functional erection, nonfunctional expansion and no effect. When papaverine hydrochloride (40 mg/ml) was administered, 4 of the 6 patients had nonfunctional expansion. By administering a mixture of 80 mg/ml papaverine hydrochloride with 1.0 mg/ml phentolamine mesylate, nonfunctional expansion was seen in four patients and incomplete functional erection in two patients. As for the effects of prostaglandin E1, incomplete functional erections were seen in all patients, while complete functional erection was seen in two patients. These findings indicated that the patients reacted differently to the same vasoactive drug, and that the papaverine and phentolamine mixture had a stronger effect than papaverine alone, while prostaglandin E1 had the strongest effect on erection. Intracavernous injection of the these drugs is useful for the treatment and differential diagnosis of impotence

    Excess bodyweight is a major public health concern. Obesity, as measured by body-mass index (BMI), has increased inrecent decades in many populations in the world. Obesity is an important risk factor for mortality and morbidity from cardiovascular diseases,diabetes, cancers, and musculoskeletal disorders, causing deaths worldwide. The bariatric surgery is the most efficacious therapy for morbidobesity today. The global total number of bariatric operations performed in 2011 was more than three hundred thousand. In our country, thenumber of bariatric operations performed in 2011 was more than 150 and it is increasing. Obese patient often has psychological and/or socialproblems, which induce a condition that is difficult to treat. Therefore multidisciplinary team approach is needed as American Society forMetabolic & Bariatric Surgery (ASMBS), International Federation for the Surgery of Obesity & Metabolic Disorders (IFSO) and JapaneseSociety for the Surgery of Obesity and Metabolic Disorders(JSSO)mentioned in their guidelines. We have performed bariatric surgery andhave taken a multidisciplinary team approach since 2008. Our team consists of doctors, nurses, dieticians, clinical psychotherapists, socialworkers and Nutrition Support Team (NST).     Oral appliance therapy was approved by national health insurance in Japan in 2004 and oral appliances(OAs)have since been widely used in the treatment of obstructive sleep apnea(OSA). We herein described the process of making clinical practice guidelines by the task force of the Japanese Academy of Dental Sleep Medicine as a work report. In Japan, OAs are covered by national health insurance. In consideration of the balance between medical treatment fees and the price of technical materials, we used a single-piece(monoblock)OA that advanced the mandible forward and limited mouth opening in OSA patients in Japan. The Japanese Academy of Dental Sleep Medicine(JADSM)focused on OAs frequently used for the treatment of OSA in Japan, and considered an evaluation of their effects to benecessary. Clinical practice guidelines were developed using the Grading of Recommendations, Assessment, Development, and Evaluation(GRADE)system. We recommend OAs that advanced the mandible forward and limited mouth opening for patients with OSA.However, CPAP should be used by patients for whom it has been indicated. OAs are desirable for those who cannot use CPAP(GRADE 1B, strong recommendation/quality of evidence, “Moderate quality”). The long-term effects and side effects, OSA severity, and comorbidities of OA therapy were not examined, which represented a limitation to the present study. In future studies, the Japanese Academy of Dental Sleep Medicine plan to update clinical practice guidelines for oral appliances used in OSA
